Acute solitary diverticulitis of the caecum. Case report.

In two cases of suspected appendicitis, laparotomy revealed an inflammatory mass medially in the caecum. Colonic resection was performed in both cases and the final diagnosis was solitary caecal diverticulitis. When inflammation of a caecal diverticulum is recognized at laparotomy, simple diverticulectomy is the procedure of choice, but colonic resection is recommended if malignancy cannot be excluded or inflammatory changes are severe.

Acute diverticulitis of the cecum and ascending colon diagnosed by computed tomography.

Between June 1984 and August 1985, computed tomography (CT) was performed upon seven patients with diverticulitis of the cecum and ascending colon who presented with acute right lower quadrant symptoms of unknown origin. Three of these patients had undergone an appendectomy, while in the remaining four patients, the history and physical findings were atypical of acute appendicitis. CT findings suggestive of acute diverticulitis including thickening of the intestinal wall and pericolonic inflammation were present in all seven patients. In four patients, the colonic inflammation was limited to the segment of ascending colon superior to the ileocecal valve. In one patient, the inflammation involved both the ascending colon and the cecum, while in the two remaining patients, the inflammatory changes were limited to the cecum. An associated diverticular abscess was present in five patients and all were correctly identified preoperatively by CT. One patient in whom the diagnosis of uncomplicated diverticulitis of the ascending colon was made on the basis of the CT scan, was successfully managed nonoperatively with antibiotics. Five patients required a right hemicolectomy for perforated diverticulitis with abscess. The remaining patient underwent a right hemicolectomy for recurrent episodes of acute diverticulitis. Our experience suggests that CT is useful in the early diagnosis of diverticulitis of the cecum and ascending colon.

[Acute solitary diverticulitis of the cecum, a rare cause of acute abdomen. Apropos of 9 cases and review of the literature].

The natural history of colonic diverticulitis rarely includes acute solitary localization in the cecum. Complications are even more rarely seen. We report retrospectively nine cases seen in our unit. There were 5 males and 4 females, mean age 54 years. The patients presented with a more or less painful abdomen with localization in the right iliac region and signs of peritoneal irritation. Physical examination revealed a painful mass in this zone. Fever and hyperleukocytosis were not found in all patients. Barium enema demonstrated signs of pseudocompression of the cecum in four cases suggesting a tumoral formation in two and diverticulitis in the two others. A right hemicolectomy was performed in five patients as peroperative pathological diagnosis could not be ascertained on the macroscopic specimen. Diverticulectomy in the four other patients was unsuccessful in direct relation with the disease course and late diagnosis.
