Cephalometric analysis of profile nasal esthetics. Part I. Method and normative data.

A comprehensive method for the measurement of profile nasal esthetics from standard lateral cephalograms was used for a group of 30 white male and 30 white female patients with skeletal Class I relationships and dental malocclusions only. Linear, horizontal, vertical, and angular hard and soft tissue measurements were made. Results for the two groups were analyzed to determine if there were statistically significant morphologic differences between the sexes. To eliminate differences related to the absolute size differences that characteristically exist between men and women, the results were taken as ratios of a fixed referent (middle third facial height), so that actual intergender differences in shape and form could be determined. The results indicated that male and female noses have essentially identical profile characteristics. The range for both sexes extended from an aquiline nasal type (more classically male) to an upturned type (more classically female). The characteristics of the normative nose for males and females are presented. The proposed methodology provides a standard objective means of evaluating changes in nasal esthetics after maxillary or rhinoplasty surgery.

Esthetic restoration of a single-tooth implant using a precision ceramic coping.

Previously, osseointegrated implants were used almost exclusively for edentulous arches. In recent years, the development of abutments has allowed more esthetic restorations of single and multiple abutments. The recent introduction of a precision ceramic coping for single-tooth implants has enabled the restorative dentist to achieve a highly esthetic result. Proper treatment planning, surgical-guide fabrication, and technique of placement for the various components are addressed in this article.

[Free bone flaps in esthetic and functional rehabilitation after segmental mandibulectomy. Apropos of 38 cases].

Development of microvascular free tissue transfers gives the surgeon the opportunity to realise complex reconstructions after segmental mandibular resections for tumor. We reviewed 38 cases of oromandibular reconstruction using various free bone grafts (25 fibula, 7 scapular, 3 iliac and 3 forearm free flaps). The mandibular defect was anterior in 24 cases, lateral in 9 cases and subtotal in 5 cases. In the post-operative course there were two total flap losses. Long-term esthetic results were excellent or good for 82% of the patients, 81% were able to eat a solid or a soft diet. The causes of esthetic or functional failures were studied as well as the specific interest of each donor site. The use of free bone flaps improved cosmetic and functional rehabilitation in cases of anterior mandibular defects. The secondary reconstructions or reconstructions of lateral bony defects were more disappointing. The free fibula flap allows reconstruction of wide bony defects. Scapular flap or forearm flap are indicated for smaller bony defects and in patients with severe peripheral vascular disease.

Cephalometric analysis of profile nasal esthetics. Part II. Patients with vertical maxillary excess.

In part I of this study, a method for the objective evaluation of profile nasal esthetics was detailed, and normative values for males and females were presented. In part II, identical methodology was applied to the preoperative lateral cephalograms of 13 female patients with vertical maxillary excess to determine how the nasal profile esthetics of these patients varies from the norm. This data was compared to the normative data established for females in part I. Results determined that patients with vertical maxillary excess have increased nasal length caused by a more superiorly positioned soft tissue nasion, decreased thickness of soft tissue at rhinion, increased nasal form angle, and increased absolute nasal tip angle related to an increased inclination of the cranial base, increased forward projection of the anterior nasal spine (tip support), and decreased incidence of supratip break. These findings suggest a characteristic appearance similar to the type II nasal characteristic (aquiline form) established in part I.

Esthetic recontouring. A team approach.

The presence of irregular incisal edges, a worn or abraded dentition, and imbalances in tooth morphology compromise a harmonious esthetic smile. A team approach is indicated after extensive orthodontic or prosthetic treatment to reach a final cosmetic result. Intentional changes in tooth form by recontouring provide an improved outcome. This article presents the components of a harmonious smile, evaluation, guidelines and a stepwise approach for the esthetic recontouring of anterior teeth. Two cases illustrate the effects that are achieved with this methodology.

Periodontal procedures to enhance esthetics.

Dentistry today can provide excellent esthetics for the patient. Periodontal procedures have transcended the era where the treatment and scope are limited to pocket reduction and disease control. Periodontal plastic surgery will prove to be an excellent adjunct when properly applied in conjunction to elective esthetic dentistry.

Esthetic improvements and in vitro testing of In-Ceram Alumina and Spinell ceramic.

The original In-Ceram material (In-Ceram Alumina), composed of sintered aluminum oxide subsequently infused with a glass, features interesting mechanical properties. In-Ceram Spinell was marketed more recently to improve the esthetic potential. This study compared the flexural strength of various combinations of sintered alumina and spinel infiltrated with the associated glasses. In addition, the influence of vacuum during the infusion process was investigated using density measurements. The characteristic strength (load at which 63% of the specimens had failed) was 530 MPa for the original material, 523.7 MPa when the same material was infiltrated under vacuum, 481.4 MPa when sintered alumina was infused under vacuum with the glass originally marketed for the spinel, and 283.1 MPa for the sintered spinel infiltrated under vacuum with the associated glass. A significant increase in density was observed when the infiltration firing of sintered alumina was performed under vacuum. Furthermore, the in vivo evaluation of specific esthetic parameters inherent to different types of cores was made and revealed the relative opacity of alumina; spinel was found to have the ability to blend in with the underlying substrate. Both materials demonstrated a general lack of fluorescence.

Restoring function and esthetics in a patient with amelogenesis imperfecta.

Amelogenesis imperfecta is a rare dental disease and presents a major challenge to the dentist. With the tremendous advances in the field of esthetic dentistry, especially in bonding to dentin, it is today possible to restore function and esthetics to an acceptable level. The need for full crown preparation has been decreased to an absolute minimum. A case of amelogenesis imperfecta, complicated by a malocclusion, is presented. A combination of periodontal treatment and resin-bonded porcelain onlays and nobel alloys resulted in a highly successful outcome. The virtual absence of enamel was overcome with the aid of dentin bonding.

Forced eruption in the esthetic zone.

This article describes the background, technique, and benefits of forced eruption vs conventional osseous surgery in maintaining anterior esthetics. Four case reports will demonstrate the benefits of the procedure for maintaining esthetics and phonetics and preventing open interproximal spaces, which are seen frequently after conventional surgical procedures.

Esthetic crown lengthening for maxillary anterior teeth.

In the maxillary anterior region, the gingival labial margin position is an important parameter in the achievement of an ideal smile. The relationship between the periodontium and the restoration is critical if gingival health and esthetics are to be achieved. Periodontal therapy is a necessary and useful adjunct when any anterior restoration is undertaken. Anterior surgical crown lengthening may be undertaken to avoid restorative margin impingement on the biologic width. Crown lengthening is also used to alter the gingival labial profiles. This article discusses the esthetic parameters of ideal gingival labial positions and presents a classification of crown-lengthening procedures and the procedure for a two-stage crown-lengthening technique. The two-stage crown-lengthening technique is surgically precise because healing is predictable.

Multidisciplinary approaches: esthetic restorations after orthodontic treatment.

After orthodontic treatment prosthetic restorations are sometimes still necessary to improve the patient esthetics. After routine orthodontic procedures (alignment of teeth and achievement of functional occlusion) conventional crowns and bridges are used. In this case study, the follow-up results of four patients are presented. They had been treated by various disciplines; orthodontics, plastic surgery and prosthodontics. Satisfactory esthetics and function were achieved with all patients.

Evaluation of the porcelain laminate veneers as a new esthetic and reconstructive modality.

Veneers are being used for the esthetic improvement of anterior teeth, and porcelain veneers are a recent development in this field. This article will described briefly the porcelain laminate technique and attempts to evaluate it as a new esthetic and treatment modality. Though there are some shortcomings yet the advantages afforded by this system are considerable enough to make the technique deserving of further investigation.

Long-term esthetic provisional restorations in dentistry.

The role of interim dental restorations used for indirect restorative and prosthodontic procedures has changed dramatically in the past several years. These restorations are no longer regarded as temporary restorations but rather as provisional restorations with distinct functions and purposes. Provisional restorations have become a vital diagnostic and assessment tool to evaluate function color, shape, contour, occlusion, periodontal response, implant healing, and overall esthetics. An accurate fit and margination is essential to insure and maintain pulpal health of the tooth. With increased demands being placed on provisional restorations, new materials and techniques are being developed and some existing protocols are being refined to accomplish the desired goals. Provisional restorations are often in the oral environment for several months, requiring a well-fabricated and stable restoration. This review is designed to report on current materials, techniques, and concepts in fabricating and maintaining long-term esthetic provisionals.

Esthetic dentistry in the twenty-first century. Healthy beauty.

The goal of esthetic dentistry in the twenty-first century is to attain healthy beauty by harmonizing morphology and function. Morphologic beauty is achieved by using restorative techniques such as ceramo-metal restoration, all-ceramic restoration, laminate veneer, bleaching, and the treatment of malocclusion. Functional beauty is obtained by facilitating the primary functions of the stomatognathic organ, namely, mastication, speech, and craniomandibular stability. In the next century, esthetic dentistry must combine the present focus of morphologic beauty with the goals of functional beauty to attain true, healthy beauty.

Achieving optimal gingival esthetics around restored natural teeth and implants. Rationale, concepts, and techniques.

The role of prosthetic restorations in the final appearance of the surrounding soft tissues has long been recognized. Innovative prosthodontic concepts as described should be used to enhance the biologic as well as the esthetic data of the supporting tissues, in natural teeth and implants alike. Combined dental treatment modalities of different kinds (i.e., orthodontics, periodontal treatment) are often required for optimal results. Meticulous care and attention to the delicate soft tissues should be given throughout all phases of the treatment, with a view to achieving a functional, healthy, and esthetic oral environment.

Anterior esthetic considerations when splinting teeth.

There are many techniques for stabilizing and splinting teeth. No matter which restorative technique is chosen, the technical elements of marginal fit, psychologic contour, cleansibility, and occlusion must be met. When the restoration is in the esthetic zone of the oral cavity, there is an additional element of achieving an acceptable cosmetic result. This article presents an overview of concepts to achieve acceptable esthetic results when teeth are joined together.

[Sense of smell before and after functional esthetic rhinoplasty].

BACKGROUND: The aim of the present study was to determine possible changes in olfactory threshold after functional and esthetic nasal operations and to evaluate whether these changes were recognized by the patients. METHODS: The study included 41 patients before and after nasal surgery (septoplasty, septorhinoplasty). Nasal airway resistance, olfactory thresholds, and subjective symptom scores were evaluated and compared pre- and postoperatively. The mean follow-up was 5.4 months. Olfactory performance was assessed using the Sniffin' Sticks. RESULTS AND CONCLUSIONS: The postoperative values in the screening test for olfactory sensitivity were not significantly different from those before nasal surgery. Nasal ventilation was improved postoperatively in all patients, assessed by determination of nasal airway resistance and subjective symptom scores. Besides an increase in nasal ventilation, functional and esthetic nasal operations can lead to improvement of olfactory function. The rarely seen postoperative increase in olfactory threshold does not seem to be subjectively recognizable by the patients.
