Randomised controlled trial of therapeutic massage in the management of stress.

BACKGROUND: Many patients presenting in general practice are suffering from stress. Approaches using complementary therapeies are gaining popularity but have not been systematically evaluated. AIM: To compare the effect of six sessions of therapeutic massage with the use of a relaxation tape on stress, as measured by The General Health Questionnaire (GHQ-30). DESIGN OF STUDY: Randomised controlled trial. SETTING: A stress reduction clinic in general practice. METHOD: There were three treatment groups. Patients in the first group received six sessions of therapeutic massage, which was carried out by a nurse trained in this technique. Patients in the second group were given six sessions using a relaxation tape in the surgery and those in the third group were given a relaxation tape to use at home. The main outcome measures were: the GHQ-30, the Adapted Well Being Index (AWBI); a sleep scale; general practitioner (GP) consultations for any reason in the six weeks before treatment, during treatment, and the six weeks following treatment; and patient satisfaction. RESULTS: Sixty-nine patients completed the treatment. There were significant improvements across all four outcome measures and all three treatment groups. Following treatment the majority of patients felt far less emotional disturbance, were sleeping better, and consulting their GP less. CONCLUSION: Despite very strong patient preference for therapeutic massage, it did not show any benefits over either a relaxation tape used in the surgery or a relaxation tape used at home.

[Massage and health].

In this first article, the author, Dr. Sagrera Ferrandiz, introduces us to the most generic aspects of massage and its relationship with health. In order to do so, he carries out a historical review in the use of massage beginning with the Vedas, Hinduism's sacred texts, and ending in our days. In future articles, the author will deal with different types and uses of massage and their relationships with health.

Abdominal massage: another cause of maternal mortality.

BACKGROUND: Abdominal massage is an age long practice among obstetric and non obstetric patients in different parts of the riverine areas of Southern Nigeria. AIM: To highlight the consequences associated with this practice in pregnancy and labour even among literate clients. METHODS: Two hundred and eighty four pregnant women who presented as obstetric emergencies in pregnancy and labour at the University of Port Harcourt Teaching Hospital from 1st May 1988 to 31st July 1998 were studied. RESULTS: Forty two (14.79%) of the 284 patients had abdominal massage. Maternal mortality rate was 4.76% while the perinatal mortality rate was 14.29%. Twenty four of the patients were literate though 15 of them only had primary education. Most of the patients were unbooked (76.19%). The complication of pregnancy noted were cephalo pelvic disproportion /obstructed labour (28. 57%) and abruption placenta (23.80%); placenta previa (9.52%); retained placenta (4.76%) abortion / preterm labour (19.04%); genital injuries and tears (7.14%); perinatal death (14.28%); uterine rupture (9.52%); maternal mortality (4.76%). CONCLUSION: Abdominal massage is a silent killer, which has added to matemal and perinatal mortality and morbidity in this region. Ignorance, illiteracy and poverty have sustained this practice, while underdevelopment, inaccessibility and ill-equipped medical facilities have kept this traditional practice flourishing even amongst the educated group. The time to stop this form of avoidable losses is now. This can be achieved through public health approach, mass literacy, even distribution of affordable healthcare services, improved social services and transportation, robust economy and a grass root involvement of rural programmes aimed at eradicating harmful traditional practice.

A blended infant massage--parenting enhancement program for recovering substance-abusing mothers.

Interventions that build upon the natural components of early mother-infant interactions are critical to reversing the sequelae of maternal substance abuse and breaking the cycle of addiction. This paper proposes a theoretical model that blends infant massage (IM) into a planned parenting enhancement program (PEP) to promote improved health outcomes in recovering substance- abusing mothers (SAMs) and their babies. With 4.6 million women of child-bearing age regularly using cocaine in the United States and 750,000 drug-exposed births annually, maternal substance abuse highlights the multigenerational impact of drug use in high-risk populations and its risks to our children. The proposed IMPEP model provides a means to assist recovering SAMs in making cognitive-behavioral changes through new knowledge about parenting and parenting skills, with a special focus on infant stimulation via massage. The goal is to enable recovering SAMs to become confident and responsive mothers, empowering them to become effective parents. Pilot data suggest the Infant Massage Parenting Enhancement Program (IMPEP) is effective for both mother and infant, and merits a controlled systematic study.

Hysteroscopic resection of a large submucosal fibroid using intermittent bimanual uterine massage and a bipolar resectoscope: a case report.

BACKGROUND: Hysteroscopic resection of large sub-mucosal myomas embedded deep in the myometrium is often unsuccessful due to absorption of large amounts of nonionic fluid that may result in water intoxication or incomplete resection of the intramural portion of the tumor, which later protrudes within the cavity. CASE: A 35-year-old woman with a large, sessile, sub-mucosal leiomyoma underwent hysteroscopic resection using a bipolar resectoscope under laparoscopic monitoring with intermittent uterine massage. An isotonic electrolyte solution was used for uterine distention and visualization. Frequent massaging of the uterus during the procedure resulted in the expulsion of the fibroid into the endometrial cavity and facilitated complete resection. CONCLUSION: A bipolar resectoscope, which can be used with electrolyte solutions, such as 0.9% NaCl, combined with uterine massage, allows complete removal of large leiomyomas without adverse electrolyte imbalances.

[Comparison and analysis on therapeutic effects of acupuncture plus massage therapy and drug on infantile diarrhea].

OBJECTIVE: To compare therapeutic effects of acupuncture plus massage therapy and western medicine on infantile diarrhea. METHODS: A total of 120 cases of infantile diarrhea were randomly divided into a treatment group of 80 cases and a control group of 40 cases. The treatment group were treated by acupuncture and massage therapy, and the control group by smecta. RESULTS: The cured rate of 55.0% in the treatment group was better than 35.0% in the control group (P < 0.05). CONCLUSION: Acupuncture plus massage therapy has obvious therapeutic effect on infantile diarrhea.

[Clinical observation on 136 cases of chilblains treated by acupuncture combined with massage].

OBJECTIVE: To compare therapeutic effects of acupuncture plus massage and medicine on chilblains. METHODS: Two hundred and sixty-four cases of chilblains were randomly divided into an acupuncture group of 136 cases and a medicine group of 128 cases. The acupuncture group were treated by acupuncture at 9 acupoints such as Yamen (GV 15), Laogong (PC 8), Sanyinjiao (SP 6), etc. plus massage, and the medicine group by dong chuang Plaster. RESULTS: In the acupuncture group, 111 cases were cured and 136 cases were effective, the effective rate being 100.0%; and in the medicine group, 55 cases were cured and 98 cases were effective, the effective rate being 76.6%, with a very significant difference between the two groups in the therapeutic effect (P < 0.01). CONCLUSION: Acupuncture plus massage is superior to dong chuang plaster in therapeutic effect on chilblains.

[Clinical observation on electroacupuncture combined with massage for treatment of knee osteoarthritis complicated by gonarthromeningitis].

OBJECTIVE: To observe therapeutic effect of acupuncture and moxibustion combined with massage on gonarthromeningitis complicated with knee osteoarthritis of Bengalese. METHODS: Acupuncture combined with electroacupuncture was given at Heding (Ex-LE2), Nei- and Wai-Xiyan (Ex-LE4, Ex-LE5) and others. and massage was given around the knee joints to treat 51 Bengalese patients of gonarthromeningitis complicated by knee osteoarthritis for one course (10 sessions of treatment). RESULTS: Seventeen cases were markedly-effective, 25 were effective, and 9 were ineffective, with a total effective rate of 82.4%. CONCLUSION: Chinese traditional acupuncture, moxibustion and massage have satisfactory therapeutic effect on gonarthromeningitis complicated by knee osteoarthritis of Bengalese.

[Controlled study on treatment of cervical spondylopathy of the nerve root type with acupuncture, moxibustion and massage as main].

OBJECTIVE: To probe into clinical value of comprehensive program of acupuncture, moxibustion and massage as main for treatment of cervical spondylopathy of the nerve root type. METHODS: Five centers, single blind, randomized controlled method were used, 660 cases were divided into a treatment group of 317 cases and a control group of 311 cases. They were treated respectively with comprehensive program of acupuncture, moxibustion and massage as main, and comprehensive program of physical therapy as main. Establish syndrome detection scale and multiply dimensional effect assessment indexes, and evaluate the therapeutic effects and safety. RESULTS: The cured rate, the cured-markedly effective rate were 42.9%, 64.4% in the treatment group, respectively, better than 16.7%, 36.3% in the control group (P<0.01); after treatment of 2 weeks, clinical symptoms improved in the both groups, but the treatment group was better than the control group in the improvement degrees of neck-shoulder-limb pain, neck rigidity, abnormality of cervical anteflexion, etc. (P<0.01 or P<0.05); the treatment group was shorter than the control group in the time of producing the effect and therapeutic course (P<0.01). CONCLUSION: Comprehensive program of acupuncture, moxibustion and massage as main is safe and effective for treatment of cervical spondylopathy, with a better therapeutic effect compared with the comprehensive program of physical therapy.

[Clinical study on round-sharp needle therapy combined with massage for prolapse of lumbar intervertebral disk].

OBJECTIVE: To search for a better therapy for prolapse of lumbar intervertebral disk. METHODS: One hundred and sixteen cases were randomly divided into a treatment group treated with round sharp needle plus massage, and a control group with filiform needle plus massage, 58 cases in each group. Changes of symptoms and signs were observed before and after treatment. RESULTS: The effective rate was 98.2% in the treatment group and 82.8% in the control group (P < 0.01). CONCLUSION: Round sharp needle combined with massage has a better therapeutic effect on prolapse of lumbar intervertebral disk.

[Effect of acupuncture and massage at tendon on F-wave of electromyogram in the patient of flexor spasm of the upper limb after operation of cervical spondylosis].

OBJECTIVE: To explore the mechanism of acupuncture and massage at the tendon in treatment of the upper limb flexor spasm after cervical operation. METHODS: Fifty-six cases were randomly divided into a treatment group (n = 30) treated with acupuncture and massage at the tendons, and a control group treated with traditional acupuncture and medication. Changes of F-wave, time limit and threshold value in electromyogram before and after treatment were investigated in the both groups. RESULTS: Before treatment, the F-wave in the patient of the upper limb spasm post-cervical operation was characterized by high amplitude, long time limit and low threshold value, with no significant difference between the two groups (P > 0.05); after treatment, the amplitude lowered (P < 0.01), the time limit shortened (P < 0.001) in the treatment group and no significant changes were found in the control group (P > 0.05), but the amplitude of F-wave in the treatment group was significantly lower than that in the control group (P < 0.05). The threshold value significantly increased in the two groups, with the treatment group more significantly increased than the control group (P < 0.05). CONCLUSION: Acupuncture and massage at tendons can relieve the spasm through lowing the excitement in the cells of the anterior horn of the spinal cord, decreasing the amplitude and time limit of the F-wave, increasing the threshold value and reducing traction reflection.

[Effectiveness of climatic health resort complex treatment of patients with pneumoconiosis and dust-induced bronchitis using underwater massage shower].

The article provides the results of studies of 167 cases affected with occupationally-related pneumoconiosis and dust bronchitis. 117 patients were treated with complex resort therapy including under-water massage, and 50 patients were given the same complex therapy without shower massage. The effectiveness of the therapeutic techniques were evaluated basing on both objective and subjective criteria, including supplementary laboratory and functional techniques. The results were positive in both groups, but in the major group, the favourable shifts were more expressed but statistically were not reliable. Thus, the underwater shower massage may be regarded as a positive but not decisive factor in the complex resort treatment of occupational diseases of lungs.

[Indirect cardiac massage as a cause of pulmonary fat embolism].

55 cases of non-traumatic death with preterminal treatment by indirect heart massage showed fat pulmonary embolism in 42 per cent. Cases with thoracal fractures caused by massage were positive in 67 per cent. Controls were represented by a group of traumatic deaths with multiple fractures (80 per cent positivity) and a group of non-traumatic deaths without application of massage (6 per cent positivity).

[Study on "massage to activate the meridian" apparatus in the treatment of cholecystolithiasis].

This apparatus is designed in accordance with the doctrine and principles of traditional Chinese medicine and based on the theory of meridian and specific massage procedure in conjunction with the "Information Theory" and electron utilizing technique. It utilizes a programmed induction "Special Wave" to stimulate a series of acupuncture points, combined with administration of western drugs from a regime of "General Attack" treatment on the gallbladder stones, to enhance their evacuation. By stimulating the specific acupuncture points with this apparatus through the action of "Special Wave" strong contraction of the gallbladder can be elicitated. Experiments on dogs showed that under such stimulation the intra-gallbladder pressure can be increased 26 times, producing a jet of bile flow which facilitates the expulsion and evacuation of stones in the gallbladder. Method of use: first place a conductive rubber pad on the acupuncture point to be stimulated, then use a hand-held electrode to massage-stimulate the meridian. It is carried out forward and backward along the direction of meridian. The whole procedure is carried out under direct visualization with the help of ultrasonic B-scan. When dilatation of the common bile duct is observed, the hand-help electrode is used to perform push-compress massage on the meridian, and acupuncture points making the stones enter the common bile duct B-scan give proper orientation for the manipulation. This method has been tested clinically on 410 patients with good results. The evacuation rate of the gallbladder stones was 92.7% with complete evacuation achieved in 17.3%.(ABSTRACT TRUNCATED AT 250 WORDS)

Cryosurgery versus dilation and massage for the treatment of recurrent urethral syndrome.

One year after the initiation of a prospective, randomized, crossover trial comparing dilation and massage to urethral cryosurgery utilizing a specially designed urethral cryoprobe, our results show that cryosurgery was more effective in the treatment of recurrent urethral syndrome in women. Ninety-one percent of patients first treated with cryosurgery were successful in achieving relief from their symptoms whereas only 33% of the women initially treated with dilation and massage were successful (P = .005). In addition, cryosurgery was more successful in treating women when they crossed over after failing the other therapy (75% vs. 0%). In total, 21 of 24 patients (87.6%) were treated successfully with one of the two modalities. Nineteen of the 22 patients (86.4%) in this select group treated with cryosurgery during the trial were successful, whereas only 5 of 15 (33.3%) treated with dilation and massage were successful (P = .001).

Admittance plethysmographic evaluation of undulatory massage for the edematous limb.

A new apparatus, called Hadomer has been developed for the treatment of peripheral lymphedema and venous disorder. It has cuffs with 5 rooms, through which the air pressure moves from periphery to proximal point, just like surging waves. The peripheral stagnant lymph and venous blood are displaced toward the heart by this pneumatic massage. Hadomer has been applied to more than 400 patients with the edematous limbs, with satisfactory results, such as decrease of swelling, pain and induration. These clinical results have been also confirmed by electrical admittance plethysmography which is useful to measure noninvasively the edematous volume and the blood flow. The admittance of the edematous limbs is high and after massaging with Hadomer it approaches normal range. The blood flow in the diseased limbs is less than the healthy one and it is observed better after pneumatically massaging.

[Heart massage. Liver rupture and thrombolysis].

Complications to external cardiac massage with special regard to liver damage are shortly reviewed. A case story, in which a young healthy woman suffered rupture of the liver followed by hypovolaemic shock after a few minutes of cardiac massage and subsequent thrombolysis, is presented. It is discussed whether the common use of thrombolytic treatment will cause more cases of clinically important haemorrhage after external cardiac massage.

[Value of prostatic massage for detection of Chlamydia trachomatis in the male urethra].

The aim of this work was to evaluate prostatic massage (PM) as diagnostic method of chronic chlamydial infection (possible etiologic agent of male infertility or chronic prostatitis). 105 men were examined. One urethral swabbing and one urine sampling are done before PM. After PM, prostatic fluid is collected spontaneously and in a minimum volume of urine. Chlamydial isolation samples are treated by 3 methods: culture on HeLa 229 cells, immunoenzymology and direct immunofluorescence. Of 107 samplings, 22 were positive. In 13 patients (14 samplings) the result was positive by PM alone. In those cases, prostatic massage was the only way to establish the Chlamydial infection, when urethral swabbing, usual method, is negative. If confirmed, the interest of prostatic massage rests on detection of unknown genital Chlamydial infection, and this, by a non surgical method.
