[Anesthesia for bronchoscopy: application of laryngeal mask airway and "minidose midazolam anesthesia"].

Bronchoscopy distresses most of the patients. Therefore, we combines a small dose of midazolam (ca. 2 mg) with laryngeal mask airway. This method of anesthesia has the following features: (1) mild discomfort, (2) day care, (3) putting bronchoscope in or out easily, (4) mild effect on respiration and circulation, and (5) easy respiratory care. But improper cases for this method include non-fasting patients, those at high risk for regurgitation, and those with low pulmonary compliance. This is a very usefull anesthetic method for bronchoscopy.

[Preoperative adrenergic beta receptor blockade and anesthesia. II. Effect on peroperative anxiety and cardiovascular response in epidural anesthesia].

The effects of a preoperative beta-adrenoceptor antagonist, metoprolol, on peroperative anxiety, relaxation, general feeling, blood pressures and heart rate, were studied in patients undergoing orthopaedic surgery during epidural anesthesia. The study was double-blind and placebo controlled. Twenty non-consecutive patients were randomized to receive either metoprolol 100 mg or placebo orally 1-2 hours before anaesthesia. Patients receiving metoprolol scored statistically significantly better on anxiety, and general feeling. The average height of the epidural block was ThVIII. The average blood pressure was reduced by 5% during the first 45 minutes in both groups. The average maximum falls in mean blood pressure from preanaesthetic to the lowest measured after the epidural were 14% (metoprolol) and 7% (placebo). The heart rates were significantly lower after metoprolol. In conclusion, the patients felt subjectively better after metoprolol and no significant differences were found in blood pressure after metoprolol compared to placebo.

[Sedation, combined anesthesia, and total intravenous anesthesia (TIVA) with propofol in the pediatric surgical patient].

Total intravenous anaesthesia (TIVA) has recently obtained a wide diffusion in paediatrics, thanks to the pharmacological properties of propofol. The authors make a review on sedation, combined anaesthesia and Tiva in paediatric anesthesia, particularly on propofol and its characteristics. They describe the practical use of TIVA with recent knowledge in this field.

[Relationships between course of anesthesia, depth of anesthesia and acetylcholine content of the brain after urethane and pentobarbital administration in the rat].

Urethane (1.25 g/kg) and pentobarbital (0.06 g/kg) 30 min after i.v. application increase the content of acetylcholine in telencephalic areas but not in the brain stem. These acetylcholine levels are normalized again 120 min p.i.; but urethane treated animals show an elevated acetylcholine content in the brain stem at this time. A different course of anesthesia by pentobarbital and urethane respectively correlated with this result: Only in the presence of urethane a persistent depression of spontaneous motility was seen, whereas the effectivity of both anesthetics on pinna and corneal reflexes and in respect of pentobarbital also on active movements occurrence declined in the end of the test period.
