Drug information for prescribers and dispensers: toward a model system.

Four alternative ways of providing health care professionals with drug information are examined in terms of medical and social values. It is assumed that maximum patient benefit will result when prescribing decisions can be made in an environment of enlightened neutrality. The alternative of maintaining the status quo is discarded because it does little to provide practitioners with complete data. The second alternative notes major reforms that would further regulate promotional efforts and upgrade the qualifications of detailmen. Because this approach yields a suboptimum solution, a third alternative introduces the concept of a National Drug Education Foundation. This countervailing force would disseminate objective drug information to clinicians through 2,000 therapeutic consultants, subsidize professional journals and schools, develop regional drug information centers, and conduct a variety of related functions. Foundation operations are estimated to cost $167 million per year or about one-seventh of current expenditures for drug promotion and information. Since commercial inputs would not only be redundant but also tend to negate Foundation efforts, the fourth model contends that social benefits can be optimized only when such outlays are terminated.

Developing a clinical teaching handbook and reference manual for part-time clinical faculty.

To orient nurses who are already clinical experts to their new role as clinical educators, we developed a department-specific handbook and a reference manual that included materials which could not, as a practical matter, be photocopied extensively. These materials allowed us to quickly orient new clinical instructors to the responsibilities and skills they would need for clinical instruction and supervision. Providing these orientation and training materials also helped solve the clinical nursing faculty shortage at our institution by supporting nurses who, despite expertise in clinical areas, lacked formal training or experience as educators.

The PDA as a reference tool: libraries' role in enhancing nursing education.

"The PDA as a Reference Tool: The Libraries' Role in Enhancing Nursing Education" is a pilot project funded by the University of Massachusetts President's Office Information Technology Council through their Professional Development Grant program in 2004. The project's goal is to offer faculty and students in nursing programs at two University of Massachusetts campuses access to an array of medical reference information, such as handbooks, dictionaries, calculators, and diagnostic tools, on small handheld computers called personal digital assistants. Through exposure to the variety of information resources in this digital format, participants can discover and explore these resources at no personal financial cost. Participants borrow handhelds from the University Library's circulation desks. The libraries provide support in routine resynchronizing of handhelds to update information. This report will discuss how the projects were administered, what we learned about what did and did not work, the problems and solutions, and where we hope to go from here.

Suicide with inert gases: addendum to Final Exit.

Suicide by asphyxiation with a plastic bag placed over the head is a method that has been advocated by right-to-die groups. Recently, such groups have proposed the introduction of helium into the plastic bag as a means of hastening death. Helium is readily available at toy stores, where it is sold in tanks for balloon inflation. It produces asphyxiation by the exclusion of oxygen in enclosed spaces. We report 7 fatalities throughout an 18-month period involving plastic bag suffocation in conjunction with helium use. These fatalities coincide with publication of an update to a popular right-to-die text in which this method is described. Although right-to-die literature was absent from all scenes, this method was not previously observed in our jurisdiction, and the deaths likely reflect exposure to this information. Because of analytical difficulties in testing for helium in biologic specimens, death certification rests on scene investigation.

Getting the right tool for the job.

In summary, getting the right tool for the job requires several steps prior to using the tool in an anticipated study. Finding obtaining and evaluating the tool are all critical to a successful use of the tool in the study. If any of these factors are not followed, the researcher could be ethically or legally in trouble. Most tool developers want their tools to be used. It is up to the researchers to ensure that credit is given where credit is due, and that due process is followed.

Comparative assessment of four drug interaction compendia.

AIMS: To assess the consistency of inclusion and grading of major drug interactions for 50 drugs in four leading international drug interaction compendia. METHODS Four international drug interaction compendia were compared: the drug interactions appendix of the British National Formulary, the interaction supplement in the French drug compendium Vidal, and two US drug interaction compendia, Drug Interaction Facts and the Micromedex (Drug-Reax) program. Major interactions were defined as potentially hazardous in BNF or with the warning 'contraindication' or 'avoid' in Vidal or with the significance grading 1 or 2 in DIF. Major interactions for a list of 50 drugs were searched in all four compendia. RESULTS: A total of 1264 interactions meeting the inclusion criteria were identified for these 50 drugs. After deletion of 169 duplicates, 1095 interactions were included in the analysis. Of the drug interactions classified as major in any one compendium between 14% and 44% were not listed in the other compendia. The grading systems used for the severity and the quality of the supporting evidence in Micromedex and DIF were inconsistent. CONCLUSIONS: There is a lack of consistency in the inclusion and grading of drug interactions of major significance for 50 drugs across the four drug compendia examined. This may reflect the lack of standardization of the terminology used to classify drug interactions and the lack of good epidemiological evidence on which to base the assessment of the clinical relevance of drug interactions.

An experimental resource centre on handicap in children. 1. Preliminary analysis of need for such a centre.

An experimental information and resource centre on handicap in children for the use of staff in the health, social and educational services was set up in an area of London serving four adjacent boroughs. As a first step, a questionnaire was widely distributed to potential users of the centre asking for information about their work, training and recent reading in this subject. Of 192 replies, those from health visitors, medical officiers, teachers and therapists are analysed. Lack of adequate training, both at basic and post-qualification level, as well as a failure to keep up to date with specialized journals, encouraged the establishment of the resource centre, which also planned to provide a course of lectures and seminars. The nature of the material collected is briefly described.
