Monolithic diphasic gels of mullite by sol-gel process under ultrasound stimulation.

Diphasic gel in the mullite composition was prepared from a colloidal sol of boehmite mixed with a hydrolyzed tetraethoxisilane (TEOS) solution. The boehmite sol was obtained by peptization of a poorly crystallized or very small mean crystallite size (approximately 34 A) precipitate, resulting from the reaction between solutions of aluminum sulfate and sodium hydroxide. Ultrasound was utilized in the processes of the TEOS hydrolysis and the boehmite peptization, and also for complete homogenization of the mixture to gel. The wet gel is almost clear and monolithic. The gel transparency is lost on drying, when syneresis has ended, so that the interlinked pore structure starts to empty and is recovered upon water re-absorption. Cracking closely accompanies this critical drying process. Differential thermal analysis (DTA) and X-ray diffraction (XRD) show that the solid structure of the gel is composed of an amorphous silica phase, as a matrix, and a colloidal sized crystalline phase of boehmite. Upon heat treatment, the boehmite phase within the gel closely follows the same transition sequence as in pure alumina shifted towards higher temperatures. Orthorhombic mullite formation was detected at 1300 degrees C.

Hydrothermal preparation, structures, and NLO properties of the rare earth molybdenyl iodates, RE(MoO2)(IO3)4(OH) [RE = Nd, Sm, Eu].

The reactions of RE(IO3)3 [RE = Nd, Sm, Eu] with I2O5 and MoO3 in a 1:2:2 molar ratio at 200 degrees C in aqueous media provide access to RE(MoO2)(IO3)4(OH) [RE = Nd (1), Sm (2), Eu (3)] as pure phases as determined from powder X-ray diffraction data. Single crystal X-ray diffraction experiments demonstrate that these compounds are isostructural and crystallize in the chiral and polar space group P2(1). The structures are composed of three-dimensional networks formed from eight-coordinate, square antiprismatic RE3+ cations and MoO2(OH)+ moieties that are bound by bridging iodate anions. The Mo(VI) centers are present in distorted octahedral environments composed of two cis-oxo atoms, a hydroxo group, and three bridging iodate anions arranged in a fac geometry. There are four crystallographically unique iodate anions in the structures of 1-3, one of these is actually present in the form of a IO3+1 polyhedron where a short interaction of 2.285(4) A is formed between the iodate anion and the hydroxo group bound to the Mo(VI) center. This interaction results in significant distortions of the iodate anion similar to those found in tellurites with TeO3+1 units. Two of the four iodate anions are aligned along the polar b-axis, imparting the required polarity to these compounds. Second-harmonic generation (SHG) measurements on sieved powders of 1 show a response of 350 x alpha-quartz. Crystallographic data: 1, monoclinic, space group P2(1), a = 6.9383(5) A, b = 14.0279(9) A, c = 7.0397(5) A, beta = 114.890(1) degrees, Z = 2; 2, monoclinic, space group P2(1), a = 6.9243(6) A, b = 13.963(1) A, c = 7.0229(6) A, beta = 114.681(1) degrees, Z = 2; 3, monoclinic, space group P2(1), a = 6.9169(6) A, b = 13.943(1) A, c = 7.0170(6) A, beta = 114.542(1) degrees, Z = 2.

The Synthesis, NMR Spectroscopy, and X-ray Structure of a New Rhenium N(2)S(2) Chelate Complex.

A new chelate, mercaptoacetyl-L-histidinyl-S-benzyl-L-cysteine methyl ester was synthesized by standard peptide coupling techniques and reacted with ReOCl(3)(PPh(3))(2) to give two diastereomers, 7a and 7b. The two isomers were separated by reversed-phase HPLC and characterized by NMR spectroscopy and electrospray mass spectrometry. An X-ray structure of one isomer, 7a, confirmed that the chelated complex was analogous to other Re-N(2)S(2) compounds in that it formed a square pyramidal complex where the four donor atoms were the base of the pyramid and the oxygen attached to the rhenium was at the apex. The S-benzyl group, as expected, was cleaved during the formation of 7, and the resulting complex was a zwitterion where the rhenium was formally -1 and the counterion was the protonated imidazole ring.

Normalization of epidermal calcium distribution profile in reconstructed human epidermis is related to improvement of terminal differentiation and stratum corneum barrier formation.

Calcium plays an important role in the regulation of cellular differentiation and desquamation of epidermal keratinocytes. In this study, we examined the calcium distribution in reconstructed epidermis in an attempt to understand the physiology of keratinocyte differentiation and desquamation in vitro. Ion capture cytochemistry (the potassium oxalate-pyroantimonate method) was employed to localize ionic calcium in reconstructed epidermis generated under three different culture conditions (in serum-containing medium, serum-free medium, and serum-free medium supplemented with retinoic acid), allowing a comparison of the physiology of incompletely and well-differentiated keratinocytes. The reconstructed epidermis generated in serum-containing medium showed features of incomplete differentiation, and compared with the native skin, a high calcium content within incompletely differentiated cells in the stratum corneum. Use of serum-free medium containing vitamin and lipid supplements led to a marked improvement of the stratum corneum ultrastructure and penetration pathway across the stratum corneum, indicating improved barrier formation of the reconstructed epidermis. In parallel, the calcium distribution pattern was normalized showing the highest levels of calcium in the stratum granulosum and low levels in the inner stratum corneum. Addition of retinoic acid to the serum-free medium resulted in an altered keratinocyte differentiation and re-appearance of large quantities of calcium precipitates in the stratum corneum. Proton probe X-ray microanalysis was applied to investigate the calcium distribution quantitatively in native and reconstructed epidermis generated in serum-free medium, and verified the calcium distribution demonstrated by the precipitation technique. Regardless of the presence or absence of calcium in the stratum corneum, all examined culture systems exhibited insufficient desquamation, which correlates with the finding that stratum corneum chymotryptic enzyme was present predominantly as an inactive precursor. This study demonstrates that improvement of the stratum corneum barrier properties in vitro is concurrent with the normalization of the epidermal calcium gradient, whereas deregulation of terminal differentiation correlates with an accumulation of calcium ions within incompletely differentiated corneocytes.

Dilated cardiomyopathy.

On account of the rapidly progressive nature of the disease, the high incidence of systemic embolism and arrhythmias, and the risk of sudden death, dilated cardiomyopathy is inconsistent with aircrew duties. In the small proportion of subjects who apparently make a satisfactory recovery , re-licensing may be considered once there is no requirement for medication and chest X-ray, electrocardiogram, echocardiogram, exercise electrocardiogram, 24 h ambulatory electrocardiogram and left ventricular ejection fraction are all normal. These tests should initially be repeated annually and be associated with cardiological review.

Understanding enzyme structure and function in terms of the shifting specificity model.

The purpose of this paper is to suggest that the prominence of Haldane's explanation for enzyme catalysis significantly hinders investigations in understanding enzyme structure and function. This occurs despite the existence of much evidence that the Haldane model cannot embrace. Some of the evidence, in fact, disproves the model. A brief history of the explanation of enzyme catalysis is presented. The currently accepted view of enzyme catalysis--the Haldane model--is examined in terms of its strengths and weaknesses. An alternate model for general enzyme catalysis (the Shifting Specificity model) is reintroduced and an assessment of why it may be superior to the Haldane model is presented. Finally, it is proposed that a re-examination of many current aspects in enzyme structure and function (specifically, protein folding, x-ray and NMR structure analyses, enzyme stability curves, enzyme mimics, catalytic antibodies, and the loose packing of enzyme folded forms) in terms of the new model may offer crucial insights.

[Tension pneumothorax after simultaneous bilateral lung resection].

A 72-year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray film. Chest CT showed a mass 6 cm in diameter in left S1 + 2, and a small round mass in right S2. Left side mass was diagnosed squamous cell carcinoma by transbronchial biopsy, but right side mass was unidentified. We performed partial resection for a right S2 mass with VATS, and then left upper lobe lobectomy and mediastinal lymph node dissection simultaneously. Pathological examination revealed the right mass was hamartoma. He discharged on 16 postoperative days uneventfully. But 3 days after he was sent to our hospital on emergency because dyspnea and unconscious. Chest X-ray revealed right side tension pneumothorax, then he was recovered by chest tube insertion. At re-thoracotomy we confirmed air leakage was occurred from a ruptured bulla that was leaved at first operation.

Pulmonary lymphangioleiomyomatosis in a 29-year-old woman with bilateral spontaneous pneumothorax: case report.

Pulmonary lymphangioleiomyomatosis is a rare, progressive and fatal interstitial lung disease that affects women of child-bearing age. It is characterized by the nonneoplastic proliferation of atypical smooth muscle cells around lymphatics, venules, and bronchioles. Patients with pulmonary lymphangioleiomyomatosis most commonly present with exertional dyspnea and a cough. Pneumothorax is almost always found on the initial chest radiograph. We report a case of a 29-year-old woman who presented with abrupt right chest pain and progressive dyspnea. Physical examination in the emergency department was significantly diminished breath sounds over the right lung, and a chest X-ray revealed a right-sided pneumothorax. She required a chest tube for complete lung re-expansion. On the sixth day of admission, left-sided pneumothorax occurred and another chest tube was inserted. She underwent high-resolution computed tomography of the thorax with the findings of numerous cysts evenly distributed throughout the parenchyma of both lungs. Bilateral thoracotomy with lung biopsies and pleurodesis were performed and the surgical findings confirmed the presence of multiple, small, pink-colored cystic lesions on the surfaces of both lungs. Pathological examination showed the characteristic findings of pulmonary lymphangioleiomyomatosis.

[A case of spontaneous regression of malignant lymphoma, complicated with miliary tuberculosis, liver dysfunction and pancytopenia].

An 82-year-old woman was admitted to Iwamizawa Rosai Hospital, Cardiovascular Medicine, for detailed examination of an aneurysm of abdominal aorta. CT scan revealed marked swelling of para-aortic lymph nodes which regressed spontaneously in three months. She was re-admitted to our hospital because of general malaise and gait disturbance. Her chest X-ray on the second admission revealed miliary disseminated shadows, which were confirmed to be tuberculous granuloma by lung biopsy. She was initially well controlled with anti-tuberculosis drugs, followed by severe liver dysfunction and pancytopenia, and died of respiratory and cardiac failure. At autopsy, wide-spread malignant lymphoma and miliary tuberculosis of the lung were found. The cause of liver dysfunction was strongly suspected to be due to infiltration of the lymphoma cells to portal triads of the liver. Hemophagocytosis found in the bone marrow, spleen and liver might be consistent with clinically so-called "hemophagocytic syndrome", causing pancytopenia, associated with infection.

Strategy for membrane protein crystallization exemplified with OmpA and OmpX.

The bacterial outer membrane proteins OmpA and OmpX were modified in such a manner that they yielded bulky crystals diffracting X-rays isotropically beyond 2 A resolution and permitting detailed structural analyses. The procedure involved semi-directed mutagenesis, mass production into inclusion bodies, and (re)naturation therefrom; it should be applicable for a broader range of membrane proteins.

Exploring the melting of a semirigid-chain polymer with temperature-resolved small-angle X-ray scattering.

The thermal behavior of semirigid semicrystalline polymers differs significantly from that of flexible-chain polymers. The origin of the differences is believed to lie in the higher energy expenditure associated with the formation of adjacent re-entry folds at the crystalline surface in the case of semirigid chains. The effect of constraints imposed by the interlamellar amorphous regions on the neighboring crystals was studied with temperature-resolved synchrotron radiation small-angle X-ray scattering (SAXS). The analysis of SAXS patterns with a generalized paracrystalline lamellar stack model indicates that melting of a semirigid-chain polymer is not a random process but that the crystals grown in the smallest amorphous gaps melt first. This suggests that the hitherto largely neglected geometrical confinement effects may play an important role in determining the thermodynamic stability of semirigid-chain polymer crystals.

New paramagnetic Re(II) compounds with nitrile and cyanide ligands prepared by homolytic scission of dirhenium complexes.

The preparation and characterization of three new paramagnetic complexes of the 17-electron Re(II) ion are reported. The salts [Re(triphos)(CH(3)CN)(3))][X](2), X = [BF(4)](-) (1), [PF(6)](-) (2), and [Et(4)N][Re(triphos)(CN)(3)] (3) were prepared by homolytic cleavage of the Re-Re bond in [Re(2)(CH(3)CN)(10)][BF(4)](4) or by disruption of the chlorine bridges in [(triphos)Re(mu-Cl)(3)Re(triphos)]Cl (1) (triphos = 1,1,1-tris(diphenylphosphino-methyl)ethane) and characterized by single-crystal X-ray diffraction, infrared and (1)H NMR spectroscopies, cyclic voltammetry, and magnetic susceptibility measurements. Compound 2 undergoes reversible reduction and irreversible oxidation processes while 3 undergoes a reversible reduction, an irreversible oxidation, and a reversible oxidation. The magnetic susceptibility data for 2 and 3 exhibit a strong temperature independent paramagnetic component which is in accord with a highly anisotropic S = (1)/(2) magnetic ground state. The results of this study indicate that dinuclear Re(2)(II,II) starting materials are viable precursors for producing unusual mononuclear Re(II) complexes.

Head-to-head (HH) and head-to-tail (HT) conformers of cis-bis guanine ligands bound to the [Re(CO)3]+ core.

We have prepared four complexes of the type [Re(guanine)(2)(X)(CO)(3)] (guanine = 9-methylguanine or 7-methylguanine, X = H(2)O or Br) in order to understand the factors determining the orientation of coordinated purine ligands around the [Re(CO)(3)](+) core. The 9-methylguanine ligand (9-MeG) was chosen as the simplest N(9) derivatized guanine, and 7-methylguanine (7-MeG) was chosen because metal binding to N(9) does not impose steric hindrance. Two types of structures have been elucidated by X-ray crystallography, an HH (head-to-head) and HT (head-to-tail) conformer for each of the guanines. All complexes crystallize in monoclinic space groups: [Re(9-MeG)(2)(H(2)O)(CO)(3)]ClO(4) (2) in P2(1)/n with a = 12.3307(10) A, b = 16.2620(14) A, c = 13.7171(11) A, and beta = 105.525(9) degrees, V = 2650.2(4) A(3), with the two bases in HT orientation and its conformer [Re(9-MeG)(2)(H(2)O)(CO)(3)]Br (3) in P2(1)/n with a = 15.626(13) A, b = 9.5269(5) A, c = 15.4078(13) A, and beta = 76.951(1) degrees, V = 2234.5(3) A(3), and the two bases in an HH orientation. Similarly, [Re(7-MeG)(2)(H(2)O)(CO)(3)]ClO(4) (4) crystallizes in P2(1)/c with a = 13.0708(9) A, b = 15.4082(7) A, c = 14.316(9) A, and beta = 117.236(7) degrees, V = 2563.5(3) A(3), and exhibits an HT orientation and [ReBr(7-MeG)(2)(CO)(3)] (5) in P2/c with a = 17.5117(9) A, b = 9.8842(7) A, c = 15.3539(1) A, and beta = 100.824(7) degrees, V = 2610.3(3) A(3), and shows an HH orientation. When crystals of any of these complex pairs are dissolved in D(2)O, the (1)H NMR spectrum shows a single peak for the H(8) resonance of the respective coordinated purine indicating a rapid equilibrium between HH and HT conformations in solution. DFT calculations simulating the rotation of one ligand around its Re-N bond showed energetic barriers of less than 8.7 kcal/mol. We find no hypochromic effect in the Raman spectrum of 3, which showed base stacking in the solid state. Neither steric interactions nor hydrogen bonding are important in determining the orientation of the ligands in the coordination sphere.

New calculations of the atmospheric cosmic radiation field--results for neutron spectra.

The propagation of primary cosmic rays through the Earth's atmosphere and the energy spectra of the resulting secondary particles have been calculated using the Monte Carlo transport code FLUKA with several novel auxiliary methods. Solar-modulated primary cosmic ray spectra were determined through an analysis of simultaneous proton and helium measurements made on spacecraft or high-altitude balloon flights. Primary protons and helium ions are generated within the rigidity range of 0.5 GV-20 TV, uniform in cos2theta. For a given location, primaries above the effective angle-dependent geomagnetic cut-off rigidity, and re-entrant albedo protons, are transported through the atmosphere. Helium ions are initially transported using a separate transport code called HEAVY to simulate fragmentation. HEAVY interfaces with FLUKA to provide interaction starting points for each nucleon originating from a helium nucleus. Calculated cosmic ray neutron spectra and consequent dosimetric quantities for locations with a wide range of altitude (atmospheric depth) and geomagnetic cut-off are presented and compared with measurements made on a high-altitude aeroplane. Helium ion propagation using HEAVY and inclusion of re-entrant albedo protons with the incident primary spectra significantly improved the agreement of the calculated cosmic ray neutron spectra with measured spectra. These cosmic ray propagation calculations provide the basis for a new atmospheric ionising radiation (AIR) model for air-crew dosimetry, calculation of effects on microelectronics, production of cosmogenic radionuclides and other uses.

Repeated bone lead levels in Queensland, Australia--previously a high lead environment.

Bone lead levels (finger and tibia) were measured, both in vivo and in vitro, with an x-ray fluorescence technique. In vivo finger-bone lead levels of 105 volunteers were estimated and subsequently re-estimated after a 5-y period. In many instances, elevated levels in some of these individuals probably resulted from lead ingested during childhood (i.e., more than 60 y ago). A decline in finger-bone lead from the initial value was accompanied by a mean finger-bone lead half-life of 6.2 y (n = 25); many individual's half-life values were less than 3.0 y. Perhaps one reason for these low values is the special propensity of finger bone to resorption during the involutional period, at which time there are high levels of circulating parathormone.

[Tension pneumothorax manifested after extubation in a patient who underwent total gastrectomy].

A patient developed tension pneumothorax immediately after extubation. The patient was a 53-year-old man, who underwent total gastrectomy under general anesthesia combined with epidural anesthesia. The posterior mediastinum drainage tube was placed near the site of esophago-jejunum anastomosis. Surgeons reported that they might have injured left diaphragmatic pleura during the procedure. Postoperative chest X-ray showed no abnormal findings in the both lung fields. Patient's trachea was extubated when he emerged from anesthesia. However, Spo2 rapidly dropped from 100 to 88. Re-intubation was performed, and positive pressure ventilation was resumed. The Spo2 returned quickly to 100 without hemodynamic change. Auscultation revealed reduced respiratory sound from the left lung. Diagnosis of tension pneumothorax was made from emergency chest X-ray. Patient's respiration improved when chest tube was inserted, but a large amount of air was continuously drained. Air leakage decreased significantly when the mediastinum drainage tube was tentatively occluded. The possible mechanism of the positive pressure in the thoracic cavity was assumed that air was introduced with spontaneous inspiration from the drainage tube, and damaged pleura played as a check valve.

Miliary tuberculosis: clinical manifestations, diagnosis and outcome in 38 adults.

OBJECTIVE: The aim of the study was to determine the clinical, radiographic and laboratory characteristics, diagnostic methods, and prognostic variables in patients with miliary tuberculosis (TB). METHODOLOGY: The records of 38 patients (15 male, 23 female; mean age 41 years, range 16-76 years) with miliary TB from 1978 to 1998 were analyzed. Patients were evaluated also as to whether they presented with a fever of unknown origin (FUO). Criteria for the diagnosis of miliary TB were (i) miliary pattern on chest X-ray or (ii) biopsy or autopsy evidence of miliary organ involvement. Paraffin-embedded tissues with granulomata (n = 15) were re-evaluated for the presence of Mycobacterium tuberculosis DNA by polymerase chain reaction (PCR). RESULTS: Predisposing conditions were present in 24% of the patients. The findings were fever, weakness, night sweats, anorexia/weight loss (100% for each), hepatomegaly (37%), splenomegaly (32%), choroidal tubercles (13%), neck stiffness (11%), altered mental status (8%), anaemia (76%), leukopenia (26%), thrombocytopenia (16%), lymphopenia (76%), pancytopenia (8%) and hypertransaminasemia (55%). Eighteen patients (47%) met the criteria for a FUO. Miliary infiltrates were found on chest X-rays of 32 of 38 cases (84%). In six cases without miliary infiltrates, the diagnosis was made by laparotomy in four cases, and autopsy in two cases. Tuberculin skin test was positive in 32% of cases. Acid-fast bacilli were demonstrated in 37% (16/43), and cultures for M. tuberculosis were positive in 90% (9/10) of tested specimens (predominantly sputum and bronchial lavage). Granulomas were found in 85% (11/13) of lung, 100% (15/15) of liver, and 56% (9/16) of bone marrow tissue specimens. Acid-fast bacilli staining was negative in all (0/21), while PCR was positive in 47% (7/15) of specimens with granulomata. Mortality was 18%. Stepwise logistic regression identified male sex (P = 0.005), non-typical miliary pattern (P = 0.015), altered mental status (P = 0.002) and failure to treat for TB (P = 0.00001) as independent predictors of mortality. CONCLUSIONS: Miliary infiltrates on chest X-ray or FUO should raise the possibility of miliary TB. Therapy should be administered urgently to prevent an otherwise fatal outcome.
