[Stomatologic approach in patients with congenital coagulation deficit].

BACKGROUND: The purpose of the present paper was to resolve postoperative bleeding after oral surgery in patients with congenital coagulative defects, avoid transfusion of clotting factors and improve the patient's quality of life. METHODS: Since 1988, 1598 oral surgery procedures have been performed on patients with congenital coagulative defects treated with a protocol including pre- and postoperative tranexamic acid rinses. RESULTS: The use of tranexamic acid mouth washing has obtained effective hemostasis in 86% of cases and only 4% of surgical procedures required clotting factor transfusions. CONCLUSIONS: The results suggest that mouth washing with tranexamic acid prevents excessive oral bleeding in patients with congenital deficit of the coagulation system. Furthermore, a total elimination of the granulation tissue, that reduces the possibility of activation of the fibrinolytic mechanism, is also important.

[Micromechanical characteristics of restoration stomatological materials and human dental enamel and dentin].

Polished sections of human teeth and 11 Russian and foreign restoration materials (silver amalgam, cements, and polymer-based materials) were studied with an MTI-3M microhardness meter. Microhardness immediately under loaded diamond indentor of the device, elastic and plastic deformations were evaluated. Dental enamel is characterized by the greatest microhardness. Inorganic filler essentially (3-fold) increases the microhardness of polymer-based material. Microhardness of dental tissues and restoration materials notably decreased as the strength of indentor loading, i.e. the depth of its submerging in material, increased. Dental enamel is characterized by the most elastic deformation; dentin approximates the enamel by this parameter, as well as some restoration materials (prodigy, degufil-H, degufil-SC) when exposed to the saliva.

The influence of selected bioadhesive polymers on physical-chemical properties of stomatological dressing.

Dressings made from polyvinyl alcohol (PAV) and Hydroxypropylmethylcellulose (HPMC) with the addition of glycerol (GLY) differ in adhesiveness and the time of wash out in water, both characteristics depended on polymer and GLY concentration. The research on the release speed which is applied in topical therapy of chlorhexidine digluconate antiseptic in biopharmaceutical model proved the existence of close to rectilinear relation between the amount of released substance and release time for selected formulations.

[Stomatologic significance of masticatory system types].

The author gives analysis on the importance of the constitutional types of the masticatory system in the fields of restorative dentistry, oral surgery, prosthodontics and orthodontics. According to his research there are two types: I. High alveolar process, the teeth have long roots and short crowns. II. Low alveolar process, the teeth have shorter roots and longer crowns. The two groups may be differentiated by measurement of the height of the upper alveolar process in the median line (anthropologic points: nasospinale, prosthion) and the lengths of the anatomic crown of the upper medial incisor.
