Incidence of bacteremia with elective upper gastrointestinal endoscopic laser therapy.

The incidence of bacteremia associated with endoscopy varies widely for different endoscopic procedures. This study prospectively evaluates the incidence of bacteremia in patients undergoing elective endoscopic laser therapy for two diseases of the upper gastrointestinal tract. In five patients (eight treatment sessions) with arteriovenous malformations, no bacteremia developed. In 15 patients (26 treatment sessions) with esophagogastric carcinoma, bacteremia developed at some time during the procedure in six (40%). However, bacteremia appeared to be associated with the endoscopic insertion through the tumor rather than the laser treatment per se. These findings lead us to conclude that for patients undergoing endoscopy and laser therapy for esophagogastric malignancy, but not for those being treated for arteriovenous malformations, antibiotic prophylaxis to prevent endocarditis may be indicated.

Prostatic abscess owing to anaerobic bacteria.

Abscess of the prostate is seen infrequently. We report a prostatic abscess owing to anaerobic bacteria in a 46-year-old man with a 10-year history of irritable voiding symptoms. Preoperative computerized tomography confirmed the diagnosis of prostatic abscess, which was treated with transurethral resection and broad-spectrum antibiotics.

Relationship between clinical symptoms and enzyme-producing bacteria isolated from infected root canals.

The object of this study was to determine the correlation between clinical symptoms and the activity of enzymes such as collagenase, chondroitinase, and hyaluronidase produced by bacteria isolated from infected root canals. The materials examined consisted of 28 teeth with apical periodontitis from 25 patients. Bacteria producing collagenase or chondroitinase and hyaluronidase were found to be significantly related to subacute clinical symptoms involving percussion pain. The frequency of bacteria producing collagenase was higher in isolates from root canals with a radiolucent area over 5 mm in diameter than in those from canals having a radiolucent area less than 5 mm in diameter.

Antibacterial effects of various endodontic medicaments on selected anaerobic bacteria.

This study was undertaken to determine the antibacterial effects of various endodontic medicaments against six selected anaerobic bacteria. The experiment involved the testing of the vapors of six medicaments using agar plates streaked with the bacteria. A zone of inhibition was recorded for each plate and the results were analyzed statistically. Formocresol produced significantly larger zones of inhibition than any of the other medicaments.

Localization and identification of root canal bacteria in clinically asymptomatic periapical pathosis.

Twenty-one teeth with clinically asymptomatic periapical pathosis (class 3) were extracted and the isolation, identification, and localization of bacteria in the root apex were examined. Mixtures involving several bacteria were isolated from more than 60% of the cases. Scanning electron microscopy revealed bacterial masses to be associated with the apical part of the root canal, but not with the area of apical foramen or on the surface of root apex. Our results indicate that the bacteria in class 3 cases may be derived from organisms which colonized before or during endodontic treatment, but not from anachoresis. The bacteria-positive cases of asymptomatic periapical pathosis have the potential to progress to symptomatic periapical pathosis.

Isolation of genital mycoplasmas from blood obtained shortly after vaginal delivery.

Blood obtained from three hundred and twenty-seven women within a few minutes of vaginal delivery was cultured for genital mycoplasmas (Mycoplasma hominis and T mycoplasmas). Twenty-six (8%) of the women had genital mycoplasmas isolated from their blood. Ten women had blood-cultures which contained M. hominis. Fifteen women had blood-cultures which contained T mycoplasmas. Both M. hominis and T mycoplasmas were isolated from the blood of one woman. Only one of two hundred and seventy-three blood-cultures obtained 1 or more days after delivery contained mycoplasmas. The isolation of M. hominis, but not of T mycoplasmas, from the blood was associated with vaginal colonisation and a serological response to the homologous isolate. Two of ten women whose blood contained M. hominis gave birth to stillborn infants. In contrast, there were only four (1.3%) stillbirths among the infants born to the three hundred and one women whose blood did not contain mycoplasmas. This difference is statistically significant.

Lactobacilli do not cause frequency and dysuria syndrome.

Mid-stream specimens (MSU) of urine were collected from 142 healthy women (pregnant and non-pregnant) and cultured for lactobacilli and other fastidious bacteria. The latter either require CO2 or are obligate anaerobes. Lactobacilli were present in counts of 10(4)/ml or more in 34.8% of the women, and in counts of 10(5)/ml or more in 20.2%. Besides lactobacilli, which were the bacteria most frequently isolated, anaerobic gram-positive cocci (peptococci and peptostreptococci) were often found. This flora is typical of that of the lower vagina, and none of these women had either symptoms of urinary infection of pyuria. Therefore, the bacteria isolated were commensals or contaminants. Cultures of MSUs taken from 26 women with symptoms of dysuria and/or frequency, but without significant numbers of conventional pathogens such as Escherichia coli, contained commensals and contaminants of the same variety and in similar numbers. Urine samples from 50% of these patients contained at least 10(4) lactobacilli/ml and 27% had 10(5) or more/ml. Lactobacilli were absent from the suprapubic urine specimens cultured from a further 44 women. There was no significant difference between the isolation rate of lactobacilli in urine cultures from healthy women and the rate in women with dysuria and frequency.

Anaerobic bacteria in dentoalveolar abscesses.

The present study was initiated to determine the bacteriology of 40 orofacial abscesses of dental origin in patients who had taken antibiotics for several days. Bacteria were isolated from all but 2 specimens. Aerobic or facultative anaerobic bacteria were isolated in 21 cases, obligate anaerobic bacteria in 17 cases, whereas in 11 cases, polymicrobial growth was revealed. The average number of bacterial species was 2.1 per specimen. Gram positive aerobic micro-organisms predominated, namely, Staphylococcus epidermidis followed by Streptococci (group A) and Staphylococcus aureus. Among obligate anaerobes, Gram positive micro-organisms, peptostreptococci and peptococci were more often isolated, in the following decreasing order: Ps. productus, Ps. intermedius, Ps. parvulus, Ps. anaerobius, Pc. constellatus, Pc. prevotii. Gram negative anaerobic rods were detected in a small number of cases, namely B. corrodens, B. fragilis, B. melaninogenicus, B. ochraceus, B. oralis. Quantitative determination did not show any meaningful difference between aerobic and anaerobic isolates. Susceptibility tests against a series of antibiotics showed that ampicillin was the most active in aerobes and cefoxitin in anaerobes.

Syntheses of Curcumin Bioconjugates and Study of Their Antibacterial Activities against beta-Lactamase-Producing Microorganisms.

In the present study curcumin bioconjugates, viz. di-O-glycinoylcurcumin (I), di-O-glycinoyl-C(4)-glycylcurcumin (II), 5'-deoxy-5'-curcuminylthymidine (5'-cur-T) (IV), and 2'-deoxy-2'-curcuminyluridine (2'-cur-U) (V) have been synthesized and characterized by elemental analysis and (1)H NMR. The turmeric peptide (Tp) was isolated from the aqueous turmeric extract of the turmeric rhizome. The antibacterial activity of these four bioconjugates and also of the turmeric peptide and sodium salt of curcumin (III) have been tested particularly for beta-lactamase-producing microorganisms.

Determination of nonligand amino acids critical to [4Fe-4S]2+/+ assembly in ferredoxin maquettes.

The prototype ferredoxin maquette, FdM, is a 16-amino acid peptide which efficiently incorporates a single [4Fe-4S]2+/+ cluster with spectroscopic and electrochemical properties that are typical of natural bacterial ferredoxins. Using this synthetic protein scaffold, we have investigated the role of the nonliganding amino acids in the assembly of the iron-sulfur cluster. In a stepwise fashion, we truncated FdM to a seven-amino acid peptide, FdM-7, which incorporates a cluster spectroscopically identical to FdM but in lower yield, 29% relative to FdM. FdM-7 consists solely of the. CIACGAC. consensus ferredoxin core motif observed in natural protein sequences. Initially, all of the nonliganding amino acids were substituted for either glycine, FdM-7-PolyGly (.CGGCGGC.), or alanine, FdM-7-PolyAla (.CAACAAC.), on the basis of analysis of natural ferredoxin sequences. Both FdM-7-PolyGly and FdM-7-PolyAla incorporated little [4Fe-4S]2+/+ cluster, 6 and 7%, respectively. A systematic study of the incorporation of a single isoleucine into each of the four nonliganding positions indicated that placement either in the second or in the sixth core motif positions,.CIGCGGC. or.CGGCGIC., restored the iron-sulfur cluster binding capacity of the peptides to the level of FdM-7. Incorporation of an isoleucine into the fifth position,.CGGCIGC., which in natural ferredoxins is predominantly occupied by a glycine, resulted in a loss of [4Fe-4S] affinity. The substitution of leucine, tryptophan, and arginine into the second core motif position illustrated the stabilization of the [4Fe-4S] cluster by bulky hydrophobic amino acids. Furthermore, the incorporation of a single isoleucine into the second core motif position in a 16-amino acid ferredoxin maquette resulted in a 5-fold increase in the level of [4Fe-4S] cluster binding relative to that of the glycine variant. The protein design rules derived from this study are fully consistent with those derived from natural ferredoxin sequence analysis, suggesting they are applicable to both the de novo design and structure-based redesign of natural proteins.
