Effect of total and rumen undegradable protein on the performance of cows fed low fiber diets.

Twelve multiparous and 18 primiparous Holstein cows were fed a 17.3% CP, 21.0% ADF diet during wk 2 through 6 postpartum. Cows then were assigned from wk 7 through 14 to one of three low fiber (10.7% ADF) dietary treatments containing either 14.4 or 18.7% CP, the latter with or without a soybean meal enhanced with rumen undegradable protein. Treatments had no effect on milk yield or composition in multiparous cows, although milk fat percentage was not depressed in multiparous cows receiving the low fiber diets. The soybean meal diet enhanced with rumen undegradable protein increased yields of milk, 4% FCM, fat, protein, and DMI compared with the 14.4% CP diet in primiparous cows; it also increased yields of 4% FCM and fat versus the 18.7% CP, untreated diet in primiparous cows. Blood urea N concentrations were greater for high CP diets than for the low CP diet in both parity groups. Rumen acetate: propionate ratios were higher for both high CP diets than for the low CP diet in multiparous cows. Soybean meal enhanced with rumen undegradable protein improved yields of milk and its components in primiparous cows fed low fiber diets, even when high protein diets were fed.

Multi-farm use of bovine somatotropin for two consecutive lactations and its effects on lactational performance, health, and reproduction.

In a two-lactation study, 352 Holstein cows (124 primiparous) from six commercial dairy herds were assigned to daily injections of bST. Dosages were 0 (control), 5.15, 10.3, or 16.5 mg/d of bST; injections began 28 to 35 d postpartum. During yr 1, FCM production increased linearly as bST dose increased for primiparous and multiparous cows. However, FCM production for primiparous cows that were retained for yr 2 was not increased by bST, and the increase in FCM for multiparous cows was only 67% of that observed during yr 1 for doses of 10.3 and 16.5 mg/d. Milk SCC were not increased with the use of bST during either lactation. Days to conception of multiparous cows during yr 1 tended to increase linearly with increasing dose. Multiparous cows administered bST during yr 2 tended to have a lower conception rate than control cows (71 vs. 87%, respectively). Body condition was decreased linearly by bST dose at the end of yr 1 and 2. Prior to receiving bST during yr 2, bST-treated cows regained less body condition than control cows. Number of health disorders at parturition and assisted births of cows that received bST during yr 1 were unaffected by previous treatment. General health, ratio of single to multiple births, and BW gain of calves was similar among treatments.

Winter evaluation of a postmilking powdered teat dip.

A powdered teat dip designed for winter usage was evaluated for bacteriological efficacy and teat conditioning qualities. A positive control, natural exposure field trial was conducted for 3 mo on 509 lactating cows. Two sets of cows, primiparous and multiparous, were used. The trial compared efficacy of a powdered teat dip with a teat dip of 1% iodine plus 10% glycerin. Bacteriological efficacy among primiparous cows was equivalent for all major mastitis pathogens, environmental pathogens, and streptococci other than Streptococcus agalactiae. Efficacy was not equivalent against coagulase-negative staphylococci and all mastitis pathogens. Results suggested that the positive control product was more efficacious. Among multiparous cows, efficacy was equivalent against environmental mastitis pathogens and bacteriologically negative, clinical mastitis. The products were not equivalent against Staphylococcus aureus, coagulase-negative staphylococci, or all major mastitis pathogens, once again suggesting that the positive control product was more efficacious. Data indicated that germicidal activity of the powdered dip was not sufficient to reduce the incidence of new IMI caused by contagious or minor pathogens normally associated with teat skin. Application of a powdered postmilking teat dip during 3 winter mo in Idaho resulted in improved teat end condition among primiparous and multiparous dairy cows. Teat skin condition improved among primiparous but not among multiparous cows.

Production responses of cows to recombinantly derived bovine somatotropin and to frequency of milking.

To determine whether bST additively increases milk production in cows milked at different frequencies per day, 118 Holstein primiparous and multiparous cows were milked two or three times daily beginning at parturition and received either 14 mg of bST or no injection beginning at d 75 of lactation. Increased milking frequency from two to three times daily increased 3.5% FCM in multiparous (4.7 kg/d) and primiparous (4.1 kg/d) cows over 305 d. Injection of bST increased FCM 4.3 kg/d in multiparous cows and 5.0 kg/d in primiparous cows over 230 d. Increased milking frequency from two to three times daily reduced milk fat and protein percentages in milk, but bST generally did not affect these percentages. Injection of bST prevented increases in body condition score as lactation advanced, but increasing milking frequency from two to three times daily did not. In general, bST and increased milking frequency additively increased FCM in multiparous and primiparous cows.

Induced abortion among women attending antenatal clinics in Yaounde, Cameroon.

OBJECTIVES: Unsafe abortion is a public health concern because of its impact on maternal morbidity and mortality. The objective of this study was to document on induced abortion in Yaounde, Cameroon. DESIGN: Cross-sectional study. SETTING: Six antenatal clinics in Yaounde, Cameroon. METHODS: Women attending antenatal clinics between October and December 1998 were included in the study and interviewed. Nulliparous were women with no previous delivery and multiparous were defined as women who had at least one previous delivery. RESULTS: Out of the 1532 women, five hundred seventy-two were nulliparous and 960 were multiparous. Of the nulliparous women 17% reported a previous abortion ever; this proportion exceeded 35% in those over 24 years. For multiparous women, the proportion who reported an abortion (between the last birth and present pregnancy) was 22%. In multivariate analysis on the group of nulliparous women, older age, having used modern contraception and having spent more than two years in the city were significantly associated with induced abortion. In the multiparous group, older age, having a full time job and antenatal clinic were significantly associated with induced abortion. CONCLUSIONS: This study shows that induced abortion is a common practice in urban Cameroon. Because of restrictive laws, a substantial proportion of these abortions are likely to be unsafe, with the risk of associated complications. There is a need for expanded comprehensive sexual and reproductive health services.

[Contribution to the anatomical-morphological study of the pelvic floor in the asymptomatic female: the use of MRI imaging].

OBJECTIVES: To confirm the results of previous studies demonstrating the morphology of the levator ani muscle in the living subject is different to that described in classic anatomical works; to evaluate the anatomical-morphological differences of the pelvic floor between nulliparous and multiparous women in order to analyze the influence of pregnancy and delivery. METHODS: Comparative study of the morphological variations of the iliococcigeous fascicle of the levator ani muscle between two groups of females using T2 MRI.: the first group included 11 nulliparous women and the second group 9 multiparous women. The curvature radium of the fascicles was calculated in the frontal projections. The differences in height were also calculated. The anterior limit of the iliococcygeal muscle dome was determined in the sagittal plane. The software Image Tool 3.0 was employed for the measurements. The Kolmogorov-Smirnov test was employed to analyze the distribution of the study populations; mean value comparisons between groups were performed by the Student's t test. Finally, the various morphological measurements were compared in relation to various parameters: parity, side, body mass index (BMI), sports practice, menstrual cycle phase, and presence or absence of episiotomy. Data obtained were analyzed using the Fisher's exact test, with a statistical significance of p < 0.05. RESULTS: Morphological measurements: We observed that the concavity of the iliococcygeal fascicle dome is larger in nulliparous women (p = 0.03 for the right side and p = 0.04 for the left). Moreover, these women have the domes significantly more anterior (p < 0.001 for both sides). Comparisons between other variables: an association between nulliparous status and the presence of a higher and more anterior left dome, and multiparous status and a higher and more anterior right dome were (p = 0.02). CONCLUSIONS: The hypothesis of the pelvic floor morphology being a double dome with inferior-posterior concavity in the living asymptomatic female is confirmed. We also demonstrate the existence of anatomical-morphological differences in the iliococcygeal muscle of the levator ani between nulliparous and multiparous females, which seems to confirm a relationship with pregnancy and/or delivery.

[Aging and the immune system. Experimental aspects].

So far no comparative studies have been conducted to know whether physiological influences related to sex hormonal differences affect the age-related changes of the immune system. The aim of this study was to investigate whether pregnancies and sex influence the age-related changes in the peripheral lymphoid compartment and functions of T cells in mice. Using flow cytometry, we examined changes in (Thy1.2+) T cells, (B220+) B cells and (CD11b/Mac-1+) macrophages in the spleen of multiparous and virgin females and males at 2, 8, 15 and 23 months of age. The development of naive (CD44low) and memory (CD44high) cells were investigated in CD4+ and CD8+ T cell subsets. To analyze the age-related changes in functions of T cells, we examined the secretion of some T cell immunoregulatory cytokines (IL-2, IL-4, gamma-interferon and GM-CSF) of in vitro Concanavalin A-activated spleen cells of C57BL/6 mice. Both short term (8 months) and long term (15-23 months) effects of pregnancies were obvious in the age-related changes of the immune system. Short term effect included delayed appearance of memory CD4+ cells and the preserved IL-2 production. At eight months, shortly after pregnancies, both parameters were higher in multiparous females. Later effects of pregnancies were evidenced by a higher level of macrophages (Mac-1+) than in other groups throughout life. The increased gamma-interferon, IL-4 and GM-CSF productions appeared earlier, at 15 months, IL-4 and GM-CSF levels remained higher in multiparous females than in virgin females and males in late adulthood. Sex differences were also noticed: males exhibited lower macrophage levels after one year and gamma-interferon secretion capacity than females in late life. This study underlines that the onset, magnitude and kinetics of the age-related changes in the distribution of immune cells and T cell functions are parity- and sex-dependent. These changes may influence the incidence of age-related diseases and may explain the greater longevity of women, especially the multiparous ones.

Comparison of three intrauterine contraceptive devices: the Antigon-F, the Ypsilon-Y, and the Copper-T 200.

A comparative study of three intrauterine contraceptive devices, the Antigon-F, the Copper-T, and the Ypsilon-Y, was carried out at The New York Hospital Cornell Medical Center during an 18-month period from March 1, 1972, through August 31, 1973. At the cutoff date of February 28, 1974, all patients had been followed for at least 6 months. During the period of study, 884 Antigon-F devices, representing 14,436 woman-months of use, were inserted. The over-all pregnancy rate was 0.88, with an explusion rate of 5.1 and a medical removal rate of 9.1/100 woman-years. This device was best tolerated by multiparous women. There was a total of 910 insertions of the Ypilson-Y, representing 14,348 woman-months of use. The over-all pregnancy rate was 1.8, with an explusion rate of 1.9 and a medical removal rate of 3.7/100 woman-years. The pregnancy rate in nulliparous women was 0.78, with a continuation rate of 88.9. The pregnancy rate in multiparous woman was higher, i.e., 2.4. Nine hundred thirty-nine Copper-T 200 devices, representing 15,558 woman-months, were inserted. The over-all pregnancy rate was 0.85, with an expulsion rate of 1.8, a medical removal rate of 4.5, and a continuation rate of 88.4/100 woman-years. The pregnancy rate was 0.82 for nulliparous women and slightly higher, i.e., 1.04, for multiparous women. Thus, all three devices had considerable merit. The Antigon-F and the Copper-T seemed the best in multiparous women; the Ypsilon-Y in the size used was best in nulliparous women. The only perforations occurred with the Copper-T.

A comparison of the outcomes of partnership caseload midwifery and standard hospital care in low risk mothers.

BACKGROUND: Maternal and infant clinical outcomes were compared for low risk mothers receiving a partnership caseload model of midwifery care, known as Primary Health Midwifery Care (PHMC), and standard hospital care (SHC). METHODS: Using secondary analysis of data from the Obstet Data System routine collection (PHMC n=976, SHC n=976) from a large metropolitan hospital, maternal and infant clinical outcomes were examined. RESULTS: Odds ratios (OR) demonstrated reduced rates of interventions for multiparous women (OR 0.62 [CI 0.49-0.80]), with multiparous women receiving PHMC being more likely to have a normal delivery (OR 1.75 [CI 1.22-2.5]). A higher proportion of both primiparous and multiparous women receiving PHMC received pethidine during labour (OR 1.78 [1.33-2.39], OR 1.55 [1.19-2.01] respectively). Primiparous women receiving PHMC underwent fewer episiotomies with an associated increase in the proportion of women experiencing perineal tears (OR 1.93, CI 2.35-2.78), although perineum trauma rates were similar for both care models. Similar and very small numbers of infants in both parity groups and care models had an Apgar of less than seven at five minutes or were admitted to the neonatal intensive care unit or special care unit. CONCLUSION: This study, within the limitations of its design, supports the safety of the partnership caseload midwifery care model, in addition to reduced rates of interventions experienced by multiparous women and fewer episiotomies in primiparous low risk English-speaking women receiving caseload care.

Reducing the cesarean section rate in a rural community hospital.

OBJECTIVE: To determine the success of a program designed to reduce the cesarean section rate in a rural community hospital, to identify reasons for any reduction in the rate and to identify any accompanying increases in the maternal and neonatal morbidity and mortality rates. DESIGN: Longitudinal study of modes of delivery. SETTING: A 44-bed community hospital with a medical staff of nine family physicians serving a population of 9000. PATIENTS: All 1161 women who gave birth at the hospital from Jan. 1, 1985, to Dec. 31, 1989. Routinely recorded data were manually extracted from medical charts and entered into a computer database. INTERVENTION: The guidelines of the National Consensus Conference on Aspects of Cesarean Birth (NCCACB) for vaginal birth after cesarean section (VBAC), management of breech presentation and the diagnosis of dystocia requiring cesarean section were introduced at the hospital in 1985. OUTCOME MEASURES: The annual overall cesarean section rates and the rates among nulliparous women, multiparous women eligible for VBAC and multiparous women ineligible for VBAC. RESULTS: The overall cesarean section rate decreased from 23% in 1985 to 13% in 1989 (p = 0.001). Among the nulliparous women the rate decreased from 23% to 12%, but the difference was insignificant (p = 0.069); this decrease was due to a drop in the number of dystocia-related cesarean sections. The rate among VBAC-eligible multiparous women decreased from 93% to 36% (p less than 0.001) because of an increased acceptance of VBAC by the patients and the physicians. The rate among multiparous women ineligible for VBAC was virtually unchanged. There were 20 neonatal transfers to an intensive care unit, with no tendency toward an increase over the study period. None of the mothers died; one newborn, of a nulliparous woman, died from a prolapsed umbilical cord. CONCLUSIONS: The program was accompanied by a significant decrease in the cesarean section rate. Rural hospitals with facilities and personnel for emergency cesarean sections should consider the introduction of a similar program.

Changes in the electric resistance of the vaginal mucus in oestrous sows.

Changes in the electric resistance of the vaginal mucus were monitored in a large pig herd at 4-h intervals with a heat detector (Hauptner, Federal Republic of Germany) in multiparous sows (n = 16; litter size = 11.4 +/- 2.9) that showed a positive standing test and conceived. The initial value of resistance was 74.4 +/- 7.4 ohms. In 36 h this value gradually increased to 93.6 +/- 7.6 ohms. One to 2 h after the artificial inseminations (AI) lower resistance values were obtained, though the difference was not statistically significant. In the second part of the experiment 73.9% and 82.7% of the gilts (n = 92) and multiparous sows (n = 98) conceived, respectively, with an average litter size of 9.6 +/- 2.2 and 11.0 +/- 2.7, respectively. The conception rate of animals that immediately before AI had vaginal mucus resistance values between 75 and 90 ohms was about 20% higher. The only exception were 8 multiparous sows which had an average vaginal mucus resistance of 70 +/- 4.6 ohms after the first insemination. Litter size was also the biggest in sows with vaginal mucus resistance between 75 and 90 ohms. This difference was statistically significant for the multiparous sows. The heat detector is considered to be a useful complement to conventional methods of oestrus detection.

Labor patterns in women with previous cesareans.

Little information exists to help determine the presence or significance of labor abnormalities in women attempting vaginal birth after previous cesarean. A case-control study was performed to obtain information on patterns of labor progress and the incidence of dysfunctional labor in patients having a trial of labor after previous cesarean delivery. Sixty-eight such women were matched to nulliparous and multiparous controls. Labor-curve characteristics for the group of women with previous cesarean differed significantly from those of both the nulliparas and multiparas. When stratified by history of previous vaginal birth, however, those with no previous vaginal birth were indistinguishable from nulliparous controls and those with a previous vaginal birth were indistinguishable from multiparous controls. Parity-specific criteria for the diagnosis of dysfunctional labor were thus indicated. Labor disorders were present most frequently in the previous-cesarean group with no previous vaginal birth (41.9%). This incidence did not differ significantly from that in the control nulliparas (27.1%) (P = .15), but did differ from that in the multiparas (15.8%) (P less than .01). Previous-cesarean patients with a previous vaginal birth had a frequency of labor disorders (14.3%) not significantly different from that of multiparous controls. We conclude that trial of labor in women with a previous cesarean should be evaluated by standard criteria for nulliparas if there has been no previous vaginal birth, and by criteria for multiparous women if there has been any previous infant born vaginally.

Relationship between test-day measures of somatic cell count and milk production in California dairy cows.

The relationship between test-day measures of milk somatic cell count and milk yield was evaluated using the November 1985 test data from 8352 Holstein cattle (2923 primiparous and 5429 multiparous cows) located in ten Tulare County, California dairies. Following correction for herd and stage of lactation effects, design variable regression was used to create separate models for primiparous and multiparous cows predicting the changes in milk production associated with milk somatic cell count class. Cell counts were stratified by 1/2 loge cell count (x1000 cells/mL) units, permitting comparisons with previous studies. Cell counts less than 148,000/mL were not found to be associated with significant reductions in milk yield when compared to the reference class (cell counts less than 20,000/mL). Consistent incremental decreases in milk production were not noted with increasing cell count strata, even following the natural log transformation. The most dramatic production losses were noted in the range of 148,000 to 665,000 cells/mL. Primiparous cattle in the 403,000 to 665,000 cell count strata experienced a 5.22 kg (19.72%) decrease in test-day milk yield. Multiparous cattle in the same class experienced 3.01 kg (7.82%) reductions in milk production. Primiparous and multiparous cows had similar production losses. The study population differed from previous studies on the basis of herd size, milk production and the level of udder health, measured by milk somatic cell count. These differences and the choice of experimental design may in part explain differences in study results and conclusions.

Transforming growth factor-alpha promotes mammary tumorigenesis through selective survival and growth of secretory epithelial cells.

Transforming growth factor (TGF)-alpha stimulates the growth and development of mammary epithelial cells and is implicated in the pathogenesis of human breast cancer. In this report we evaluate the consequences of overexpressing TGF-alpha in the mammary gland of transgenic mice and examine associated cellular mechanisms. When operating on a FVB/N genetic background (line MT100), TGF-alpha induced the stochastic development of mammary adenomas and adenocarcinomas f secretory epithelial origin in 64% of multiparous females. In contrast, tumors were exceedingly rare in virgin MT100 females, MT100 males, and multiparous FVB/N females. In MT100 females multiple foci of hyperplastic secretory lesions preceded the development of frank tumors; these initial lesions appeared during the involution period after the first lactation. Serial transplantation of these hyperplasias indicated an absence of proliferative immortality. Nevertheless, they gave rise to tumors at a low frequency and after a prolonged latency in virgin hosts; in multiparous hosts, tumors developed earlier and at a high incidence. The TGF-alpha transgene was highly expressed in hyperplasias and tumors but not in virgin and nonlesion-bearing tissue, suggesting that TGF-alpha overexpression provides a selective growth advantage. TGF-alpha also induced at lactation a 6.4-fold increase in DNA synthesis in MT100 epithelial cells, many of which were binucleated. MT100 mammary tissue experienced an obvious delay in involution, resulting in the postlactational survival of a significant population of unregressed secretory epithelial cells. In contrast, another line of transgenic mice on a CD-1 genetic background (MT42), in which TGF-alpha overexpression induced liver but not mammary tumors, failed to demonstrate postlactational epithelial cell survival. These data show that TGF-alpha promotes mammary tumorigenesis in multiparous MT100 mice by stimulating secretory epithelial cell proliferation during lactation and prolonging survival during involution. These points support the notion that TGF-alpha can act as a mitogen and also as a differentiation factor in mammary epithelium.

Sole haemorrhages and heel horn erosion in dairy cows: the influence of housing system on their prevalence and severity.

Altogether 94% of the primiparous and 66% of the multiparous cows had haemorrhages of the sole at trimming 2-4 months after calving. Within cows, and independent of age and stall system, the lateral hind claws had higher scores of haemorrhages than the other claws. The primiparous cows had higher scores than the multiparous cows both in the tie-stalls and in the cubicles. The palmar/plantar region of the claw capsule was divided into zones ("white line zones" and "sole zones"). For the multiparous cows the haemorrhage scores of the "white line zones" were higher among the cows housed in the cubicles than among those housed in the tie stalls. There was no difference between stall systems for haemorrhages of the "sole zones". Heel horn erosion was absent or rare in the fore hooves of both the primiparous and multiparous cows in the tie-stalls. The cows in the cubicles had higher heel horn erosion scores in their fore hooves than the cows in the tie-stalls. The scores of the hind hooves were generally higher than those of the fore hooves in both systems. Heel horn erosions were reduced in association with the introduction of a footbath. A higher incidence of lameness was observed in the cubicle system.

Risk of perinatal death associated with labor after previous cesarean delivery in uncomplicated term pregnancies.

CONTEXT: Trial of labor after previous cesarean delivery is associated with increased risk of uterine rupture. However, no reliable data exist on the effect of a trial of labor on the risk of perinatal death in otherwise uncomplicated term pregnancies. OBJECTIVE: To determine the risk of intrapartum stillbirth or neonatal death not related to congenital abnormality among women with uncomplicated term pregnancies who had a trial of labor after previous cesarean delivery, compared with women having a planned repeat cesarean delivery, and multiparous and nulliparous women at term not delivered by planned cesarean method. DESIGN AND SETTING: Population-based, retrospective cohort study of data from the linked Scottish Morbidity Record and Stillbirth and Neonatal Death Enquiry encompassing births in Scotland between January 1, 1992, and December 31, 1997. POPULATION: A total of 313 238 singleton births between 37 and 43 weeks' gestational age in which the fetus was in a cephalic presentation. MAIN OUTCOME MEASURE: Delivery-related perinatal death, defined as intrapartum stillbirth or neonatal death unrelated to congenital anomaly, compared among the 4 groups. RESULTS: Among women who had a trial of labor following previous cesarean delivery (n = 15 515), the overall rate of delivery-related perinatal death was 12.9 (95% confidence interval [CI], 7.9-19.9) per 10 000 women. This was approximately 11 times greater (odds ratio [OR], 11.6; 95% CI, 1.6-86.7) than the risk associated with planned repeat cesarean delivery (n = 9014), more than twice (OR, 2.2; 95% CI, 1.3-3.5) the risk associated with other multiparous women in labor (n = 151 549), and similar to the risk among nulliparous women in labor (n = 137 160; OR, 1.3; 95% CI, 0.8-2.1). The associations were not explained by differences in maternal height, smoking status, socioeconomic status, age, fetal growth, or week of gestation at delivery. Among women having a trial of labor, the rate of death due to mechanical causes, including uterine rupture, was 4.5 (95% CI, 1.8-9.3) per 10 000 women. This was more than 8 times greater than other multiparous women (OR, 8.5; 95% CI, 3.2-22.3) and nulliparous women (OR, 8.8; 95% CI, 3.2-24.2). CONCLUSIONS: The absolute risk of perinatal death associated with trial of labor following previous cesarean delivery is low. However, in our study, the risk was significantly higher than that associated with planned repeat cesarean delivery, and there was a marked excess of deaths due to uterine rupture compared with other women in labor.

Hippocampal morphology is differentially affected by reproductive experience in the mother.

Pregnancy and mothering result in a number of hormonal, neurological, and behavioral changes that are necessary to ensure reproductive success. With subsequent reproductive experience (multiparity and mothering), further neurological and behavioral changes may result. Recent research has shown that previous motherhood enhances both hippocampus-dependent learning and memory and long-term potentiation (LTP); together with decreases in hippocampus volumes during pregnancy it is suggested that the hippocampus is affected by pregnancy and/or mothering. The present experiment aimed to investigate the effect of reproductive experience (nulli, primi-, and multiparity and mothering) on dendritic morphology in the CA1 and CA3 regions of the hippocampus. Brains were stained with a modified version of the single-section Golgi impregnation technique, and dendritic length, number of branch points, and spine density was analyzed for apical and basal regions of CA1 and CA3 pyramidal neurons. Primiparity and/or mothering resulted in dendritic remodeling in both the CA1 and CA3 hippocampal regions, and multiparity resulted in enhanced spine density in the basal CA1 region, which was positively correlated with number of male pups in a litter. These findings point to the effect of reproductive experience and offspring on plasticity in the hippocampus, an area not traditionally associated with motherhood.

Buschke-Ollendorff syndrome in a grande multipara: a case report and short review of the literature.

A 36-year-old short-statured grande multipara (gravida 10, para 8) with diabetes mellitus and hyperlipidaemia was incidentally found to have Buschke-Ollendorff syndrome (osteopoikilosis and dermatofibrosis lenticularis disseminata). The pelvis and hips, followed by the knees, were the sites mainly affected by the osteopoikilosis. The lumbosacral spine was also affected. She had a single connective tissue naevus on the right thigh. Apart from two abortions, the repeated pregnancies were uneventful and all her deliveries were normal at full-term or nearly full-term. With regard to the relationship between Buschke-Ollendorff syndrome and multiparity, it was apparent that multiparity did not affect the involved weight bearing bony structures, nor did the disseminated osteopoikilosis interfere with the endurance of multiple pregnancies. Buschke-Ollendorff syndrome is known generally to have a benign course, a feature that is illustrated in this case, even when associated with the stresses of multiparity. The report also provides a short and updated review of various clinical aspects of the syndrome and its associations, some of which are of a serious nature.
