How global brands compete.

It's time to rethink global branding. More than two decades ago, Harvard Business School professor Theodore Levitt argued that corporations should grow by selling standardized products all over the world. But consumers in most countries had trouble relating to generic products, so executives instead strove for global scale on backstage activities such as production while customizing product features and selling techniques to local tastes. Such "glocal" strategies now rule marketing. Global branding has lost more luster recently because transnational companies have been under siege, with brands like Coca-Cola and Nike becoming lightning rods for antiglobalization protests. The instinctive reaction of most transnational companies has been to try to fly below the radar. But global brands can't escape notice. In fact, most transnational corporations don't realize that because of their power and pervasiveness, people view them differently than they do other firms. In a research project involving 3,300 consumers in 41 countries, the authors found that most people choose one global brand over another because of differences in the brands'global qualities. Ratherthan ignore the global characteristics of their brands, firms must learn to manage those characteristics. That's critical, because future growth for most companies will likely come from foreign markets. Consumers base preferences on three dimensions of global brands--quality (signaled by a company's global stature); the cultural myths that brands author; and firms' efforts to address social problems. The authors also found that it didn't matter to consumers whether the brands they bought were American--a remarkable finding considering that the study was conducted when anti-American sentiment in many nations was on the rise.

Curare: the South American arrow poison.

The history of curare is both curious and convoluted. A product of South American culture it emerged in the sixteenth century from the mists of antiquity at the same time as quinine, coca, and chocolate. Like quinine, at first came the extract but no plant, and later the plant but no chemical compound. It took more than 300 years and the efforts of many explorers and scientists to resolve the problem. These included Condamine, Humboldt, Brodie, Waterton, Bernard, Dale, Walker, and King. Finally, the pure compound d-tubocurarine was isolated from the liana Chondrodendron and synthesised. Its specific physiological action was blockade of the effect of acetylcholine at the neuro-muscular junction. Such a paralytic poison could be used to kill oneself or others. The bizarre plot to kill the Prime Minister, Lloyd George, during the First World War is described. Fortunately this nefarious plan was thwarted by the Secret Service!

[Diet as an important factor in the etiology of dental erosions].

A questionnaire study was carried out on 120 healthy subjects of both sexes. The age of the examined population was between 18-30 years. The subjects were divided into 2 groups comprising the same number of participants. The first and second group consisted of patients with and without enamel erosions respectively. The consumption of food like fruit juice, apples, citrus fruit, coca-cola, fizzy drinks and yoghurt and its influence on the frequency of enamel erosions were estimate. The analysis of the obtained results pointed to an existing relationship between diet and the existence of enamel erosions.

[Atopic dermatitis. Epidemiology].

It was in 1923 that Coca and Cooke made a distinctive entity of a group of diseases of allergy, including asthma, and hay fever. They remarked that these affections were frequently associated and/or preceded by eczema localised to certain regions (flexor folds), sometimes generalised, perhaps appearing suddenly, from the third month in infants. In effect, the majority of constitutional eczema is in infants of 6 months, 75% of the cases are in one-year-old children. There is recovery in 5 years following the start of the affection in 90% of cases. Only 5% of cases persist to adulthood.

Effect of five staining solutions on the colour stability of two acrylics and three composite resins based provisional restorations.

The effect of coffee, tea, coca-cola, orange-juice and red wine on the colour stability of acrylic and composite based provisional materials were evaluated. Two acrylic resins and three composite resins were studied. 48 standardized specimens for each provisional material were prepared. Each group was divided into 6 subgroups. Specimens from each group were immersed in staining solutions at room temperature for 30 days. Red wine and tea caused the most significant colour changes and orange juice showed the least significant colour changes. deltaE of all of the provisional restorations materials was changed after the immersion in all of the staining solutions during the experimental process.

Health behavior among Lithuania's adolescents in context of European Union.

AIM: To compare health behavior patterns of adolescents in Lithuania with health behavior of adolescents in European Union (EU) in 2001/2002. METHODS: The study was carried out in conformity with the methodology of Health Behavior in School-aged Children: a World Health Organization cross-national study. Three country representative samples of schoolchildren, aged 11, 13, and 15, were surveyed in 25 EU countries and regions in 2001/2002 school year. The study instrument was a standardized questionnaire that included questions on alcohol consumption, smoking, illegal drugs use, physical activity, and other patterns of health behavior. The rate or mean values of the targeted health behavior of Lithuanian students were assessed and compared to those calculated as an average for the EU countries and regions. RESULTS: We found an early onset of alcohol consumption among Lithuanian boys. Lithuanian boys and girls across all age groups reported being drunk two and more times more often than their peers from other EU member states. Lithuanian 15-year-old boys smoked more often that did their European peers, while girls smoke more rarely. The prevalence of drugs (marihuana group) use among Lithuanian students is relatively low: the prevalence of drug use among 15-year-olds in Lithuania is two times lower than the prevalence in other EU countries and regions (11.2% vs 24.5% for boys and 4.5% vs 18.3% for girls, respectively). Physical activity of Lithuanian adolescents is rather high in comparison with their EU peers, although many students watch television for > or =4 hours a day. Lithuanian students did not eat sweets often, or drank soft drinks (Coca Cola and other), and they ate fruits and had breakfast every school day. With respect to hygienic habits, approximately only one in 3 boys and every second girl brushed their teeth more than once a day. CONCLUSIONS: There are health behavior differences between adolescents in Lithuania and those in other EU countries. The disparities among health behavior of young people in EU countries and regions require Lithuanian and EU health policy to develop initiatives aimed at decreasing health behavior inequalities.

Staining of light-cured aesthetic resin restorative materials by different staining media: an in vitro study.

Aesthetic restorative resins have been characterised by clinical discolouration. New light-cured anterior and posterior resins are currently available to the profession and this study investigated the in vitro colour stability at 24 hours and 7 days of ten such resins in three different staining media. Disc specimens were prepared and stained at 37 degrees C and 100 per cent R. H. and then evaluated by double blind visual assessment. All the materials undergo some staining in coffee and red wine. Coca-Cola, however, does not cause any staining of the restorative resins. Adaptic II is the most stain-resistant material and most of the staining occurs within the first 24 hours after immersion in the staining media.

Disaster relief, inc.

When disaster strikes, many corporations respond generously. After the 2004 tsunami, for instance, U.S. firms alone contributed more than half a billion dollars in cash and in-kind donations. But a host of reactive efforts don't produce the best results-and may even get in the way. To make the most of their humanitarian efforts, companies need to address two fundamental questions: What kind of aid do we want to contribute--philanthropic (money and in-kind donations) or integrative (backroom, operational assistance)? And how do we want to contribute it--by working one-on-one with a single agency or by joining a consortium? The permutations of those two decisions lead to four different approaches, each with its own strengths and challenges. Single-company philanthropic partnerships work well when there's a good match between what a company wants to contribute and what an agency needs, as with Coca-Cola's donations of water to the Red Cross. More diffuse, but also potentially more effective, are the benefits of joining a multicompany philanthropic partnership, which enables the resources of many firms to be matched to the missions of many agencies. More difficult to establish but more fundamental in its impact is a single-company integrative partnership, in which a corporation works to improve the way an aid agency operates, as the logistics giant TNT has done to help the distribution efforts of the World Food Programme. And most difficult to implement--but potentially most effective-is a multicompany integrative partnership, which brings to bear the collective best practices of many companies to improve the response capabilities of multiple agencies. It's easy to see why the image of a relief worker carrying a sack of grain delivers an emotional wallop, but the behind-the-scenes work of process enhancement is just as crucial to humanitarian efforts. The sooner executives realize this, the better positioned the world will be to respond to global disasters.

[Status of dentition in Bolivia according to altitude, sex and age].

The author's quantitative evaluation of the condition of teeth among peasants in several rural communities in Bolivia gave the following results: As altitude decreases the health status of human dentition deteriorates. Further study of possible explanations of this phenomenon is proposed, specifically: the effect of diet, the fluorine content of drinking water, the habit of chewing coca leaves, and inadequate oral hygiene. Whatever the ecological conditions, it was found that younger girls have healthier teeth than boys, but that for all other ages male teeth are in better condition. Studies undertaken by other authors show that younger girls' molars exhibit more attrition than boys'; at other periods in life, the opposite is the case. Currently, there is no satisfactory explanation for the differential health status of dentition by age and sex.

Bilateral hydronephrosis from uretero-pelvic (U-P) obstruction: some clinico-pathological aspects.

Three cases of bilateral hydronephrosis from ureteropelvic obstruction in Nigerian children are presented. The unique association of this disease with congenital musculoskeletal disorders is shown in one of the cases. Frank haematuria was not a presenting complaint but presence of altered blood in urine described here as "Coca cola" coloured urine is common in all of the cases probably a feature of advanced disease process. Intermittent pyrexia mistaken for malaria in this environment and the tendency to mimic gastrointestinal disorders could lead to unnecessary delay in recognition of this disease. The morbidity attendant to operation on two compromised kidneys and the dangers posed by infection especially pseudomonas species have also been highlighted. The good reparative capacity of the kidneys in Young children enhance the chances of success in salvage operations on the kidney. Even though preoperative urinary tract infection may not be evident post operative infection often supervenes hence prophylactic antibiotics is to be recommended in this Surgery.

Cocaine metabolites in pre-Columbian mummy hair.

Chewing of coca leaves was an integral part of many Andean pre-Columbian populations. Since the cocaine metabolite benzoylecognine is entrapped in hair of the user, this could serve as a convenient specimen for analysis. We tested the scalp hair of 8 Chilean mummies with dates ranging from 2,000 BC to 1,500 AD. This article reports that benzoylecognine is very stable and can be found in mummy hair.

[Atopy: definition--pathogenesis].

After a short historical review about the detection of reagins by Coca and Prausnitz-Küstner 1921-1923 and about their physico-chemical characterisation as IgE-antibodies by Ishizaka and Johansson 1967-1968 the definition and the clinical relevance of atopy is discussed. The IgE-regulation by interleukins and the pathomechanism of IgE-mediated allergies is reviewed. Of special importance is the IgE-mediated inflammatory late-reaction and the--therefore caused--unspecific hyperreactivity of the stricken tissue, both characteristic features of asthma bronchiale as well as atopic dermatitis.

Consumption of dependence-producing substances in Colombia.

A survey examining the use of six dependence-producing substances (alcohol, tobacco, tranquilizers, marijuana, coca paste or "basuca," and cocaine) was conducted in Colombia in 1987. The survey population consisted of 2,800 urban residents in four cities (Barranquilla, Bogotá, Cali, and Medellín) between the ages of 12 and 64. The results indicated that substantially more men than women were using all the substances involved except tranquilizers, that high proportions of study subjects used alcohol and tobacco, that 8.1% of the study subjects could be considered alcoholics, and that another 7.3% were at risk of becoming alcoholics. User prevalences of the three illegal substances (marijuana, basuca, and cocaine) were much lower, and the prevalence of marijuana users exceeded that of the other two drugs combined. However, 1% of the male study subjects reported using basuca within the past year. The high prevalence of basuca use has important public health implications, because the drug typically does great harm to its users within a short period of time.

[Microbial pathogenesis of Whipple's disease].

In a case of Whipple's disease with electron microscopic demonstration of rod-shaped bacteria we could isolate Propionibacterium acnes (former name: Corynebacterium anaerobium), Actinomyces, and Lactobacillus from duodenal biopsies by the aid of modern anaerobic culture technique. Simultaneously we found a dysbiosis (increase of anaerobic coca and anaerobes) in du odenal juice. After antibiotic treatment Propionibacterium and Lactobacillus disappeared and an other type of dysbiosis (appearance of Bacteroides) was to be seen. In discussion is a change of immunologic state and following disturbed regulation of microbial colonization in the small bowel of patients with Whipple's disease.

A review of the history, actions, and legitimate uses of cocaine.

The history of cocaine is charted with specific reference to its usefulness as a medicine and local anesthetic. It is common knowledge that coca leaves were used as a panacea and local anesthetic throughout the history of the Incan Empire of Peru. In Europe, however, its medical usefulness was not fully recognized until Carl Koller used it to anesthetize the cornea of the eye. Over the next 20 years, cocaine became a popular medicine and tonic in Europe and America, where it was credited with curing a wide variety of diseases and illnesses. However, reports soon started to appear claiming that cocaine was a drug with a high social abuse potential and in America it seemed to underpin growing crime figures. As a result, cocaine was misclassified as a narcotic and its use was restricted to specific surgical procedures and medicinal preparations. Today, cocaine and its derivatives are still popular local anesthetics in operations of the ear, nose and throat and it is also used in a preparation given to alleviate the pain (physical and mental) of terminal diseases. Although cocaine has a high public profile as a drug of addictive potential, this drug has also had a long and distinguished history as a medicine and local anesthetic. The legitimate uses of cocaine exacerbate the problems of controlling this substance of abuse and should provide a stimulus for generating local anesthetics that lack addictive potential.

[Consumption of dependency-producing substances in Colombia].

In 1987 a study was made of the consumption of dependency-producing substances in the urban population of Colombia. For this purpose the prevalence survey method was applied to a representative sample of 2,800 individuals between the ages of 12 and 64. The descriptive analysis was supplemented by the exploration of causal associations and measurement of the strength of such associations by means of the prevalence ratio coupled with calculation of the degree of statistical significance. The study included three substances whose consumption is socially accepted--alcohol, tobacco, and tranquilizers--and another three considered to be illicit--basuco (coca-paste), cocaine, and marijuana. Alcohol and tobacco were the two drugs most used by both sexes (560 and 297 per 1,000 subjects studied, respectively). Tranquilizers, the only one of the drugs in the study that was used more by women, ranked third (60 per 1,000). Reported in much smaller proportions were marijuana (11 per 1,000), basuco (6 per 1,000), and cocaine (3 per 1,000). It may be noted that the consumption of basuco has recently reached a level double that of cocaine. Analysis of the use and abuse of these substances by age, marital status, socioeconomic situation, and other variables indicates that the prevalence of consumption is higher in the medium age groups, that unmarried persons are at excess risk compared with those who are married, that men from the upper classes tend to use cocaine and marijuana, and that both sexes in the lower classes use basuco as the drug of preference. Differences in suicide rates between users and nonusers were statistically significant in the population aged 15 to 54, and it was determined that the substances of greatest risk, generally for women, were basuco and marijuana.

Cocaine inhibits muscarinic cholinergic receptors in heart and brain.

(-)-Cocaine inhibits M2 muscarinic cholinergic binding measured with [3H]quinuclidinyl benzilate in heart and brain with a Ki of 18.8 microM. The cyclic nucleotide 5'-guanylylimidodiphosphate does not shift the competition curve, suggesting that (-)-cocaine is an antagonist. (-)-Cocaine also reverses the methacholine-induced inhibition of guinea pig atrial contractions at a similar concentration. Although (+)-cocaine is about 8-fold more potent than (-)-cocaine, (+)-cocaine is not present in extracts of the coca plant. Of the many compounds tested, only (-)-cocaine and lidocaine have a higher affinity at M2 muscarinic receptors than at M1 receptors; other compounds such as (+)-cocaine, norcocaine, procaine and dimethocaine are equipotent at the M1 and M2 subtypes. These results indicate that cocaine can act as an antimuscarinic agent, particularly at higher, toxic doses.
