The radiographic incidence of chronic sinus disease in the pediatric population.

The otolaryngologist is frequently consulted for paranasal sinus disease which has been identified as an incidental finding on computed tomography (CT) in children. The significance of such a finding is difficult to interpret without prior knowledge of the actual incidence of chronic sinus mucosal hypertrophy in the general pediatric population. In this study of pediatric CT scans from busy otolaryngology and ophthalmology clinics, the radiographic incidence of sinus disease in the general pediatric population is estimated. One hundred forty-two CT scans of the orbits and temporal bones obtained over an 18-month period were reviewed. Fifty-eight (41%) scans showed some mucosal thickening or opacification in at least one sinus. When patients were stratified by age, or by the presence or absence of chronic otitis media or respiratory tract disease, exclusive of clinical sinusitis, there was no statistical difference between groups. The authors conclude that the asymptomatic child with an incidental paranasal sinus finding on CT scan need not be worked up further unless clinical symptoms and signs are elicited.

Trace elements in chronic sinusitis.

Trace elements are indispensable for life and play an important role in the immunological system. Since it is believed that serum trace element values decrease in chronic infections, 43 patients with chronic sinusitis were evaluated for Zn, Mg and Cu levels. While all trace elements were found to be within normal ranges, Cu values were found to be significantly lower than in a control group of 20 age- and sex-matched volunteers. The reasons for this are unclear in the present study.

Clinical effect of low-dose, long-term roxithromycin chemotherapy in patients with chronic sinusitis.

We studied the clinical efficacy of roxithromycin (RXM) administered at the daily dosage of one tablet (150 mg) for 3 months in 30 patients with chronic sinusitis. The effectiveness of this drug was evaluated on a four-point scale. Subjective and objective symptoms disappeared or decreased markedly, especially postnasal drip and nature of discharge in 80 percent or more of the patients. All symptoms significantly decreased (P < 0.001; headache P < 0.05), except for the sensation of foul odor. Symptoms improved even in those cases in which Haemophilus influenzae was detected. It is suggested that RXM produce some clinically beneficial effect through an immunological and or anti-inflammatory mechanisms in addition to its antibiotic effect.

Impact of gender on general health-related quality of life in patients with chronic sinusitis.

BACKGROUND: The aim of this study was to evaluate the impact of gender on health-related quality of life (HRQOL) in patients with chronic sinusitis METHODS: A prospectively administered survey was performed. For measuring HRQOL, we used the German versions of the Short Form 36 (SF-36; eight subscales) and the European Quality of Life-5 Dimensions (EQ-5D) (two subscales). For all SF-36 subscales we used norm-based scoring of the general German population. RESULTS: One hundred twenty-three patients (45 women and 78 men) underwent HRQOL evaluation at baseline (preoperatively) and 3 months after endonasal sinus surgery. Compared with male patients, female patients scored a significantly worse baseline in all SF-36 and one EQ-5D subscales and 3 months later in only one SF-36 subscale. Compared with the norm-based scores, both women's and men's SF-36 scores reached normal levels in seven subscales after 3 months. CONCLUSION: Compared with men, women assessed preoperative HRQOL as being worse. Most women's and men's SF-36 subscales reached normal levels at the 3-month follow-up.

Nasal mucociliary clearance of chronic sinusitis in relation to rheological properties of nasal mucus.

The role of the viscosity (eta') and elasticity (G') of nasal mucus in nasal mucociliary clearance (MCC) in chronic sinusitis (CS) was studied. The nasal mucus was collected from 17 patients with CS, and both eta' and G' were determined by an oscillating sphere magnetic rheometer. Following the nasal mucus collection, nasal MCC was measured by a radioisotope method in the patients and 22 normal subjects. Nasal MCC in the patients was significantly reduced compared with that in the normal subjects (p < .05). No significant correlation was noted between nasal MCC and eta', or between nasal MCC and G'. The results suggest that factors other than eta' and G' of nasal mucus may control nasal MCC in CS.

Neutrophil elastase and its complex with alpha 1-antitrypsin in soluble and insoluble fractions of nasal secretions of chronic sinusitis.

Immunoreactive neutrophil elastase (NE) and its complex with alpha 1-antitrypsin (AT) was measured by double antibody enzyme linked immunosorbent assay (ELISA) in nasal secretions of chronic sinusitis (CS). Nasal secretions were separated into two fractions: PBS-soluble and insoluble fractions. Elastolytic activity was also examined. Mean value of total NE level was 31.0 micrograms/ml in the soluble fraction, which was significantly lower than that in the insoluble fraction (71.9 micrograms/ml, p less than 0.01). On the other hand, the percentage of complexed NE in total NE in the soluble fraction (33.7%) was significantly higher than that in the insoluble fraction (12.1%, p less than 0.01). Elastolytic activity in the soluble fraction (23.4 RFU) was significantly lower than that in the insoluble fraction (170.5 RFU, p less than 0.01). NE with elastolytic activity exists in nasal secretions of CS, and active-free NE in the insoluble fraction could be a major source of enhancement and continuation of mucosal inflammation.

Microscopic antrostomies in children: a review of the literature in chronic sinusitis and a plan of medical and surgical treatment.

Potential complications, morbidity, treatment failures and "nasal cripples" make one cautious about advising radical sinus surgery. Though antibiotics have greatly reduced the need for radical surgery, some patients do not respond to conservative treatment. Reviewed is pertinent literature concerning treatment of chronic, refractory sinusitis in children, particularly intranasal antrostomy. Medical treatment for chronic sinusitis precedes surgical intervention. Allergic history, nasal cytology and radiographic examination are essential. Since principles of adequate surgical drainage and ventilation have long been established, controlled antrostomy procedure is done with the aid of the operating microscope, creating a nasal mucosal flap. A new instrument is presented---a self-retaining stabilized retractor speculum which allows adequate visualization with the microscope while protecting the mucosal flap and freeing both hands for the procedure.

Cytomegalovirus infection presenting as chronic sinusitis and nasal polyposis: a case report.

Cytomegalovirus (CMV) is the most common viral opportunistic infection in patients with acquired immunodeficiency syndrome (AIDS), affecting up to 90% of patients. CMV can manifest as primary, latent or chronic persistent infection, most commonly appearing as retinitis, encephalitis or gastrointestinal disease. Other less common presentations, including otolaryngologic expression of the virus, have also been noted. Described here is the first known documented case of CMV with both chronic sinusitis and nasal polyposis in an AIDS patient.

Compressive optic neuropathy secondary to chronic sinusitis.

The paranasal sinuses surround the orbit on three sides. The thin bony walls of the sinuses are an ineffective barrier to the spread of infection to either the orbit or the intracranial cavity. In this report a case of compressive optic neuropathy secondary to chronic sinusitis is presented. The anatomical and physiologic relation between the sinuses and the orbit are reviewed and sinusitis is discussed.

Traumatic intracavernous aneurysm of the internal carotid artery following surgery for chronic sinusitis.

A case of traumatic intracavernous aneurysm is presented. The aneurysm at the siphon of the internal carotid artery was produced as a result of injury during an operation for chronic sinusitis. In addition to the formation of the aneurysm, the angiogram showed an irregular intimal dissection along the internal carotid artery distal to the aneurysm. To prevent disastrous bleeding from the aneurysm and further progression, a trapping operation was made in combination with a superficial temporal-middle cerebral artery anastomosis.

Necrotizing sialometaplasia in the setting of acute and chronic sinusitis.

Necrotizing sialometaplasia is a benign lesion that may be mistaken for mucoepidermoid or squamous cell carcinoma. A case report is presented in which necrotizing sialometaplasia was noted as an incidental finding in the setting of acute and chronic sinusitis. It is critical to recognize the lesion as benign so as to avoid overdiagnosis and overtreatment.

Nasal interleukin-5, immunoglobulin E, eosinophilic cationic protein, and soluble intercellular adhesion molecule-1 in chronic sinusitis, allergic rhinitis, and nasal polyposis.

OBJECTIVE: To compare concentrations of interleukin-5 (IL-5), immunoglobulin E (IgE), eosinophilic cationic protein (ECP), and soluble intercellular adhesion molecule-1 (sICAM-1) in nasal secretion and serum of patients with chronic nonallergic sinusitis, allergic rhinitis, and nonallergic nasal polyposis to obtain information about the pathogenesis of these diseases. METHODS: Nasal secretion and serum were analyzed by routine enzyme-linked immunosorbent assay techniques. Nineteen patients with chronic nonallergic sinusitis, 24 patients with seasonal allergic rhinitis, and 18 patients with nonallergic nasal polyposis were included in the study. Eight healthy, nonallergic probands served as control subjects. RESULTS: Significantly elevated concentrations of IL-5 (5-fold, P < .05) and IgE (15-fold, P < .01) were detected in nasal secretion of patients with allergic rhinitis (IL-5, 51.8 +/- 13.2 pg/mL; IgE, 41.9 +/- 20.9 kU/L) or nonallergic nasal polyposis (IL-5, 57.9 +/- 36.9 pg(mL; IgE, 40.5 +/- 20.2 kU/L) compared with controls (IL-5, 10.6 +/- 7.8 pg/mL; IgE, 2.8 +/- 0.5 kU/L) or with patients with chronic nonallergic sinusitis (IL-5, 16.5 +/- 13.2 pg/mL; IgE, 5.4 +/- 3.1 kU/L). There were no significant differences between patients with allergic rhinitis and those with nonallergic nasal polyposis. Concentrations of ECP were significantly elevated (sixfold, P < .01) in patients with allergic rhinitis (297.8 ng/mL +/- 173.1) compared with controls (52.4 +/- 28.0 ng/mL) or patients with chronic nonallergic sinusitis (44.8 +/- 40.1 ng/mL), whereas twofold higher concentrations (not significant) of ECP were observed in patients with nonallergic nasal polyposis (107.1 +/- 26.6 ng/mL). Significantly elevated concentrations of sICAM-1 in nasal secretion (threefold, P < .05) were detected only in patients with chronic nonallergic sinusitis (79.4 +/- 45.6 ng/mL). The elevated sICAM-1 nasal secretion values in this group correlated significantly (P < .05) to the serum values. CONCLUSIONS: Equally elevated concentrations of IL-5 and IgE in patients with allergic rhinitis and nonallergic nasal polyposis implicated similar pathogenic processes in both diseases. Whereas the pathogenesis of allergic rhinitis is IgE-specific, the pathogenesis of nasal polyps is not as clear. IL-5 was suggested to play a pivotal role in tissue eosinophilia, which was confirmed by data in the present study. Elevated concentrations of ECP were suggested to result from tissue eosinophilia--a characteristic of both diseases. Elevated concentrations of sICAM-1 in patients with chronic nonallergic sinusitis pointed to its key role in the recruitment of neutrophils into the inflamed tissue, whereas an important role in eosinophil recruitment was ruled out.
