["Telescopic" pancreatico-jejunal anastomosis after duodenocephalopancreatectomy for cancer].

The Authors report their experience and a review of the literature on the "telescopic" pancreatojejunostomy following pancreatoduodenectomy. The alternative modalities proposed for the management of the pancreatic stump and their results are also described. Although no technique seems free from complications, the relatively low rate of anastomotic leaking (7.9%) encountered convinced the Authors to adopt this technique on routine basis.

["Telescopic" terminoterminal pancreatico-jejunal anastomosis after duodenocephalopancreatectomy].

Pancreaticojejunostomy represents the most important step of the reconstructive process following pancreaticoduodenectomy. Anastomotic dehiscence at this level accounts for two thirds of total postoperative mortality. In order to reduce the incidence of anastomotic complications, we have recently adopted a new technique of "telescopic" end-to-end-pancreaticojejunostomy where, differently from our previous technique, we are not any longer invaginating the small bowel over the pancreatic stump. Our preliminary results obtained in 5 consecutive patients appear to be promising.

[Care of edentulous mandible with implant stabilized telescope complete denture].

In comparison to the present junction elements of the implantologic-prosthetic treatment of the toothless lower jaw the dental implant stabilized overdenture is a good alternative. The movable positioning of the resilient telescopes needs the observance of the prosthetic criteria. Having only a guiding function the number of the implants can be reduced to two implants.

Treatment and retention of a mandibular arch telescoped within the maxillary arch: case report.

A patient in the early mixed dentition was experiencing "difficulty in eating." The right mandibular posterior teeth were telescoped within the maxillary arch. The mandibular midline deviated to the left of the maxillary midline. With the midlines aligned, posterior occlusion was minimal. By activating a lingual arch, the mandibular arch width was increased to establish a posterior occlusion. Also, the lingual arch was used as a retainer. The posterior occlusion remained stable throughout the mixed dentition. An unexpected second phase of treatment was indicated in the permanent dentition because the maxillary right first and second premolars and the maxillary right second molar were completely buccal to the mandibular teeth. Successful treatment was accomplished in spite of broken appliances and noncompliance.

[The usefulness of the double-light telescopic catheter in the diagnosis of bacterial pneumonia in patients under mechanical ventilation].

Fifty-eight patients who presented clinical criteria of pneumonia (fever, leukocytosis, purulent tracheo-bronchial secretions, and lung infiltrate of recent appearance in X-ray) were prospectively studied in order to determine the cost effectiveness [correction of rentability] of quantitative culture of bronchial secretions by means of a telescopic catheter (TC) in the diagnosis of bacterial pneumonia in patients under mechanical ventilation. In 25 patients (43%) a positive culture was obtained above 10(3) UFC/ml, confirming the pneumonia diagnosis in 17 cases and detecting a false positive result in one patient. In no case with a count less than 10(3) UFC/ml pneumonia diagnosis could be confirmed, being this diagnosis excluded in 24 of 33 patients with a count lower than 10(3) UFC/ml. These results suggest that the presence of fever, purulent tracheobronchial secretions and lung infiltrate in an intubated patient under mechanical ventilation does not indicate the presence of pneumonia in a high percentage of cases and that CT scan can identify those patients who require antimicrobial treatment, and this avoiding its use in patients who do not need it, with the resulting decrease in morbidity and high cost.

[Behaviour of domestic high gold alloys in telescoped crown systems].

A great variety of elements and systems have been used in prosthetic dentistry for retention of removable partial dentures. The aim of this paper was to examine the possibilities for retention of telescoped crowns with parallel axial planes, made from domestic alloys, and influence of these alloys on the crowns durability. Two domestic alloys, Platinijum MPa-20 and Dentor-S, were used. Comparison between Platinijum MPa-20 and Dentor-S, and the results related to Degulor-M alloy suggested the excellent quality of domestic alloys, which recommends them as suitable materials for these purposes.

Experience obtained during liver resections made under experimental conditions by a telescopic compressor (AKE).

Liver resection experiments in dogs made by the AKE telescope compressor are reviewed. It is established that the instrument can be applied to the liver parenchyma of varying size and compressibility. The time of resection is short and the intervention entails only minimal loss of blood. Using this solution, it is not necessary to produce the transitory warm ischaemia of the liver.

[Is the telescope system still reasonable?].

Parallel milled abutments in partial dentures are the anchors of the so called telescoping system. Excellent function, easy cleaning and the possibility of modification on the case at a later date - if further losses of teeth should occur - are the great advantages of this design. The application of this suitable attachment seems to be limited by economic considerations and patients' desire for fixed brigdework. The state of the art is presented and proved by pretentious case-reports.

[Application of low noble alloys in telescoped crowns].

It is known that low noble alloys (Hera SG, Aurea-Heraus, Stabilor-Degussa, Midgold-Bego etc.) are used woredwide for the preparation of prosthetic constructions. For this reason attempts to produce low noble alloys with the properties equal to those of more expensive high noble alloys, are justified, as they are cheap and available to a great number of patients. The Yugoslav producer of dental materials "Zlatarna Celje" has produced a low noble alloy named Midor S. The comparative investigations of resistance to the mouth, performed at the University School of Dentistry in Ljubljana and in the Laboratory of Metallurgy of "Zlatarna Celje", have shown that this alloy was more resistant to corrosion than 22 karat, high noble alloy. In our Department Mikdor S was experimentally tested on telescoped crowns. The obtained results showed the satisfactory hardness and resistance to attrition between the internal and external crowns within the limits of tolerance which made it suitable for use.

The pull-through technique for the advancement of the unfolded epididymal tubule and the tubulo-deferential telescopic reconstruction.

A new reconstructive microsurgical model of the epididymo-deferential complex was performed in the rat to study the plastic adaptation of the distal epididymal tubule in transposition and/or substitution procedures. The model consists of three steps: endoluminal stripping of the mucosa of the proximal half of the vas deferens; pull-through technique for the advancement of the unfolded epididymal tubule of the "zone 9" into the stripped vas deferens; end-to-end tubulo-deferential telescopic reconstruction.

[Analysis of the bacteriologic data obtained with a double-lumen distally-plugged telescopic catheter in the diagnosis of pulmonary infiltrates in patients on low mechanical ventilation].

In order to assess the bacteriology of bronchial secretion samples, 40 patients in the intensive care unit who had developed fever and pulmonary infiltrates during mechanical ventilation have been studied. In each patient bronchial secretion samples were obtained by a double lumen distally-plugged telescopic catheter (DTC) inserted under direct view through the fiber bronchoscope (FB) as well as from simple bronchial aspiration (SBA) done simultaneously. A week later DTC and SBA were repeated. A statistically significant difference was found between the positive cultures obtained by SBA and those obtained by DTC (p less than 0.005). However, in 9 samples (14.27%) other microorganisms were isolated with DTC which were not detected by SBA and a lower number of colonizing microorganisms were found by DTC (p less than 0.05). The isolation of microorganisms by DTC allowed more precise management and moreover, a better clinical course was observed in those patients in whom chemotherapy was based on the data given by DTC. The relationship between the cultures obtained by DTC and the previous antibiotic treatment was statistically significant, finding a greater number of positive cultures when they were taken 2 hours after the last doses of the antibiotic. This relationship was not found in the cultures obtained with SBA. The most frequently isolated microorganisms were diverse types of Pseudomonas and Acinetobacter. No complication caused by the techniques arose.

Femoral neck fractures. A biomechanical study of a new form of internal fixation using multiple telescoping variable length compression screws.

Biomechanical tests of a new method of fixation for intracapsular hip fractures compared with other types of mechanical fixation were investigated under one specific loading condition. Human femora fixed in formalin were classified by the Singh index for degree of osteoporosis. Specimens of Grade III or less were grouped as osteoporotic, and Grade IV or greater, normal. Both normal and osteoporotic bones were tested. A transverse osteotomy in the subcapital region was fixed by one of two techniques: (1) Asnis screws (AS) or (2) variable length telescoping compression screws (VLCS). Each specimen was roentgenographed after fixation to assess the placement of the devices and reduction. Each femur was held in an angle vise that was placed on rollers on a table mounted on the servohydraulic testing machine. Compression was applied at a constant displacement rate of 10 mm per second to failure. Load and displacement were monitored; specimens were photographed and roentgenographed after failure. In osteoporotic bone, the VLCS provided yield and ultimate load values several times greater than those of the AS. Resistance to combined compression varus loading was significantly higher with the experimental system in osteoporotic bone. In normal bone there was no significant difference between the two systems.

A telescoping collection cup changer for insect traps.

A description is provided of a time interval insect sampler that has been used in suction traps but would also be useful with light traps. The device is capable of taking 27 successive samples. Principle of operation involves 2 telescoping concentric cylinders. The collection cups, mounted in the motor driven inner cylinder, are closed by the wall of the outer cylinder as the inner cylinder spirals inward about a helix screw.

[Comparison of the shielded telescoping brush catheter with bronchial lavage in the diagnosis of pulmonary infiltrates].

The results of cultures from bronchial lavage (BL) were compared with those obtained with a telescoping protected brush catheter (PBC) in 56 nonselected patients presenting with a pulmonary infiltrate of unknown etiology. At the time of bronchoscopy, 34 patients were under antibiotics. In 20 cases, the cultures from PBC and/or BL were positive. On the basis of quantitative bacteriological analysis and medical chart review, we conclude that PBC is more sensitive and more specific than BL for the diagnosis of clinically significant infections of the lower respiratory tract.

[Replacement of Bauhin's valve with an end-to-end telescope anastomosis between the ileum and transverse colon. Experimental study in dogs].

A method of a telescope anastomosis for prevention of reflux was developed experimentally in dogs. After the resection of 40 cm of ileum and colon ascendens the ileal part was intussuscepted into the colon and fixed by seromuscular stiches. The vessels were left untouched. The intussuscepted part measured 3-4 cm. No ileus was observed in the 24 weeks. Glass balls of 1.2 cm diameter could pass through. Barium enema as well as air insufflation of different pressures showed no reflux. The serosa was covered pretty soon by a granular tissue and later on by normal mucosa.
