An investigation into dystocia in a south country Cheviot flock.

An abnormally high incidence of dystocia was encountered in the south country Cheviot flock at the Hill Farming Research Organisation's research station at Sourhope, Yetholm, Roxburghshire over the past four years, 1970 to 1973. Lambing records were only available for 1972 and 1973 for Sourhope ewes and gimmers of all breeds and showed the following: (see article) This meant that in terms of attention and vigilance, a high shepherding input was necessary with the South Country Cheviot flock. Intensive observation and measurements of some relevant parameters were therefore made in the gimmer age group of this flock, numbering 126, during the lambing season of 1974; 39-7 per cent required assitance. This paper describes the behavioural pattern in lambing and demonstrates differences between those animals which were assisted and those which were not. The features of the dystocias and their possible cause are discussed.

[Deliveries in foreign women (author's transl)].

The question is to be answered whether the delivery in foreign women has in any case to be valued as riskful birth. The development of deliveries in 1941 foreign women from 1968-1973 is therefore analysed and compared with 9009 deliveries in German women. The share of deliveries in foreign women increased from 3% in 1968 to 32% in 1973. Wifes from Turkey and Jugoslavia are prevailing (68,1% and 19,5%). A significant moving of parity to a higher number of children can be noticed in foreign women. The incidence of operative deliveries in foreign women amounting to 12,5% was 5% lower than in German women. The incidence of sectio amounted to 9.4% in German women and to 7.5% in foreign women. The rate of premature births in foreign women figured up to 9,5% surpassed the rate of premature births in German women by 1%. The comparison of uncleared as well as of cleared perinatal mortality did not show any statistically significant difference. Merely the mortality of sectio-children amounting to 6.9% was significantly higher in foreign women than in German women with 3.7% - owing to the accumulation of vital indications in caesarean sections. Authors come to the conclusion that based on their material examined deliveries in foreign women cannot be juged as riskful births. Particularities in foreign deliveries are specially indicated.

[Premature rupture of membranes seen at the Befelatanana maternity, Antananarivo University Hospital Center].

Generally preterm ruptures of membranes (PRM) are harmless, but they become serious if the labor doesn't occur in the following 24 hours. Then, they might generate neonatal infections which provoke heavy fetal and maternal mortality. A retrospective study was carried out in 1998 at the Maternity Hospital of Befelatanana, Antananarivo in order to sum up knowledges on epidemiology and fetal prognosis of this disease, and to draw up measures to aim to reduce causes of PRM. 4232 cases of PRM were registered for the study period. The average age of parturient women was of 27 years old. PRM occur frequently among primiparas and high level multiparas. Risk factors and determinative causes are gyneco-obstetrical history as abortion, preterm delivery, cicatricial uterus, urogenital infections; uterine malformation; placenta praevia; hydramnios; dystocic labor presentation; uterine distension due to either multiple pregnancy or disproportion of fetus and birth canal; irregular and poor prenatal visits quality; low standard of living. Numerous premature infants of PRM outcomes had infections: 1,619 out of 4315 new-borns. Infant perinatal mortality rate was of 11.7 per cent. Maternal complications were infections, uterine rupture, hemorrhages. 5 deaths were noted. The reduction of PRM rate might be obtained by improvement of standard of living and hygiene, correct cares during pregnancy and intergenesic periods.

Uterine anomalies. Frequency of diagnosis and associated obstetric complications.

Uterine anomalies are associated with an increase in cases of breech presentation, premature labor, abnormal presentation with dystocia, and the necessity for cesarean section. The yearly frequency of diagnosis of an anomaly has varied from 0.06 to 0.48 at this medical center depending on the physician's clinical suspicion. Thus, if the true incidence and the decrease of obstetric complications are to be accomplished, an increased interest in diagnosing uterine anomalies is essential.

[Current practice and results of labor induction].

INTRODUCTION: The role of labor induction has been gradually increasing in the last decade all over Europe due to the early detection of fetal jeopardy, improvement of neonatal therapy and availability of cervical ripening agents. Success rate of vaginal delivery depends on maternal, fetal condition and cervical status. PATIENTS: Authors report the outcome of 795 labor inductions during the period from 1996 to 2000 at the Department of Obstetrics and Gynecology at the University of Debrecen, Hungary. Preinduction cervical ripening and induction method was based on Bishop score and clinical situation. RESULTS: The rate of induced labor was 5.97% out of 13312 consecutive deliveries. The outcome of induction is discussed in details. The caesarean section rate after induction of vaginal delivery was 25% in this high risk group, and 33.3% among the total number of inductions. CONCLUSIONS: The clinical application of prostaglandins for cervical ripening among high risk pregnant women facilitates the decision of labor induction and provides a favourable rate of vaginal deliveries.

Internal fixation of radial neck fracture in a fracture dislocation of the elbow. A case report.

A 27-year-old manual laborer presented with dislocation of the right elbow, complete separation of the radial head at the neck level, and avulsion of the coronoid process. Closed reduction of the elbow resulted in ulnohumeral instability. As an alternative to silastic implant, the radial head was reduced, internally fixed, and used as a spacer to restore elbow stability. The radial head fracture healed with no sequelae. At two years follow-up examination, the patient had a stable elbow with nearly full range of motion.
