[A new sieve-test for testing colour vision (author's transl)].

From our results we can be sure that the colour test disk is at least as reliable for mass testing for colour blindness as the Ishihara isochromatic colour plates. In practice the test disk has the advantage of constancy of testing conditions and easy and quick procedure. If it is necessary to differentiate between protopia and deuteropia the colour test disk is significantly better Ishihara's colour plates.

Performance of red-green color deficient subjects on the Holmes-Wright lantern (Type A) in photopic viewing.

BACKGROUND: The Holmes-Wright lantern (Type A) is an approved occupational color vision test for airline pilots in the European Economic Community and for specific occupations in the British Armed Forces. The colors shown are red, green and white signal lights. HYPOTHESIS: The Holmes-Wright lantern is a sensitive screening test for red-green color deficiency in photopic viewing and the pass/fail level is similar to that of the Farnsworth Lantern (Falant) if the same scoring method is applied. METHOD: There were 138 color deficient subjects identified with the Ishihara plates and diagnosed with the Nagel anomaloscope, completed a color vision test battery which included three runs of the nine color pairs of the Holmes-Wright lantern at high brightness in normal room illumination. RESULTS: Screening sensitivity on a single error was found to be 97% compared with the Ishihara plates. Using the Falant scoring method, 20 subjects passed. These were 1 deuteranope, 2 protanomalous trichromats and 17 deuteranomalous trichromats (22% of 88 anomalous trichromats). The mean error score was greater for protans than for deutans but the mean number of qualitative error categories was smaller. Green/white confusions were the most frequent errors. It was not possible to predict who would pass the lantern test from other test results but all subjects with a Nagel anomaloscope matching range > 15 scale units who failed the Farnsworth D15 test or were grading as moderate/severe with the American Optical Company (Hardy, Rand and Rittler) plates failed. CONCLUSIONS: The Holmes-Wright lantern is a sensitive screening test for red-green color deficiency. Although a similar percentage of anomalous trichromats fail the Holmes-Wright lantern as fail the Falant, if the same scoring method is used, the superior correlation between the Holmes-Wright result and other color vision tests designed to grade the severity of color deficiency suggests that the two lantern results are not equivalent.

Improvement in colour vision parameters following successful trabeculectomy.

PURPOSE: To determine whether colour vision improves following reduction of intraocular pressure (IOP) in glaucoma patients. METHODS: The medical records of 29 glaucoma patients (41 eyes) were reviewed. Inclusion criteria required subjects to have made more than four visits to the Glaucoma Service Laboratory and to undergo a thorough eye examination including a Farnsworth-Munsell 100-hue colour vision test and Goldmann tonometry before and after pressure lowering. Colour vision parameters of total error score (TES), yellow-blue score (YBS) and red-green score (RGS) were measured. The study group consisted of 21 eyes of glaucoma patients who underwent uncomplicated trabeculectomy with an IOP reduction of >/= 20% from baseline. The control group consisted of 21 eyes of glaucoma patients matched for age and colour vision, who received medication and/or underwent surgery with a post-intervention IOP reduction of < 20% from baseline. The primary outcome was a comparison of pre- and post-intervention colour vision parameters between the two groups. RESULTS: There was a statistically significant improvement in TES (43 +/- 44, p < 0.001), RGS (19 +/- 27, p = 0.0077) and YBS (23 +/- 29, p = 0.0007) in the study group compared with the control group. The improvement in TES (r = 0.52, p < 0.001), RGS (r = 0.55, p < 0.001) and YBS (r = 0.40, p = 0.008) was correlated with the percentage of IOP reduction. There was no statistically significant difference between improvement in Y-B and R-G scores in the study group. CONCLUSION: Intraocular pressure reduction of >/= 20% post-trabeculectomy was associated with an improvement in colour vision. Colour vision tests may be useful as an adjunctive outcome measure for therapeutic interventions.

Colour vision defects in asymptomatic carriers of the Leber's hereditary optic neuropathy (LHON) mtDNA 11778 mutation from a large Brazilian LHON pedigree: a case-control study.

AIMS: To determine if asymptomatic carriers from a previously identified large pedigree of the Leber's hereditary optic neuropathy (LHON) 11778 mtDNA mutation have colour vision deficits. METHODS: As part of a comprehensive analysis of over 200 members of a large Brazilian LHON pedigree spanning seven generations, colour vision tests were obtained from 91 members. Colour vision was tested one eye at a time using the Farnsworth-Munsell 100 (FM-100) hue colour vision test. The test was administered under uniform conditions, taking into account: ambient light levels, daylight colour temperature of 6700 kelvin, and neutral uniform background. Tests were scored using the FM-100 MS-Excel computer scoring program. Defects were determined and categorised as tritan, deutan, or protan. Categorisation of each dyschromatopsia was based on review of demonstrated axis computer generated plots and age adjusted error scores which coincided with Verriest 95% confidence intervals. Only the axis with the greatest magnitude error score was used to classify the defect. 55 of the 91 test subjects were LHON mtDNA 11778 J haplotype mutation carriers, proved by mtDNA analysis. The remaining 36 subjects were age matched non-blood relatives (off pedigree), who served as controls. RESULTS: 27 of 55 carriers (49.10%) were shown to have colour vision defects in one or both eyes. 13 of the 27 (48%) abnormal tests in the carrier group were tritan defects and the remaining 14 (52%) were deutan defects. Nine of the 27 (33%) abnormals in the carrier group were identified as having bilateral defects. Six of these were deutan, and the remaining three were tritan dyschromatopsias. Only six of the 36 (16.66%) age matched controls were found to have any type of dyschromatopsia. Five (83.3%) of these were deutan defects. The remaining one was a tritan defect. The difference between the two groups using a chi(2) test with one degree of freedom was statistically significant with a p value less that 0.001. CONCLUSIONS: Until now, LHON has always been characterised by a sudden, devastating vision loss. Asymptomatic carriers, those without vision loss, were considered unaffected by the disease. It now appears that asymptomatic carriers of the LHON mutation are affected by colour vision defects and may manifest other subtle, yet chronic, changes.

The effect of anthocyanosides in a multiple oral dose on night vision.

PURPOSE: In view of research demonstrating the ability of anthocyanosides in a multiple oral dose to improve night vision in normal individuals, we assessed their effect on three night vision tests: full-field absolute scotopic retinal threshold (SRT), dark adaptation rate (DAR) and mesopic contrast sensitivity (MCS). METHODS: In a double-masked, placebo-controlled, cross-over study, 18 young normal volunteers were randomly assigned to one of three different regimens of multiple oral administrations of 12 and 24 mg anthocyanosides, and a placebo, given twice daily for 4 days. A 2 week washout period was allowed between each 4 day treatment period. SRT, DAR and MCS was tested 1 day before and at days 1, 2, 3 and 4 during the treatment period. RESULTS: No significant effect was found on any of the three above-mentioned night vision tests. The study had a power of 0.95 to detect a 0.1 log unit improvement in SRT and 0.5 log unit improvement in MCS. CONCLUSIONS: Multiple oral administrations of 12 and 24 mg anthocyanosides twice a day appear to lack significant effect on night vision tests.

Appraisal of psychomotor skills of dental students at University Complutense of Madrid.

The aim of this study was to evaluate the psychomotor skills of the dental students at University Complutense of Madrid (UCM), using a battery of psychometric tests. The sample comprised 306 students of the 1st, 2nd, 3rd and 4th years of Odontology, who, during the last month of the academic term, took the test on rotation of solid shapes, the Embedded Figures test in its group form (GEFT), the O'Connor Tweezer test, the MacQuarrie test for mechanical ability and the indirect vision test. Before these tests began, some personal data of all students were collected. The results showed some statistically significant correlation between several personal variables (sex, manual dominance and previous skills) and performance in the psychometric tests, although no significant relation was found between these variables and the students' academic qualifications. When comparisons were made between the psychomotor tests and the academic results of the students, significant differences appeared in the tracing and dotting subtests of the MacQuarrie test, whereby those students with higher practical qualifications reached the best punctuation. In the O'Connor Tweezer test, the students who obtained the worst results were those with lower qualifications. These data could serve to detect at an early stage those students who need greater educational support. No relation was found between the GEFT and the indirect vision test.

Early detection of inborn dyschromatopsias in preschoolers and young schoolchildren.

Color vision is one of the most important factors of human vision. Early detection of color vision malfunction in children is of utmost importance, allowing parents and teachers to take appropriate and timely measures for children's future professional orientation. A cohort of 300 children aged 4-6 years (49-86 months) were examined by color vision testing and divided into three age groups of 4-5, 5-6 and > 6 years. Their ability to name the fundamental spectral colors and read Ishihara's pseudoisochromatic plates were recorded. The results obtained are presented in tables and figures. Data analysis showed the youngest group of children to be unsuitable for such testing of color vision. Numerical data revealed considerable percentage deviations as compared to the older groups. In addition, a great difference between the girls and boys indicated psychomotor development to be much faster in girls of that age. Generally, data obtained by the distribution of the results for the two older age groups were consistent with those found in the literature; thus, the authors propose an early examination for the detection of inborn chromatopsia to be performed in preschool children, but not before the age of five. Ishihara's pseudoisochromatic test can be useful in differentiating between dyschromatic and other children, but it is very difficult to implement for singling out dyschromates according to the protan or deutan type.

Impaired color vision in cocaine-withdrawn patients.

BACKGROUND: The main reinforcing effect of cocaine happens by altering dopaminergic neurotransmission in the brain reward systems. Dopamine is found in high concentrations in the retina in which it plays an important role in color vision. Therefore, we investigated whether cocaine-dependent patients might have impaired color vision. METHODS: We compared patients recently withdrawn from cocaine (n = 31) with matched normal controls (n = 31) on 2 color vision tests. RESULTS: Cocaine-withdrawn patients had significantly higher error scores than matched controls on the Farnsworth-Munsell 100-hue and Lanthony desaturated D-15 color vision tests. Also, 23 of the 31 cocaine-withdrawn patients had blue-yellow color vision losses on the Farnsworth-Munsell 100-hue test compared with 3 controls (P < .001, chi 2 test) and 15 had blue-yellow color vision loss on the Lanthony desaturated D-15 test compared with 2 controls (P < .001, chi 2 test). CONCLUSIONS: These significantly higher test error scores and blue-yellow color vision losses suggest that color vision is impaired in cocaine-withdrawn patients. Color vision testing may be useful in future studies of cocaine-dependent patients.

The new Richmond HRR pseudoisochromatic test for colour vision is better than the Ishihara test.

AIM: The Hardy-Rand-Rittler (HRR) pseudoisochromatic test for colour vision is highly regarded but has long been out of print. Richmond Products produced a new edition in 2002 that has been re-engineered to rectify shortcomings of the original test. This study is a validation trial of the new test using a larger sample and different criteria of evaluation from those of the previously reported validation study. METHODS: The Richmond HRR test was given to 100 consecutively presenting patients with abnormal colour vision and 50 patients with normal colour vision. Colour vision was diagnosed using the Ishihara test, the Farnsworth D15 test, the Medmont C-100 test and the Type 1 Nagel anomaloscope. RESULTS: The Richmond HRR test has a sensitivity of 1.00 and a specificity of 0.975 when the criterion for failing is two or more errors with the screening plates. Sensitivity and specificity become 0.98 and 1.0, respectively, when the fail criterion is three or more errors. Those with red-green colour vision deficiency were correctly classified as protan or deutan on 86 per cent of occasions, with 11 per cent unclassified and three per cent incorrectly classified. All those graded as having a 'mild' defect by the Richmond HRR test passed the Farnsworth D15 test and had an anomaloscope range of 30 or less. Not all dichromats were classified as 'strong', which was one of the goals of the re-engineering and those graded as 'medium' and 'strong' included dichromats and those who have a mild colour vision deficiency based on the results of the Farnsworth D15 test and the anomaloscope range. CONCLUSIONS: The test is as good as the Ishihara test for detection of the red-green colour vision deficiencies but unlike the Ishihara, also has plates for the detection of the tritan defects. Its classification of protans and deutans is useful but the Medmont C-100 test is better. Those graded as 'mild' by the Richmond HRR test can be regarded as having a mild colour vision defect but a 'medium' or 'strong' grading needs to be interpreted in conjunction with other tests such as the Farnsworth D15 and the anomaloscope. The Richmond HRR test could be the test of choice for clinicians who wish to use a single test for colour vision.

[Influence of Eqb 761 on the function of the retina in children and adolescent with long lasting diabetes mellitus--preliminary report].

PURPOSE: Early detection of patological function of the retina, before anatomical changes, plays very important role in monitoring of visual complications in patients with diabetes mellitus. The aim of the study was the evaluation of anatomical and functional changes in visual organ in children and adolescents with long lasting diabetes mellitus type 1 and taking Egb 761 (Tanakan Beaufour Ipsen) as an adjuvant. MATERIALS AND METHODS: Group consists of 15 patients, age between 11 and 19 years, with diabetes mellitus lasting 6-12 years. All patients had full ophthalmologic examination and color vision tests (Panel D 15 saturated and desaturated). The examination was done 3 times, every 3 months. Egb 761 was given: 1 tablet - 3 times a day, during 3 months. RESULTS: No diabetic retinopathy was found. The results of color vision test were better after therapy (25% of pathological results) and 3 months later (only 4 % of patients). CONCLUSIONS: 1. Egb 761 seems to be good adjuvant in patient with long lasting diabetes mellitus. 2. Color vision tests are sensitive tests of the retinal function and are easy to perform.

Binocular contrast sensitivity testing for low vision patients.

Contrast sensitivity testing is often included in primary care and low vision examinations. This test is designed to provide information regarding the quality of a patient's vision. This clinical study provides a method to assess the binocularity of a low vision patient with contrast sensitivity testing. The method allows the practitioner to determine if monocular or binocular conditions are optimal for the use of a near low-vision device.

Visual function in young adults following intrauterine growth retardation.

BACKGROUND: Intrauterine growth retardation (IUGR) resulting in infants born small for gestational age is a known risk factor for neurologic deficits and may predispose to poor cognitive development later in life. We recently found an association between IUGR and a reduced neuroretinal rim area at 18 years of age. We evaluated the possible association between IUGR and visual function. SUBJECTS AND METHODS: We studied 26 subjects who had been born small for gestational age and 20 subjects whose birth weights were appropriate for gestational age (controls) using letter acuity thresholds, color vision testing, full-threshold frequency doubling technology perimetry, and rarebit perimetry at 18 years of age. gestational age had a rarebit hit rate below the normal range as compared with none of the controls (P = .006). These 8 subjects had a significantly smaller rim-disc ratio compared with the subjects who were small for gestational age who had a normal rarebit hit rate (P = .047). The frequency doubling technology indices did not differ significantly between the control group and the group that was small for gestational age, nor did the visual acuity, refraction, and color vision test results. CONCLUSION: These data indicate that IUGR is associated with an increased rate of impaired visual function, which can be detected by using rarebit perimetry but not frequency doubling technology perimetry, visual acuity, or color vision tests.

Assessment of visual acuity via a telephone interview.

We compared the visual acuity of 89 patients obtained via a telephone interview with visual acuity elicited from the same patients in an ophthalmologist's office. Agreement within one line of vision between the telephone interview, in which patients used a modified Rosenbaum near card with attached patch, and the clinic near vision test was observed in 91% of eyes, while agreement within one line of vision between the telephone interview and clinic distance vision test was found in 86% of eyes. We conclude that testing visual acuity via a telephone interview is a potentially useful approach to assessment of visual acuity and prevalence of visual impairment in the larger population.

Predicting Farnsworth Lantern success with a six-plate series of the Ishihara pseudoisochromatic plates.

The Farnsworth Lantern (FALANT) is the definitive color vision test for the Navy, Marine Corps, and Coast Guard. It is also the definitive color vision test for aviation personnel in the Army. Results from it are also accepted by the Federal Aviation Administration. For various reasons, it is desirable to have an alternative test that can predict success (i.e., a passing score) on the FALANT. In 1991, it was shown that a 14-plate series of Ishihara pseudoisochromatic plates could predict FALANT success provided the proper passing criteria was chosen. Interest has arisen regarding whether or not a 6-plate series of Ishihara plates can predict FALANT success. A study was undertaken to answer this question. It appears that a 6-plate series of Ishihara pseudoisochromatic plates can predict FALANT success.

Color vision in 42 Congolese patients with tuberculosis receiving ethambutol treatment.

PURPOSE: To study color vision in Congolese patients with tuberculosis receiving ethambutol therapy. METHODS: A prospective, descriptive study of color vision test in patients with systemic tuberculosis receiving ethambutol was performed between April 1995 and January 1998 at the Department of Ophthalmology, University of Kinshasa. Color vision tests were assessed with pseudoisochromatic plates (the Ishihara Pseudo-isochromatic Plates), the AO-HRR (American Optical Handy Rand Rittler), the Bölle and Kastel anomaloscope, Farnsworth-Munsell test (the D-15 and the FM-100). RESULTS: There were 42 patients with a mean age of 33 years (range, 14 to 75 years). The color vision of all the patients was found to be normal as measured by the Ishihara pseudoisochromatic plates. One (2%) patient showed color vision defect (anarchic axis] with the OA-HRR test. Three (7%) of 42 patients displayed blue-yellow color axis or anarchic axis color vision test on the D-15 test. Fifteen (36%) of 42 patients had high total error scores at the Farnsworth-Munsell 100 test. The color axis was as follows: anarchic axis (13.1%), red-green-color and blue-yellow-color combined axis (13.1%), blue-yellow color axis (7.5%). Results of the Bölle and Kastel anomaloscope were normal in all patients. CONCLUSION: Our results confirm the importance of color vision examinations in the detection of the complications of ethambutol treatment.

The effect of anthocyanosides on night vision.

SRT, DAR and MCS were measured immediately before, and 4, 8 and 24 h after treatment. RESULTS: No significant effect was found on any of the three night vision tests during the 24 h following a single oral administration of 12, 24 or 36 mg anthocyanosides. The study had a power of 0.95 to detect a 0.1 log unit improvement in SRT and 0.5 log unit improvement in MCS. CONCLUSIONS: Single oral administration of 12-36 mg of anthocyanosides appears to lack significant effect on militarily relevant night vision tests.
