Mesenchymal chondrosarcoma. A clinicopathologic analysis of 35 patients with emphasis on treatment.

Almost a quarter of a century has passed since mesenchymal chondrosarcoma, a rare, insufficiently studied and still poorly understood cartilage neoplasm has been described. Based on 35 cases diagnosed and treated at this Cancer Center, this study found 20 males and 15 females with an average age of 26 years (range, 6-70 years). All but five of the tumors arose in the skeleton in the femur, humerus, and ilium in five cases each, while the os calcis (a rare site for any other osseous tumor) gave rise to this tumor in four instances. Pain was the cardinal symptom in 27 patients. The lesional size varied from 4 to 18 cm (average, 9.5 cm). Histologic examination revealed nine of the tumors to be of the small cell undifferentiated types while the others were of the "hemangiopericytomatoid" variant. According to this subclassification, patients with the small cell type of lesions responded to combination chemotherapy and irradiation, as usually do other small cell sarcomas. The addition of surgical resection may be of value especially in the patients with the hemangiopericytomatoid variant. Preliminary results in the treatment of five such patients with evaluable disease suggest that this combined treatment approach is encouraging. Follow-up analysis of all patients revealed a 37.9 months median survival, and 28% to be alive at ten years.

Fatty replacement of lymph nodes mimicking lymphoma relapse.

A 70-year-old man was treated successfully for diffuse histiocytic lymphoma involving the left ilium and bone marrow. He received radiation therapy to the left hip for stabilization, then eight cycles of intense (COP-BLAM) chemotherapy. Restaging at the completion of therapy demonstrated a normal physical examination, laboratory values, bone scan, bone survey, and abdominal computerized tomographic scan. One month after restaging he developed rapidly enlarging lymphadenopathy. Biopsy of an axillary node showed complete fatty replacement without lymphoma, and the patient remains in remission 16 months later. Recent reports of residual masses after lymphoma therapy have demonstrated fibrous replacement, necrotic tissue, thymic hyperplasia, and vascular anomalies mimicking lymphadenopathy. Lymph node enlargement may be due to nonmalignant causes, including fatty replacement, and not due to recurrent lymphoma.

Usefulness of FDG-PET to diagnose intravascular lymphomatosis presenting as fever of unknown origin.

Intravascular lymphomatosis (IVL) is a rare systemic disease characterized by proliferation of lymphoid cells within the lumina of small arteries, veins, and capillaries. Diagnosis requires skin, liver, lung, bone marrow, renal, meningeal, or brain vessel biopsy but is often made only when the illness has progressed or post mortem because early involvement of organs was not evident. We report a case of IVL presenting as fever of unknown origin (FUO). In this case, gallium scintigraphy and computed tomography (CT) showed no evidence of malignancy, whereas (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET) showed increased FDG uptake in the sternum, left and right vertebra, humerus, femur, and especially ilium. The diagnosis was made on iliac bone marrow biopsy examination. FDG-PET was useful for the detection of spread of disease in a patient with IVL suffering from FUO.

Skeletal growth of the protohistoric Arikara.

The longitudinal growth of the long bones and growth in breadth of the ilium are assessed for a population of protohistoric Arikara Indians from South Dakota through the correlation of skeletal measurements with estimates of chronological age at death. Comparison of the Arikara growth data with those from other Indian samples (Indian Knoll and Late Woodland, Illinois) reveals similar rates of bone growth, when compensation is made for methodological variation. As predicted from documented variation in adult statures, the Indian samples indicate slower growth rates than those of Whites but faster than those of Eskimos.

Bone resorption of the pubis and preauricular area in humans and nonhuman mammals.

Some innominates of adult human females show areas of bone resorption on the dorsal aspect of the pubic corpus and preauricular area of the ilium. For both sites, many studies have shown a positive association between degree of resorption and parity. The present study tested hypotheses concerning resorption of the pubis and preauricular area. Samples of innominates from three prehistoric Amerindian populations were used. Within each population, only a minority of females, in general, showed resorption more severe than that which characterized males. The severity of resorption of the pubis was not significantly associated with that of the preauricular area. Pitting of the pubis, but not of the preauricular area, was significantly correlated with age-at-death in each sample of Amerindian females. Also, sacral angulation was not significantly associated with resorption of the preauricular area. Observations on resorption of the pubis and preauricular area in samples of Pan troglodytes and Gorilla gorilla are reported. In previous studies on resorption of the pubis and preauricular area in the human innominate, the proposed etiologies involve ligamentous hyperplasia and joint trauma. However, a number of studies on resorption of the pubis in the nonhuman mammalian innominate have been overlooked by anthropologists. These latter studies demonstrate that estrogen alone can induce resorption of the mammalian pubis by stimulating the synthesis of osteoclastic enzymes. Partial resorption of the pubis may be obstetrically advantageous in some mammals, as resorption would delay or inhibit synostosis of the interpubic joint. The relationship between estrogen and the preauricular area is an issue that requires further research.

Sex determination of infant and juvenile skeletons: I. Morphognostic features.

Ancient cemeteries are often characterized by a considerable number of infants and young children. Sex differences in childhood mortality, however, could rarely be studied up to now, mainly because there were only few proven traits for sexual determination of immature skeletons. Based on a historic sample of sixty-one children of known sex and age from Spitalfields, London (37 boys, 24 girls), sexually distinctive traits in the mandible and ilium are presented for morphognostic diagnosis. Besides other features, boys typically show a more prominent chin, an anteriorly wider dental arcade, and a narrower and deeper sciatic notch than girls. Most of the traits presented in this study allow individuals between birth and five years of age to be successfully allocated to either sex in 70-90% of the cases.

Statistical study of sexual dimorphism in the human fetal sciatic notch.

Whether human fetal skeletal remains exhibit sexual dimorphism has been the subject of considerable debate. Most attention in this debate has focused on the greater sciatic notch of the ilium, since it is a gross morphological characteristic with known sex differences in the adult and is easily seen in fetal skeletal remains. Unfortunately, previous traditional morphometric analyses of the fetal sciatic notch have led to ambiguous results. The purpose of this study is to determine whether differences between the sexes can be discerned when modern morphometric techniques are applied to the fetal sciatic notch. Photographs of the ventral side of 133 fetal ilia of known age and sex from the Trotter Collection of Washington University were digitized, and the trace coordinates used for all subsequent analyses. The results of the analysis demonstrate that there is significant sexual dimorphism in the anterior to posterior location of the maximum depth of the sciatic notch, but that the depth of the notch itself is not dimorphic. While there is significant sexual dimorphism in the shape of the sciatic notch, the amount of overlap between males and females is too great for the sciatic notch to be used as a reliable indicator of sex.

Ultrastructural study of mineralization of a strontium-containing hydroxyapatite (Sr-HA) cement in vivo.

The purpose of this study was to investigate the mineralization leading to osseointegration of strontium-containing hydroxyapatite (Sr-HA) bioactive bone cement injected into cancellous bone in vivo. Sr-HA cement was injected into the ilium of rabbits for 1, 3, and 6 months. The bone mineralization area was found to be largest at 3 months, then at 1 month, and smallest at 6 months (p < 0.01) measured with tetracycline labeling. Osseointegration of Sr-HA cement was achieved at 3 months as observed by scanning electron microscopy. A high calcium and phosphorus area was observed at the interface of bone-Sr-HA cement determined by energy-dispersive X-ray analysis. Transmission electron microscopy gave evidence of the mechanism of bone formation. Dissolution of Sr-HA into debris by the bone remodeling process was thought to increase the concentration of calcium and phosphorus at the interface of bone-Sr-HA cement and stimulate bone formation. Crystalline Sr-HA formed an amorphous layer and dissolved into the surrounding solution, then apatite crystallites were precipitated and formed new bone at 3 months. This young bone then becomes mature bone, which bonds tightly to the Sr-HA cement with collagen fibers inserted perpendicularly after 6 months.

Regeneration of bicortical defects in the iliac crest of estrogen-deficient sheep, using new biodegradable polyurethane bone graft substitutes.

Porous scaffolds for cancellous bone graft substitutes were prepared from new experimental biodegradable aliphatic polyurethane elastomers with varying hydrophilicity. The ratios of the hydrophilic-to-hydrophobic content in the polymers were 30-70, 50-50, and 70-30%, respectively. The hydrophilic component consisted of poly(ethylene oxide) diol and the hydrophobic component of poly(epsilon-caprolactone) diol. To promote the materials' biological performance, the calcium complexing moiety, the polysaccharide, and vitamin D(3) were incorporated into the polymer chain upon synthesis. The scaffolds had an interconnected porous structure with an average pore size in the range of 300-2,000 microm and pore-to-volume ratios of (85 +/- 5)%. The bone substitutes were implanted (press-fit) in biocortical 10 x 10 mm(2) defects created in the tuber coxae of 21 skeletally mature Warhill ewes, which were ovariectomized 12 months prior to implantation. At the time of euthanasia at 18 and 25 months, all the defects in the ilium implanted with polyurethane bone substitutes had healed with new bone. The extent of bone healing depended on the chemical composition of the polymer from which the implant was made, although for the same material there were animal-related differences in healing. The structure of the newly formed cancellous bone was radiographically and histologically similar to the native bone. The implants from polymers with the incorporated calcium-complexing additive were the most effective promoters of bone healing, followed by those with vitamin D(3) and polysaccharide-containing polymer. There was no bone healing in the control defects.

Concentration of bone elements in osteoporosis.

In aging and in osteoporosis, decreased bone density is associated with decreased bone mass. However, changes in the bone mineral phase remain a matter for investigation. In particular, it is unknown whether bone mineral loss is directly related to reduction in bone mass or associated with changes in the concentration of mineral elements in mineralized bone tissue. In this study, the cortical bone concentration of elements was determined in biopsies of the ilium from 33 subjects (12 controls and 21 individuals with untreated severe osteoporosis). Calcium and phosphorus concentrations were evaluated in cortical and trabecular bone using energy-dispersive x-ray (EDX) microanalysis and inductively coupled plasma optical emission spectrometry (ICPOES). Bone concentrations of Na, K, Mg, Cu, Zn, Fe, Sr, Al, B, and Si were also determined in cortical bone using ICPOES. Additionally, the concentration of F in cortical bone was measured with a specific ion electrode and the concentration of Pb was determined by atomic absorption spectrometry. In mineralized bone tissue there was no significant age-dependent variation in the concentration of Ca, P, or other elements either in controls or in osteoporotic subjects. Moreover, the concentration of elements in bone tissue did not differ in the two groups. These results suggest that the decrease in bone density in osteoporosis is directly related to evolution of the bone mass, without detectable changes in the concentration of elements in bone.

Iliac trabecular bone formation predicts radial trabecular bone density changes in type 1 osteoporosis.

In 28 patients with idiopathic or postmenopausal type 1 (spinal crush fracture) osteoporosis, resorption indices and dynamic measurements of trabecular bone formation based on in vivo tetracycline labeling in 7.5 mm transiliac biopsies have been compared with trends in radial cortical and trabecular bone density measured with computed tomography. Positive correlations were observed between trabecular bone density trends in the radius and indices of bone formation in the ilium. These were improved when one of the two resorption indices was included with a formation index in bivariate regressions. Marked interindividual variations in radial bone density trends were also seen in cortical bone. These correlated poorly with trends in trabecular bone. Weak negative relationships between cortical bone trends and indices relating to bone formation and resorption were observed, but a positive association was seen with single-labeled surfaces on iliac trabeculae. If, as has been suggested, there are periodic variations in bone formation, the results suggest that axial and peripheral trabecular bone density trends are synchronized in osteoporosis, perhaps in response to systemic factors, such as circulating hormones.

Agenesis of the scapula in Emx2 homozygous mutants.

The shoulder and pelvic girdles represent the proximal bones of the appendicular skeleton that connect the anterior and posterior limbs to the body trunk. Although the limb is a well-known model in developmental biology, the genetic mechanisms controlling the development of the more proximal elements of the appendicular skeleton are still unknown. The knock-out of Pax1 has shown that this gene is involved in patterning the acromion, while the expression pattern candidates Hoxc6 as a gene involved in scapula development. Surprisingly, we have found that scapula and ilium do not develop in Emx2 knock-out mice. In the homozygous mutants, developmental abnormalities of the brain cortex, the most anterior structure of the primary axis of the body, are associated with important defects of the girdles, the more proximal elements of the secondary axis. These abnormalities suggest that the molecular mechanisms patterning the more proximal elements of the limb axis are different from those patterning the rest of appendicular skeleton. While Hox genes specify the different segments of the more distal part of the appendicular skeleton forming the limb, Emx2 is concerned with the more proximal elements constituting the girdles.

Hernia through iliac crest defects. A report of four cases.

Herniation of abdominal contents through the donor site in the ilium occurred in 5% of a series of 59 bone grafts. Four patients had this condition at a 7 year follow up, and 2 were repaired with synthetic mesh. Age, gender, obesity and surgical technique are important causative factors. Preventive measures are proposed which have been successful during the past 3 years.

Radiology of the paraglenoid sulcus.

The paraglenoid sulcus is a variable groove in the ilium adjacent to the inferior end of the sacroiliac joint. This study investigates the hypothesis that the groove is due to resorption at the insertion of the anterior sacroiliac ligament in response to stress. The anatomical relationships of the groove are shown with plain films, arteriography and computed tomography. Relationship to parity was tested in 70 adult female subjects; a statistically significant relationship was found, with deep grooves occurring only in parous women. An association with osteitis condensans ilii was also demonstrated. Relationship to lumbosacral posture was tested in 102 adult females, and an association with increased lumbar lordosis was found. These findings are consistent with our initial hypothesis.

Metastatic and pseudomalignant osteoblastoma: a report of two unusual cases.

Two unusual cases of osteoblastoma with different responses to radiation therapy are reported. A 15-year-old boy with an aggressive osteoblastoma of the ilium was treated initially with curettage and subsequently with radiation and systemic chemotherapy. The patient died 35 months after his initial presentation with distant metastases which were verified at autopsy. A 12-year-old girl developed a lesion of the femur with clinical and histologic features of aggressive osteoblastoma. Amputation was recommended at the time of recurrence, but the patient declined and radiation therapy was given. The patient was alive and well 14 years after her initial presentation. These cases highlight the biologic diversity of osteoblastoma-like lesions and illustrate the difficulties which may be encountered in attempting to differentiate between benign and malignant forms of this tumor.

The direct fixation system of total hip prosthesis. Research of the possibilities of fixing total hip prosthesis directly to bone.

There are two methods for the fixation of prostheses to bone; one is a method using bone cement and another is the direct fixation of the prostheses to bone. Four types of animal experiments were performed to compare the two methods of fixation. A small pyramid shaped test piece was used to compare the fixation capacity of the socket spike. Also, a special test piece was used to study the time of bone formation, to check the fixation capacity of bone bar formation occurring in the hole in the based part of the prosthetic component. furthermore, models of prosthetic stem and socket were made and attached to the femur and ilium of dog to perform fixation tests and fatigue tests. As a result, any of the tests proved that the direct fixation method without the usage of bone cement was superior to the fixation method using bone cement.

Primary osteoliposarcoma of bone.

Radiologic and pathoanatomic findings in two patients with the very rate primary osteoliposarcoma of bone are reported. In one case, there was a manifestation on the left femoral shaft which led to pathologic fracture. Amputation of the femur was followed by a 2.5-year tumor-free interval. In the second case, there was a manifestation in the region of the right upper part of the ilium. After radiation and cytostatic therapy, pronounced intratumoral ossifications occurred and the patient's condition was good up to exitus lethalis 3 years after diagnosis. Osteoliposarcoma thus shows a markedly more favorable prognosis than the osteosarcoma and rather corresponds to that of a liposarcoma. Typical clinical and radiologic symptoms are lacking.

Local application of basic fibroblast growth factor into the bone increases bone mass at the applied site in rabbits.

The effect of basic fibroblast growth factor (bFGF) applied locally into the bone under physiological conditions was investigated. An aqueous solution containing 0 microgram (vehicle), 100 micrograms or 400 micrograms recombinant human bFGF was percutaneously applied through a needle into the right ilium in rabbit, and the ilia were harvested 4 weeks after the application. Compared with vehicle-treated animals, bone mineral density measured by dualenergy X-ray increased in the 400 micrograms bFGF group. The width of trabeculae in the bFGF-treated groups was greater than in the vehicle group. These results showed that bFGF applied locally into the bone under physiological conditions affected bone formation, and suggested that such an application might have potential for increasing bone.
