A method of assessing the efficacy of memory rehabilitation techniques using a "real-world" memory task: learning a computer language.

A software program developed to assess the effectiveness of memory rehabilitation techniques is described. This project addresses current weaknesses in the research literature on memory rehabilitation: 1) remediation strategies are evaluated in terms of performance on a practical, real-world memory task, rather than a laboratory learning task; 2) the program allows maximum flexibility in assessing a wide variety of remediation strategies and training schedules; and, 3) the software, which provides sensitive outcome measures and detailed feedback about performance, is adaptable to the needs of patients with widely varying degrees of memory impairment. The results of a demonstration project are included to illustrate the use of this technology in determining the usefulness of different mnemonic strategies for patients with specific patterns of memory deficit. The importance of assessing maintenance of rehabilitation training over time is stressed.

Dynamite: a flexible code generating language for dynamic programming methods used in sequence comparison.

We have developed a code generating language, called Dynamite, specialised for the production and subsequent manipulation of complex dynamic programming methods for biological sequence comparison. From a relatively simple text definition file Dynamite will produce a variety of implementations of a dynamic programming method, including database searches and linear space alignments. The speed of the generated code is comparable to hand written code, and the additional flexibility has proved invaluable in designing and testing new algorithms. An innovation is a flexible labelling system, which can be used to annotate the original sequences with biological information. We illustrate the Dynamite syntax and flexibility by showing definitions for dynamic programming routines (i) to align two protein sequences under the assumption that they are both poly-topic transmembrane proteins, with the simultaneous assignment of transmembrane helices and (ii) to align protein information to genomic DNA, allowing for introns and sequencing error.

General purpose model and a computer program for combined segregation and path analysis (SEGPATH): automatically creating computer programs from symbolic language model specifications.

A general purpose model and a flexible computer program, called SEGPATH, have been developed to assist in the creation and implementation of a variety of genetic epidemiological models. SEGPATH is a computer program which can be used to generate programs to implement linear models for pedigree data, based upon a flexible, model-specification syntax. SEGPATH models can perform segregation analysis, path analysis, or combined segregation and path analysis using any user-specified path model and can be structured to analyze any number of multivariate phenotypes, environmental indices, and/or measured covariate fixed effects (including measured genotypes). Population heterogeneity models, repeated-measures models, longitudinal models, auto-regressive models, developmental models, and gene-by-environment interaction models can all be created under SEGPATH. Pedigree structures can be defined to be arbitrarily complex, and the data analyzed with programs generated by SEGPATH can have any missing value structure, with entire individuals missing, or missing on one or more measurements. Corrections for ascertainment can be done on a vector of phenotypes and/or other measures. Because the model specification syntax is general, SEGPATH can also be used in non-genetic applications where there is a hierarchical structure, such as longitudinal, repeated-measures, time series, or nested models. A variety of applications are demonstrated.

Preparing the educational interpreter. A survey of sign language interpreter training programs.

Despite rapid growth in the field of educational interpreting little is known about the formal training of educational interpreters. This gap in the research is the focus of this study. A questionnaire was sent to the directors of 50 interpreter training programs nationwide asking for information about their course work in educational interpreting and related areas; instruction in signed English systems; and the directors' opinions on certain ethical and professional questions facing the educational interpreter. The results suggest that graduates of interpreter training programs who obtain employment as public school interpreters are not adequately prepared. Training programs provide few courses on the education of deaf children, on the language systems used, and on issues specific to classroom interpreting. The directors of these programs overwhelmingly support the development of guidelines on the educational interpreter's role. They show some support for the "interpreter as tutor" role but are equivocal about the development of a special certification for educational interpreters.

A comprehension monitoring program for language-impaired children.

Four language-impaired children (5:10-8:2) participated in an intervention program designed to improve their comprehension monitoring skills. A set of audiotaped adequate and inadequate messages was presented through the baseline and treatment phases of a multiple-baseline across subjects design, with the dependent variable being the percentage of inadequate messages that were verbally queried. Results revealed rapid increases in verbal queries of five types of inadequate messages in conjunction with progress through the comprehension monitoring treatment program. High levels of comprehension monitoring were maintained following a nontreatment interval of 3-6 weeks. Facilitation of comprehension monitoring skills appears to be a feasible and fruitful adjunct to traditional language intervention objectives.

Neural dynamics of speech and language coding: developmental programs, perceptual grouping, and competition for short-term memory.

A computational theory of how an observer parses a speech stream into context-sensitive language representations is described. It is shown how temporal lists of events can be chunked into unitized representations, how perceptual groupings of past item sublists can be reorganized due to information carried by newly occurring items, and how item information and temporal order information are bound together into context-sensitive codes. These language units are emergent properties due to intercellular interactions among large numbers of nerve cells. The controlling neural networks can arise through simple rules of neuronal development: random growth of connections along spatial gradients, activity-dependent self-similar cell growth, and competition for conserved synaptic sites. Within these networks, a spatial frequency analysis of temporally evolving activity patterns leads to competitive masking of inappropriate list encodings in short term memory. The neurons obey membrane equations undergoing shunting recurrent on-center off-surround interactions. Several design principles are embodied by the networks, such as the sequence masking principle, the long-term memory invariance principle, and the principle of self-similar growth.

Language sample analysis in the 21st century.

Time requirements inherent in transcription and analysis of spontaneous language samples represent a significant barrier to the regular use of language sample analysis in clinical settings. Taking advantage of the options provided by new large, fast, and affordable personal microcomputers, two language analysis programs, the Systematic Analysis of Language Transcripts and The Child Language Data Exchange System have been developed to provide automated analysis of a wide range of language indices, as well as tools designed to reduce and simplify the time-intensive process of transcribing and analyzing children's language samples from both typical and clinical populations. In this article we provide a historical context for the development of these language analysis programs and a brief introduction to the transcription editors and language sample databases for both systems. In addition, a case study highlighting the interactive use of both analysis systems is provided.

Community-based Spanish language arthritis education program: a randomized trial.

OBJECTIVES: To determine 4-month and 1-year health-related outcomes of a 6-week, lay-led, and community-based arthritis self-management program for Spanish-speaking participants and to determine the role of self-efficacy in predicting health status for this population. METHODS: Three hundred and thirty one subjects were randomized to the program or to a 4-month wait list control group. One hundred ninety eight subjects continued in a 1-year longitudinal study. Data were collected via mailed questionnaires with telephone follow up. RESULTS: At 4 months, treatment subjects, compared with controls, demonstrated positive changes in exercise, disability, pain, and self-efficacy (P < 0.05). At 1 year, compared with baseline, treatment subjects demonstrated improvements in exercise, general health, disability, pain, self-efficacy, and depression (P < 0.05). Baseline and 4-month changes in self-efficacy predicted health status at 1 year. CONCLUSIONS: Spanish-speaking participants of an arthritis self-management program demonstrate short- and long-term benefits (improved health behaviors, health status, and self-efficacy).

An adapted language training strategy in the treatment of an electively mute male child.

The present study investigated the usefulness of an Adapted Language Training Program in the treatment of an electively mute seven-year-old male. Training utilized a multiple baseline across behaviors single subject design. Four phases of treatment focused on non-verbal attending, verbal imitative responding, and functional language responding. Systematic attempts at promoting generalization of verbal responses across situations and persons were employed throughout the language program. A 90% appropriate response criterion was reached for all measures. Non-verbal attending and imitative verbal responding remained above criterion at the termination of treatment, but functional language responding regressed to below criterion. Generalization of verbal responses was noted outside of the training sessions.

Real-time computer simulations of excitable media: JAVA as a scientific language and as a wrapper for C and FORTRAN programs.

We describe a useful setting for interactive, real-time study of mathematical models of cardiac electrical activity, using implicit and explicit integration schemes implemented in JAVA. These programs are intended as a teaching aid for the study and understanding of general excitable media. Particularly for cardiac cell models and the ionic currents underlying their basic electrical dynamics. Within the programs, excitable media properties such as thresholds and refractoriness and their dependence on parameter values can be analyzed. In addition, the cardiac model applets allow the study of reentrant tachyarrhythmias using premature stimuli and conduction blocks to induce or to terminate reentrant waves of electrical activation in one and two dimensions. The role of some physiological parameters in the transition from tachycardia to fibrillation also can be analyzed by varying the maximum conductances of ion channels associated with a given model in real time during the simulations. Establishing pre-language signalling behaviour with profoundly mentally handicapped students: a preliminary investigation.

The purpose of this investigation was to ascertain the effectiveness of a specific pre-language communication programming technique on the development of pre-language communication signalling behaviour in a group of profoundly mentally handicapped students. Results indicate that the procedure produced desired effects. Problems related to defining pre-language communication behaviours and communicative stimuli for this population, as well as issues pertaining to generalization, were discussed. Concluding comments stress the preliminary nature of the results and the need for future research in the validation of pre-language communication programming techniques.

Design and implementation of a relational database used in the management of patients with retinoblastoma.

Clinical data are most useful in the management of patients with complex medical problems if they are accurate, reliable, and easily accessible by physicians and the medical community at large. Furthermore, the data are most valuable when they can be shared among cooperating institutions. We describe a computer system which exhibits a uniform taxonomy, an integrated on-line dictionary of clinical terms, a coherent temporal layout, and persistent spatial integrity with regard to the values of the variables. The system is user friendly and provides real time data access which can be retrieved by structured query language or may be programmed to be used as part of an international network in the management of patients with retinoblastoma, a malignant and potentially fatal tumor of childhood. Furthermore, because of its design flexibility, this system provides for potential application to other ophthalmic disorders, such as malignant uveal melanoma, and other areas of medicine as well.

Computer assisted medical diagnosis using the Web.

The ADM (Aide au diagnostic Medical) project was started 15 years ago and was the first telematic project for physicians in France using the MINITEL terminal. The knowledge base contains information on more than 10000 diseases from all pathological fields, using more than 100000 signs or symptoms. The ADM system has two main functionalities for physicians: consultation of diseases descriptions and list of diseases containing one or more symptoms. The ADM knowledge base is supported by a relational database management system (DBMS ORACLE) and we developed a Web interface using the Perl language to produce HTML pages for the web server. We will describe our experience on redesigning a large existing medical knowledge base for diffusion on the web Internet.
