Anti-Ro/SSA associated recurrent annular erythema: autoimmune response to recombinant 60- and 52-kDa Ro/SSA proteins.

Recurrent annular erythema associated with anti-Ro/SSA antibody response has recently been recognized as a distinct clinical entity. Serum samples from 15 anti-Ro/SSA positive patients with recurrent annular erythema were tested by enzyme-linked immunosorbent assay (ELISA) for reactivity with full-length recombinant human 60-kDa and 52-kDa Ro/SSA proteins. All the 15 sera were positive for anti-60-kDa Ro/SSA, and 11 sera (73.3%) contained anti-52-kDa Ro/SSA. These results suggest the importance of 60-kDa polypeptide component of the Ro/SSA particle as a potential target in anti-Ro/SSA associated annular erythema.

[Erythema nodosum and adenopathy in a 15-year-old boy: uncommon signs of cat scratch disease].

UNLABELLED: Erythema nodosum rarely occurs in childhood and can be caused by cat scratch disease, as a result of agent Bartonella henselae. We report the case of a teenager who presented erythema nodosum and bilateral inguinal adenitis. Cat scratch disease diagnosis was confirmed by anti-Bartonella henselae serologies. Despite an appropriate antibiotic therapy, evolution was unfavourable with adenitis abcédation requiring surgical drainage. CONCLUSION: Erythema nodosum in children must let think to cat scratch disease among others etiologies.

Immunohistochemical detection of hemoglobin in frost erythema.

Reddish discoloration of exposed skin areas, called frost erythema, is an important criterion for the diagnosis of fatal hypothermia. In the present study, we used immunohistochemistry in a prospective trial to show that on the molecular level, the correlate of frost erythema is hemoglobin without hemorrhage. Furthermore, we compared routine histological and immunohistochemical features of frost erythema, hematoma and livor mortis and established some criteria for their histological differentiation.

Reticulate erythema following diode laser-assisted hair removal: a new side effect of a common procedure.

The popularity of laser-assisted hair removal has grown rapidly since April 3, 1995 when the Food and Drug Administration approved the introduction of the first hair removal laser system. Lasers with wavelengths in the red and infrared portion of the electromagnetic spectrum are most often used for hair removal because they effectively target melanin in the hair follicle and can potentially penetrate to the appropriate depth of the dermis. Despite all efforts to protect the skin from damage, photoepilation may result in clinically significant adverse reactions. The most common and known side effects of laser hair removal include transient erythema, perifollicular edema, pain, folliculitis, hyper-pigmentation, hypopigmentation, crusting, purpura, erosions and scarring. The present report describes the appearance of a reticulate erythema after diode laser treatment for hair removal, encountered in 10 patients in our clinics in London and Israel. To the best of our knowledge, this is the first report of this side effect. The aim of this work is to detail the clinical manifestations, histological findings, and follow-up of these patients in order to expand the clinical spectrum of laser-assisted hair removal side effects and to alert dermatologists to the possibility of this type of net-like erythema.

Annually recurring erythema annulare centrifugum: a distinct entity?

Four patients presenting a peculiar clinical variant of erythema annulare centrifugum are reported. The lesions were clinically and histopathologically indistinguishable from classic superficial erythema annulare centrifugum but constant annual and seasonal recurrences for many years or decades were observed. No clear precipitating factor could be identified. No associated symptoms were present and the eruption regressed spontaneously after a variable period of days to months. Annually recurring erythema annulare centrifugum seems to represent a rare distinct clinical entity that has received little attention in literature. Clinicopathologic features of this peculiar clinical disorder and the differential diagnosis with other recurrent seasonal eruptions are reviewed.

Topical tacrolimus neither prevents nor abolishes ultraviolet-induced erythema.

A dose- and vehicle-controlled study on the pretreatment and posttreatment effect of 0.1% tacrolimus ointment on erythema induced by solar-simulated ultraviolet (UV) radiation was performed. Surprisingly, comparisons of clinical erythema scores and colorimetric data obtained 24 and 72 hours after UV exposure did not reveal significant differences between the control, tacrolimus, and vehicle preirradiation and postirradiation treated sites (P > .05). Our data strongly indicate that topical tacrolimus neither prevents nor abolishes UV-induced erythema in a clinical meaningful degree.

Peripheral keratitis associated with erythema elevatum diutinum.

PURPOSE: To describe a case of peripheral keratitis associated with erythema elevatum diutinum (EED), an unusual immune-complex-mediated cutaneous vasculitis. DESIGN: Observational case report. METHODS: A 25-year-old man who was diagnosed 15 months previously with erythema elevatum diutinum presented with an inflammatory peripheral keratitis of the left eye. RESULTS: Serologic investigations for systemic disorders associated with vasculitic peripheral ulcerative keratitis were unremarkable. As the sclerokeratitis was thought to represent an ocular extension of the patient's cutaneous vasculitis, dapsone therapy was initiated and resulted in a rapid response of both the cutaneous and the ocular inflammation. CONCLUSIONS: Erythema elevatum diutinum should be included in the differential diagnosis of vasculitic peripheral keratitis.

Nodular scleritis and panuveitis with erythema elevatum diutinum.

PURPOSE: To report a case of nodular scleritis and panuveitis associated with erythema elevatum diutinum, a rare immunocomplex-mediated skin disease. DESIGN: Observational case report. METHODS: A 22-year-old woman who was diagnosed with erythema elevatum diutinum developed nodular scleritis and panuveitis of the right eye. She had experienced peripheral ulcerative keratitis with corneal perforation. RESULTS: All other known causes of nodular scleritis and panuveitis were investigated and ruled out. CONCLUSIONS: Erythema elevatum diutinum should be considered as an underlying systemic disease associated with nodular scleritis and panuveitis as well as peripheral keratitis.

Human parvovirus (B19) and erythema infectiosum.

An outbreak of erythema infectiosum ("fifth disease") was studied in Fukuoka, Japan, in 1980-1981. Human parvovirus (HPV) antigen was not detected in any patients, but anti-HPV, measured by countercurrent immunoelectrophoresis, was found in 33 of 34 affected children and in 21 (15%) of 141 children of the same ages without the disease. Immunoglobulin M class anti-HPV was present in all 25 children with erythema infectiosum tested. In a survey of hospital patients, the prevalence of anti-HPV detected by CIE was 12% in the cohort 5 to 9 years of age, 19% in the cohort 10 to 14 years, and 32 to 55% in the cohorts greater than or equal to 30 years. The antibody reactions in the cases of erythema infectiosum, which were already well established at the onset of disease, indicate that HPV was the cause of the outbreak.

[Reticular erythema signalling the onset of episodes of hereditary angioedema in a child].

INTRODUCTION: Hereditary angioedema is characterized by episodes of subcutaneous, digestive or laryngeal edema. In some cases, non-pruritic reticular erythema may precede the episodes of edema. OBSERVATION: Every 4 to 6 weeks since infancy, a girl presented non-pruritic widespread reticular erythema, sparing the face. Two or three times every year, abdominal pain or edema of the lower limb joints followed the skin eruption. At 12 years of age, she was hospitalized because of an edema of the face associated with the eruption. Exploration of the complement confirmed the diagnosis of type I hereditary angioedema. DISCUSSION: The mean delay before diagnosis of hereditary angioedema is of 7 years. Reticular erythema in hereditary angioedema is frequent (40p. 100 of cases) and it usually occurs early in childhood, even in the absence of any episode of angioedema. When present, these eruptions usually precede an episode of angioedema. Recognition of this eruption as a symptom of hereditary angioedema would shorten the delay before diagnosis and anticipate appropriate management of the episodes.

Erythema chronicum migrans disease in the Federal Republic of Germany.

Epidemiology and clinical presentation of Erythema chronicum migrans disease are not well known yet. During a period of only 19 months, serological and clinical investigation of 2955 patients rendered 1106 cases of infection whose widespread incidence was remarkable: of the 328 administration districts of the FRG, 205 were affected. Accordingly, positive antibodies against Borrelia burgdorferi could be demonstrated in an average of 15.7% of the investigated rural population (2830 persons). Typical clinical signs were encountered in 817 of 1106 infected persons. Erythema (458 cases) and meningopolyneuritis (404 cases) were especially prominent. In comparison to Lyme disease the occurrence of arthritis (63 cases), carditis (13 cases) multiple erythema, recurrence, and central nervous symptoms in meningopolyneuritis (10%) were rare. On the other hand, progressive borrelia encephalomyelitis (45 cases) was surprisingly common. Acrodermatitis chronica atrophicans occurred in 72 cases; lymphadenosis benigna cutis in 5 patients. The variability of this disease is demonstrated by the combined syndromes occurring in only 27% of the cases.

Complement deposition in the skin of patients with herpes-associated erythema multiforme.

Granular staining for C3 by direct immunofluorescence is a frequent finding along the dermoepidermal junction and in papillary blood vessels in the early skin lesions of erythema multiforme. In order to evaluate whether the complement cascade is activated by the classical or alternative pathway, ten biopsies from patients with herpes-associated erythema multiforme, which were positive for granular C3 along the dermoepidermal junction, were stained by an immunofluorescence technic for other complement components. Staining for the components of classical pathway, C1q and C4, were found in none of the ten biopsies. However, in nine of ten biopsies, granular staining for properdin was present along the dermoepidermal junction. These findings suggest complement activation by the alternative complement pathway in herpes-associated erythema multiforme.

Erythema induratum of Bazin.

Erythema induratum of Bazin is a rare entity, the cause and treatment of which are topics of continuing controversy. This report documents the cases of two patients with ulcerating leg nodules. One patient had concurrent active pulmonary tuberculosis, and the other had a history of previous treatment for pulmonary tuberculosis. The treatment regimen for these patients is outlined. First described by Bazin, the rare entity of erythema induratum was thought to be of tuberculous origin or a tuberculid reaction. Opinion has varied, and some authors have classified this condition as a vasculitis involving subcutaneous vessels. Because of the rarity of this disorder and the controversy surrounding its cause and pathogenesis, we share our experience with two patients whose clinical and pathologic findings were consistent with the diagnostic criteria for erythema induratum and in whom there was a recent or remote history of tuberculosis.

Annular erythema associated with primary Sjögren syndrome: analysis of T cell subsets in cutaneous infiltrates.

Immunohistochemical analysis was performed on the annular erythema associated with Sjögren syndrome. This type of annular erythema is characterized by a doughnutlike appearance with an elevated border and central pallor; it is distinct from subacute cutaneous lupus erythematosus. Histologically this erythema is characterized by coat sleeve-like infiltration of lymphocytes around blood vessels and by nuclear debris in the connective tissue. Vasculitis or epidermal changes suggestive of lupus erythematosus were not observed in any of the cases, although immunoglobulin or complement deposition along the basement membrane zone and focal liquefaction degeneration of the basal layer in involved skin were seen in some cases. Major dermal infiltrates consisted primarily of CD4+ and 4B4+ lymphocytes.

Lupus erythematosus associated with erythema multiforme: does Rowell's syndrome exist?

We describe a patient with lupus erythematosus who experienced an unusual erythema multiforme-like eruption suggestive of Rowell's syndrome. We compare our case and 9 other reports of lupus erythematosus associated with erythema multiforme to the 4 cases reported by Rowell. Our findings indicate that Rowell's original criteria are not well preserved. The coexistence of lupus erythematosus with erythema multiforme does not impart any unusual characteristics to either disease, and the immunologic disturbances in such patients are probably coincidental.

[Outbreak of infectious erythema at a urban health center].

OBJECTIVES: To analyse the clinical and serological characteristics of an outbreak of infectious erythema in patients attending the two paediatrics clinics at the Soria Norte Health Centre. DESIGN: Cross-sectional, descriptive study. SETTING: Paediatrics clinics of the Soria Norte Health Centre. PATIENTS: Once the outbreak was suspected, the study was conducted on children with a clinical picture suggesting infectious erythema who attended successively two Soria Norte paediatrics clinics between April and August 1998 and who consented to the analyses. INTERVENTIONS: The clinical symptoms, serology, haemogram and evolution of 25 patients were recorded. RESULTS: 25 patients of the total were studied and diagnostic suspicion was confirmed in 84% of the cases. There were no differences between sexes, with an average age of 6.1, SD 2.015. The most constant clinical sign was exanthem on the cheeks, present in 100% of the cases confirmed, followed by 57% exanthem on the trunk and limbs, 9.5% adenopathies and 4.7% temperature. No complications were found in any case. CONCLUSIONS: The results of this study confirm the existence of an outbreak of infectious erythema in our area. Serological confirmation of an outbreak is useful and can lay the basis for a proper therapeutic and preventive focus (against possible complications). We highlight the generally benign nature of the disease.

The 30 degree tilt position vs the 90 degree lateral and supine positions in reducing the incidence of non-blanching erythema in a hospital inpatient population: a randomised controlled trial.

Manual repositioning of patients by nursing staff is a recognised technique for preventing pressure ulcer formation. The 30 degree tilt is a method of positioning patients that, in the laboratory setting, reduced the contact pressure between the patient and the support surface. A randomised controlled trial was used to examine the effects of the 30 degree tilt position in reducing the incidence of non-blanching erythema (i.e. established pressure damage) in a hospital inpatient population (n=23) when compared to the use of the 90 degree lateral and supine position (n=23). The primary outcome of the trial was the incidence of pressure damage, defined as non-blanching erythema. In this study no subject developed pressure damage that presented with visible breaks in the epidermis, but all damage was restricted to areas of non-blanching erythema (five of the 39 subjects who completed the study exhibited such injury). The main findings of this study were that patient positioning using the 30 degree tilt method did not reduce the incidence of pressure damage compared with either the 90 degree lateral or supine positions. This study also investigated the feasibility of using the 30 degree tilt position with medical inpatients; it found that 78% of subjects experienced difficulty in adopting and maintaining the position. This finding seriously questions the practicality of using the 30 degree tilt method with a predominantly ill population.

Erythema multiforme after application of aural Gentisone HC drops.

Erythema multiforme is an uncommon acute self-limiting condition characterized by mucocutaneous lesions of varying severity with a variable pattern of recurrence. This is a case of a four-year-old girl who developed erythema multiforme secondary to topical aural application of Gentisone HC drops (hydrocortisone acetate one per cent, gentamicin 0.3 per cent (as sulphate)) prescribed as a treatment for otorrhoea following grommet insertion. The clinical features are described and the literature reviewed. To my knowledge, this is the first reported case in the literature, of erythema multiforme secondary to a topical aminoglycoside/steroid preparation.
