[Mortality due to acute diarrheal diseases in Cuba, 1987-1993].

Data available at the National Statistics Division of the Ministry of Public Health were studied aimed at characterizing mortality due to acute diarrheal diseases (ADD). 2,934 persons died for this reason in Cuba from 1987 to 1993. Data were tabulated according to age groups, sex, occupation, province of residence, death date and necropsy. Lost potential years of life were calculated and tests of statistical significance were applied to the differences found. It was observed that mortality due to ADD tended to increase during the last 4 years studied, and that 62% of the dead patients were retired and housewives. 16% of the total of the lost potential years of life by all infectious causes was missed due to ADD. There was a predominance of mortality in the group 65 and over. Mortality was higher among males and the most affected provinces were the eastern provinces, Camagüey and the Isle of Youth special municipality.

Dengue haemorrhagic fever in Cuba. II. Clinical investigations.

Clinical and serological studies were carried out on 114 patients admitted to hospital in Havana, Cuba with Dengue Haemorrhagic Fever and Dengue Shock Syndrome (DHF/DSS). Serological confirmation of dengue was obtained in 90% of cases, with 5% of cases primary and 95% secondary. Fever, haemorrhagic manifestations, vomiting and headache were the most frequent signs and symptoms. Among haemorrhagic manifestations, petechiae and vaginal bleeding were reported in a larger number of patients. 21 patients presented shock and, of these, 20 were secondary infections. The disease appeared more frequently in white persons and in women. The aetiopathogenicity of the syndromes is discussed. 95% of the cases could be explained on the basis of the secondary infection hypothesis.

Enterococci spp. isolated from Cuba: species frequency of occurrence and antimicrobial susceptibility profile.

This study investigated the species distribution and antimicrobial resistance among 99 enterococci isolated from hospitalized patients in 12 hospitals in Cuba from October 2000 to September 2001. Species identification was performed by WIDER Automatic System (Francisco Soria Melguizo, Madrid, Spain), and the susceptibility testing was performed by disk diffusion, agar dilution, and E-test methods. Enterococcus faecalis was the most prevalent (85%) species, followed by E. faecium (10%), E. gallinarum (2%), E. casseliflavus (2%), and E. durans-hirae (1%). A higher percentage of resistance to ampicillin (50%), fosfomycin (40%), ciprofloxacin (30%), norfloxacin (20%), and tetracycline (90%) was detected in E. faecium isolates, whereas E. faecalis strains showed higher rates of resistance to erythromycin (52.4%), chloramphenicol (34.5%), rifampicin (62.5%), moxifloxacin (3%), and nitrofurantoin (2.4%). Resistance to glycopeptide was detected in E. faecalis (1.2%) and E. faecium (10%). Thirty-one E. faecalis (37%) and 3 E. faecium (30%) showed a high-level resistance to gentamicin. The results of this work will be very helpful to guide empirical antimicrobial therapy and the implementation of infection control measures in Cuban hospitals.

Out of Cuba: overwater dispersal and speciation among lizards in the Anolis carolinensis subgroup.

Overwater dispersal and subsequent allopatric speciation contribute importantly to the species diversity of West Indian Anolis lizards and many other island radiations. Here we use molecular phylogenetic analyses to assess the contribution of overwater dispersal to diversification of the Anolis carolinensis subgroup, a clade comprising nine canopy-dwelling species distributed across the northern Caribbean. Although this clade includes some of the most successful dispersers and colonists in the anole radiation, the taxonomic status and origin of many endemic populations have been ambiguous. New mitochondrial and nuclear DNA sequences from four species occurring on small islands or island banks (Anolis brunneus, Anolis longiceps, Anolis maynardi, Anolis smaragdinus) and one species from the continental United States (A. carolinensis) are presented and analysed with homologous sequences sampled from related species on Cuba (Anolis allisoni and Anolis porcatus). Our analyses confirm that all five non-Cuban species included in our study represent distinct, independently evolving lineages that warrant continued species recognition. Moreover, our results support Ernest Williams's hypothesis that all of these species originated by overseas colonization from Cuban source populations. However, contrary to Williams's hypothesis of Pleistocene dispersal, most colonization events leading to speciation apparently occurred earlier, in the late Miocene-Pliocene. These patterns suggest that overwater dispersal among geologically distinct islands and island banks is relatively infrequent in anoles and has contributed to allopatric speciation. Finally, our results suggest that large Greater Antillean islands serve as centres of origin for regional species diversity.

Genetic relatedness and antimicrobial resistance determinants among clinical isolates of enterococci from Cuba.

This study describes the genetic relationships and antimicrobial resistance determinants found among 99 clinical isolates of enterococci from 15 different hospitals in Cuba. Pulsed-field gel electrophoresis SmaI analysis demonstrated a high degree of genetic diversity. A limited number of multiresistant Enterococcus faecalis clones, showing resistance to three or more families of antimicrobial agents, were detected simultaneously in different institutions, suggesting inter-hospital circulation of selected clones, and/or selection of particular clones following their introduction into the hospital environment. Antimicrobial resistance determinants, including erm(B), aac(6')-aph(2'), aph(3'), ant(6), vanB (E. faecalis) and vanA (Enterococcus faecium) were detected by PCR in various isolates.

Ambiguities in sound-duration selectivity by neurons in the inferior colliculus of the bat Molossus molossus from Cuba.

This study examines duration selectivity in auditory neurons of the inferior colliculus of the bat Molossus molossus (Molossidae, Chiroptera) from Cuba. Three main types of duration selectivity, short-, band-, and long-pass, as previously described in other species, are present in M. molossus. The range of best durations in the inferior colliculus of this species approximates the durations of their echolocation calls, suggesting that, as has been shown in other species of bats and frogs, the filter mechanism that produces duration tuning is selective for species-specific sounds relevant to behavior. Duration coding in M. molossus is not unambiguous because approximately 30% of the short- and band-pass neurons respond best to two different stimulus durations. This bimodal duration selectivity could be explained by time delayed excitatory inputs that coincide with an inhibitory rebound. In addition, the effect of stimulus intensity on duration selectivity was tested. For most of the neurons (78%), duration selectivity was affected by absolute sound pressure level and/or small changes of sound pressure. In this respect, the processing of stimulus duration by collicular neurons seems to be more complex in M. molossus than in other species studied so far.

Ultrasound measurement using CUBA clinical system can discriminate between women with and without vertebral fractures. Contact Ultrasound Bone Analyzer.

The purpose of this study was to assess the ultrasound (US) parameters measured by Contact Ultrasound Bone Analyser (CUBA) clinical system for discriminating the subjects with vertebral fractures from those without vertebral fractures. The subjects consisted of 114 postmenopausal women over age 50 (mean +/- SD: 72.2 +/- 8.7). Seventy-three had vertebral fractures (fracture group) and 41 had no vertebral fractures (control group). Values of all US parameters and bone mineral density (BMD) in the fracture group were significantly lower than those in the control group, even after adjusting for age. Areas under the receiver operating characteristic (ROC) curve was 0.768 +/- 0.056 (mean +/- SE) for broadband ultrasound attenuation (BUA) 0.828 +/- 0.045 for velocity of sound (VOS), 0.707 +/- 0.058 for lumbar spine, 0.872 +/- 0.050 for femur neck, 0.790 +/- 0.050 for trochanter, and 0.695 +/- 0.060 for Ward's triangle. There were no significant differences among the areas under the ROC curves in BUA, VOS, lumbar spine, femur neck, trochanter, and Ward's triangle. US parameters (BUA and VOS) had the same discriminatory power as spine and hip BMD for evaluating the vertebral fracture risk. As far as disadvantages of the use of BMD measurement against US measurement, US measurement is potentially useful for screening of vertebral fractures. However, the bias concerning the propotion of the number of patients with or without vertebral fractures could not be neglected in this study.

[Pathohistology of allotransplanted kidneys. A 10-year report from Santiago, Cuba].

Of altogether 201 allotransplantations in Santiago de Cuba the material was histologically examined 328 times. Transplant biopsies and explants most frequently revealed a rejection nephropathy, in which case the forms of the prognostically unfavourable rejection vasculopapty were more frequent than the types of the tubulointerstitial rejection nephropath which are better to be influenced therapeutically. A histologically diagnosed acute failure of the kidney was casually based on (reversible) lesions of the donor kidney, infections and haemorrhages after the transplantation as well as on the combination with a rejection. A transplant pyelonephritis and transplant glomerulonephritis were rarely observed. Complete necroses of a transplant biopsy cylinder allow the urgent suspicion of an obliterating rejection vasculopathy with secondary transplant necroses.
