Free flap cover of acute hand injuries.

Eighteen free flap reconstructions of acute hand trauma were compared with 12 groin flap repairs as historical controls. The free flap group had a similar success rate, with fewer operations (though the same total operating time) and shorter hospital stay than the groin flap group. Complications were the same in both groups, but less secondary procedures were necessary in the free flap group. If flap cover is necessary, free flaps are a better way of obtaining skin cover in acute hand trauma.

Sequential multiple free flap transfers for reconstruction of devastating hand injuries.

Restoring function to a severely damaged hand can require complex reconstructions using multiple tissue transfers to replace skeletal, soft tissue, and nerve components. We present 2 patients with severe hand trauma treated with serial multiple flap transfers, an operative sequence not previously reported in detail. One patient underwent five flap transfers in three operations, whereas the second patient underwent four flap transfers in two operations. All the flaps survived. Total anesthesia time for these patients was 43.5 and 34.5 hours, respectively. Both patients obtained measurable functional restoration, and suffered no significant perioperative morbidity. These patients illustrate the clinical feasibility of serial multiple microvascular transplantations for complicated reconstructions.

Work-related hand injuries in Ontario: an historical perspective.

Worker's compensation legislation was enacted in Ontario almost 90 years ago. Workers injured on the job gave up their right to sue employers and received no-fault compensation from an independent, employer-funded body called the Workmen's Compensation Board. Three academic health sciences centers in Ontario that are recognized for their commitment to patient care, research, and education compose part of the Specialty Program network with the Ontario Workplace Safety and Insurance Board (WSIB). Statistical data from the WSIB database for workers with hand injuries from 1996 to 2003 show an increase in fractures from fall injuries in the group of women older than 60 that may be related to osteoporosis, a common condition in this group.

[Hand injuries in the ready-made clothing industry].

We collect during 1997, 30 work accidents in the ready-wade clothe sector. These accidents have concerned 28 females and 2 males with a mean age of 21 years. Injury affect predominatly the left hand (17 patients) and the finger of pince. Severity of the lesions was variable according to the traumatic cause. In the distal fingers trauma, skin cicatrisation have been observed after 21 days and bone consolidation within 45 days.

On-site work evaluations: desensitisation for avoidance reactions following severe hand injuries.

This study examined on-site work evaluations as an environmental exposure strategy to promote return to work in 15 recalcitrant patients who had failed to benefit from established methods of reducing post-traumatic stress disorder symptoms. Following the on-site work evaluation, 87% of these patients were able to use visualisation of the work setting to further desensitize themselves and returned to work within the next eight weeks. All 87% have continued to be employed at six and twelve month follow-ups. This approach holds promise for assisting patients with hand injuries who develop post-traumatic stress disorder and fail to respond to traditional psychological strategies.

Mechanical cornpicker hand injuries.

The mechanical cornpicker causes tearing anse injuries from 1962 to 1975 were studied with regard to mechanism and extent ohe time of injury in 36% of hands, and in 73% of the remainder following treatmophylaxis, and antibiotics, and 73% hands required some form of delayed surgical treatment. Antibiotics did not appear to be helpful. Eighty-nine ps the most common cause given for farmers for their injuries. Excluding four pr permanently disabled patients, the average length of disability was 135 days. Eighty-nine per cent of patients experienced some permanent impairment of hand function. The mechanical cornpicker is described, and the importance of its proper use and physician's emphasis on accident prevention as well as treatment are stressed.

[Combined tissue transplantation for complicated hand injury].

OBJECTIVE: To investigate the effect of combined types in treating cases with 2 to 5 tissues transplantation. METHODS: 305 cases treated from December 1989 to December 2002 were analyzed and summarized. 214 cases were constructed with 2 combined tissues transplantation, the combined types were:toe combined flap,the second toe at both sides transplanted, 2 flaps combined transferred; 75 cases were constructed with 3 combined tissues transplantation, the combined types were: the second toe at both sides combined flap, 2 flaps combined toe, 3 flaps combined transferred; 11 cases were constructed with 4 combined tissues transplantation, the combined types were: 2 flaps combined 2 toes,1 toes combined 3 flaps, 4 flaps combined transferred; 5 cases were constructed with 5 combined tissues transplantation, the combined type was: the wrap flap and the second toe with 1 vascular pedicle and another second toe combined bilateral femoris anterior flaps were transferred. The principles of repair were: the thumb was reconstructed first,the wrap round flap was used for the thumb's skin, the second toe transplantation was used for the thumb defect. The opposite toe was transferred to finger to reconstruct the pinch function. 1 or 2 flaps were selected for repair according to the area of damage. The anterolateral thigh flaps were used for the large areas and the lateral arm flaps were used for the small areas in selecting the donor area. RESULTS: Of the 722 tissues in 305 cases, 14 of them were necrosis, in which the toe were 6 and the flap were 8,and other transferred tissues all survived, the survival rate of tissue transplantation was 98.1%. 251 cases were followed up for 1 year to 7 years (2. 6 years in average). All the patients could care themselves in daily life. The pinch and opposition functions of the constructed thumbs and fingers were recovered, the two-point discrimination was 6 to 14 mm, but 2 cases recovered adduction function only because the thumb was 6 degree defect without the thenar muscle and did not fix the reconstructed thumb in opposite side in operation. All transplanted flaps recovered protecting sensory with S2, 56 flaps required plastic operation because of their swelling appearance. The donor areas gained primary stage heal in 285 cases, 20 cases gained secondary stage heal, in which part grafted skin necrosis in donor of wrap round flap were 12 cases, the distal of donor big toe necrosis were 2 cases and grafted skin necrosis in donor of anterolateral thigh flap were 8 cases and skin grafted were successful. CONCLUSION: Different combined types can be used according to the traumatic situation. Surgical operation and early rehabilitation is conducive to the final function. Combined tissue transplantation is the best way to repair complicated hand injuries.
