Hypersecretion of melatonin following diurnal exposure to bright light in seasonal affective disorder: preliminary results.

Night-time melatonin secretion was measured in five depressed inpatients with seasonal affective disorder before and after 1-week of morning and evening exposure to bright (3000 lux) and dim (300 lux) light. Analysis of variance by ranks showed no differences in timing of melatonin secretion but statistically significant differences in plasma melatonin levels. There was a decrease of the area under the curve after bright light and a very robust rebound after exposure to dim light. The failure to constitute a parallel group of patients in a crossover design did not permit to control for an ordering effect of light exposure. These findings raise many questions concerning the diurnal sensitivity to different intensities of light in seasonal affective disorder.

Rupture of herniated small intestine from blunt abdominal injury: a report of 2 cases.

Two cases of perforation of the small bowel following blunt abdominal injury in patients with an abdominal hernia are presented. In one case the bowel was present in a large incisional hernia and the injuring force caused transection of the bowel at three different sites. The bowel was resected and the hernia was repaired at the same time. The patient died of associated chest injuries. In the second case, the bowel was caught in an inguinal hernia; although the injuring force was relatively mild it resulted in two perforations in the herniated terminal ileum. The patient survived following closure of the perforations and simultaneous repair of the hernia.

CA-125 in menstrual discharge in patients with chronic pelvic pain.

CA-125 levels in menstrual discharge were determined in 55 patients with chronic pelvic pain to evaluate whether this test would be useful in differentiating between pelvic pain due to endometriosis and other causes. Of the 28 women with endometriosis, 25 (89%) had CA-125 concentration greater than or equal to 72,000 units/ml. The frequencies of elevated levels in Stage I, Stage II and Stages III/IV were 85.7, 85.7 and 92.8%, respectively. When used for the detection of endometriosis, the test had a sensitivity of 89.3% and a specificity of 96.3%. These results suggest that CA-125 in menstrual discharge may be helpful in the evaluation of women with chronic pelvic pain.

Reproductive outcome following abdominal metroplasty.

OBJECTIVE: To evaluate pregnancy outcome following uterine unification procedures in patients with uterine anomalies. METHODS: A retrospective survey included all abdominal metroplasty procedures performed on 43 patients at the American University of Beirut Medical Center between January 1, 1974 and December 31, 1991. Age at metroplasty, type of anomaly and surgical procedures, as well as preoperative and postoperative reproductive performance were all recorded. RESULTS: Forty (93%) out of 43 patients who underwent metroplasty had postoperative live births compared with five (12%) prior to surgery. The fetal wastage rate dropped from 93% pre- to 16% postoperatively. All seven patients with a history of primary infertility conceived and had live births. CONCLUSION: Our data suggest a remarkable improvement following abdominal metroplasty in patients with both typical and uncharacteristic preoperative reproductive performance.

Management of persistent lymphocele by sclerotherapy with doxycycline.

Three cases of lymphocele after lymphadenectomy for gynecologic malignancy are presented. These patients had large, symptomatic lymphoceles resistant to percutaneous catheter drainage. Sclerotherapy with a single dose of doxycycline was administered percutaneously to each patient. Single treatment achieved resolution in two patients. The third had a persistent lymphocele requiring excision. To our knowledge, doxycycline has not been used for sclerotherapy for lymphocele following radical gynecologic surgery. It may represent a viable addition to the conservative management of lymphoceles.

Fluoxetine potentiates nitrazepam-induced behavioral sleep in young chicks.

Nitrazepam (0.5-10 mg/kg, IP) dose dependently induced behavioral sleep in day-old chicks. Fluoxetine (0.1-1 mg/kg) did not produce sleep in the young birds, but the 5-HT reuptake inhibitor (0.1 and 0.5 mg/kg) potentiated nitrazepam (0.5 and 1.0 mg/kg)-induced hypnosis. Doses (0.5 and 1 mg/kg) of the benzodiazepine that did not produce sleep in any or all the chicks, when administered alone, induced sleep in some or all the chicks in the presence of fluoxetine (0.1 and 0.5 mg/kg). Ketanserin (0.5 and 2.5 mg/kg) effectively antagonized the effect of fluoxetine on nitrazepam-induced behavioral sleep. These results suggest that enhancement of 5-HT level by fluoxetine may be the mechanism involved in the potentiation of nitrazepam-induced sleep in the young chicks.

Virulence for guinea pigs of tubercle bacilli isolated from the sputum of participants in the BCG trial, Chingleput District, South India.

This study, conducted in Madras, India and in Madison, Wisconsin, USA, was concerned with the virulence of isolates of Mycobacterium tuberculosis obtained from the sputum of individuals living in the Chingleput district of south India. The following results were obtained. 1. The findings of Mitchison with respect to the predominance of low virulence for guinea pigs among isolates from persons living Madras, were confirmed on isolates from the sputum of residents of the Chingleput district. 2. A high correlation was found between the log10 number of tubercle bacilli recovered from the spleen of guinea pigs infected intramuscularly with 1.0 mg of tubercle bacilli and the root index of virulence. 3. A high correlation was found between the log10 number of tubercle bacilli recovered from the spleen of guinea pigs infected intramuscularly with 1.0 mg of tubercle bacilli and the number recovered from the spleen of guinea pigs infected by the respiratory route with 5-10 tubercle bacilli. 4. Relatively low correlations were found between RIV and the susceptibility of isolates to thiophene-2 carboxylic acid hydrazide or to hydrogen peroxide.

Adenoid cystic carcinoma of the cervix: a report of 17 cases.

Seventeen cases of adenoid cystic carcinoma of the cervix are presented. The long-term survival was in early stage IB-IIA. Patients with more than stage IIB had no survival rate over 5 years. One thing of interest was the fact that lung metastasis was 29.4%.

Endosalpingiosis in association with ovarian surface papillary tumor of borderline malignancy.

A 40-year-old woman underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic/paraaortic lymphadenectomy, and omentectomy for Stage IA ovarian surface papillary tumor of borderline malignancy. Microscopic examination revealed endosalpingiosis of ovaries, the peritoneum of uterus, pelvic lymph nodes, and omentum. There was no evidence of disseminated peritoneal malignancy. The several proposed theories of the pathogenesis of endosalpingiosis are reviewed, but that involving metaplasia of the multipotential peritoneal cells is presented as the most acceptable.
