Short sequences of high homology between the primary structures of alpha-fetoprotein and albumin.

Several reports have shown a high percentage of homology between the primary structures of alpha-fetoprotein (AFP) and serum albumin (Alb). With the aid of a program which searches for all identical sequences, we compared the aminoacid sequence of AFP and Alb from several species. Two short pieces of very high homology were always present in all the AFPs and Albs studied, but in none of the other analyzed proteins. One is in Domain I (16 amino acids long) and the other in Domain III (17 residues long). The implications of this finding in relation to the function and evolution of AFP and Alb are discussed.

[Production of fibronectin and PGE2 by cultured human alveolar macrophages].

Production of fibronectin (Fn) and prostaglandin E2 (PGE2) by human alveolar macrophages (AM) which were obtained by bronchoalveolar lavage was studied in vitro. AM obtained from patients with idiopathic interstitial pneumonia (IIP) produced much more Fn than those from normal volunteers but produced less amounts of PGE2. We also tested the effect of lipopolysaccharide (LPS), phorbol 12-myristate 13-acetate (PMA), zymosan and albumin-anti albumin complex (alb-anti alb) on production of Fn and PGE2 from AM. LPS, PMA and zymosan suppressed Fn production but stimulated PGE2 production by AM from patients with IIP but indomethacin reversed the suppressive effect of LPS, PMA and zymosan on Fn production. On the contrary, alb-anti alb stimulated Fn production by AM. Furthermore, exogenous PGE2 suppressed Fn production by AM from patients with IIP in a dose-dependent manner. These data suggest that Fn production by AM may be changed by different stimuli or different states of disease, and there is close relationship between the production of Fn and PGE2 by AM.

Attributes of microorganisms that contribute to recurrence and intractability of vaginal mycosis.

Recurrence or intractability of vaginal mycosis may be ascribable to various factors. Attributes of microorganisms that contribute to recurrence and intractability of vaginal mycosis have not fully been elucidated. We studied this problem by means of a modern fungal identification method, strict measurement of the minimum inhibitory concentration and close clinical analysis. As for the detection rate for Candida albicans (C. alb.) and Torulopsis glabrata (T. glab.) as pathogens of vaginal mycosis, we found a relative increase was observed in T. glab. that so far has shown strong adaptability, even under unfavorable environments, to general fungi. The MIC values for clotrimazole (CTZ) against T. glab. were about 4 times as high as those against C. alb. No acquired resistance to CTZ was observed in these 2 groups. Conventional identification methods have been unable to provide conclusive evidence of the self-infection of vaginal mycosis from the anus to the vagina via the vulva. In some of the cases examined in this study, the modern fungal identification method succeeded in demonstrating that the fungal species detected in these three regions were of the same origin. Some of the C. alb. and T. glab. strains which reappeared after treatment were found to be different from those detected before treatment, a finding indicating that reinfection, apart from recurrence, occurred. Besides, treatment occasionally resulted in replacement of C. alb. with T. glab.

Extracellular fluid volume and distribution in relation to changes in plasma colloid osmotic pressure after major surgery. A randomized study.

This prospective randomized study deals with the changes in magnitude and distribution of the extracellular fluid volume (ECV) and the relation between such changes and the plasma colloid osmotic pressure (COPP) in patients having abdominal aortic surgery. Two groups of thirteen patients each received whole blood to replace the blood loss. One group (ALB) had additionally 80 g albumin administered on the day of operation and 20 g the following 3 days, the other group (NON-ALB) did not receive extra albumin. In the ALB group ECV decreased from 9.3 +/- 1.71 (= 147 ml/kg) to 8.4 +/- 2.01 (NS) on the first postoperative day (p.o.d.) and to 9.2 +/- 2.5 1 (NS) on the fourth p.o.d. In the NON-ALB group the preoperative ECV of 8.1 +/- 1.11 (= 125 ml/kg) was unchanged on the first p.o.d. and 8.4 +/- 1.41 (NS) on the fourth p.o.d. The differences between the groups were non-significant. The post-operative changes observed in ECV were not related to COPP in the range 33 mmHg to 21 mmHg. The ratio between plasma volume (PV) and ECV was 0.35 +/- 0.06 preoperatively in both groups. Postoperative changes were non-significant and no correlation between COPP and PV/ECV could be found. In the present study the distribution of ECV between plasma and interstitium was found to be independent of COPP in the interval 33-21 mmHg. Furthermore, no obligatory contraction or expansion of ECV occurred after major elective surgery.

Removal of bilirubin by hemoperfusion with ion exchange resins.

Removal of bilirubin by hemoperfusion with macroporous resins is suggested to replace exchange transfusion of blood from jaundiced infants. Anion exchangers tested for BIL binding rate and the effects of various system parameters, i.e. particle size, temperature, bed volume, fluid flow rate and particle porosity are reported. The dissociation equilibrium constant K of the BIL-ALB (albumin) complex was determined from the equilibrium adsorption isotherms data, at different ALB concentrations. Increasing ALB concentration decreased the BIL adsorption rate. For a given ALB concentration, the ratio of unbound to total BIL was constant and the initial BIL adsorption rate was independent of total BIL concentration.

Distribution of albumin- and/or alpha-fetoprotein-positive cells in hepatocellular carcinoma.

Albumin (ALB)-positive cells were identified by an immunoperoxidase technique in 52 of 53 autopsy cases and in all of 13 surgical cases of hepatocellular carcinoma (HCC). The distribution pattern of ALB-positive cells could be classified into three groups: diffuse, localized, and sparse. The diffuse type was the most common pattern and was usually seen in well or moderately differentiated HCC, showing a trabecular growth pattern. The localized or sparse patterns were more frequent in poorly differentiated HCC showing a compact growth pattern. alpha-Fetoprotein (AFP)-positive cells were detected in 37 of the 53 autopsy cases of HCC and 11 of the 13 surgical cases. The number of AFP-positive cells and the intensity of the immunoperoxidase reaction were roughly proportional to serum AFP levels in most cases. In most regions of HCC, there seemed to be an inverse relationship between the number of ALB- and AFP-positive cells, suggesting tht most HCC cells synthesized only one of the two antigens studied. ALB-positive hepatocytes were found in all of the normal or cirrhotic livers examined and in the tumor-free regions of the HCC-containing livers. In contrast, AFP was not detected in nonneoplastic hepatocytes.

Immunocytochemical localization of alpha-fetoprotein in the developing rat brain.

The immunocytochemical localization of the alpha-fetoprotein (AFP), serum albumin (ALB), transferrin (Tf) and immunoglobulins (IgG) in developing rat brain has been studied by the indirect peroxidase technique using rabbit antibodies to AFP, ALB, Tf and IgG followed by goat anti-rabbit immunoglobulins conjugated with peroxidase. Evidence of a high selective staining for AFP and ALB is presented. The pattern of morphological localization of both proteins was similar throughout fetal and post-natal development up to 20-25 days of age. Large areas and groups of cells in all regions of the brain from the olfactory bulb to the medulla oblongata appeared positively stained at a given moment in the development. The labeling was cytoplasmic and, in the neuronal elements, extended to their axonic and dentritic prolongations. The intracellular localization of AFP and ALB in the developing rat brain may be related to the binding affinity of these proteins for oestrogens and/or essential fatty acids. The morphological data presented here suggest that both proteins are actively involved in the uptake of such substances by the cellular structures of the brain.

Heterogeneity of alpha-fetoprotein(AFP) and albumin containing cells in normal and pathological permissive states for AFP production: AFP containing cells induced in adult rats recapitulate the appearance of AFP containing hepatocytes in fetal rats.

The cellular localization of alpha-fetoprotein (AFP) and albumin (ALB) in permissive states for AFP synthesis has been examined. The cells containing AFP associated with permissive states in the adult are similar in appearance to cells that are present during the development of fetal liver. In fetal liver, AFP is seen in most developing hepatocytes ranging from small 'oval' like cells to larger dividing hepatocytes and in cells organized in glandular structures. Following exposure to some chemical hepatocarcinogens, AFP can also be seen in small 'oval' cells, ductal-like cells, and larger atypical hepatocytes that form glandular-like structures. Following partial hepatectomy or galactosamine-induced live injury, AFP is seen in a few large parenchymal cells usually containing identifiable chromatin Cells which contain AFP almost always contain ALB as well, but for each cell type there are many more ALB containing cells than AFP containing cells. ALB and AFP containing hepatoma cells are more frequently located adjacent to tumor vessels and AFP production by hepatoma 777 in vitro is associated with the growth state of the tumor. The AFP containing cells that are seen during restitutive proliferation most likely arise from deregulation of proliferating adult hepatocytes. The non-hepatoma AFP containing cells that appear early during carcinogenesis may arise in the adult by retrodifferentiation of hepatocytes or by proliferation of stem cells. These morphologically different AFP containing cells may or may not be precursors of the hepatocellular carcinomas which develop later.

Nutritional assessment and postoperative morbidity. A prospective study in 286 consecutive surgical patients.

286 consecutive surgical patients went through a nutritional assessment following admission to the hospital. Serum albumin (S-alb), serum transferrin (S-transf), triceps skinfold (TSF) and arm muscle circumference (AMC) were recorded. The frequency of pathological values of TSF and AMC varied greatly depending on the reference population used to set the standard. There was no correlation between TSF or AMC and postoperative morbidity. S-alb less than 36 g/l and S-transf less than 2.0 g/l were associated with a marked increase in postoperative morbidity (29.4% and 20.0% respectively as compared to 7.6% in all operated patients). Thus S-alb and S-transf seemed to be able to predict postoperative morbidity. However, this ability varied depending on the diagnosis. Age was another factor with a marked influence on postoperative morbidity. It is suggested that attempts to make nutritional interpretations from S-alb and S-transf take into account patient age and the diagnosis. TSF and AMC seem to be of less value.

Effect of inhibited endocytosis in sinusoidal liver cells on catabolism of equine haptoglobin and haemoglobin in the hen.

Catabolism of equine haptoglobin (Hp), haptoglobin-haemoglobin complex (Hp-Hb) and haemoglobin (Hb) in the hen was studied in the liver reticuloendothelial system (RES) in which endocytosis of protein was inhibited by aggregated denatured albumin (Agr-Alb). Intravenous injection of Agr-Alb together with equine [125I]Hp, [125I]Hp-Hb or [125I]Hb partially inhibited elimination of these proteins from hen circulation. The decrease of clearance was less pronounced when the labelled proteins were introduced 30 min after Agr-Alb injection due to stimulation of phagocytosis in RES by Agr-Alb. Elimination of equine proteins (Hp, Hp-Hb and Hb) from hen circulation by RES is only one of the possible metabolic fates of these proteins.

Effect of albendazole on Ancylostoma caninum larvae migrating in the muscles of mice.

When mice inoculated with 1,000 third-stage larvae of Ancylostoma caninum for 1 week were treated intragastrically (ig) with albendazole (Alb) 75, 150 or 300 mg/kg.d for 3 days, the mean larva numbers collected from the muscles of each group were 2.7 +/- 1.7, 2.0 +/- 1.5 and 1.0 +/- 1.0, respectively, being much less than that 205 +/- 68 of the control group. In mice treated ig with Alb 150 mg/kg.d for 3 days, the concentrations of Alb and its effective metabolite, albendazole sulfoxide (AlbSO), were determined in plasma and the muscles at different intervals after the last medication using high performance liquid chromatography. The results showed that only low concentrations of Alb were detected in both plasma and the muscles. However, higher concentrations of AlbSO were found not only in the plasma (5.4-10.5 micrograms/ml), but also in the muscles (2.2-4.6 micrograms/g). The higher contents of AlbSO in the muscles would be helpful for killing the Ancylostoma larvae migrating in the muscles of mice.

[Long-term effects of breathing exercises and yoga in patients with bronchial asthma].

To compare the effects of breathing exercises (BE) or Yoga (Y) on the course of bronchial asthma we studied 36 subjects with a mild disease. The patients were randomly divided into 3 groups. 2 of them participated in a 3 weeks training program of BE or Y while the third group rested without any additional treatment (control group, C). At the end of the training period the patients were asked to practise BE or Y on their own. Drug therapy and lung function parameters before and after a beta 2-agonist metered dose inhaler (albuterol, ALB) were recorded prior to the training program and in 4 weeks intervals for 4 months thereafter. The response to the beta 2-agonist was documented continuously in 28 patients. The mental state of the patients was elucidated by questionnaires.--Prior to the study a significant effect of inhaled ALB on the FEV1 was shown without any significant between group differences. Both, BE and Y, caused a significant amelioration of the mental state but only the BE induced a significant improvement of lung function parameters compared to the individual baseline values. The FEV1 increased significantly by 356.3 +/- 146.2 ml (p < 0.05) and the VC by 225.0 +/- 65.5 ml (p < 0.01). These long-term changes were not significantly different from the actual response to ALB. BE decreased the RV significantly by 306.3 +/- 111.6 ml (p < 0.05), an effect significantly higher compared to the beta 2-agonist (p < 0.01). BE in combination with ALB caused an additive effect.(ABSTRACT TRUNCATED AT 250 WORDS)

Usefulness of fecal alpha-1-antitrypsin for detection of gastrointestinal bleeding in patients with gastric cancer.

Hemoglobin (Hb), albumin (Alb), immunoglobulin G (IgG) and alpha-1-antitrypsin (AAT) were measured in 87 fecal samples from 29 normal volunteers and 68 fecal samples from 33 patients with gastric cancer before surgery to evaluate the usefulness of fecal AAT as an indicator of gastrointestinal bleeding in patients with gastric cancer. Mean values and standard deviation of fecal Hb, Alb, IgG and AAT in normal volunteers were 0.13 +/- 1.19 micrograms/g, 6.96 +/- 20.48 micrograms/g, 17.52 +/- 10.16 micrograms/g, and 0.483 +/- 0.315 mg/g, respectively. These parameters in the patients with gastric cancer were significantly higher than those in the normal volunteers. No statistically significant correlations were observed between these fecal parameters, except between fecal Alb and IgG. Data obtained from peripheral blood also showed no significant correlation with these fecal parameters. When the cut-off levels of fecal Hb, Alb, IgG and AAT were set at M + 2SD of the values obtained from the normal volunteers, sensitivity, specificity and accuracy were 80.0%, 86.2% and 84.1% for fecal AAT, and 66.7%, 96.6% and 86.4% for fecal Hb in the patients with advanced gastric cancer in a cohort consisting of patients with advanced gastric carcinoma and normal volunteers. Since fecal AAT showed higher sensitivity in patients with advanced gastric cancer with a low number of false negative cases, we concluded that measurement of fecal AAT could be a promising method for assessment of gastrointestinal bleeding in patients with advanced gastric carcinoma instead of immunological detection of fecal Hb.

[Screening antihydatid drugs using cultivated germinal cells of Echinococcus granulosus].

Germinal cells isolated from Echinococcus granulosus cysts harbored in mice have been maintained in an in vitro culture system containing RPMI 1640 supplemented by 20% calf serum, and used as a model for screening anti-hydatid drugs. When the germinal cells were maintained in the medium for 6 days, the cell proliferation rate was rather high in the first four days but declined in the last two days. In screening drugs, 1.4 x 10(6) germinal cells were exposed to known effective drugs against metacestodes of E. granulosus in mice, such as mebendazole (Meb), albendazole (Alb) or praziquantel (Pra) at various concentrations. One to three days after exposure, cell counts were made daily in 3 samples of each drug concentration. The mean cell number of each group was compared with that of the control and the inhibition rate of the cell was then calculated. The results showed that the minimal effective concentrations of Meb, Alb and Pra, were 1.0 (48 h), 2.5 (24 h) and 10.0 (72 h) micrograms/ml, respectively, while the inhibition rates of the cell were 34.1, 55.7 and 18.5%. Interestingly, the in vitro effects of Meb, Alb and Pra were consistent to those obtained from the in vivo tests, ie Meb > Alb > Pra. Nevertheless, after exposure of germinal cells to Meb at 2.5 micrograms/ml for 24 h, the cells appeared in roughness, indistinction, shrunk or swelling, collapse, deformation and hole-like feature detected by light microscopy and scanning electron-microscopy, while the ultrastructure alterations of the cells noted by transmission electron-microscopy were lysis in cytoplasm, disruption or disappearance of nucleus and even darkness of the whole cell.(ABSTRACT TRUNCATED AT 250 WORDS)

[Endoscopic and clinical parameters in assessing th degree of portal hypertension: the value of the serum-ascitic fluid albumin gradient].

A prospective study in thirty one patients with ascites, who were hospitalized at Cayetano Heredia National Hospital (H.N.C.H.), to investigate the association between the high serum-to-ascites albumin concentration gradient (high GRAD-Alb) with the degree and development of oesophageal varices, studied by endoscopy, is here reported. It was also studied its relationship with the degree of impairment of liver function, determined by the Child-Pugh's score. In addition our series found that the degree of high GRAD-Alb could discriminate patients with oesophageal varices finding like a signal of its presence a value of high GRAD-Alb greater than 1.435 +/- 0.015 g/dl. It is here demonstrated that the degree of high GRAD-Alb does not have any relationship with the degree of impairment of liver function (Child-Pugh's score), prothrombine time, serum bilirrubin, degree of encephalopathy neither grade of ascites; however, we found a light association and correlation with serum albumin.

[Plasma fibronectin in pediatric patients].

OBJECTIVE: To determine the levels of plasma fibronectin (FBN) in pediatric patients and to correlate them with serum albumin (ALB). METHODOLOGY: The FBN was measured by nephelometry and ALB by enzymatic analysis in samples from 95 healthy Mexican pediatric patients seen in the ambulatory surgery department. RESULTS: The levels of FBN was similar in boy and girls (mean +/- SD) 273 +/- 90 and 268 +/- 106 micrograms/mL respectively. FBN was significantly lower in children less than 6 months old. We found seven patients with ALB < 3.0 g/dL who had significantly lower values of FBN than the normoalbuminemic patients. The correlation of FBN and ALB was low (r = 0.23). CONCLUSIONS: FBN in pediatric patients is significatively lower than in adults. The lowest concentrations were seen in children less than 6 months old. These lower FBN may be an additional factor for immunodeficiency in this group at risk.

Immunostimulation and growth faltering in UK infants.

The purpose of the study was to determine whether chronic immunostimulation could explain growth faltering in disadvantaged children in the UK, as it does in developing countries such as The Gambia. In all, 216 infants, age 10-21 months, were recruited when blood samples were taken for the routine or clinical purposes of a longitudinal study tracking a larger cohort of children. Aliquots of blood were collected on Guthrie cards to determine blood concentrations of albumin (Alb), alpha(1)-antichymotrypsin (ACT), and immunoglobulin G (IgG). Haemoglobin concentrations were determined by routine hospital laboratory analysis. Heights and weights were measured and converted to z-scores; birth weights were used with recruitment weight to calculate a 'thrive index' for each child. Age-corrected plasma IgG concentration was negatively associated with both height- and weight-for-age z-scores (P = 0.042 and 0.038, respectively) but not with the thrive index or body mass index z-scores. Blood haemoglobin levels were positively related to height- and weight-for age z-scores, as well as to the thrive index (P = 0.026, 0.014, and 0.007, respectively). Although significant, these relationships could only account for a small part the observed growth variation. Although the relationships were weak, the results suggest that some of the observed variation in growth of these UK infants may be explained on the basis of persistent immunostimulation or poor iron status. In terms of markers of immunostimulation (Alb, ACT, ACT:Alb ratio, IgG), both absolute levels and relationships with height-for-age are substantially different than those previously observed in cohort studies of infants in The Gambia.

Spirometric function in non-smoking workers exposed to aluminum.

BACKGROUND: Exposure to aluminum (Al) causes the onset of respiratory disorders. This study is aimed at providing further information on how occupational exposure to Al affects the respiratory apparatus in healthy non-smokers, with particular attention to respiratory function. METHODS: A group of 50 male shipyard workers who were exposed to Al underwent medical examination, standard chest X-rays and spirometry in accordance with the C.E.C.A. protocol. The data were compared with those of a homogeneous group of controls, all with blood aluminum (AlB) levels below 7.5 ng/ml. Statistical analysis was performed on the following spirometric parameters: vital capacity (VC), forced vital capacity (FVC), maximum forced expiratory volume in 1 s (FEV1), and mean forced expiratory flow during mid-half of FVC (FEF(25-75%)). Environmental Al levels were also measured at the various workstations. RESULTS: Fifty male workers with an average age 31.82 +/- 5.05 years, occupational exposure of 11.81 +/- 3.71 years, presented with average AlB levels of 32.64 +/- 8.69 ng/ml. Environmental monitoring displayed Al levels higher than TLV TWA for all the workstations studied. None of the sample displayed significant pathological conditions. Statistical comparison of the spirometric parameters showed a decrease in the examined values in exposed workers. This decrease was found to be directly proportional to the AlB level. CONCLUSION: The authors conclude that Al affects respiratory function and that limit values should be reassessed.
