Case study: analyzing the market using DRGs and MDCs.

Union Memorial Hospital, in response to the growing need and pressure to plan, developed a strategic planning model whereby it could assess its competitive position in the marketplace, examine the strength of hospitals in geographic areas targeted for future growth, and analyze options available to improve its competitive standing. The model consists of a database that lists by DRG and MDC the number of cases at Union Memorial and area hospitals. This database can be used to analyze opportunity cases, physician specialties, and strategic alternatives to be considered for improved competitive standing.

Complex ankle instability.

Complex ankle instability can be linked to a variety of causes. It is essential to evaluate these patients adequately, including a clinical history, evaluation of prior records and diagnostic studies (if available), and current radiographic studies. With appropriate evaluation and assessment, the underlying cause can [figure: see text] be determined and a reasonable surgical plan developed. With appropriate planning, adequate improvement usually can be expected.

PSRO: current status of the professional standards review organization program.

Since Public Law 92-603 was enacted in October 1972 considerable progress has been made in the establishment of Professional Standards Review Organizations (PSROs) for the purpose of determining the necessity, appropriateness, and quality of medical care provided beneficiaries of the major programs authorized in the Social Security Act. Sixty-five conditional PSROs are implementing review in acute care hospitals in their geographic area, and 55 planning groups are developing plans to qualify for conditional PSRO designation. The PSRO hospital review system is based on three interrelated review mechanisms. These are concurrent review, which includes admission certification, and continued stay review through discharge; medical care evaluation studies; and analysis of hospital, practitioner, and patient profiles. This article describes the review system in some detail and the potential opportunities the PSRO program offers to occupational therapists.

National nutrition planning in developing countries via gaming-simulation.

A nutrition game designed for the Food Policy and Nutrition Division of the Food and Agriculture Organization of the United Nations to aid in planning national nutrition education programs in Third World countries is described. The Simulated Nutrition System Game allows high-level ministerial staff in developing countries to discuss, via a common language created by SNUS I, the issues, problems, and complexities of national nutrition programs.

Guidelines for pragmatic assessment for health planning in developing countries.

Although the primary health care strategy implemented since the Alma Ata declaration of 'health for all' appears to have contributed to improvements in selected health outcomes, the current ad hoc approach to health assessment and planning has impeded more substantial gains. A comprehensive yet pragmatic framework for country-level health programmers is needed that would permit consideration of the multiple steps involved in policy formulation and implementation. In the present paper, drawing upon an epidemiologic model (Iterative Measurement Loop) and an economic model (Cost-Effectiveness Analysis), we present guidelines for a pragmatic assessment for health planning. A format is provided for the conduct of these tasks which is operational in nature, is specific to the target country (or relevant region), can simultaneously consider multiple interventions, and is comprehensible to persons without sophisticated medical and/or economic backgrounds. Such a format enables articulation and consideration of local concerns as well as national and global considerations.
