[The role of congenital Enterovirus infection in the pathogenesis of suppurative and inflammatory diseases in newborn infants].

Newborns with suppurative-inflammatory disease were found to be at high risk of intrauterine infection with Coxsackie enteroviruses from mothers with persistent enterovirus infection; in 54.9%, congenital Coxsackie virus infection was confirmed by virus antigen identification in the urine sediment cells and autopsy material. Coxsackie A viruses were identified in 68.7% of sepsis cases, 42.6% with local purulent infection foci, and in only 6.7% of practically healthy neonates. Specific features of the clinical course are analysed together with the pathohistological picture of congenital enterovirus infection associated with the vertical virus transmission from the mother having a persistent form of this infection. A suggestion is proposed that the severe course of suppurative-inflammatory conditions and the general character of neonatal bacterial infection are largely determined by the immunodeficient states which are etiologically related to congenital enterovirus infections.

Acute suppurative thyroiditis in a malignant thyroid gland.

A case of acute suppurative thyroiditis in a malignant thyroid gland is presented. The patient was an elderly woman with several years' history of pre-existing goitre. A diagnosis of acute suppurative thyroiditis was not difficult to make because clinical features of an infected thyroid swelling appeared obvious. With the help of intravenous antibiotics and early incision and drainage the patient did well and is now receiving radiotherapy.

[Risk factors of wound suppuration after appendectomy].

The effect of some factors on the development of wound suppuration was studied in prospective examinations including 241 operations for destructive appendicitis. It was found that the presence of exudate, monoantibacterial prophylaxis, and the method of wound closure had no essential effect on the proportion of suppurations. The determinant factor is the surgeon's qualification, not his experience in general, but his experience in appendectomy in particular and individual technical traits.

[Optimization of the preoperative preparation of patients with chronic suppurative lung diseases].

Complex preoperative preparation of 87 patients with chronic suppurative diseases of the lungs included sanitation of the tracheobronchial tree by means of bronchoscopies (in 45 patients) and method of continuous nasobronchial catheterization (42 patients). The use of the latter resulted in rapid arrest of inflammatory alterations in the bronchial tree, effective sanitation of the suppuration focus, shorter period of preoperative preparation and less amount of postoperative complications.

[Effect of a controlled abacterial environment on the Pseudomonas aeruginosa infection of suppurative wounds].

The present work deals with the data on the isolation rate of P. aeruginosa from suppurative wounds of different origin during their treatment by the commonly used methods under dressings and by the open method under the conditions of controlled germ-free environment. The results of the immunotyping of P. aeruginosa strains isolated from patients treated by different methods are presented. The dynamics of changes in the isolation rate of P. aeruginosa at different periods of treatment, both by the open method and with the use of dressings, is shown. Among P. aeruginosa strains isolated from suppurative wounds, those belonging to immunotypes 6, 7 and 2, as well as nontyping strains, occurred most frequently. Treatment in the controlled germ-free environment permits the protection of the wound surface from hospital infection. During treatment with the use of dressings the cases of hospital infection were revealed (31.3%). Such infection occurred, as a rule, at a later period of treatment.

[Use of urea in the treatment of patients with suppurative diseases of the lungs and pleura].

Under analysis were results of treatment of 96 patients with suppurative diseases of the lungs and pleura. The 30% solution of urea was locally used in 59 patients as pleural punctures, introduction of the solution through drains into the purulent cavity by dropping irrigation of bronchi and tamponage of open purulent cavities. Gram-positive microorganisms were found to die in the 30% solution of urea within 30 min.--3 h, gram-negative ones--within 30 min.--1 h. The introduction of urea directly into the purulent infection focus against the background of general therapy allowed to quickly block the acute phase of the suppurative process, to prepare the patients to planned operations and to reduce the amount of postoperative complications as compared with a control group of patients who were treated by routine local antiseptic drugs.

[Clinical and experimental studies of suppurative surgical infection in cattle].

Investigated were a total of 30 cattle 6 of which had experimental surgery infection and 24 showed spontaneously developing suppurative infection. The animals were studied in terms of general status, blood picture, quality of urine and synovia, and rumen content. The dynamis was followed up of the development of sepsis and purulent artritis. It was found that in case of suppurative surgical infections in cattle the general status was invariably affected, the severity of the process depending on its localization and developmental phase. Changes were observed also in the eye fundus and in the urinary and the digestive system. There were anemia and leukopenia which alternated with leukocytosis, and transient changes in the values of the erythrocyte sedimentation rate and the blood electrolytes. The SGOT values almost constantly increased, and the hemoculture was not positive in all cases.

[Treatment of suppurative mastitis using laser irradiation and continuous electric current].

Laser radiation and direct electric current were used in the complex therapy of acute suppurative mastitis. The wound cavity was treated by a focussed laser till the appearance of a coagulation crust followed by galvanization of the mammary gland. Such an approach to the associated application of the laser radiation and electric field of direct current is justified pathogenetically and increases the efficiency of treatment of suppurative mastitis.

[Congenital piriformo-thyroid fistula as a cause of recurrent suppurative thyroiditis].

A previously healthy five year old boy presented with suppurative, febrile thyroiditis of the left lobe with pronounced general and local signs of inflammation, normal thyroid function and lack of thyroid antibodies. Intravenous antibiotic therapy improved the condition quickly. Six and nine months later however recurrent left lobe thyroiditis occurred. Endoscopic examination showed an internal fistula from the left piriform sinus to the left thyroidal lobe which was cut out. The case report confirms previous communications, that recurrent suppurative thyroiditis is nearly always caused by a piriform sinus fistula, probably a fourth pharyngeal pouch remnant which can be cured surgically.

[Prevention of wound suppuration].

The results of 0.3% sodium chloride solution application for the prevention of wound suppuration are reviewed. The experiments were carried out on animals, by making several pricks around the place of injection of potentially pathogenic microorganisms (staphylococcus, E. coli and yeast-like fungi) and forming a "tight" infiltrate. No inflammation was observed in the place of injection and the following "tight" infiltration by 0.3% sodium chloride solution. The control animals revealed suppuration in the infiltration area, and bacteriological analysis isolated initial cultures of microorganisms.

[Etiology and chemotherapy of suppurative surgical infection in horses and cattle].

A total of 24 horses and 42 cattle with a variety of local suppurative processes (wounds, abscesses, phlegmons, tendovaginitis, etc.) were studied under conditions of the clinic with regard to the most commonly found microbial species and the opportunity to work out antibioticogrammes in terms of the effectiveness of 11 of the chemotherapeutic agents widely used in the practice. It was found that prevailing in horses were staphylococci and streptococci. In cattle Corynebacterium pyogenes was predominantly established. Besides, there was a rising trend in the part played by representative agents of the occasionally pathogenic microflora (Ps. aeruginosa, Pr. vulgaris, Escherichia coli) as the causative species of local suppurative processes. These organisms were fairly often isolated in association with other species, while they were more rarely found as the only acting ones in a particular case. Best effects of the antibiotics tested could be expectedly produced by gentamycin (82.5 per cent), carbenimycin (74.2 per cent), oxacyllin (72.7 per cent), ampicillin (68.2 per cent), and canamycin (61.8 per cent). At the same time, the effectiveness produced by some of the antibiotics widely employed in the practice, such as chloramphenicol, tetracycline, streptomycin, and penicillin was below 50 per cent.

[Immunologic status and leukocyte function of patients with suppurative lactational mastitis].

In patients with a severe course of suppurative lactation mastitis there develops a relative deficiency in the cellular and humoral links of immunity. The compensatory activation of factors of nonspecific antiinflammatory resistance of the organism fails to exert a substantial positive effect on the development of the destructive process in these patients. On the basis of the data obtained, in order to improve results of the surgical treatment of patients with suppurative lactation mastitis means of the cellular anti-infection therapy, transfusions of donor leukocyte masses in particular, must be included in the complex of curative measures in the postoperative period.

[Effect of mytilan--a polysaccharide from Crenomytilus grayanus--on local suppurative processes].

The study on the effect of mytilan on the local suppurative processes caused by administration of a virulent staphylococcal culture to the mouse pad showed that this polysaccharide isolated from Crenomytilus grayanus had a pronounced antiexudative and antinecrotic effect. The positive results of the experiments may be explained by stimulation of the phagocytic activity of polymorphonuclear leukocytes and macrophages by mytilan which results in acceleration of the reparative processes in local staphylococcal infection. Hence, nonspecific stimulation of the defense mechanisms in local suppurative infection by mytilan and its antiexudative and antinecrotic effect are promising for investigation of the possibility of the mytilan clinical use.

[Extensive resection of the hip for suppurative arthritis in the paraplegic. Evaluation of 170 cases].

Trochanteric or ischial sores in paraplegia may lead to suppurative arthritis of the hip, followed by anaerobic infection, septicaemia, gangrene and amyloidosis. In 121 patients this complication was bilateral in 49. Extensive resection of the hip and the upper quarter of the femur avoided surgical disarticulation of the limb. This type of correction makes it possible to treat hip infection and to perform muscle and skin plastic procedures. The operation was performed 170 times in the course of twenty years. Post-operative care must be very precise to allow organisation of the new arthrosis and to prevent ossification. Seven patients died. Five disarticulations had to be performed secondarily despite the resection. The results were satisfactory in the remaining patients from the point of view of suppuration and the procedure provided satisfactory adaptation to a wheelchair. Only five cases developed a disabling hip ankylosis.

Postvaricella suppurative meningitis. Case reports and review of the literature.

Suppurative meningitis should be recognized as being a complication of varicella. The clinician must assiduously exclude suppurative meningitis that at times may be clinically indistinguishable from the typical postinfectious encephalomyelitis of varicella. Misdiagnosis of the cause of CNS alterations during the course of varicella is possible.

Suppurative thyroiditis: an unusual case caused by Actinomyces naeslundi.

A case of suppurative thyroiditis due to A.naeslundi is recorded. The clinical features (including the difficulty in establishing a clinical diagnosis) have been noted. This is the first instance of A.naeslundi being considered the etiological agent of actinomycosis of the thyroid. It is also the first report incriminating A.naeslundi as a pathogen, since up to now it was considered only a saprophyte. The etiological role of A.naeslundi in this case of suppurative thyroiditis is strongly supported by the following findings:1. A.naeslundi was isolated without the presence of other pathogenic micro-organisms.2. A sulphur granule was demonstrated in the caseous pus after incision of the abscess.3. There was a favourable response to specific antibiotic therapy.

Acute obstructive suppurative cholangitis in a man with an enlarged inferior pancreaticoduodenal lymphnode.

A case report of a 55-year-old male, who was seen with epigastric pain and hyperamylasemia, is to be presented. He failed to respond to the intensive antibiotic therapy, and subsequently acute obstructive suppurative cholangitis fully developed. At laparotomy, an inflammatory enlarged inferior pancreaticoduodenal lymphnode, which apparently compressed the common bile duct, was found. To our knowledge, no prior case of acute obstructive suppurative cholangitis provoked by an enlarged lymphnode has been reported. A plea is made for attention to lymphnodes involved by inflammatory or neoplastic disorders, as well as common provoking agents such as calculi.
