Effectiveness of the male latex condom: combined results for three popular condom brands used as controls in randomized clinical trials.

BACKGROUND: Although public health programs invest heavily in the male latex condom, its efficacy in preventing pregnancy and sexually transmitted disease has been based primarily on in vitro and retrospective studies. METHODS: We combine the results from two randomized, controlled contraceptive efficacy trials that used commercial latex condoms brands (Ramses Sensitol, LifeStyles, Trojan-Enz) in the control arms. Combining data from the two studies, we obtained longitudinal data covering 3526 menstrual cycles contributed by approximately 800 couples who used latex condoms exclusively for up to six menstrual cycles. Both trials also collected 3715 detailed breakage and slippage reports from the first five study condom uses. The second trial also tested 243 postcoital vaginal samples collected after the first study condom use for the presence of prostate-specific antigen (PSA) and spermatazoa. RESULTS: The combined clinical breakage rate for the first five condom uses was 0.4% for the three latex brands and the combined clinical slippage rate was 1.1%. The combined six-cycle typical-use pregnancy rate for the latex condoms was 7.0% (95% confidence interval 5.0-9.0). The combined six-cycle consistent-use pregnancy rate was 1.0% (95% confidence interval 0.0-2.1). PSA was detected in only 1.2% of postcoital vaginal samples collected after the first use of an intact study condom. There were no differences in performance or efficacy among the three latex brands tested. CONCLUSIONS: The male latex condoms rarely broke or slipped off during intercourse and provided high contraceptive efficacy, especially when used consistently. Risk of semen leakage from intact condoms was very low.

Species identification using sequences of the trnL intron and the trnL-trnF IGS of chloroplast genome among popular plants in Taiwan.

Forensic botanical comparison can be hampered by the lack of appropriate DNA databases. While DNA sequence databases for many mitochondrial loci have been established for the identification of animal species, less is known regarding the genomes of plants. We report on the use of the trnL intron and the trnL-trnF intergenic spacer (IGS) in the chloroplast genome and establish a DNA sequence database for plant species identification. The DNA sequences at these two loci from commonly encountered plants, including monocots and dicots, were aligned to establish a DNA database of local plants. The database comprises 373 individual sequences representing 80 families, 206 genera and 269 species. These plant species can be grouped to species level using both sequence and length polymorphisms at these loci. To validate the database for future forensic purposes, we sequenced 20 blind samples and searched the local database and the databases of GenBank and EMBL. Fifteen of these 20 samples used in blind trial testing matched their respective species from our local DNA database but only 6 matched species registered in the GenBank and EMBL databases. The sequences of two species used in the blind trial did not match any sequence registered in any of these databases. Cluster analysis was performed to demonstrate the family and genus distribution of samples. Neighbor-joining trees of the two DNA regions from 70 samples of the local database and 10 of the species used in the blind trials were constructed and clustered to both family and genus. The bootstrap values of the trnL intron were higher than most of those of the trnL-trnF IGS. The sequence database described in this study can be used to identify plant species using DNA sequences of the trnL intron and trnL-trnF IGS of chloroplast genome and illustrates its value in plant species identification.

Effect of storage on the chemical composition and biological activity of several popular South African medicinal plants.

The in vitro biological activity of nine frequently used medicinal plants in South Africa was assessed and re-assessed after various lengths of storage. The plants investigated were Alepidea amatymbica, Leonotis leonurus, Drimia robusta, Vernonia colorata, Merwilla natalensis, Eucomis autumnalis, Bowiea volubilis, Helichrysum cymosum and Siphonochilus aethiopicus. Water, ethanol and hexane extracts of fresh, 90-day-old and 1-year-old material were assayed for antibacterial activity against four strains of bacteria and for COX-1 inhibition activity. TLC-fingerprints of the fresh and stored extracts were produce to document chemical changes. Alepidea amatymbica, Eucomis autumnalis, Helichrysum cymosum, Leonotis leonurus, Siphonochilus aethiopicus and Vernonia colorata were investigated further as to the effect of 1 year's storage. Elevated temperature and humidity (55 degrees C and 100% relative humidity) were used to accelerate the ageing process of Alepidea amatymbica, Leonotis leonurus and Vernonia colorata plant material for further investigation. The TLC-fingerprints indicated that there was chemical breakdown during storage in certain species. The degree of changes in biological activity and chemistry due to storage were species-specific. In general, antibacterial activity was retained in most species while COX-1 inhibition activity was lost rapidly.

Hepatoprotective effects of Pittosporum neelgherrense Wight&Arn., a popular Indian ethnomedicine.

The stem bark of Pittosporum neelgherrense Wight&Arn. is used by the Kani and Malapandaram tribes of Kerala as an effective antidote to snake bite and for the treatment of various hepatic disorders. In the present study, the effect of the methanolic extract of the stem bark of Pittosporum neelgherrense was studied against carbon tetrachloride (CCl(4))-, d-galactosamine (D-GalN)- and acetaminophen (APAP)-induced acute hepatotoxicity in Wistar rats. Significant hepatoprotective effects were obtained against liver damage induced by all the three liver toxins, as evident from decreased levels of serum enzymes, glutamate oxaloacetate transaminase (SGOT), glutamate pyruvate transaminase (SGPT) and an almost normal architecture of the liver in the treated groups, compared to the toxin controls. Thus the present study provides a scientific rationale for the traditional use of this plant in the management of liver diseases.

Purification of antibacterial agents from Tragia involucrata--a popular tribal medicine for wound healing.

Tragia involucrata has been widely used in traditional systems of medicine for a variety of diseases. In the present study, in vitro antibacterial properties of nine different compounds including vinyl hexylether, shellsol, 2,4-dimethyl hexane, 2-methylnonane and 2,6-dimethyl heptane were isolated from the leaf of Tragia involucrata studied against Escherichia coli, Proteus vulgaris and Staphylococcus aureus using the disc-diffusion method at 50 microg/ml concentrations. The compound vinyl hexylether showed a broad spectrum of activity. The highest activity was found in shellsol (50 microg/ml) against Proteusvulgaris and Staphylococcus aureus. Minimum inhibitory concentrations were determined for the effective compounds (MICs 2.5-40 microg/ml), shellsol and vinyl hexylether showed inhibitory action at the lowest dilution (10 microg/ml) than 2-methylnanone. Shellsol inhibited the growth of Staphylococcus aureus very effectively than the other compounds. These compounds showed bactericidal effects against all the tested bacteria (MBC, 12.25 microg/ml). However, the compound shellsol showed effective killing of wound causing bacteria (Staphylococcus aureus). So, the study was focused on the constituent to evaluate wound healing in rat model. Rats that received 50 microg/kg, b.w. of shellsol showed complete healing after 24 days. Histological examination revealed an increase in the fibroblast, neovascularization, granulation and thickness of scar tissue after the treatment of shellsol as compared to control. The topical application of shellsol did not cause any toxic response on rat skin. Thus, the antibacterial properties of the constituents give some scientific basis to its usage in traditional medicine.

Pleurodesis in recurrent pleural effusions: a randomized comparison of a classical and a currently popular drug.

STUDY OBJECTIVES: Pleurodesis is generally regarded to give the best palliation in recurrent pleural effusion. Talc is now increasingly recommended but in our department quinacrine has been used successfully for many decades with good results and only minor side effects. It was therefore decided to make a prospective randomized clinical study comparing quinacrine (500 mg) with talc (5 g) with regard to efficacy and safety. METHODS: One hundred and ten eligible consecutive patients with recurrent and or malignant effusions, from 1 March 1996 till 31 March 1999 were randomized to chemical pleurodesis with either talc or quinacrine through a chest drainage tube after medical thoracoscopy. Patients were evaluated with chest radiographs at 2 weeks and 2, 4, and 6 months after pleurodesis. RESULTS: Chi-square test showed 84% power to distinguish between the groups and 10% to determine the primary endpoint. Primary success (fluid production < 50ml/24h within the first 6 days) was 96% of 56 patients with talc and 91% of 54 patients with quinacrine, a non-significant difference (P = 0.46). Quinacrine patients needed a repeated treatment in 31% (17 patients) and talc patients in 7% (4 patients) (P < 0.05). Side effects were minor with no significant difference between the substances. CONCLUSIONS: Both substances are effective. Talc treatment had less often to be repeated. This indicates that the recommendation of talc for pleurodesis is well founded. However, quinacrine is a good alternative.

Selectivity assessment of popular stationary phases for open-tubular column gas chromatography.

The solvation parameter model is used to study the influence of temperature and composition on the selectivity of nine poly(siloxane) and two poly(ethylene glycol) stationary phase chemistries for open-tubular column gas chromatography. A database of system constants for the temperature range 60-140 degrees C was constructed from literature values with additional results determined for HP-50+, DB-210, DB-1701, DB-225 and SP-2340 columns. The general contribution of monomer composition (methyl, phenyl, cyanopropyl, and trifluoropropyl substituents) on the capacity of poly(siloxane) stationary phases for dispersion, electron lone pair, dipole-type and hydrogen-bond interactions is described. The selectivity coverage of the open-tubular column stationary phases is compared with a larger database for packed column stationary phases at a reference temperature of 120 degrees C. The open-tubular column stationary phases provide reasonable coverage of the range of dipole-type and hydrogen-bond base interactions for non-ionic packed column stationary phases. Deficiencies are noted in the coverage of electron lone pair interactions. None of the open-tubular column stationary phases are hydrogen-bond acids. The system constants are shown to change approximately linearly with temperature over the range 60-140 degrees C. The intercepts and slopes of these plots are used to discuss the influence of temperature on stationary phase selectivity.

LSD. Its rise, fall, and renewed popularity among high school students.

LSD is making a comeback nationwide among Caucasian middle-class high school and college students. Compared to the mean values during the years 1985 to 1990, an approximately 25% increase occurred in lifetime-, annual-, and monthly-use rates of LSD, in 1993, according to the annual NIDA high school senior survey. In 1992, an increase in LSD-related arrests occurred, as did seizures of larger quantities of consumer-ready LSD. The number of LSD-related emergency room visits by adolescents increased, and intermittent LSD-related violent behavior, including suicide, homicide, and accidental death, were higher than in the previous 5-year period. Fewer high school students attribute great danger in trying LSD once or twice. The painfully learned lessons about LSD seem, sadly, destined to be relearned by the youth of the 1990s.
