Effect of Glycyrrhiza glabra roots and glycyrrhizin on the glucuronidation in rats.

As an approach to elucidate the possible in vivo interaction of synthetic drugs and herbs which are frequently used in combination in Asia, the effect of Glycyrrhiza glabra on the metabolism of acetaminophen (AAP) was examined in male Sprague-Dawley rats. The pretreatment of the methanol extract of Glycyrrhiza glabra roots (Glycyrrhizae Radix, GR, 1 g/kg, p.o.) for 6 days significantly increased the cumulative biliary (156%) and urinary (132%) excretions of AAP-glucuronide conjugate within 120 min after the administration of AAP (150 mg/kg, i.v.) without affecting thioether and sulfate conjugates. These findings suggest that GR might enhance the glucuronidation pathway of AAP. In order to study the effect of GR on the glucuronidation in rat liver, we examined enzymatic activity of p-nitrophenol UDP-glucuronosyltransferase (UGT), which is also called UGT1A, and intracellular concentrations of hepatic UDP-glucuronic acid, upon the administration of GR (1 g/kg, p.o.) or glycyrrhizin (23 mg/kg, p.o.), a major component of GR, for 6 days. GR and glycyrrhizin caused increases in specific activities of UGT1A by 111% and 96%, respectively. Concentration of UDP-glucuronic acid was increased 257% by GR and 484% by glycyrrhizin. These data indicate that GR and glycyrrhizin activated glucuronidation and thus suggest the possibility that GR may influence detoxification of xenobiotics in rat liver. Using the p-nitrophenol UGT1A1 cDNA as a probe, we found that the activation of UGT1A by GR was not due to the induction of mRNAs for the enzyme.

Specificity of aerobic and anaerobic work capacities and powers.

Thirty-three untrained subjects of both sexes, 18-31 years of age, performed several tests on cycle ergometers. Maximal aerobic power (MAP) was obtained in a progressive work test. Maximal aerobic capacity (MAC) was measured in a 90-min maximal test and was computed as the total work output during that period. Two all-out cycle ergometer work tests lasting 10 s and 90 s were used to estimate the anaerobic alactic capacity (AAC) and lactic capacity (ALC). Anaerobic alactic power (AAP) was computed as the highest output in 1 s in the AAC test and anaerobic lactic power (ALP) was obtained as the mean output during the last 5 s in an all-out test of 30 s. Correlation coefficients were computed between all measurements of capacity and power expressed per kg of body weight as well as with scores adjusted for sex differences. Common variances (r2 X 100) between measurements of power were either low (MAP-AAP, 40%) or moderate (MAP-ALP, 61%; AAP-ALP, 62%) while common variances between measurements of capacity were sometimes low (MAC-AAC, 49%) or higher (MAC-ALC, 76%; AAC-ALC, 77%). The common variances between tests of power and capacity reached high values when calculated with metabolic criteria of the same class (MAP-MAC, 81%; AAP-AAC, 92%). These results provide quantitative evidence to support the notion of specificity between the aerobic and the anaerobic work performances and support the distinction between capacity and power of the three energy systems.

A nascent polypeptide domain that can regulate translation elongation.

The evolutionarily conserved fungal arginine attenuator peptide (AAP), as a nascent peptide, stalls the translating ribosome in response to the presence of a high concentration of the amino acid arginine. Here we examine whether the AAP maintains regulatory function in fungal, plant, and animal cell-free translation systems when placed as a domain near the N terminus or internally within a large polypeptide. Pulse-chase analyses of the radiolabeled polypeptides synthesized in these systems indicated that wild-type AAP functions at either position to stall polypeptide synthesis in response to arginine. Toeprint analyses performed to map the positions of stalled ribosomes on transcripts introduced into the fungal system revealed that ribosome stalling required translation of the AAP coding sequence. The positions of the stalled ribosomes were consistent with the sizes of the radiolabeled polypeptide intermediates. These findings demonstrate that an internal polypeptide domain in a nascent chain can regulate eukaryotic translational elongation in response to a small molecule. Apparently the peptide-sensing features are conserved in fungal, plant, and animal ribosomes. These data provide precedents for translational strategies that would allow domains within nascent polypeptide chains to modulate gene expression.

A comparison of methods for the estimation of microbial nitrogen in duodenal digesta of sheep.

1. Six sheep, each fitted with a rumen cannula and re-entrant cannulas in the proximal duodenum and distal ileum, were given two diets (600 g dry matter (DM)/d) consisting of either grass silage (32.1 g nitrogen/kg DM) or dried grass (18.3 g N/kg DM). A net loss of N occurred between mouth and duodenum with the silage diet, indicating extensive ruminal degradation of dietary N, compared with a net gain on the dried-grass diet. Consequently, despite higher N intakes when silage was given, N flow at the duodenum was similar for both diets. 2. The proportion of microbial N in duodenal digesta N was estimated using diaminopimelic acid (DAPA), [35S]methionine (35S), 15N-enriched non-ammonia-N (15NAN) and amino acid profiles (AAP) as microbial markers. Isotopic labelling of rumen micro-organisms was achieved by intraruminal infusions of Na2 35SO4 and (15NH4)2SO4. 3. A comparison of all methods was made based on the marker concentrations in microbial fractions isolated by differential centrifuagation of strained rumen contents. With both diets, DAPA gave the highest estimates and AAP the lowest. Estimates based on 35S and 15NAN were intermediate and did not differ significantly (P greater than 0.05). 4. For the 15NAN, 35S and AAP methods, the effect of site of sampling of the microbial fraction, i.e. from rumen contents or duodenal digesta, was examined and in all instances mean estimates based on duodenally-derived microbes were higher. However, the differences were significant for only 15NAN with both diets (P less than 0.001), for 35S with the dried grass (P less than 0.05), and for AAP with the silage (P less than 0.05). Estimates based on duodenally-derived microbes were higher (P less than 0.05) using 15 NAN than those obtained using 35S with both diets. 5. Depending on the method used for estimating microbial N, estimates of the efficiency of microbial N synthesis in the rumen (g microbial N flow at duodenum/kg organic matter apparently digested in the rumen) ranged between 16 and 38 for the silage diet and 10 and 46 for the dried grass diet. Similarly, estimates of feed N degradability in the rumen ranged between 0.62 and 0.97 for the silage and 0.00 and 0.93 for the dried grass.

Specialized membrane-localized chaperones prevent aggregation of polytopic proteins in the ER.

The integral endoplasmic reticulum (ER) membrane protein Shr3p is required for proper plasma membrane localization of amino acid permeases (AAPs) in yeast. In the absence of Shr3p AAPs are uniquely retained in the ER with each of their twelve membrane-spanning segments correctly inserted in the membrane. Here, we show that the membrane domain of Shr3p specifically prevents AAPs from aggregating, and thus, plays a critical role in assisting AAPs to fold and correctly attain tertiary structures required for ER exit. Also, we show that the integral ER proteins, Gsf2p, Pho86p, and Chs7p, function similarly to Shr3p. In cells individually lacking one of these components only their cognate substrates, hexose transporters, phosphate transporters, and chitin synthase-III, respectively, aggregate and consequently fail to exit the ER membrane. These findings indicate that polytopic membrane proteins depend on specialized membrane-localized chaperones to prevent inappropriate interactions between membrane-spanning segments as they insert and fold in the lipid bilayer of the ER membrane.

Formation and removal of DNA adducts in target and nontarget tissues of rats administered multiple doses of 2-acetylaminophenanthrene.

2-Acetylaminophenanthrene (2-AAP) is carcinogenic for the mammary gland, small intestine and ear duct in rats, but is not hepatocarcinogenic. In order to understand the tissue specificity of tumor induction, the formation and removal of DNA adducts in target and nontarget tissues have been compared in rats administered 2-AAP. Male and female Sprague--Dawley rats were treated i.p. with up to four weekly doses of 5 mg 2-AAP per kg body weight. 32P-Post-labeling analysis of the DNA from all tissues showed two predominant adducts (greater than 85-95% of the total binding) and at least four minor adducts. By comparing the results obtained from reacting N-hydroxy-2-aminophenanthrene with DNA at pH 5, the major adducts were identified as N-(deoxyadenosine-8-yl)-2-aminophenanthrene and 1-(deoxyguanosin-N2-yl)-2-aminophenanthrene. Two minor adducts, N-(deoxyadenosine-8-yl)-2-aminophenanthrene and 1-(deoxyadenosin-N6-yl)-2-aminophenanthrene, were also formed in the in vitro reactions. The distribution of adducts, extent of binding and adduct persistence were similar between target and nontarget tissues, which indicates that the tissue specificity of 2-AAP for tumor induction is due to factors in addition to adduct formation.

Reciprocal modifying effects of isomeric forms of aminophenol on induction of neoplastic lesions in rat liver and kidney initiated by N-ethyl-N-hydroxyethylnitrosamine.

The effects of four isomeric forms of aminophenols, ortho-, meta-, para-aminophenol and acetaminophen (o-, m-, p-AP and AAP, respectively) on liver and kidney carcinogenesis initiated by N-ethyl-N-hydroxyethylnitrosamine (EHEN) were tested. Rats were given 0.1% EHEN (in their drinking water for 2 weeks) and then diet containing these compounds at concentrations of 0.8% for 49 weeks. Administration of o-AP and AAP significantly decreased the number and area of preneoplastic foci staining immunohistochemically for glutathione S-transferase placental type (GST-P) per unit area of liver section as compared with the values for rats given EHEN alone. o-AP and AAP also significantly decreased the incidence of hepatocellular carcinoma. In contrast, p-AP and AAP significantly increased quantitative values for kidney preneoplastic lesions and renal cell adenoma. It is suggested that the cytotoxic effects of these chemicals preferentially suppressed the proliferation of preneoplastic liver cells, but stimulated the mitotic activity of preneoplastic tubular lesions in the kidney.

Effect of modifying agents on the phenotypic expression of cytochrome P-450, glutathione S-transferase molecular forms, microsomal epoxide hydrolase, glucose-6-phosphate dehydrogenase and gamma-glutamyltranspeptidase in rat liver preneoplastic lesions.

The expression of A and P forms of glutathione S-transferase (GST-A and P), two cytochrome P-450 isoenzymes (P-450 PB3a and P-450 MC2), microsomal epoxide hydrolase (mEHb), glucose-6-phosphate dehydrogenase (G6PD) and gamma-glutamyltranspeptidase (gamma-GT) was compared in preneoplastic liver lesions and background parenchyma of F344 rats post-treated with butylated hydroxyanisole (BHA), ethoxyquin (EQ) or acetaminophen (AAP). These latter three compounds have been shown to inhibit hepatocarcinogenesis after initial treatment with N-ethyl-N-hydroxyethylnitrosamine (EHEN) and a significant decrease in the number of enzyme-altered foci and nodules positive for GST-P, GST-A, G6PD and gamma-GT and negative for P-450 PB3a, P-450 MC2 was associated with their administration. Whereas in the foci case the decrease was most prominent for non-discrete (heterogeneous) type lesions, the results of quantitation of nodules revealed a most significant alteration in the discrete homogeneously staining population. This indicates that BHA, EQ and AAP have the potential to inhibit the growth of the phenotypically stable lesions thought most likely to be the immediate precursors of hepatocellular carcinomas. The two anti-oxidants were associated with periportal increase of all enzymes investigated, whereas AAP induced GST species and mEHb in the perivenular zone. Irrespective of slightly elevated enzyme levels in surrounding parenchyma, mEHb antibody binding levels within lesions showed a reciprocal shift from positive to negative in rats treated with BHA, EQ and AAP.

Commensal isolates of methicillin-resistant Staphylococcus epidermidis are also well equipped to produce biofilm on polystyrene surfaces.

OBJECTIVES: To study biofilm production and to detect icaAD, atlE and aap genes in 137 isolates of methicillin-resistant Staphylococcus epidermidis (MRSE) obtained from healthy individuals from the community (35 isolates), from hospitalized patients at the Antônio Pedro University Hospital (25 isolates) and from individuals from a home-care system (HCS; 77 isolates). METHODS: Biofilm production was determined in vitro using polystyrene inert surfaces. icaAD, atlE and aap genes were detected using PCR. Hybridization experiments were also carried out to confirm some PCR results. Antimicrobial susceptibility testing was carried out using the NCCLS methods. RESULTS: Although many of the commensal MRSE isolates produced biofilms, the percentage of biofilm producers was significantly higher (P = 0.0107) among hospital isolates (76%) than among isolates from the community (60%) and from the HCS (57%). An association was observed between multiresistance and biofilm production for isolates obtained from healthy individuals from the community and from household contacts from the HCS (P < 0.0001). The concomitant presence of the ica operon and atlE and aap genes was associated with the strong biofilm-producer phenotype (P < 0.0001). CONCLUSION: Because many of the commensal MRSE isolates obtained from nares produced biofilms and carried icaAD, aap and atlE genes, biofilms or such genetic elements should not be used as markers for clinical significance. The biofilm environment seems to increase genetic exchanges and hence may contribute to multiresistance phenotypes.

The unusual occurrence of 4-aminoantipyrine (4-aminophenazone) in human biological fluids.

During toxicology screening of human biological fluids, 4-aminoantipyrine (4-AAP) was detected in 19 patients and identified by two-dimensional TLC and UV spectrophotometry with whole blood concentrations of 4-AAP ranging from 0.4 to greater than 4.9 mg/dL. Of the 19 patients, 15 had received prior treatment at a Mexican health-care facility before transfer to this medical center. Since 4-AAP is a known metabolite of aminopyrine, it is likely that the patients had ingested this anti-inflammatory, analgesic agent in Mexico; however, the clinical histories in this respect were vague. Recognition of the presence of 4-AAP in biological fluids is important, since the parent drug may produce fatal agranulocytosis and the compound may complicate the detection of other compounds that simultaneously partition into the weak-base fraction in toxicology screening.

Biological activity of p-methylaminophenol, an essential structural component of N-(4-hydroxyphenyl)retinamide, fenretinide.

Fenretinide, N-(4-hydroxyphenyl)retinamide (4-HPR), is a synthetic amide of all-trans-retinoic acid (RA), which inhibits cell growth, induces apoptosis, and is an antioxidant, and cancer chemopreventive and antiproliferative agent. These findings led us to investigate which structural component of 4-HPR contributes to these potent activities. Our approach was to examine 4-aminophenol (4-AP), p-methylaminophenol (p-MAP), and p-acetaminophen (p-AAP). It was found that vitamin E, 4-AP and p-MAP scavenge alpha,alpha-diphenyl-beta-picrylhydrazyl (DPPH) radicals in a 1:2 ratio, in contrast to 4-HPR and p-AAP, for which 1:1 and 1:0.5 ratios were observed relative to DPPH radicals. However, RA was inactive. Lipid peroxidation in rat liver microsomes was reduced by compounds (RA > p-MAP = 4-HPR > 4-AP) in a dose-dependent manner, while p-AAP was inactive. In addition, both p-MAP and 4-HPR are potent inhibitors of cell growth and inducers of apoptosis in HL60 cells. p-MAP exhibits the same level of antiproliferative activity as 4-HPR against HL60R cells, which are a resistant clone against RA, and it inhibits the growth of various cancer cell lines (MCF-7, MCF-7/Adr(R), HepG2, and DU-145) to an extent greater than 4-AP and p-AAP, but is less potent than 4-HPR. Thus, although the antioxidant activity of p-MAP is more potent than that of 4-HPR, p-MAP is less potent than 4-HPR in anticancer activity. These results suggest that both the anticancer and antioxidative activities shown by 4-HPR are due to the structure of p-MAP. The retinoyl residue or long alkyl chain substituent attached to an aminophenol may be significant for anticancer properties.

Role of lipid peroxidation as a mechanism of liver injury after acetaminophen overdose in mice.

Mitochondrial oxidant stress and peroxynitrite formation have been implicated in the pathophysiology of acetaminophen-induced (AAP-induced) liver injury. Therefore, we tested the hypothesis that lipid peroxidation (LPO) might be involved in the injury mechanism. Male C3Heb/FeJ mice fed a diet high in vitamin E (1 g d-alpha-tocopheryl acetate/kg diet) for 1 week had 6.7-fold higher hepatic tocopherol levels than animals on the control diet (8.2 +/- 0.1 nmol/g liver). Treatment of fasted mice with 300 mg/kg AAP caused centrilobular necrosis with high plasma alanine aminotransferase (ALT) activities at 6 h (3280 +/- 570 U/l) but no evidence of LPO (hepatic malondialdehyde, 4-hydroxynonenal). Animals on the vitamin E diet had similar injury and LPO as mice on the control diet. To verify a potential effect of the vitamin E diet on drug-induced liver injury, animals were pretreated with a combination of phorone, FeSO4, and allyl alcohol. We observed, 2 h after allyl alcohol, massive LPO and liver cell injury in the livers of animals on the control diet, as indicated by a 32-fold increase in malondialdehyde levels, extensive staining for 4-hydroxynonenal, and ALT activities of 2310 +/- 340 U/l. Animals on the vitamin E diet had 40% lower hepatic malondialdehyde levels and 85% lower ALT values. Similar results were obtained when animals were treated for 3 days with alpha- or gamma-tocopherol (0.19 mmol/kg, ip). Both treatments reduced LPO and injury after allyl alcohol but had no effect on AAP hepatotoxicity. Thus, despite the previously shown mitochondrial oxidant stress and peroxynitrite formation, LPO does not appear to be a critical event in AAP-induced hepatotoxicity.

Alanine aminopeptidase activity in urine: a new marker of chronic alcohol abuse?

BACKGROUND: The purpose of this study was to evaluate the activity of urinary alanine aminopeptidase (AAP), the enzyme released from the brush border membranes of renal proximal tubules, as a new biological marker of chronic alcohol abuse. METHODS: The AAP activity was assayed and compared between a group of 76 alcoholics undergoing detoxification and a group of 37 alcoholics abstaining from alcohol for at least 6 weeks. In all patients, the enzyme activity was measured both in untreated urine (uAAP) and after removal of endogenous AAP inhibitors by molecular filtration on Sephadex (eAAP). RESULTS: There was a correlation between the uAAP and eAAP activities in both groups of patients (r = 0.61 and r = 0.81 in abstinent alcoholics and in alcoholics undergoing detoxification, respectively), and both the uAAP activity and the eAAP activity were significantly and markedly higher in alcoholics being detoxified than in their abstinent counterparts. As revealed by receiver operating characteristic analysis, the discriminative power of the eAAP activity assay was higher than that of the uAAP activity assay. The area under the corresponding receiver operating characteristic curves was 0.84 +/- 0.04 and 0.78 +/- 0.05 (mean +/- SE), respectively. CONCLUSIONS: The results of this study demonstrate that the assays of urinary AAP activity, which relate to the nephrotoxic effects of alcohol abuse, could be a valuable complement to the other presently used markers of chronic alcohol abuse that are generally based on ethanol hepatotoxicity. Compared with the uAAP activity test, the eAAP activity test is of clear diagnostic advantage.

Guidelines for palivizumab prophylaxis: are they based on infant's risk of hospitalization for respiratory syncytial viral disease?

BACKGROUND: The American Academy of Pediatrics (AAP) guidelines for respiratory syncytial virus (RSV) prophylaxis aim to prioritize palivizumab administration to infants at highest risk for RSV disease. Multiple studies have been published that assess the risk of hospitalization for RSV disease by gestational age (GA) at birth and severity of lung disease. OBJECTIVE: To evaluate whether the AAP guidelines for RSV prophylaxis correlate with the available data in the literature on the degree of risk of hospitalization for RSV disease by GA at birth and severity of lung disease. METHODS: We considered a hypothetical population of infants with and without chronic lung disease. This population was then divided into hypothetical cohorts depending on GA at birth and month of neonatal intensive care unit discharge. We assumed that infants are discharged from neonatal intensive care unit at 36 to 37 weeks postconceptional age. By applying the AAP policy for RSV prophylaxis, the numbers of palivizumab injections were determined for the different cohorts. RESULTS: In some instances infants who are currently known to be at higher risk of hospitalization for RSV disease receive fewer palivizumab injections than infants known to be at lower risk. CONCLUSION: Some discrepancies exist between the RSV prophylaxis guidelines and the published data on the level of risk of hospitalization for RSV disease by GA and lung disease. AAP policy for RSV prophylaxis must be amended to better correlate the amount of palivizumab prophylaxis with the level of risk of hospitalization for RSV disease as determined by the above factors.

Pediatric first aid knowledge among parents.

OBJECTIVE: The American Academy of Pediatrics (AAP) has published simple guidelines for the first aid management of ill or injured children. We determined knowledge of these first aid practices in parents. METHODS: Design-Descriptive survey. PARTICIPANTS: A convenience sample of adult parents presenting to the emergency department. SURVEY INSTRUMENT: Standardized collection of demographics was performed and adults were given a multiple-choice questionnaire concerning the proper management of stings and bites, burns and scalds, eye injuries, fractures and sprains, fever, head injuries, fainting, poisoning, dental emergencies, nosebleeds, seizures, and skin wounds. OUTCOMES: Percentage of adults responding to questions appropriately based on AAP guidelines. DATA ANALYSIS: Descriptive statistics and chi2 tests. RESULTS: Six hundred fifty-four adults were surveyed. Mean age (SD) was 38.5 (13.8), 56% were female, 69% were white, 56% had at least a high school education. None of those surveyed answered all questions correctly with roughly half being familiar with 60% of the questions. Knowledge of specific guidelines ranged from 21% to 92%. Subjects especially lacked knowledge regarding the need to rapidly remove all bee stingers (only 36% aware); keep wounds moist and covered (79% felt that drying wounds was beneficial); the need to cover victims of large burns (only 43% aware); and the need to seek medical attention after tick bites (only 47% aware). Knowledge was unaffected by age, gender, and education. CONCLUSIONS: Many adults are unfamiliar with AAP first aid measures according to the AAP guidelines and most believe that wounds should be allowed to dry. Further education is required to improve knowledge of first aid practices.

Age does not alter acetaminophen absorption.

Twenty-eight healthy volunteers (age range, 22-78 years) received 650 mg of acetaminophen (AAP) on three separate occasions. The modes of administration were 1) intravenous, 5-minute infusion; 2) oral, with two 325-mg tablets; and 3) oral, with 650 mg as an elixir preparation. Plasma levels of AAP were determined in blood samples drawn up to 12 hours after the dose. The mean (+/- sd) kinetic variables for absorption of AAP from tablets in young and elderly were peak plasma concentration, 11.8 (+/- 4.2) vs 10.9 (+/- 4.1) micrograms/ml; peak time, 0.79 (+/- .54) vs 0.69 (+/- .40) hours after the dose; absorption half-life, 12.6 (+/- 9.8) vs. 8.2 (+/- 5.3) minutes; and absolute systemic availability, 79 (+/- 9) vs 72 (+/- 11) per cent. For AAP elixir, the corresponding values were 12.6 (+/- 5.4) vs 13.7 (+/- 6.0) micrograms/ml; 0.52 (+/- .24) vs 0.54 (+/- .51) hours; 8.6 (+/- 6.2) vs 6.1 (+/- 6.6) minutes; and 87 (+/- 9) vs 80 (+/- 9) per cent. Absolute bioavailability of both oral dosage forms was significantly less then 100 per cent in all groups. Elderly subjects tended to show lower availability of both oral preparations, but the difference was of borderline significance (P less than .50). Age did not influence any other measures of absorption. Since the absorption rate of acetaminophen may be indicative of the gastric emptying rate, age does not appear to alter this rate-limiting step in drug absorption.

SIDS risk factor awareness: assessment among nursing students.

OBJECTIVE: To test the hypotheses that nursing students (a) have limited knowledge of risk factors for sudden infant death syndrome (SIDS) and the American Academy of Pediatrics (AAP) recommendation of placing infants on their backs or sides for sleep, and (b) after careful education about SIDS, would retain this information and teach it to parents. DESIGN: A pretest questionnaire was used to identify knowledge of 13 risk factors and personal recommendation for sleep position. Participants attended a lecture on SIDS and received written educational material. A posttest was completed after 6 weeks. SETTING: Nursing students were assessed during their family practice course. PARTICIPANTS: Fifty-one 4th-year nursing students (mean age 28.4 years, SD=7.6) enrolled in the BSN program completed the pretest; 29 completed the identical posttest. INTERVENTION: A lecture on SIDS and SIDS risk factors with written educational material after the pretest. MAIN OUTCOME MEASURES: Pretest and posttest responses. RESULTS: The Mann-Whitney test and McNemar chi-square analyses were used to determine factors on the pretest which influenced the awareness of prone position and to compare pretest and posttest data. On the pretest, 69% of students were aware of the AAP recommendation but significantly more (92%) were aware on the posttest. Posttest responses increased significantly for 12 of the 13 risk factors; 41% of the students identified all 13 risk factors on the posttest, in contrast to 0% on the pretest. On both the pretest and posttest, 93% of students stated that it was important to discuss their infant's sleep position with parents. Posttest results showed that students unanimously recommend the side or back sleep position. CONCLUSION: Nursing students were aware of the AAP recommendation, yet their knowledge of other risk factors was limited. A careful educational process resulted in retention of information about the AAP recommendation and other risk factors. In addition, students were willing to teach parents to place their infants on the back or side to sleep.

Evaluation and comparison of colorimetric, radiometric and high performance liquid chromatographic assays for aminopyrine-N-demethylation by rat liver microsomes.

Aminopyrine (DMAP) is metabolized by two successive N-demethylations to monomethyl-4-aminoantipyrine (MMAP) and 4-aminoantipyrine (AAP). Separation and quantification of DMAP and its metabolites in microsomal incubation mixtures by h.p.l.c. showed that other reactions also occur. In the radiometric method where [Me2-14C]DMAP is used as substrate, [14C]formaldehyde is formed during the N-demethylation. However since commercial [14C]DMAP, is not completely double labelled, and both MMAP and AAP are formed, it is impossible to calculate the formaldehyde formation accurately from the specific activity of [14C]DMAP. Moreover, it was shown that DMAP, AAP and particularly MMAP may all develop considerable colour intensity with the Nash reagent, which is used to determine formaldehyde. Despite this difficulty, DMAP may still be used as a model substrate in vitro, with the Nash assay being used to determine formaldehyde if low substrate concentrations and a short incubation time are used. Thus the interference of DMAP or its metabolites with the Nash assay is negligible. As the same Km and Vmax values were obtained from both the radiometric and the relatively precise colorimetric assay it is suggested that there is little wrong with either method, at least under our experimental conditions. The h.p.l.c. method however, underestimates formaldehyde formation, probably because metabolites other than MMAP and AAP are formed. The latter method however may be used to analyse the aminopyrine metabolism in more detail.
