Endopyelotomy and pyeloplasty: face to face.

46 symptomatic adult patients with documented ureteropelvic junction obstruction were treated with pyeloplasty (n = 23) or endopyelotomy (n = 23). Basic characteristics in both the groups were comparable. The technical aspects, complications and outcome, in the form of improvement in function and drainage patterns, were compared in both the groups. Endopyelotomy enjoyed the significant advantages of a shorter operating time and hospital stay, and obviously better cosmetic acceptance. The major complications in the endopyelotomy groups were related to external drainage and secondary infection in the form of fever, secondary hemorrhage and slippage of tubes in 44, 9 and 13% of cases, respectively. Complications associated with pyeloplasty were prolonged urinary leak, wound infection and urinary tract infection in 12, 17 and 22% of cases, respectively. Using 99Tc-DTPA diuretic scan, an improvement of more than 10% in split renal function could not be documented for any case from either group. In none of the patients did the function deteriorate either. Of all evaluable cases, only 1 in the pyeloplasty group showed a persistent obstructive pattern. The rest all demonstrated adequate drainage across the ureteropelvic junction. Overall in 12 cases (8 pyeloplasty, 4 endopyelotomy) drainage could not be determined postoperatively due to poor radionuclide uptake. A nephrostogram and/or pressure flow study, however, demonstrated a nonobstructive pattern in all these patients. It is concluded that endopyelotomy scores over pyeloplasty with a shorter operating time and hospital stay. The complication rate and outcome following surgery, however, are comparable in both the groups. Using external drainage following endopyelotomy, early resumption of work, however, could not be obtained.

Patient satisfaction and information gain after the preanesthetic visit: a comparison of face-to-face interview, brochure, and video.

In this study we compared 3 methods of conducting the preanesthetic visit. We prospectively studied 197 consecutive surgical patients who were to undergo general anesthesia. The patients were randomized to a routine preanesthetic interview, a brochure plus an interview, or a self-made documentary video plus an interview. After the preanesthetic visit, the degree of patient satisfaction and information gain was quantified by a questionnaire for each method. The questions on patient satisfaction were assessed on a six-point scale, and those on information gain were assessed on a multiple-choice basis. The video plus interview group showed the highest point scores (98% of the possible maximum sum point score in patient satisfaction and 93% of the maximum sum score in information gain). In contrast, the patients of the brochure plus interview group revealed 93% for patient satisfaction and 80% for information gain, and in the standard interview group, the corresponding figures were 91% and 72%, respectively. The maximum sum scores in patient satisfaction and information gain were significantly different between the interview and the video groups, but not between the interview and the brochure groups. Therefore, these data suggest that the use of a documentary video to supplement a preoperative interview may enhance patient satisfaction and maximize information gain.

The validity and reliability of self-reported data from heroin addicts: mailed questionnaires compared with face-to-face interviews.

Immediately following receipt of their 1-year follow-up mailed questionnaire, 55 former patients from an addiction treatment program were interviewed personally, and a urine sample was collected. Results indicate a high degree of correspondence of self-reported drug use with the urinalysis reports, and moderately high correspondence between the information on employment, school attendance, legal status, and length of heroin run given in the mailed questionnaire with that provided in the personal interview.
