Breast biopsy and lesion localization were performed with gadolinium-enhanced magnetic resonance (MR) imaging guidance, with use of a stereotaxic biopsy system consisting of a flexible circular surface coil with an acrylic cylinder as an add-on guidance device. Twenty-five procedures (23 localizations, two core biopsies) were performed in lesions depicted at diagnostic MR imaging, without technical failure. The guidance system permitted easy, precise, and flexible biopsy and localization of all breast lesions.

As part of a multimodality therapy program for intracranial tumours, 105 stereotactic and implant procedures have been carried out utilizing the CT-compatible Leksell stereotactic system. In the iridium implant series, 86 catheters have been implanted for an average of 3.6 targets per patient. There have been no deaths or missed targets and only two incidential haemorrhages detected. In order to facilitate the reliability, safety and speed of multiple catheter insertion, several techniques have been developed including: (a) a standardized single-length catheter and flange system; (b) a ceramic catheter for microwave hyperthermia; (c) a mnemonic card for ease of calculation; (d) a radiation shield for nursing; (e) a stereotactic drill and surgical approaches to far lateral and posterior fossa targets. Principles for the use of these technical aids are discussed.
Herpes simplex virus type 1 (HSV-1) plasmid vectors have a number of attractive features for gene transfer into neurons. In particular, the large size of the HSV-1 genome suggests that HSV-1 vectors might be designed to accommodate large inserts. We now report the construction and characterization of a 51 kb HSV-1 plasmid vector. This vector was efficiently packaged into HSV-1 particles using a helper virus-free packaging system. The structure of the packaged vector DNA was verified by both Southern blot and PCR analyses. A vector stock was microinjected into the rat striatum, the rats were sacrificed at 4 days after gene transfer, and numerous X-gal positive striatal cells were observed. This 51 kb vector was constructed using general principles that may support the routine construction of large vectors. Potential applications of such HSV-1 vectors include characterizing large promoter fragments or genomic clones and co-expressing multiple genes.
On May 12th 1984 a meeting on stereotactic biopsies of intraencephalic lesions has been held in Marseilles. All French Neurosurgeons and Neuropathologists involved with this technique were present. This report presents, in one hand, the result of an investigation on 3 052 BS and, in the other hand, the synthesis of discussions held on the limits and dangers of the BS, morphology of the biopsy instrument, means of the definition of the target and the microscopic histologic technics and results.

The aim of this report is to introduce a simple modification to the free-hand frameless stereotactic placement of ventriculoperitoneal shunts in undersized ventricles. In this technical note, we describe our experience with ventricular catheter placement in two children suffering from shunt dependent idiopathic intracranial hypertension using an image-guided instrument holder with a catheter guide. In both patients, the surgical procedure proved to be easy and accurate, with good initial clinical results. The use of an image-guided instrument holder is a modification to the free-hand frameless stereotactic placement of ventriculoperitoneal shunts in undersized ventricles.
The marmoset is becoming an important model for studies of primate vision, due to factors such as its small body size, lissencephalic brain, short gestational period and rapid postnatal development. For many studies of visual physiology (including single-cell recordings), it is a requirement that the animal is maintained under anaesthesia and neuromuscular block in order to ensure ocular stability. However, maintaining such a small animal (290-400 g) in good physiological condition for long periods requires expert attention. This becomes particularly important in the case of recordings from visual association cortex, where neuronal responses are known to be highly sensitive to factors such as the type and dose of anaesthetic, and the animal's physiological balance. The present protocol summarises our laboratory's experience over the last decade in developing a preparation for the study of marmoset visual cortex. It allows excellent recording from extrastriate areas for periods of at least 48 h, including the continuous study of isolated single cells for several hours.
In the past generation, mammography has become a major, if not the best available, diagnostic tool for the earlier detection of breast cancer. We have certainly come far from hard-to-imagine anecdotes of the early 1970s that "if I can't feel it, it's not there!" Although current controversy questions the earliest age at which screening mammography truly lowers the death rate from breast cancer, that mammography does detect breast cancer years before it might be discovered as a mass in the breast cannot be challenged. Mammographic techniques have improved so that smaller and smaller areas of suspicion may be identified, and mammographers have gained greater judgment in the interpretation of these minute radiographic abnormalities. This has inevitably led to an increase in procedures designed to explain them. The incurred costs, both emotional and economic, of positive mammographic "calls" are considerable. Whether practicing medicine as patients' advocates or, as unfortunately currently seems to be of equal importance, exercising politically correct and cost-effective mandates, the management of nonpalpable breast lesions certainly bears witness to the correlation of cognitive and procedural skills and cooperation between physicians, as well as the technical achievements of contemporary medicine.

Stereotaxic rostral mesencephalotomy was performed 19 times in 14 patients suffering from intractable pain syndromes due to malignant diseases. The satisfying results in terms of pain relief during a necessarily short follow-up period (mean 4.9 months) are outlined. Undesired side effects were mainly confined to oculomotor disorders, which partly subsided over the months following the operation. Some technical aspects of the procedure and the pertinent literature are briefly discussed.
