[Treatment of cochlear neuritis of vascular etiology by endaural phonophoresis and phonoelectrophoresis].

In an attempt to develop an adequate and pathogenetically indicated therapeutic method for treating inner ear dysfunction aggravated by angiocerebral circulation problems, we treated 32 patients using endaural phonophoresis and phonoelectrophoresis. In the treatment the following drugs were used: clopheline, sermion, cocarboxylase, sansurdal, otoneurine, theobromine, glycerol, mannitol, diphenine, proserine, ascorbic acid, ATP, B1, B6, and B12 vitamins. The effect was measured audiometrically and rheoencephalographically. Good results were recorded in 18 patients. It is concluded that treatment of the above cases should include endaural drug phonophoresis and phonoelectrophoresis used alone, if rheoencephalographic parameters are stable, or used in combination with peroral and parenteral drug administration, if cerebral circulation parameters are unstable.

Histochemical changes in the digestive tract in irradiated rats.

Wistar rats (110-125 g) were irradiated with a single dose of 500 R. Histochemical studies were done concerning the glycoproteins (GP) of sublingual glands, gastric, small intestine and colon mucosa, and some intestinal enzymes: acid and alkaline phosphatase (ACP, ALP) leucineaminopeptidase (LAP), Mg-dependent ATP-ase, NADH-diaphorase, lactic dehydrogenase (LDH). After irradiation all these reactions were diminished, with a maximal effect between 3-5 days. This impairment is in accord with the maximal lethality in this interval after such a degree of irradiation that produced the gastrointestinal syndrome. Cocarboxylase, a radioprotector, improved these changes regarding the structures of the small intestine and also the GP of sublingual glands, stomach, small intestine and colon, demonstrating there its efficiency.

[Electrocardiographic studies of patients with acute hellebore (Veratrum album) poisoning].

In 12 patients, 20 to 80 years of age, 6 men and 6 women, with acute hellebore (Veratrum album) intoxication the electrocardiographic changes were studied. In 10 of them a characteristic ECG pattern was found: sinus bradycardia 38-40/m, shortening of the interval PQ up to 0.12-0.08 s and QTc up to 0.36-0.32 s, slow intraventricular conduction--transitory right and incomplete left bundle-branch block, rhythm disorders--atrial and substitutional ventricular extrasystoles, nodal rhythm (I patient), disturbed ventricular repolarization--depression of ST-segment, low and (or) pointed T waves. The authors are of the opinion that the bradycardia is due to a reflexively increased vagal tonus but the other changes of the ECG are caused by the direct toxic action of the hellebore alkaloids on the myocardium. This suggestion is supported by the fact that atropine corrects the bradycardia but not the other pathological changes of the ECG. They are beneficially influenced by the fast elimination of the toxins and the application of cardiotropic means--atriphos, cocarboxylase, vitamins of the group B.

[A variant of pathogenetic therapy in the acute period of perinatal hypoxic-traumatic brain lesions in premature infants].

The restorative potential of reparative cellular processes was studied in 39 immature infants born in hypoxia and receiving a pathogenetic treatment. In addition to the conventional treatment the test group of 24 infants received intravenously 20% piracetam solution, 1% riboflavin-mononucleotide solution, and 0.5% lipoic acid solution beginning from the first days of life. To correct the cellular metabolic processes in a control group of 15 infants, they were given 10% glucose solution, 5% ascorbic acid solution and cocarboxylase. The activity of dehydrogenases in the peripheral blood remained virtually unaltered in the control group but increased by an average of 1.5-2 times in the test group. An 18-month follow-up ascertained that the incidence of neurological complications in the infants receiving the proposed treatment was significantly lower as against the control patients.

[Thiamine diphosphate absorption by the cells of Pliss lymphosarcoma and Ehrlich ascitic cancer].

Concentration difference of vitamin B1 coenzyme in blood and in ascitic fluid was found in mice with the Ehrlich ascites tumour. The thiamine diphosphate deficiency was detected in the liver of rats with the Pliss lymphosarcoma. The introduction of cocarboxylase to animals was accompanied by the active absorption of this coenzyme by tumours. It is observed that experimental tumours disturb the vitamin B1 balance in the organism.

Effect of ethanol administration on thiamine metabolism and transketolase activity in rats.

To clarify the effect of ethanol on thiamine metabolism and activity of transketolase, a thiamine dependent enzyme, in vivo and in vitro experiments were carried out. For the in vivo experiment, rats were separated into four groups: Group 1, thiamine sufficient diet with no ethanol administered; group 2, thiamine sufficient diet and ethanol administered by stomach tube; group 3, thiamine deficient diet with no ethanol administered; group 4, thiamine deficient diet and ethanol administered. After three weeks, the rats were killed, then thiamine levels and the activity of transketolase were assayed. Thiamine concentration in brains and livers of ethanol fed rats were lower than that in control rats. Urinary thiamine was higher in ethanol administered rats than in controls. The increase in the liver transketolase activity by the in vitro addition of cocarboxylase (TPP effect) was higher in group 3 than in group 4 although original transketolase activity was lower in group 4. For the in vitro experiment, various amounts of ethanol or acetaldehyde were added to the supernatant of brain and liver of intact rats, and transketolase activity was determined. Inhibition by acetaldehyde to transketolas was about 10 times higher than that by ethanol.

[Comparative evaluation of the effectiveness of different treatment methods of atrial fibrillation in thyrotoxicosis patients].

Comparative data are presented on the treatment of auricular fibrillation in 105 patients with severe thyrotoxicosis. All the patients were divided into 3 groups to evaluate the effectiveness of the treatment: the 1st group of 45 patients, given complex therapy (mercazolyl, diiodotyrosine, cardiac glycosides, potassium drugs, ATF, cocarboxylase); the 2nd group of 40 subjects, treated with inderal in a dose of 80 to 120 mg during 7 days, followed by complex therapy; the 3d group of patients given complex therapy and cordaron. The latter combined with complex therapy was the most effective method in the treatment of auricular fibrillation in patients who suffered from thyrotoxicosis. In 15 of 20 patients of the 3d group the sinus rhythm was recovered. The treatment effectiveness in this group of patients was not dependent on their age, thyrotoxicosis and arrhythmia duration and the degree of circulatory disturbance, whereas in patients of the 1st group good results, as a rule, were seen in young and middle-aged subjects with the disease duration, not exceeding one year and circulation disorder of the Ist and IInd stages. It is concluded that in pathogenesis of auricular fibrillation the main role belongs to the unfavourable effect of excessive thyroid hormones upon the metabolic processes in myocardial auricles.

[Analysis of the daily human requirements for thiamine in the newly arrived population of Polar regions].

The erythrocytes transketolase activity and the TDP-effect were determined in persons residing in the regions beyond the polar circle and these demonstrated a moderate thiamine deficiency. The daily thiamine intake with food in the examined did not exceed 1.5 mg, the thiamine-diphosphate of the epoenzyme amounting to 90 per cent. Following introduction of various pharmacological preparations of vitamin B1 for a space of 2 weeks a somewhat greater transketolase activity and a diminution of the TDP-effect were observed. Administration of cocarboxylase and vitamin B1 with Mg and gelatin more favourable changes were obtained. Some macro- and micro-elements were found to exercise a positive effect on the thiamine assimilation by the tissues. The results obtained indicate that the daily thiamine requirement (allowance) of man in the North does not surpass 1.5-2.0 mg. To control hypovitaminoses in the North it is more effective to use natural vitamin-carriers rather than pharmacological preparations of vitamin B1.

[Hepatorenal lesions in acute acetone poisoning].

Lesions of the parenchymal organs, not described so far, were found in two males and two females with acute acetone intoxication, besides the well known morbid changes in the gastrointestinal tract and ZNS, regardless the energetic hepato--and nephroprotective treatment. In all patients (one of them inhaled acetone vapours and the rest swallowed the poison per os) liver lesion is present with the corresponding clinical manifestations and changes in the laboratory liver indices. In two of the per orally intoxicated, renal lesion was also found, manifested in a milder degree. The state of the parenchymal organs is necessary to be followed up carefully in case of acute acetone intoxications and a timely treatment with hepato- and nephroprotective drugs in large doses to be applied: glucocorticoids, glucose and levulose solutions, cocarboxylase, lipovitan, vitamin "C", "B1", "B2", "B12", calcium gluconate.

[Fisher's syndrome].

The authors describe a 43-year-old man with ophthalmoplegia, ataxia and areflexia and with 0.42 g/l of protein in the cerebrospinal fluid. EMG disclosed some slowing down of the conduction velocity in the motor fibres of the extremities. Treatment with synacthen depot, cocarboxylase and vitamins gave a good effect. The described case corresponds closely to Fisher's syndrome and may be a special form of brain-stem encephalitis involving the mesencephalon.

Effect of medication with pravastatin sodium on hemorheological parameters in patients with hyperlipoproteinemia.

Pravastatin sodium, a newly developed potent synthesis inhibitor of HMG-CoA (beta-hydroxy-beta-methylglutaryl-cocarboxylase-A) reductase (Sankyo Co., Ltd., Japan) was medicated, 10 approximately 15 mg/day (mean: 11.1 mg/day) for 10.2 weeks in mean, in 14 patients with primary hyperlipoproteinemia of more than 230 mg/dl of serum cholesterol levels (mean age: 56.9 y.o.). The values of serum cholesterol decreased (from 242 +/- 12 to 207 +/- 22; mg/dl), and of high density lipoprotein (HDL) increased (from 42.3 +/- 8.8 to 45.3 +/- 9.2; mg/dl) significantly (p < 0.05, respectively) 10.2 weeks in mean after medication with pravastatin sodium. The whole blood viscosity, at every shear rate examined, corrected blood viscosity, for the standard hematocrit level of 45%, and plasma fibrinogen decreased significantly (p < 0.05, respectively) at the same time, without showing significant differences any more 10.2 weeks in mean after medication with those in 14 elderly normal subjects (mean age: 56.7 y.o.), which suggested that the hemorheological parameters in patients with primary hyperlipoproteinemia had improved significantly by medication with pravastatin sodium.

[Vitamin disturbances in chronic renal insufficiency. I. Water soluble vitamins].

Deficiency and/or altered metabolism of vitamins in CRI is caused by uremic toxins, dietary restrictions, catabolic illness, losses during dialysis and drug interaction. There are no reports of consistent studies on vitamin status of water soluble vitamins in CRI. Vitamin B1 (thiamine) deficiency several authors found most frequently in CAPD patients. The cause of this deficiency depends probably on increased requirement of vitamin B1 due to high glucose intake with peritoneal dialysis solution. In patients with polyneuropathy high doses of thiamine pyrophosphate (Cocarboxylase), given i.v., can be helpful in this respect. There are conflicting reports concerning plasma level of vitamin B2 (riboflavin) in CRI patients. Some authors recommend its supplementation. The majority of patients with CRI exhibit biochemical and clinical signs of vitamin B6 deficiency. There exists an univocal opinion that supplementation of this vitamin effects the cellular immune system and the amino acid metabolism as well. An adequate dose of vitamin B6 is still a matter of dispute. Evidence of vitamin B12 deficiency has been reported rarely, thus, only few authors recommend the supplementation of it, mainly in CAPD patients. According to most authors the losses of folic acid and ascorbic acid during dialysis require oral supplementation. Despite the divergences in opinions concerning the deficiency of water-soluble vitamins in CRI patients, the supplementation of these vitamins is practised in many nephrological centers. The amount and the route of vitamins, administered to CRI patients, should be individualized.

[Differentiated treatment and prevention of arrhythmias in myocardial infarct].

Monitoring and electrocardiographic observations were conducted in 900 patients during the acute and subacute periods of myocardial infarction. It was found that without preventive therapy arrhythmias develop in 85% of the patients, and with preventive antiarrhythmic treatment--in 75.5%. A combination of two and more types of arrhythmias was encountered in 42% of the cases. In the presence of routine therapy for myocardial infarction individual antiarrhythmic drugs displayed different efficacy. In supraventricular forms of arrhythmias the most effective drugs are Chinidin and Chinidin-Durules, Inderal and Hiluritmal, in ventricular forms--Lidokaine, Chinidin, Novocainamide, Inderal, Hiluritmal. A combined employment of these drugs against the background of potassium and magnesium salts administration, Inosie-F, water-soluble camphor and Cocarboxylase promoted the restoration of a normal sinus rhythm in 85% of the cases. When the drug therapy produced no antiarrhythmic effect and ventricular fibrillation developed, electroimpulse therapy was employed. In persistent atrioventricular blocks only cardiostimulation was effective.
