An historical cohort study of mortality among salaried research and development pensioners of the Allied Corporation.

An historical cohort study was conducted among salaried pensioners alive and receiving benefits on Dec 31, 1974, and followed for vital status through Dec 31, 1980. For all causes combined, this cohort experienced mortality levels at or below that of the US population. For all malignant neoplasms combined, workers with plant experience only showed a statistically significant excess (standardized mortality ratio [SMR] = 1.34). This may have been largely due to a significant excess of lung cancer in the same group (SMR = 1.90). Individuals with some plant experience exhibited a significant excess of lung cancer (SMR = 1.48). These findings were not detected among active workers in the same company. There were no causes of death for which the SMR deviated significantly from unity among individuals with research experience.

Nutritional status of the elderly. III. Vitamin nutriture of elderly pensioners in Perugia.

The thiamin, riboflavin, vitamin C, vitamin A, folacin, and vitamin B12 nutriture was assessed in a longitudinal study in 206 aged pensioners of the city of Perugia. The prevalence of thiamin and riboflavin malnutrition, particularly in men, was rather high. Unexpectedly low values of plasma vitamin C were not uncommon among men. For folacin there were a large number of individuals with low plasma levels, while the plasma levels of vitamin A and B12 is in general satisfactory. As in other studies, no significant correlation between vitamin nutriture and intake was found.

Nutritional status of the elderly. II. Anthropometry, dietary and biochemical data of old pensioners in Perugia at the fifth year follow-up.

206 aged pensioners of the city of Perugia have been examined in a longitudinal study, with emphasis on body composition, diet, life habits and clinical-biochemical data. From the anthropometric data of the individuals examined in 1981 it is evident that both in men and women obesity is common especially among the women. The opposite is true for the arm muscle area. Comparing the same men examined in 1976 and 1981 body fatness decreased in the five years interval. Food and nutrient intakes are rather similar in the 1976 and 1981 surveys. Consumption of milk, eggs, fish and legumes is not high. The men, but not the women, tended to a high consumption of meat and alcoholic beverages. The intake in 1981 of thiamin, riboflavin and vitamin A seems in general to be inadequate according to our recommended values. The biochemical data show practically no difference in the two surveys. Serum cholesterol continued to be rather high for an Italian population.

[Return to work and pensioning of state insured myocardial infarction patients (author's transl)].

With reference to an enquiry among a total of 961 patients of a longitudinal myocardial infarction study (replies from 97%) participants in the study and drop-outs were compared with regard to questions on returning to work and pensioning. The results point to the effectiveness of follow-up care within the scope of this longitudinal study which consists essentially of the combination of inpatient curative treatment and regular follow-up outpatient examinations with a six-monthly rhythm. 85% of the participants in the longitudinal study have taken up work again after the infarction compared with only 66% of the drop-outs. At the time of the enquiry in 1976, after an average of 45 months care in the study, 68% of the participants were working, on the other hand only 48% of the drop-outs with an average care of only 4 months were working.

[The effect of a three-year trial of a community dental care program for aged pensioners in Denmark].

Recently, one Danish municipality established a programme of systematic dental care free of charge to citizens 67 through 70 years of age. The old-age pensioners were offered care according to the principles of the Danish Municipal Dental Service, including comprehensive curative and preventive care as well as oral health education. The present report describes the results achieved after three years of the programme in terms of changes in oral health status, oral health behaviour, and life-quality. Longitudinal data were collected for evaluation. Personal interviews were conducted at baseline (n = 216) and at follow-up (n = 235), comprising participants in the municipal programme as well as non-participants. Moreover, clinical follow-up data of participants were provided (n = 187, at follow-up). At base-line, 46% of all respondents had regular dental visits at least once a year and the proportion increased to 86% at the follow-up. A total of 75% participated in the municipal programme and the self-assessment of dental health improved significantly over time. The percentage of participants who reported poor function of dentures declined and, at follow-up, less of the elderly felt embarrassed by teeth or preferred food that was easy to chew. The changes in self-reported oral health status were supported clinically by a reduction both in the number of untreated decayed tooth surfaces and in the number of teeth with gingival bleeding and pockets. Finally, gradually more attenders of the municipal service showed frequent oral hygiene habits and positive attitudes to oral health.(ABSTRACT TRUNCATED AT 250 WORDS)

[An epidemiological study of dental health in a population of pensioners].

OBJECTIVE: To find the health status of teeth and mouths among a pensioner population drawn from the Autonomous Community of Madrid. SETTING: The study was performed on 491 retired people over 56 and belonging to Madrid's Tetuan district. Data were recorded on indexes in line with WHO-established procedures. DESIGN: An epidemiological observation study of a crossover type was carried out. RESULTS: 52.8% of the patients never brushed their teeth. 90.6% did not regularly attend the dentist. 62.1% used the dentist as an emergency service. 57.8% of teeth had been lost. 28.3% of patients were toothless. The group under study had a 12.35 CAOD. Only 5% had a healthy periodontal status. CONCLUSIONS: The mouth and teeth hygiene of this population group is very precarious. Preventive measures must be applied to help maintain life-long oral health.

[Evaluation of disability pensions by a medical polyclinic: 1990-1995].

The question if and in what manner changes of the labour market, in first line increasing unemployment, may influence the composition of the collective demanding a disability pension on one hand and the diagnoses relevant for assessment of requests on the other hand have been investigated as well as possible influences on the criteria for assessments. To this end all expert reports elaborated by the Medical Outpatient Clinic of the University Hospital of Zürich and submitted to the disability insurance between 1990 and 1995 have been evaluated retrospectively. The results show that the fraction of men remained stable around 70% over the whole observation period. The number of persons employed in auxiliary functions remained also constantly high. Over the whole period of observation there was a high, growing percentage of foreign applicants. The most marked change during the observation period was a significant increase of unemployment in the collective. This increase particularly affected applicants with higher ranking jobs or persons over 50. Foreigners became an important part of the unemployed applicants. Rheumatoid disorders and "back pain" in particular were of increasing importance among the relevant diagnoses for assessment of disability. There was a significant decline in the extent of invalidity acknowledged in marked contrast to our initial expectation that the strictness of the applied criteria would weaken when confronted with an increasing number of applications.

Services to pensioners and how they might be improved. A report from Malmö.

As a follow-up to a previous article, the authors present an analysis of the services to pensioners in the 4th Welfare District of Malmö, as shown by their survey in 1972-1973, along with suggestions for improvement in various respects. They point to the importance of acquiring a general view of the individual and his situation. Various functions within the society's welfare program must be essentially improved, field work and adequate information being among the most important. Integration and coordination between the functional units must be operational concepts.
