Alopecia areata associated with regression of cutaneous melanoma.

A 41-year-old white female with a past medical history of hypothyroidism and alopecia universalis presented on January 24, 2002 with a recently changing mole. She indicated changes in size and color of the superior aspect of a mole that had been present for more than 8 years. She had approximately 20 lifetime peeling sunburns due to being a lifeguard. No family or previous personal history of skin cancers, including melanoma or atypical nevi, was reported. Her history of alopecia universalis began 12 years previously and has partially resolved with remaining patchy alopecia of the scalp and eyebrows. On diagnosis of alopecia universalis, she was initially treated with oral prednisone for 1 year and topical minoxidil for 3 months. Currently, she is not being treated for this condition. She denied other previous skin conditions. She had a surgical history of tonsillectomy at the age of 7 years. Her current medication includes levothyroxine (0.015 microg) for hypothyroidism diagnosed 12 years previously. She reported no known drug allergies. During the initial physical examination, she presented with phototype II skin with two adjacent pigmented lesions on her left foot within a 1.3 cm square. The first lesion on the left posterior distal heel was an irregular, brown-black, 0.5 x 0.6 cm macule. The second lesion, on the left posterior proximal heel, was an irregular, brown, speckled, 0.3 x 0.4 cm macule (Fig. 1). The patient had ophiasis of the scalp and total alopecia of the bilateral eyebrows. In keeping with the patient's wishes, alopecia lesions were not biopsied and clinical photographs of the alopecia are not included in this article. Two 3 mm punch biopsies were performed within each lesion. The left posterior proximal lesion showed malignant melanoma, with a Breslow depth of 0.4 mm, anatomic level II, marked lymphocytic response and partial regression (Fig. 2). The left posterior distal lesion showed malignant melanoma in situ, arising in a lentiginous compound nevus, with architectural disorder and cytological atypia. These two lesions were concluded to be one lesion with clinical regression. She underwent local excision with 1-cm margins and sentinel lymph node biopsy owing to the presence of regression, which showed no evidence of metastatic melanoma. Lactate dehydrogenase and chest X-ray were within normal limits. The alopecia areas were not biopsied previously or at that time.

Should physicians have facial piercings?

OBJECTIVE: The objective of this study was to assess attitudes of patrons and medical school faculty about physicians with nontraditional facial piercings. We also examined whether a piercing affected the perceived competency and trustworthiness of physicians. DESIGN: Survey. SETTING: Teaching hospital in the southeastern United States. PARTICIPANTS: Emergency department patrons and medical school faculty physicians. INTERVENTIONS: First, patrons were shown photographs of models with a nontraditional piercing and asked about the appropriateness for a physician or medical student. In the second phase, patrons blinded to the purpose of the study were shown identical photographs of physician models with or without piercings and asked about competency and trustworthiness. The third phase was an assessment of attitudes of faculty regarding piercings. MEASUREMENTS AND MAIN RESULTS: Nose and lip piercings were felt to be appropriate for a physician by 24% and 22% of patrons, respectively. Perceived competency and trustworthiness of models with these types of piercings were also negatively affected. An earring in a male was felt to be appropriate by 35% of patrons, but an earring on male models did not negatively affect perceived competency or trustworthiness. Nose and eyebrow piercings were felt to be appropriate by only 7% and 5% of faculty physicians and working with a physician or student with a nose or eyebrow piercing would bother 58% and 59% of faculty, respectively. An ear piercing in a male was felt to be appropriate by 20% of faculty, and 25% stated it would bother them to work with a male physician or student with an ear piercing. CONCLUSIONS: Many patrons and physicians feel that some types of nontraditional piercings are inappropriate attire for physicians, and some piercings negatively affect perceived competency and trustworthiness. Health care providers should understand that attire may affect a patient's opinion about their abilities and possibly erode confidence in them as a clinician.

Markers of cutaneous human papillomavirus infection in individuals with tumor-free skin, actinic keratoses, and squamous cell carcinoma.

Separately, actinic keratosis (AK) and cutaneous squamous cell carcinoma (SCC) have been associated with cutaneous human papillomavirus (HPV) infections. To further explore the association between HPV infection and SCC development, we determined markers of cutaneous HPV infection within a single population in persons with precursor lesions (AK), cancerous lesions (SCC), and without. Serum and plucked eyebrow hairs were collected from 57 tumor-free controls, 126 AK, and 64 SCC cases. Presence of HPV L1 and E6 seroreactivity and viral DNA were determined for HPV types 5, 8, 15, 16, 20, 24, and 38. Significant positive associations with increasing severity of the lesions (controls, AK, and SCC, respectively) were observed for overall HPV L1 seropositivity (13%, 26%, and 37%) and for HPV8 (4%, 17%, and 30%). In parallel, the proportion of L1 seropositive individuals against multiple HPV types increased from 14% to 39% and 45%. The overall E6 seroreactivity, however, tended to decline with AK and SCC, especially for HPV8 (21%, 11%, and 2%). HPV DNA positivity was most prevalent in the AK cases (54%) compared with the SCC cases (44%) and the tumor-free controls (40%). Among all participants, there was a positive trend between overall HPV DNA positivity and L1 seropositivity, but not E6 seropositivity. Taken together, our data suggest that cutaneous HPV infections accompanied by detectable HPV DNA in eyebrow hairs and HPV L1 seropositivity, but not E6 seropositivity, are associated with an increased risk of AK and SCC.

Microsurgical replantation of a total scalp avulsion. Case report.

In June 1982, a 9-year-old girl sustained a total scalp avulsion including the entire right eyebrow, the upper third of the skin of the dorsum of the nose and the medial half of the left eyebrow. On both sides the temporal skin was included and in the neck a border of just 1 cm of hairbearing area was left intact. The scalp was successfully replanted. The operative procedures, the pre-operative and post-operative treatment are described and discussed. The follow-up time is 3 years and the sequelae have been found to be minimal.

[The connection of visual evoked potentials with the subjective differences between emotional expressions of the "schematic face"].

Human cortical visual potentials (VEP) were studied to obtain electrophysiological data concerning face discrimination and to compare them with the direct estimates of differences between faces obtained in the previous publications. The present schematic faces varied in curvature of a mouth and/or declination of eyebrows. These features determined the emotional expression of the schematic faces. We recorded the VEP as the response to the instant replacement of one schematic face (referent stimulus) by an other one (test stimulus) rather then to presentation of a single stimulus. Thus we recorded direct electrophysiological differences between schematic faces. A characteristic feature of this approach was the application of the set of functionally connected test stimuli with monotonously increasing values of differences between the referent and test stimuli. In a result of analysis the complex of components P120-N180-P230 in sites O1, O2, P3, P4, T5, T6 was described. Interpeaks amplitudes of the components shows high correlations with subjective differences between the same pairs of stimuli as well as with physical (configurative) differences between stimuli measured as the angles of lines, defining curvature of a mouth and a declination of eyebrows. The highest correlation with subjective estimates of emotional differences between faces was shown by interpeaks amplitudes N180-P230 in sites O1 and P3. In the some time the interpeaks amplitudes P120-N180 in sites O1 and T5 reflected highest correlation between configurative measures and subjective estimates of stimuli differences.

[Tinea faciei in a newborn].

Dermatophytosis in newborns and infants has been regarded as very unusual; a case recently observed of a 2-month-old child affected by tinea faciei is described. Twenty days before, localized erythematous infiltrated patches at the upper left eyelid and eyebrows with papules and pustules, were observed. Rarefaction of the eyebrows and partially loss of the eyelashes associated with scattered areas of erythematopapulous lesions on the face, were seen. Mycological examination was positive and trichophyton rubrum colonies grew. Tinea capitis is more common than tinea faciei in newborn and infants. Differential diagnosis includes: seborrheic dermatitis, atopic dermatitis with associated impetigo, candidiasis, bacterial folliculitis. The interhuman transmission by relatives is the most probable modality of trans-mission, as previously reported in the literature. A widespread of the dermatoses was due to previous application of topical corticosteroids. The lesions resolved completely after treatment with miconazole cream, applied twice daily for three weeks.

[Supraorbital keyhole approach in the treatment of intranasal meningoencephaloceles].

OBJECTIVE: To explore the effect and significance of endoscopic surgery for intranasal meningoencephaloceles via the supraorbital keyhole approach. METHODS: From January 1999 to December 2001, seven cases of intranasal meningoencephaloceles were treated by mini-invasive endoscopic surgery via supraorbital keyhole approach. The operation was carried out through supraorbital keyhole craniotomy approximately 2.0-2.5 cm in diameter following a skin incision just in the eyebrow. The roughly 3-4 cm skin incision began medial to the supraorbital nerve and ended within the lateral edge of the eyebrow. After the frontal lobe was elevated and the skull base defects exposed, the hernia sac was cut off at bone defect and the lump was resected via nasal cavity. The bone defect were repaired using bone flap, self fat, fascia of rectus abdominis muscle and Ear-Brain Glue. RESULTS: Seven patients were successfully treated at initial operation without recurrence and cerebrospinal fluid leak during follow-up 6 months to 2 years after surgery. CONCLUSION: The supraorbital keyhole approach is a safe approach in the hands of experienced surgeons for the mini-invasive treatment of meningoencephaloceles.

[Role of deep temporal lift in the surgical treatment of face aging].

This is a new technique which is completely different from superficial temporal facelift also referred to as the mannequin facelift, which is associated with well known failures and sequelae. The principle of the deep temporal facelift is to approach the deep surface of the musculoaponeurotic plane by remaining in contact with the bone and to attach the ascension of this plane by means of sutures anchored to the temporalis mucsle and fibrin glue. This operation is technically possible due to the use of the dissectable space described by Rouvière between the galea and the temporalis muscle. This virtual, detachable space can only be easily defined by the use of hydrodissection. It is an avascular space with the exception of an unnamed vein located at the external agnle of the orbit. There is no resection of the scalp, no modification of the hairline and no modification of the sideburns. The frontal branch of the facial nerve constitutes the anatomical danger. It is situated anterior to the superficial temporoparietal aponeurosis which prolongs the galea. The two danger zones are the zygomatic arch which is crossed by the frontal nerve trunk and a quadrilateral area between the hairline and the tail of the eyebrow and a vertical line 1.5 cm lateral to the lateral canthus of the eye. The frontal nerve does not constitute any risk during deep temporal facelift, as this procedure remains about 1.5 cm away from the zygomatic arch. The nerve is obviously more superficial in the quadrilateral area. To avoid damaging it, the dissection must be performed under direct vision using a cold light, remaining in contact with the temporalis aponeurosis and avoiding any untimely instrumental manoeuvre between a subaponeurotic metallic instrument and the stretched skin. This technique causes minimal discomfort to the patient. It can be performed under local or general anaesthesia. The hospital stay is brief and the postoperative course is extremely straightforward with a very limited risk of bruising. The deep temporal facelift can be combined with the subpalpebral facelift in order to act on the upper 2/3 of the face. Its essential indications are: drooping of the tail of the eyebrow, drooping of the external canthus of the eye, erows feet.

Opposite imbalances of distal 14q in two unrelated patients.

Two unrelated children were found to have de novo opposite imbalances for distal 14q. One had a 46,XY, del(14)(q24q32) karyotype and exhibited, like three other patients with similar deletions, a distinctive facial appearance including round face, frontal hypertrichosis, thick eyebrows, horizontal narrow palpebral fissures, a short bulbous nose with a flat root, and mild micrognathia. The other had a 46,XX, dir dup(14)(q22----q32) karyotype and stigmata common to patients with comparable duplications, namely high forehead, sparse eyebrows, prominent overlip, gingival hypertrophy, and overriding fingers. Therefore, it is concluded that each of these imbalances originates a distinct syndrome.

[West syndrome as an epileptic presentation in Menkes' disease. Two cases report].

INTRODUCTION: Menkes' disease is a neurodegenerative disorder, recessive X chromosome linked (Xp13.3) that normally codify an ATPasa copper transporter. CASE REPORTS: Case 1: patient exhibit failure in the gastrointestinal copper absorption, which is insufficient to cover the needing during the first twelve months of life. The first case was a 5 months male. His developmental skills were normal until he was 5 months old, when he exhibited visual impairment and failure to continue getting normal developmental skills. One month later he had infantile spasms and hypsarrhythmia in the EEG. He had kinky hair, alopecia zones and copper serum level in 0 microg/dL (range 590-1,180 microg/dL) brain CT scan revealed diffuse cortical atrophy. The patient is 5 years old now, he is free of seizures but he has a severe neurological impairment. Case 2: he is a 7 months old male who developed during the two days of life hypotonia and weak suction. He exhibited later hypertonia, delayed neurological development and infantile spasms, microcephaly, kinky hair, blindness and EEG pattern of hypsarrhythmia. The serum copper level was 84 microg/dL (range: 590-1,180 microg/dL). The brain CT scan showed generalized atrophy, including cerebellum, extradural effusion and MRI with multiple infarcts in different stages. Electronic microscopy revealed pili torti. In both cases the diagnosis was suspected because of the hair and eyebrow features. CONCLUSIONS: We suggest a careful hair and eyebrow clinical exam in those patients with delayed milestones and early epilepsy without a documented etiology, and the copper serum level determination in those patients with suspected disease.

Cutaneous and pulmonary sarcoidosis in association with tattoos.

Sarcoidosis encompasses a heterogeneous spectrum of clinical presentations, including sarcoidosis in association with tattoos. We report the development of cutaneous and pulmonary sarcoidosis in a patient with long-standing eyebrow tattoos whose cutaneous sarcoidosis almost completely resolved when treated with tacrolimus 0.1% ointment. A 70-year-old woman with a 3-year history of an erythematous eruption circumscribing her eyebrow tattoos presented with a chronic, nonproductive cough of 8 months' duration. Skin biopsy results demonstrated naked tubercles consistent with sarcoidosis. Results of radiographs and a computed tomography scan of the chest revealed multiple pulmonary nodules with mediastinal and hilar adenopathy. The results of transbronchial biopsy were consistent with the diagnosis of pulmonary sarcoidosis. Initial treatment with oral prednisone only improved the pulmonary sarcoidosis. The cutaneous sarcoidosis almost completely resolved after the addition of tacrolimus 0.1% ointment.

Optical computer recognition of facial expressions associated with stress induced by performance demands.

Application of computer vision to track changes in human facial expressions during long-duration spaceflight may be a useful way to unobtrusively detect the presence of stress during critical operations. To develop such an approach, we applied optical computer recognition (OCR) algorithms for detecting facial changes during performance while people experienced both low- and high-stressor performance demands. Workload and social feedback were used to vary performance stress in 60 healthy adults (29 men, 31 women; mean age 30 yr). High-stressor scenarios involved more difficult performance tasks, negative social feedback, and greater time pressure relative to low workload scenarios. Stress reactions were tracked using self-report ratings, salivary cortisol, and heart rate. Subjects also completed personality, mood, and alexithymia questionnaires. To bootstrap development of the OCR algorithm, we had a human observer, blind to stressor condition, identify the expressive elements of the face of people undergoing high- vs. low-stressor performance. Different sets of videos of subjects' faces during performance conditions were used for OCR algorithm training. Subjective ratings of stress, task difficulty, effort required, frustration, and negative mood were significantly increased during high-stressor performance bouts relative to low-stressor bouts (all p < 0.01). The OCR algorithm was refined to provide robust 3-d tracking of facial expressions during head movement. Movements of eyebrows and asymmetries in the mouth were extracted. These parameters are being used in a Hidden Markov model to identify high- and low-stressor conditions. Preliminary results suggest that an OCR algorithm using mouth and eyebrow regions has the potential to discriminate high- from low-stressor performance bouts in 75-88% of subjects. The validity of the workload paradigm to induce differential levels of stress in facial expressions was established. The paradigm also provided the basic stress-related facial expressions required to establish a prototypical OCR algorithm to detect such changes. Efforts are underway to further improve the OCR algorithm by adding facial touching and automating application of the deformable masks and OCR algorithms to video footage of the moving faces as a prelude to blind validation of the automated approach.

The effect of age and gender on the anatomic structure of Caucasian healthy eyelids.

OBJECTIVES: To determine the normal values of the eyelid parameters in Caucasians and to describe the effects of age and gender on eyelid and eyebrow anatomy. METHODS: We evaluated the effects of age and gender on eyelid structures in 100 Caucasian volunteers in the Ophthalmology and Anatomy Departments, University of Gaziantep Medical School, Turkey between 2003 and 2004. Forty-five females with mean age of 36 years (5-80 years) and 55 males with mean age of 39 years (3-68 years) participated in the study. We divided subjects into 6 groups according to decades. We measured the palpebral fissure length (PFL), the distance between the pupil center and the upper eyelid (PC-UE) and lower eyelid margin (PC-LE), the eyebrow height (EH), the eye crease height (ECH) and the distance from the reference line to the pupil center (RL-PC), using a reference line through the medial canthus and vertical line through the pupil center of the frontal slides. RESULTS: The measurements of PFL were between 23.5 mm and 29 mm in females, 24.8 mm and 29.1 mm in males and showed a gradual decrease with age. The PC-UE, PC-LE, ECH and RL-PC remained stable throughout life and were identical for both gender. However, EH ranging from 10.7-12 mm in females and 6.5-11 mm in males appeared to have a significant association with gender, which was higher in females than males, but was not effected with age. CONCLUSION: These results demonstrate the importance of providing a normal anatomic relationship that relates to the patient's age and gender.

[A preliminary approach of covering corrugator with rectangle periosteous flap in rhytidectomy].

OBJECTIVE: To evaluate the aesthetic outcome after application of rectangle periosteous flap to cover corrugator. METHODS: On the basis of regular rhytidectomy, a inter-eyebrow periosteous flap is applied to cover the fascia flap of corrugator by means of turnover. This manipulation helps to separate the skin from the muscle and prevent the re-adherence between the skin and muscle. RESULTS: The approach was applied on 15 cases. The follow-up ranged from 6 months to 1 year. The results were satisfactory. CONCLUSIONS: The method is effective in elimination of inter-eyebrow crease.

A new approach to sinus relief.

You suffer from "sinus". Medical treatment isn't getting you better, and now you need surgery. It is better that you be treated surgically today rather than as in the past. Five years ago, surgery meant an incision through the eyebrow, with probable numbing of the forehead, in order to relieve the pain of acute frontal sinusitis; or a long eyebrow incision, or shaving the front part of the scalp and pulling down the forehead skin from the skull was the procedure of choice for chronic disease in these sinuses. If the cheek sinuses were chronically diseased, an incision was made through the gums above the upper teeth, often resulting in numbing of the teeth and marked swelling of the cheek.

[Contribution of videoendoscopy to the surgery of face rejuvenation].

After a delay of several years, endoscopic surgery was introduced into cosmetic surgery of the face in 1992. Initially, its indications were very limited as they only concerned resection of corrugator muscles for the treatment of wrinkles between the eyebrows. Today, it has a new lease of life, with the ambition of replacing classical forehead facelift, by constituting a surgical approach. It replaces the bicoronal incision by means of 5 small incisions which allow detachment and resection of the depressor supercilii muscles similar to that obtained via a bicoronal incision. The skin or scalp excess is absorbed by various techniques. However, endoscopy now has other ambitions: functional fronto-orbital surgery. It tries to achieve a new muscular equilibrium by resection of the depressor supercilii muscles and release of the levators: occipitofrontal muscle. In this way, this new technique achieves an entirely natural repositioning of the eyebrow. The authors describe all of the technical possibilities by endoscopy, median frontal endoscopy, endoscopic frontal facelift, endoscopic extraperiosteal masklift and endoscopic subperiosteal masklift, in the case of the forehead. A special technique has been developed: temporomalar facelift. Lastly, although cervicofacial dissection is technically possible using videoendoscopy, there nevertheless remains a major stumbling block: the excess skin. Based on an experience of more than a hundred cases over a period of more than 18 months, this technique provides extremely encouraging results leading the authors to adopt this technique definitively.

[Pilomatrixoma of the eyelids--clinical differential diagnosis and follow-up. Report of 17 patients].

Pilomatrixomas (benign epithelioma of Malherbe) are often located in the eyelid or eyebrow. Clinically the diagnosis is rarely made. Histologically, especially in partial biopsies, they can be confused with a basal cell carcinoma. We evaluated 17 tumors in 17 patients (19 months to 80 years old). The interval between the diagnosis and total excision of the mass ranged from 4 weeks to 2 years. The clinical misdiagnosis appeared to be related to the patient's age: A cystic dermoid (4x) was considered most frequently in children and sebaceous cyst or atheroma (5x) in patients of middle age. Further diagnoses were as follows: Chalazion, basal cell carcinoma or keratoacanthoma, epidermoid cyst, abscess, papilloma. Nine tumors showed calcifications that might be visible by X-ray examination. After a mean follow-up of 7.6 years no recurrence was observed. Pilomatrixoma should be included in the clinical differential diagnosis of subepidermal solid or cystic tumors, especially in the upper eyelid and eyebrow.
