Think INN, prescribe INN, dispense INN: good professional practice.

(1) International nonproprietary names (INN) for drugs were invented about fifty years ago, under the aegis of the World Health Organisation, to provide a common language for health professionals and patients worldwide. (2) No country forbids INN prescriptions. Some countries actively recommend using INNs. (3) INN prescription empowers prescribers and pharmacists in their choice of treatment. (4) The choice between a prescription based on the INN or the brand name will depend on the type of treatment (short term or chronic), the nature of the drug (especially its therapeutic margin) and any specific risks related to the patient (age, disease condition, allergy, and adherence). (5) A pharmacist's decision to dispense a brand name drug from an INN prescription must be based on usual dispensing precautions. (6) Adopting INN prescribing means having to reflect on one's knowledge of drugs, and to challenge the quality of one's initial and continuing education in pharmacology and therapeutics. The INN system is a means of improving prescribing and dispensing practices: it involves paying more attention to the patient, explaining the treatment in greater detail, and respecting his/her choice.

[Knowledge and beliefs of 4th-year medical students concerning professional practices in general medicine. Results of a survey conducted at the School of Medicine of Brest].

As part of preparations for the first seminar in general practice, organized for medical students in their forth year at the Brest faculty of medicine, students were asked to fill in a questionnaire concerning their beliefs and knowledge concerning general-practice medicine. Fifty-two students, or 89.6% of the class, completed the questionnaires. The results showed that despite the lack of formal contacts with this type of practice during their studies, the students had a good idea of what general-practice medicine is and of the role of the general practitioner in our health-care system. However, this study has illustrated the difficulties students experience in defining their future professional direction, since they receive no information during their training on the different options in medical practice.

Computer-aided learning for the education of patients and family practice professionals in the personal care of diabetes.

Diabetes Mellitus is approaching pandemic proportions across the globe. It is a disproportionately expensive condition, accounting for 5-9% of annual NHS expenditure. Family practices often play a huge role in the care of diabetic patients. Many GPs elect to play a larger role in diabetes care, but the increasing burden on the multidisciplinary secondary care team means that some of the burden has to fall to family practitioners. In order to provide a high standard of care, the practitioner requires access to continuing education regarding diabetes care. The value of patient education is undisputed. In light of this situation a computer-aided learning (CAL) system is being developed for the education of both patients and practitioners concerning diabetes and its care. The proposed system takes a two pronged approach, being aimed at both patient and practitioner. This interactive system employs multimedia technology to teach practical skills and promote and consolidate theoretical understanding. It is hoped this system will improve patient self-care, and in the long-term reduce the incidence of diabetic complications and their associated costs.

Periodontics in general practice: professional plaque control.

Traditionally the primary emphasis of preventive periodontics was daily patient performed plaque control. Recent studies indicate that frequent professional subgingival toothcleaning is a mandatory treatment for prevention of recurrent periodontitis. Pathogenic subgingival bacterial complexes are disrupted by frequent cleaning and require time to reestablish. Disease progression is prevented if the recall interval does not exceed the time required for reestablishment of a pathogenic plaque. Legally, patients have acquired the duty to comply with the prescribed recall interval. Both the patient and the practitioner will benefit from a preventive program that includes frequent professional subgingival toothcleaning.
