The development of healthcare services for drug misusers and prostitutes.

With the advent of human immunodeficiency virus (HIV) and the increase of drug misuse in the UK, the Government wishes primary care to play a greater part in treating drug problems in the hope of preventing the spread of HIV. Drug misusers do not avail themselves of traditional services and many are not registered with general practitioners. In response to this Liverpool Health Authority and Family Health Service Authority commenced a new salaried post to provide primary care services to special groups such as injecting drug misusers and prostitutes. Judgemental attitudes towards drug misusers, their high mobility and being a transient population play a part in the reasons why drug misusers find it difficult to access primary healthcare. Drug misusers have high morbidity related to their drug misuse. Many of these conditions, if treated early, can prevent the need for more intensive intervention. Although drug misusers may present with a condition requiring immediate treatment, the opportunity is used to provide other healthcare such as hepatitis B vaccinations, sexually transmitted infection screening, contraception and HIV/hepatitis B testing. The sero prevalence of anti-HBc in injecting drug misusers is 45.5%. Due to their high morbidity and associated costs, the requirements of these groups may conflict with the objectives of budget-holding practices. If general practitioners are unable to respond to their problems, then health care providers and purchasers will have to consider similar schemes in areas which have a higher prevalence of drug misuse in order to provide appropriate healthcare for these vulnerable groups.

Culture versus direct specimen test: comparative study of infections with Chlamydia trachomatis in Viennese prostitutes.

To evaluate the sensitivity and specificity of the direct specimen test with monoclonal antibody conjugated with fluorescein isothiocyanate (FITC), cervical smears from 700 Viennese prostitutes were investigated by immunofluorescence microscopy and compared with the results of cultures for Chlamydia trachomatis in McCoy cells. Inclusion bodies in cell culture and elementary bodies on the smear slide were counted and evaluated in four categories. Cultures were obtained from 12.6% (88) of all women investigated, whereas the direct specimen test gave positive results in 10.4% (73). The results indicate that in asymptomatic people with only few inclusion bodies and latent infections, culture on McCoy cells is the more adequate method of identifying Chlamydia trachomatis.

Penicillin treatment for gonorrhoea in relation to early syphilis in prostitutes.

A retrospective study of prostitutes in Singapore showed that they had received less antigonorrhoeal treatment with penicillin derivatives during the three month period before early syphilis was diagnosed than in other three month periods when they had not developed syphilis. This suggests that penicillin derivatives in doses sufficient to treat gonorrhoea will abort coincidental early syphilis.

Late seroconversion in HIV-resistant Nairobi prostitutes despite pre-existing HIV-specific CD8+ responses.

Resistance to HIV infection in a small group of Kenyan sex workers is associated with CD8+-lymphocyte responses to HIV cytotoxic T-lymphocyte (CTL) epitopes. Eleven prostitutes meeting criteria for HIV resistance seroconverted between 1996 and 1999. The occurrence and specificity of preexisting HIV-1 epitope-specific responses were examined using the IFN-gamma enzyme-linked immunospot assay, and any epitopes recognized were cloned and sequenced from the infecting viral isolate. Immunologic and behavioral variables were compared between late seroconverters and persistently uninfected sex worker controls. HIV-1 CTL epitope responses were present in four of six cases, 5-18 months before seroconversion, and their presence was confirmed by bulk CTL culture. A possible viral escape mutation was found in one of six epitopes. The key epidemiologic correlate of late seroconversion was a reduction in sex work over the preceding year. In persistently uninfected controls, a break from sex work was associated with a loss of HIV-specific CD8+ responses. Late seroconversion may occur in HIV-1-resistant sex workers despite preceding HIV-specific CD8+ responses. Seroconversion generally occurs in the absence of detectable CTL escape mutations and may relate to the waning of HIV-specific CD8+ responses due to reduced antigenic exposure.

Cytotoxic T cell responses to multiple conserved HIV epitopes in HIV-resistant prostitutes in Nairobi.

Many people who remain persistently seronegative despite frequent HIV exposure have HIV-specific immune responses. The study of these may provide information about mechanisms of natural protective immunity to HIV-1. We describe the specificity of cytotoxic T lymphocyte responses to HIV in seronegative prostitutes in Nairobi who are apparently resistant to HIV infection. These women have had frequent exposure to a range of African HIV-1 variants, primarily clades A, C, and D, for up to 12 yr without becoming infected. Nearly half of them have CTL directed towards epitopes previously defined for B clade virus, which are largely conserved in the A and D clade sequences. Stronger responses are frequently elicited using the A or D clade version of an epitope to stimulate CTL, suggesting that they were originally primed by exposure to these virus strains. CTL responses have been defined to novel epitopes presented by HLA class I molecules associated with resistance to infection in the cohort, HLA-A*6802 and HLA-B18. Estimates using a modified interferon-gamma Elispot assay indicate a circulating frequency of CTL to individual epitopes of between 1:3,200 and 1:50,000. Thus, HIV-specific immune responses-particularly cross-clade CTL activity- may be responsible for protection against persistent HIV infection in these African women.

Theological and psychological reflection on the functions of pastoral care in the context of child prostitution in Thailand.

Reports that there are 800,000 children below the age of 16 serving as prostitutes in Thailand and approximately 200,000 of these children are under the age of twelve. Estimates that 20% of these children will end up with AIDS. Relates these realities to several mental health and theological questions. Argues that God will become real for these children only if sensitive human participation takes place. Proposes that pastoral caregivers can play a crucial role in the confrontation and the resolution of this tragedy.

Crime, prostitution, drugs, and malingered insanity: female offenders' resistant strategies to abuse and domination.

Mizrahi female offenders have been described as passive victims propelled into crime, prostitution, and drug abuse as a result of traumatic childhood and life course experiences. This qualitative study adopts a postmodern critical orientation and Foucault's bottom-up microsocial analysis of power to examine the trajectories of resistance of 8 female offenders who break the silence to tell their life story. Analysis of narratives, informal conversations, and more focused, in-depth interviews with these women allow the deconstruction of the stereotype of the passive and helpless female offender. Using the sensitizing concepts of control, agency, and resistant efforts and letting the data speak, this research reconstructs female offending as a hidden script of resistance against intolerable socioeconomic deprivation and extreme forms of abuse.

Evidence of HTLV-I infection in Singapore prostitutes.

Sera obtained in 1987 from 63 male and 632 female Singapore prostitutes were screened for antibody to human T-cell leukaemia virus (HTLV)-I with a particle agglutination test. Of the 3 males and 4 females who were positive one had antibody to HTLV-I core and envelope antigen on Western Blot. Two subjects had presumptive antibody to HTLV-I core antigen and a third subject had such antibody on a repeat specimen in 1989. These sera were negative for HIV-1 antibody. There is evidence of infection with HTLV-I or a variant virus in this population. The infection is likely to have been sexually transmitted.

The adolescent female prostitute and sexually transmitted diseases.

Prostitution may be seen as an adolescent's cry for help. A review into the background of the problem, the groups and individuals involved and the incidence of sexually transmitted diseases amongst them is presented. Psychological analysis and behavioural trends, especially in relation to attitudes towards sex, sexually transmitted diseases and intravenous drug abuse are examined.

[Prostitution in adolescence: interfaces with the family institution].

Research carried out aiming at learning and analysing the context and significance of the dynamics in the family institution and its interfaces with the prostitution of adolescents. Data collection was carried out by means of life story interviews, and the content of the speeches underwent thematic analysis. We found a wide gap caused by the lack of dialogue and by how situations of abandonment, mother's death and life with family members being not a biological daughter of one of the parents have been experienced, breaking up with trust, respect and affect relationships, thus causing the loss of individual and collective identity. Violence within the family was identified through situations of sexual abuse, negligence and physical violence, thus making evident a true school of disrespect, abuse of power and gender/age hierarchy.

[The health-sickness process connected to sexual activity in prostitutes].

This articicle is a part of a research focusing on the relationship among child-prostitution, health-sickness-process linked to sexuality and violence; the three categories found in the environment of street girls. This paper, however, only deals with health-sickness-process linked to sexuality revealed by the girls themselves. After the considerations about the research as a whole and about sexuality in general, the street girls are characterized in their social scenery. Within that scenery, data about sexuality are analysed. Based on this analysis, the author concluded, among other aspects, that the education is necessary to enable Public Health to focus on this matter.

Risk factors for syphilis: cocaine use and prostitution.

In Philadelphia, a large increase in syphilis among minority group heterosexuals began in 1986 and preceded similar increases elsewhere in the United States. To determine reasons for this increase, we conducted a case-control study in the metropolitan sexually transmitted diseases clinic during 1987 and 1988. Cocaine use (odds ratio [OR] 3.1; 95% confidence interval [95% CI] = 1.5, 6.5 among men; OR 5.8; 95% CI = 1.5, 33 among women) and exchange of drugs for sex (OR 3.5; 95% CI = 1.4, 8.7 among men) were risk factors for syphilis. Although cocaine users reported more sexual partners and more frequently reported sex with prostitutes, cocaine use remained a risk factor after adjustment for these behaviors. These data suggest that sexual behavior or another factor, such as availability or utilization of health care, among cocaine users leads to increased risk of syphilis in this population. Increases in cocaine use may be partly responsible for recent increases in syphilis incidence in the United States.

Prevalence of HIV antibodies in transsexual and female prostitutes.

Human immunodeficiency virus (HIV) prevalence was studied in an unselected group of 216 female and transsexual prostitutes. Subjects were asked about age, biological sex, marital status, children, length of occupation, sexual practices, and drug abuse history. Blood was drawn on site. All 128 females who did not admit to drug abuse were seronegative; 2 of the 52 females (3.8%) who admitted to intravenous drug abuse were seropositive. In contrast, 11.1% of the 36 male transsexuals (including 3 out of 32 non-drug abusers) were seropositive. The results support the notion that vaginal transmission of HIV is less effective than anal transmission.

Seroprevalence of HIV-1 and hepatitis B and C in prostitutes in Albuquerque, New Mexico.

A survey of persons soliciting sex in an area known to be frequented by prostitutes in Albuquerque, NM, included 43 females and 66 males. Seroprevalence rates found in this population-based study were as follows: human immunodeficiency virus type 1 (HIV-1), 3%; hepatitis B, 39%; hepatitis C, 45%. Increased age, intravenous drug use, and condom use were independent risk factors for hepatitis B. Female gender and intravenous drug use were independent risk factors for hepatitis C. Neither sharing injection equipment nor engaging in receptive anal intercourse was independently associated with hepatitis B or C.

Homosexual sex as harmful as drug abuse, prostitution, or smoking.

In 2003, the U.S. Supreme Court said same-sex sexual activity could not be prohibited by law. Analyzing data from the 1996 National Household Survey of Drug Abuse (N= 12,381) and comparing those who engaged in four recreational activities-homosexual sex, illegal drug use, participation in prostitution, and smoking --against those who abstained, participants (1) were more frequently disruptive (e.g., more frequently criminal, drove under the influence of drugs or alcohol, used illegal drugs, took sexual risks), (2) were less frequently productive (e.g., less frequently had children in marriage, more frequently missed work), and (3) generated excessive costs (e.g., more promiscuous, higher consumers of medical services). Major sexuality surveys have reported similar findings for homosexuals. Societal discrimination inadequately accounts for these differences since parallel comparisons of black and white subsamples produced a pattern unlike the differences found between homosexuals and nonhomosexuals.

Alberta law confining child prostitutes upheld.

The Alberta Court of Queen's Bench has determined that the Alberta Protection of Children Involved in Prostitution Act is not in violation of the Constitution. This decision overturns the ruling of the Provincial Court, which held that the Act infringes the rights articulated in sections 7, 8, and 9 of the Charter.

Suicide and prostitution among street youth: a qualitative analysis.

This study presents the results of a qualitative analysis of the narratives of 29 street youth in which they describe their experiences with, and understanding of, suicide. A history of attempted suicide was reported by 76% of the participants and the analysis revealed themes of isolation, rejection/betrayal, lack of control, and most centrally, low self-worth as forming the basis of their experiences concerning suicide. Additionally it was found that trading sex, in which most participants had been or were currently involved, was linked with their suicidal experiences and may account for the high attempt rate. Finally, variables related to suicide were found that have not been examined previously in the literature on street youth, including loss of control, assault during prostituted sex, drug abuse as a "slow suicide," and breakups in intimate relationships.
