[Reinfusion of autologous platelet-rich plasma improves hemostasis after cardiopulmonary bypass].

Although it is reported that postoperative bleeding is reduced by reinfusing autologous platelet-rich plasma (PRP) after cardiopulmonary bypass (CPB), the effect of PRP on hemostasis is not reported in detail. We prepared PRP and fresh whole blood (WB) from the blood of seven patients each prior to their undergoing CPB, and reinfused them autologously to the patients intravenously after the CPB was terminated. In this article, the effect on hemostasis of autologous PRP and WB was described. Platelet aggregation rates and blood coagulation factors were examined before, during and after bypass. Platelet counts, ADP-induced platelet aggregation and the activities of coagulation factors II, V and VII-X were significantly greater in prepared PRP than in WB (p less than 0.01 or p less than 0.05). A mean volume of 724 +/- 109 ml of PRP or 401 +/- 63 ml of WB was reinfused within about 30 minutes after heparin was neutralized by protamine sulfate. The platelet counts increased from 4.3 +/- 1.4 x 10(4)/mm3 to 14.1 +/- 1.6 x 10(4)/mm3 after PRP reinfusion and the platelet aggregation rates increased significantly (p less than 0.01) after PRP reinfusion compared to WB transfusion. The activities of coagulation factors VII-X also increased significantly (p less than 0.05) after reinfusion of PRP when compared to transfusion of WB. The activated partial thromboplastin time decreased to 1.2 times the baseline in the PRP group but remained 1.5 times the baseline in the WB group (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Thrombosis and disorders of hemostasis in nephrotic syndrome].

Thromboses and disorders of hemostasis in nephrotic syndrome. Thromboembolic complications are common in nephrotic syndrome (NS). This article reviews the factors of thrombogenesis in NS, including: 1) a hypercoagulable state with platelet hyperaggregability, hyperfibrinogenemia and elevated factor VIII, decrease in plasma levels of coagulation inhibitors antithrombin III and free protein S, reduced fibrinolytic activity; 2) excessive intravascular thrombin formation marked by increased plasma levels of fibrinopeptide A. The intensity of hemostasis disorders coincides with that of NS. Most disorders are related to hypoalbuminemia and proteinuria. In agreement with experimental data, the role of intraglomerular activation of coagulation during active phases of glomerulopathies has to be considered. This could explain the predominance of renal vein thrombosis in several glomerulopathies with NS. Several coagulation disorders in SN have implications for therapy.

[The status of indices of immunity and hemostasis in patients with lung cancer and their changes due to general anesthesia and surgical trauma].

Phagocytosis, some factors of humoral immunity and hemostasis have been studied in cancer patients before and after surgery. Suppression of natural body resistance in the preoperative period, as well as significant inhibition of leukocyte phagocytic activity following surgery and anesthesia have been established. A correlation has been suggested between hemostasis damages in the postoperative period and an elevated IgM concentration.

[The prostacyclin-thromboxane system and platelet hemostasis following aortocoronary bypass with an uncomplicated postoperative course].

Prostacyclin-thromboxane system and platelet hemostasis have been studied in 56 patients upon aortocoronary bypass surgery with uncomplicated early postoperative period. It has been established that cardiopulmonary bypass surgery leads to a considerable increase in 6-keto-PGF1 alpha and TXB2 levels per platelet, as compared to preoperative values. By hour 18 postoperatively 6-keto-PGF1 alpha to platelet number ratio returns to baseline, while TXB2 to platelet number ratio remains higher than preoperative values, which determines a shift in 6-keto-PGF1 alpha to TXB2 ratio towards TXB2, thus ensuring, probably, enhanced platelet aggregation properties. Thrombocytopenia, a decrease in platelet aggregation properties and elevated blood plasma level of beta-thromboglobulin were observed upon aortocoronary bypass surgery. By hour 18 postoperatively the number of platelets increased significantly, their aggregation properties were enhanced, beta-thromboglobulin blood plasma level was reduced, however, the parameters under study did not reach normal values at that time. Increased levels of 6-keto-PGF1 alpha and TXB2 per platelet in the early postoperative period are considered an important component of compensatory-adaptive body reactions directed to normalization of the damaged body functions, namely hemostasis and microcirculation.

[State of hemostasis in patients with chronic gastritis dependent on the intensity of exacerbation and its morphological form].

The study of systemic hemostasis in 254 patients with chronic gastritis has revealed that the exacerbation of the disease proceeds with marked disorders of blood coagulation typical for the first and second stages of the chronic relapsing thrombohemorrhagic syndrome and thrombophilia. They manifested in intensification of coagulability, lowering of anticoagulative and fibrinolytic potentials of blood, pathological intravascular coagulation. The disorders of hemostasis were strongly expressed during the period of marked exacerbation of the disease and in patients with erosive gastritis.

[State of hemostasis in hemorrhagic vasculitis in children].

The purpose of the study was to explore hemostasis in children suffering from hemorrhagic vasculitis (HV) by means of the new amidolytic methods using chromogenic substrates. The patient's plasma was studied for the content of thrombin, trypsin, factor Xa, AT-III, kallikrein, plasmin, free heparin, urokinase, factor 3 of platelets, prothrombin and Willebrand's factor. 69 children with HV were entered into the study. All of them were examined during crises. In cutaneous HV, the content of trypsin decreased 3-fold, the content of factor Xa increased 5-fold; there was a negligible increase in the plasmin and AT-III levels; the content of kallikrein rose 2-fold, that of urokinase 60-fold; the release of platelet factor 3 was intensified 1.5-fold, the activity of prothrombin 3-fold. These data indicate that in cutaneous HV, blood coagulation increased. However, the signs of disseminated intravascular coagulation were lacking because of the high blood anticoagulant activity. In mixed HV, the phase of hypercoagulation was not made for by the blood anticoagulant activity, since the latter one was depleted. The capillary toxic nephritis may give rise to disseminated intravascular coagulation associated with the depletion of the anticoagulant component. The gravity of HV and its complications can be predicted according to the characteristics of the anticoagulant component of hemostasis, especially according to the levels of urokinase and AT-III.

[Changes in general hemostasis in patients with diabetic retinopathy].

The hemostasis system was analyzed in 106 patients with diabetes mellitus, including 86 patients with diabetic retinopathy. Parameters of coagulogram and thromboelastogram as well as platelets aggregation, stimulated by collagen, were examined in all patients. Besides, the fibrinolytic activity of blood plasma as well as the concentration of plasminogen, fibrinogen and of fibrin-degradation products were evaluated. Essential changes were detected, which correlated with an intensity of diabetic changes in the eye fundus, i.e. generalized hypercoagulation caused by a lower activity of the system antithrombin III-heparin, a higher activity of fibrinogen and activation of the fibrinolytic system. The obtained data denote an important role of impaired hemostasis in the pathogenesis of diabetic retinopathy.

[Relationship Between Lipid Lowering and Changes of Hemostasis and Inflammation During Use of Statins in Non-ST Elevation Acute Coronary Syndrome].

AIM: To analyze relationship between changes of lipid levels and parameters of hemostasis and inflammation in a previously reported comparative study of some biological effects of pravastatin and atorvastatin in patients with non ST elevation acute coronary syndrome (NSTEACS). METHODS: Ninety aspirin and heparin treated patients with NSTEACS were randomized to open pravastatin 40 mg/day (n=31) and atorvastatin 10 (n=30) or 40 mg/day (n=29). At baseline, on days 7 and 14 we measured levels of thrombin-antithrombin complex (TAT), prothrombin fragments 1+2 (F1+2), D-dimer, von Willebrand factor (vWF) and C-reactive protein (CRP) and assessed platelet aggregation. Spearman correlation coefficients were calculated for mean deltas of all parameters {[(baseline - day 7) + (baseline - day 14)]/2} for all patients. The patients were divided into quartiles according to absolute LDL CH lowering by day 14. RESULTS: Levels of LDL and total CH significantly decreased in all groups (atorvastatin 40 mg/day >> atorvastatin 10 mg/day > pravastatin). Contrary to pravastatin the use of atorvastatin was associated with increases of F 1+2, TAT, and decrease of vWF. Platelet aggregation decreased only in atorvastatin 40 mg/day group (p<0.05) and CRP decreased in combined atorvastatin group (p<0.05). Most pronounced relationship existed between changes of lipid levels and vWF. Lowering of total and LDL CH correlated positively with decreases of vWF (r=0.25, p=0.018, and r=0.23, p=0.032, respectively) and quartile analysis showed that vWF decreased only in patients with marked LDL CH lowering (quartiles 3, 4). Negative correlation was found between changes of total CH and those of TAT (r=-0.22, p=0.039), and between changes of CRP and HDL CH (r=-0.24, p=0.03). TAT and F 1+2 levels significantly increased in those patients in whom LDL CH level <2.5 mmol/l was achieved by day 14 and tended to decrease in other patients. No relationship was found between changes of lipids and platelet aggregation. CONCLUSION: Rapid (in 14 days) changes of some parameters of hemostasis occurring during treatment of patients with NSTEACS with various statins were related to degree of lipid lowering possibly irrespective of a statin used. Most evident was relationship between changes of lipids and von Willebrand factor.

Risk of local adverse events following cardiac catheterization by hemostasis device use and gender.

Hemostasis devices are primarily used to stop bleeding from the femoral artery catheterization site after cardiac diagnostic or interventional procedures. Studies assessing the effectiveness of hemostatic devices compared to manual compression have produced mixed results regarding the relative risk of serious adverse events, such as hemorrhage. This study assessed the relative risk of serious complications following the use of the two main types of hemostasis devices (as compared with manual compression), and assessed the relative rates of these complications by gender.

Endoneurosurgical hemostasis techniques: lessons learned from 400 cases.

The increasing popularity of minimally invasive neurosurgery has led to the development of transnasal expanded approaches for the treatment of skull base lesions. One of the greatest challenges in safely accomplishing resection of tumors, particularly intradural lesions, is effective hemostasis. Over the past 7 years the authors have progressively developed an organized approach to address this challenge. This has required the development of new instrumentation as well as variations on standard techniques. In this report they present the technique that has evolved at their institution for endoneurosurgical hemostasis.

[Polymorphism of genes of factors of hemostasis in patients with early development of ischemic heart disease.].

The review is devoted to analysis of data, related to contribution of genetic peculiarities of the hemostasis system to early development of ischemic heart disease (IHD). Information is presented on polymorphisms of tissue factor, coagulation factors VII, V, prothrombin, thrombomodulin, fibrinogen, thrombin activated fibrinolysis inhibitor, tissue plasminogen activator inhibitor, platelet membrane glycoproteines Ia, Iba, IIIa in patients with early development of IHD. Contradictoriness of existing data on effects of genetic heterogeneity of factors of hemostasis on risk of development of IHD at young age and necessity of further investigations in this area are underlined.

[Hemostasis in patients with ischemic stroke].

Modern informative techniques were employed to explore the coagulation, fibrinolytic and aggregation platelet systems of hemostasis in 117 patients in the most acute , acute, early, late restorative and residual periods of ischemic brain stroke. Disorders of the hemostatic system in the form of hyperaggregation, hypercoagulation, depression of fibrinolysis, and an increase of blood viscosity were revealed as were some specific features marking different period of brain stroke. Mono- and multi-systemic disorders of hemocoagulation and three types of coagulopathies were distinguished. Emphasis is laid on the necessity and feasibility of a goal-oriented selective correction of the pathological conditions of hemostasis at different periods of brain stroke by means of administering appropriate drugs.

Potential role of entactin in hemostasis. Specific interaction of entactin with fibrinogen A alpha and B beta chains.

The interactions between the endothelial basement membrane, platelets, and coagulation factors play essential roles in hemostasis. Entactin is an integral and ubiquitous component of the basement membrane. Experiments were designed to study the interactions between entactin and blood coagulation factors. We have demonstrated, for the first time, that entactin interacts with fibrinogen in a specific manner. Entactin binding sites have been localized to the A alpha and B beta chains of fibrinogen. The binding of entactin to either the A alpha chain or the B beta chain was divalent cation-independent. The binding of 35S-labeled entactin to the immobilized fibrinogen A alpha or B beta chain was concentration-dependent and saturable and could be inhibited by unlabeled entactin, soluble fibrinogen, anti-entactin antiserum, or anti-fibrinogen antiserum. In addition, we have provided evidence that entactin can be cross-linked to itself, and probably also to fibrin(ogen), by transglutaminase. These novel properties, together with its cell binding, chemotactic, and phagocytic promoting activities and ubiquitous distribution in basement membranes, suggest that entactin may play important roles in hemostasis and wound healing.

[Thyroid and hemostasis].

Several studies have reported hemostatic abnormalities, both in terms of bleeding or thrombosis, in patients with various thyroid dysfunctions. The aim of this paper is to briefly discuss the relationship between thyroid disorders and hemostasis (i.e., primary hemostasis, coagulation factors and fibrinolytic system). From the analysis of the more recent literature data, it is evident that most of the coagulation abnormalities, associated with thyroid disorders, are a consequence of a direct action of thyroid hormones on the synthesis of various hemostatic factors or a derangement of immune function. On the whole, these data suggest that a hypercoagulable state is present in hyperthyroid patients, while patients suffering from moderate hypothyroidism are at increased risk of thrombosis, contrasting with the bleeding tendency of those presenting severe hypothyroidism.

[Endoscopic hemostasis of erosive-ulcerous gastroduodenal bleeding with fibrin glue at critically ill patients].

Experience of endoscopic hemostasis of acute erosive-ulcerous gastroduodenal bleeding with fibrin glue at critically ill patients is described. This glue is adhesive substance based on high-concentrated solution of fibrinogen (concentration of protein not less 60 g/l). Application of adhesive permitted to stop the bleeding at 84 of 87 extremely seriously ill patients (mean point according to APACHE--II scale was 19.5+/-0.9). Prolonged endoscopic control with repeated application of adhesive permitted to avoid bleeding clinical recurrences, to stop repeatedly with endoscopy 4 of 6 recurrent bleedings, to avoid forced surgery at 80 of these patients. Adhesive accelerated significantly the healing of ulcers despite of hypoxic injury of mucosa. Endoscopic hemostasis permitted to avoid forced surgical aggression, to improve treatment results and to decrease lethality at critically ill patients.

[Disorders of primary hemostasis caused by albumin infusion in hemodilution].

Previous studies using the In-vitro Bleeding Test have shown that albumin impairs primary hemostasis more pronouncedly than other infusion solutions. For this reason we compared ADP (4 microM)-induced platelet aggregation in samples of platelet rich plasma containing 50% albumin. We added the same amount of platelet-poor plasma to the controls. Using this method we could show the inhibitory effect of albumin once more. This means that not only changes in rheology are responsible for the impairment of hemostasis by albumin. Platelet aggregation itself is influenced.

[Use of neodymium YAG laser in endoscopic hemostasis in thoracic surgery].

YAG-laser radiation was used for endoscopic hemostasis in 33 patients with bleeding endobronchial carcinomas and in 4 patients with chest injury complicated by bleeding from wounds of the lung and chest wall. For conducting laser photocoagulation, the authors prefer a rigid bronchoscope and a thoracoscope and pulsed laser radiation with a power of up to 40 Wt. As the results of treatment, hemostasis was achieved in 24 patients with lung carcinoma and in 4 patients with trauma of the chest. In some patients with pulmonary carcinoma laser photocoagulation creates conditions for successful surgery or chemoradiotherapy. The performance of thoracoscopic laser operations in chest injuries and traumas allows wide thoracotomy to be avoided in some patients. Complications were not registered in endoscopic laser operations.

[The effect of cyclooxygenase inhibition on the indices of the thrombocyte-vascular link in hemostasis and on the free-radical processes of lipid oxidation in experimental Salmonella intoxication].

Injection of Salmonella typhimurium endotoxin to the laboratory animals (rabbits) in dose of 1 mg/ml (LD84) induces the particular changes in the thrombocyte vessels system of hemostasis: decrease of aggregatory ability of thrombocytes, increase of thromboxane A2 and prostacyclin activation of lipid peroxidation process. Use of indomethacin--the cyclooxygenase inhibitor--leads to less progressive alterations of the studied parameters of the thrombocyte vessels hemostasis and lipid peroxidation processes.
