Acoustic effects of lightning.

The effects of lightning on the audiovestibular apparatus vary with the degree of injury. This depends on whether the individual is struck directly or indirectly by lightning. We reported two cases where lightning has caused trauma to audiovestibular apparatus and have reviewed the available literature. One patient was struck directly by lightning leading to rupture of the tympanic membrane and a conductive hearing loss. The other patient was struck indirectly via telephone cable and had a mixed hearing loss with tympanic membrane intact.

[Is cardiac monitoring required after electrical injuries? (author's transl)].

Professional or spare time contacts with electrical appliances are, due to careless handling or technical inadequacies, potentially always associated with electrical injuries. The effects of the electric current on the body depends mainly on voltages: skin and muscle lesions predominate in high voltage injuries. Out of 23 patients, 19 with electrical injuries and 4 with lightning accidents, 18 showed normal ECGs at the time of hospital admission. There were no changes at follow-up. The remaining 5 patients had cardiac arrhythmias which disappeared after 1 1/2 to 36 hours using bedrest and symptomatic treatment only. Cardiac longterm monitoring is thus not required after electrical injuries.

Bilateral macular cyst after lightning visualized with optical coherence tomography.

The authors report a 14-year-old girl who was struck by lightning with bilateral macular cyst that was diagnosed with optical coherence tomography (OCT). Her fundus examination revealed bilateral macular lesions that simulated macular holes. The Watzke-Allen sign was negative in the right eye and positive in the left eye. Fluorescein angiography showed a foveal window defect in both eyes. OCT demonstrated a small foveal cyst evident as a hyporeflective lucency, visible just anterior to the retinal pigment epithelium/choriocapillaris complex in both eyes. OCT seems to be useful in the evaluation of lighting maculopathy to rule-out full-thickness macular hole in these cases, and avoid unnecessary surgery.

Lightning risk.

Since 1969, ninety-four persons were reportedly killed annually by lightning in the United States. Reported injuries were almost three times the fatalities. These casualties were not evenly distributed across the country. The purpose of this paper was to examine the expected distribution of these occurrences using known ecologic risk factors. Lightning fatalities and injuries were related to thunderstorm days, population characteristics, and land area. The resulting predicted casualties were mapped and indicate where preventive measures are needed. This same technique could be employed to examine most natural hazard risks using existing data.
