[Investigations on the percutaneous activity of a combination of heparin, allantoin and dexpanthenol in a specific ointment base, Antiinflammatory effect on UV-erythema in the guinea pig].

Compared to the ointment base, the application of a heparin-allantoin-dexpanthenol combination ointment (Hepathrombin ointment) to guinea pigs immediately after as well as 30 and 120 min prior to UV-irradiation effects more rapid regression of the erythema along with a reduction in erythema formation. After a pretreatment of 30 min, the temperature of the erythematous skin is also significantly lower. At a pretreatment of 120 min, no temperature differences can be observed between the ointment groups. This effect might result from a superimposition of the anti-erythematous/antiinflammatory effect of Hepathrombin along with a vasodilation effect on the deeper skin layers. The antiinflammatory effect of Hepathrombin is also discussed in connection with the mechanism of UV-induced erythema. The inhibition of UV-erythema by Hepathrombin underlines the rapid skin penetration of the active substances.

[Development of erythema in thermographic images].

The intensity of erythema does not show a linear correlation with convective and conductive heat transport. In erythema induced by various means such as dermographism, the application of nicotinic acid benzylester, UV-B-irradiation, or dithranol, the mediators follow the direction of blood and lymph defluxion. As a result heat reflection occurs not only from the erythemic region, but also from the surrounding unaffected skin area. Venous blood vessel texture is increased within areas of blood defluxion. Erythemous flushing after alcohol uptake is characterized by an increase in skin temperature. This increase in temperature occurs prior to the appearance of erythema compared to exogenous skin irritation which first shows erythema and later on an increased temperature. An even further increment can be measured in the flushed area. An increase in acral temperature (hands, feet, nose) is an indication of the systemic effect of the resorbed alcohol.

Potassium iodide in the treatment of erythema nodosum and nodular vasculitis.

Twenty-nine patients with erythema nodosum, nodular vasculitis, or erythema nodosum-like lesions associated with Behçet's syndrome were treated with potassium iodide. Administration of the drug for systemic effect showed a substantial effect in 11 of 15 patients with erythema nodosum, seven of ten with nodular vasculitis, and one of four with leg lesions of Behçet's syndrome. Relief of subjective symptoms, including tenderness, joint pain, and fever, occurred within 24 hours. Substantial improvement in the eruption occurred within a few days, and the lesions disappeared completely ten to 14 days after therapy was initiated. The patients to whom the medication was administered shortly after the initial onset of erythema nodosum seemed to respond most satisfactorily. The effect of the drug was marked in the patients with positive C-reactive protein reactions, joint pains, and/or fever. Possible mechanisms by which potassium iodide exerts its effect are discussed.

No detection of Borrelia burgdorferi-specific DNA in erythema migrans lesions after minocycline treatment.

BACKGROUND AND DESIGN: Early treatment of erythema migrans is important to prevent late complications. Minocycline possesses several attributes, making it potentially useful in the treatment of borrelial infections. In our study, minocycline was administered to 14 patients with erythema migrans. Punch biopsy specimens were obtained from the (affected) skin of all patients before and after therapy. The formalin-fixed, paraffin-embedded specimens were analyzed by polymerase chain reaction for the presence of Borrelia burgdorferi-specific DNA. RESULTS: Polymerase chain reaction assay succeeded in amplifying B burgdorferi-specific DNA from the first biopsy specimen, obtained from the border of erythema migrans before initiating treatment, in eight (57%) of 14 patients. At the end of minocycline therapy, however, polymerase chain reaction analysis disclosed no B burgdorferi-specific DNA in any of the 14 patients. The good clinical response of our patients with erythema migrans substantiates our molecular findings. CONCLUSIONS: The presented polymerase chain reaction data, together with the clinical outcome, indicate that minocycline may be useful for treatment of early Lyme borreliosis.

[Study of erythema nodosum leprosum].

Erythema nodosum leprosum usually occurs after specific treatment of lepromatous disease or borderline leprosy but may be observed in patients who have not been treated. It is an immune complex vasculitis. Since it is extremely difficult to identify the histological features and the bacteriological study is often negative, this diagnosis relies on clinical examination. Although many authors emphasize the role of reaction to dapsone, there is no one cause of erythema nodosum leprosum which may be triggered by infections, drugs, treatment errors and stress. As a rational treatment of erythema nodosum leprosum may be quite difficult, the clinician must determine the bacteriological and morphological indexes. Anti-inflammatory and anti-allergic treatment should be used before specific drug therapy which should be started after the acute episode triggering the reaction. Finally long-term follow-up of erythema nodosum leprosum is required to avoid recurrence.

Erythema multiforme associated with an outbreak of Mycoplasma pneumoniae function.

AIMS: To document the association between erythema multiforme and Mycoplasma pneumoniae. METHOD: All cases of erythema multiforme presenting to the dermatology department of Waikato Hospital over the summer of 1992/3 were tested for mycoplasma serology. Trends in recorded cases of M pneumoniae infection were examined from data supplied by the virology department of Waikato Hospital. RESULTS: Four of seven cases of erythema multiforme were due to M pneumoniae infection. This was associated with a marked local increase in the number of M pneumoniae infections recorded in our hospital. CONCLUSION: Mycoplasma serology is an important investigation in the management of erythema multiforme, especially when associated with an outbreak of M pneumoniae infections.

[Erythema nodosum in sarcoidosis patients].

Erythema nodosum was registered in 45 from 502 sarcoidosis patients (9%). Erythema often arose in initial sarcoidosis, but was also recorded in the disease recurrences. Sarcoidosis of the skin and subcutaneous fat occurred in 16 and 4 patients, respectively, this incidence being half of that for erythema nodosum. Sarcoidosis of the above sites is the second in occurrence among extrapulmonary lesions following those of peripheral lymph nodes. Positive effects in the treatment of erythema nodosum were reached after administration of corticosteroids, noncorticosteroid anti-inflammatory drugs, physiotherapy, local therapy.

Acral erythema: a clinical review.

Acral erythema is a localized chemotherapy-induced cutaneous response that has been observed in patients with either hematologic malignancies or solid tumors. An illustrative case of acral erythema in a woman with acute myelogenous leukemia is reported. The patient initially presented in blast crisis with concurrent cutaneous lesions of Sweet's syndrome. She subsequently showed acral erythema after being treated with cytosine arabinoside and idarubicin (an analogue of doxorubicin). The pathogenesis and treatment of acral erythema are reviewed.

Erythema nodosum caused by omeprazole.

We report a case of erythema nodosum caused by omeprazole. This side effect of omeprazole has not been previously reported. A 35-year-old white woman developed multiple tender erythematous nodules over the anterior aspect of her upper and lower extremities, fever, malaise, and edema. Her symptoms resolved with withdrawal of all preexisting medications and conservative treatment. Her symptoms flared soon after a rechallenge with omeprazole. A thorough history, physical examination, laboratory evaluation, and roentgenogram failed to reveal another cause for erythema nodosum. It is clear from the rechallenge that this patient's erythema nodosum was caused by omeprazole. We consider that omeprazole should be added to the list of medications known to cause erythema nodosum.

[Treatment by thalidomide of chronic multiforme erythema: its recurrent and continuous variants. A retrospective study of 26 patients].

INTRODUCTION: The objective of this study was to evaluate the efficacy of thalidomide in the treatment of chronic erythema multiforme unresponsive to usual treatments. PATIENTS AND METHODS: Twenty-six patients with chronic erythema multiforme were given thalidomide 100 mg/day after other treatments had failed, particularly acyclovir and prednisone. RESULTS: Twenty patients had recurrent erythema multiforme and were given thalidomide at the beginning of an episode. The duration of the episodes was reduced by 11 days on the average. Six of the patients had subintrant erythema multiforme and were given continuous treatment. Lesions disappeared within 5 to 8 days and remission was maintained with low dose-treatment. DISCUSSION: The spectacular results obtained here should be verified in a controlled study.

Erythema ab igne caused by a car heater.

Erythema ab igne is a reticulated erythematous hyperpigmented eruption that occurs after chronic exposure to heat. In the past, the shins were the most common area of involvement, but with the widespread availability of central climate control in most buildings the incidence has decreased dramatically. New causes of erythema ab igne have been noted that have been caused by such heat sources as therapeutic chairs with built-in heaters. We present a case in which prolonged and close contact of the legs to a car heater led to erythema ab igne. Practitioners must be aware of the changing causes of erythema ab igne so that an appropriate history can be obtained.

Photodynamic treatment of skin malignancies with aminolevulinic acid. Emphasis on anatomical observations and in vivo erythema visual assessment.

BACKGROUND: Photodynamic therapy with delta-aminolevulinic acid is a promising alternative treatment for superficial skin malignancies. OBJECTIVE: Further clinical experience, study of tissue alterations leading to recovery, and correlation/prediction of the therapeutic response through in vivo skin color changes as represented by erythema development. METHODS: The therapeutic procedure, sequential histology and histochemistry, and the development of a remote machine vision system to measure, map, and monitor the erythema development. RESULTS/CONCLUSIONS: A high cure response rate with adequate follow-up was shown. A significant correlation of the clinical-histologic response of tumors subjected to treatment with the erythema measurements implies that erythema inspection and quantitative analysis offer a reliable predictor of the therapeutic outcome and a clue for optimization of this treatment modality.

Involvement of cell adhesion and activation molecules in the pathogenesis of erythema dyschromicum perstans (ashy dermatitis). The effect of clofazimine therapy.

OBJECTIVES: To assess the expression of several cell adhesion and lymphocyte activation molecules in erythema dyschromicum perstans lesions, and to evaluate the effect of clofazimine therapy on the expression of these molecules. DESIGN AND METHODS: A prospective study. Skin biopsy samples were obtained from patients before and after 3 months of clofazimine therapy, and the expression of cell adhesion and activation molecules was assessed by an immunohistochemical technique. SETTING: This study was performed in a clinical referral center and an immunology research laboratory. PATIENTS: We studied 6 patients with erythema dyschromicum perstans. A diagnosis was made on the basis of clinical and histological criteria. Two patients discontinued participation in the study: one because of adverse effects and the other for unknown reasons. INTERVENTIONS: Patients were treated with clofazimine, 100 mg/d, for 3 months. MAIN OUTCOME MEASURES: Expression of cell adhesion and lymphocyte activation molecules in skin biopsy specimens before and after clofazimine therapy. RESULTS: Before clofazimine therapy, we detected a noticeable expression of intercellular adhesion molecule 1 and major histocompatibility complex class II molecules (HLA-DR) in the keratinocyte basal cell layer. In addition, CD36, a thrombospondin receptor that is not expressed by normal skin, was detected in the strata spinosum and granulosum. The dermal cell infiltrate expressed the activation molecule AIM/CD69 and the cytotoxic cell marker CD94. After clofazimine therapy, the expression of intercellular adhesion molecule 1 and HLA-DR disappeared, as well as the mononuclear cell infiltrate. CONCLUSIONS: Our results suggest that some cell adhesion and activation molecules are involved in the pathogenesis of erythema dyschromicum perstans. Clofazimine appears to have an important effect on the inflammatory phenomenon of erythema dyschromicum perstans.

Prediction of minimal erythema dose with a reflectance melanin meter.

The relationship between skin pigmentation and sensitivity to ultraviolet (UV) radiation-induced erythema was investigated in 60 healthy subjects of sun-reactive skin types I-V. Using a portable reflectance spectrometer, skin pigmentation was measured as the 'melanin index' (MI) in all subjects. A solar-simulated array of filtered UVA and UVB-emitting fluorescent lamps was then used to determine the UVB minimal erythema dose (MED) of each subject. MI readings and MED testing were both performed on the subjects' mid to upper backs. Using this technique, we found a close correlation between MI and MED. Comparison of the mean MI or MED of subjects with different skin types revealed progressive differences in MI and MED between all five skin types. Erythema dose-response curves, which provide further information about UV sensitivity, were also calculated for 43 subjects. A significant negative correlation was found between the gradients of these curves and both MI and MED, indicating that paler subjects respond more strongly to increments of UV above the MED than subjects with greater pigmentation. Our results indicate that although traditional, subjective means of predicting UV sensitivity to erythema are not without some value, MED correlates particularly strongly with objective measures of skin pigmentation. We therefore conclude that the reflectance spectrometer can rapidly and accurately predict UVB sensitivity, and should prove clinically useful for planning and optimizing UVB phototherapy.

[Erythema gyratum repens: drug reaction following azathioprine administration in a patient with type I autoimmune hepatitis].

BACKGROUND: Erythema gyratum repens is a rare, clinically specific, and distinctive paraneoplastic syndrome. CASE REPORT: A case of erythema gyratum repens in a 76-year-old woman with autoimmune hepatitis type I treated with glucocorticoids is reported. Within 3 weeks of supplementary azathioprine treatment, the patient reported gastrointestinal discomfort and developed an erythema gyratum repens confined to the abdomen, thighs and knees. Azathioprine medication was stopped and the dermatologic features resolved completely after a period of 1 week. Absence of any demonstrable underlying malignancy was confirmed by different tests. Molecular diagnosis detected heterozygous G460A and A719G transitions in the thiopurine methyltransferase (TPMT) gene. 18 month later, complete remission on maintenance therapy (prednisone 7.5 mg) was observed with further absence of malignancy. CONCLUSION: This is the first report of an erythema gyratum repens in association with azathioprine treatment in an autoimmune hepatitis type I patient with proven common polymorphism in the TPMT gene.

Erythema elevatum diutinum: a clinical and histopathologic study of 13 patients.

BACKGROUND: Erythema elevatum diutinum is a rare condition representing a chronic leukocytoclastic vasculitis. OBJECTIVE: Clinical and laboratory features of the disease were reviewed to better understand the disease. METHODS: The medical records and histopathologic slides of 13 patients with erythema elevatum diutinum were studied. RESULTS: The lesions were violaceous, deep red, or brown and typically were papules or plaques. Lesions were most often located on the extensor surfaces of the extremities. Associated medical problems included hematologic abnormalities in six patients: IgA clonal gammopathies (four), multiple myeloma (one), and myelodysplasia (one). Erythema elevatum diutinum preceded the myeloproliferative disorders by an average of 7.8 years. All patients showed vasculitis. Leukocytoclasia was present in 27 of 35 specimens. The predominant cell type in the inflammatory infiltrate was polymorphonuclear leukocytes or a mixture of polymorphonuclear leukocytes and lymphocytes. CONCLUSION: The most significant finding of this study is the association of erythema elevatum diutinum with hematologic disease, most frequently an IgA monoclonal gammopathy.

[Erythema migrans disease. A contribution to its clinical features and relation to Lyme disease].

A largely prospective study averaging 33 months was undertaken in 30 patients with and one without (chronic) erythema migrans. In one case erythema migrans disappeared spontaneously, in the 29 others it persisted up to six months, but quickly responded to antibiotic treatment. Measured from the tick bite in 9 patients or from onset of the erythema migrans, arthritis and arthralgia appeared in ten patients on average 6.5 months (0.7-36), and persisted for eight months (0.2-42). In seven of these patients sensory disturbances appeared three weeks (1-10) later and (or) signs of meningitis which lasted for four months (0.5-16), while in three patients cardiac symptoms appeared a few weeks later, persisting for 4.5 months (0.3-12). In one patient tracheolaryngitis developed two months later, persisting for three months. These manifestations occurred in seven patients despite antibiotic treatment. Extradermal manifestations in two patients were successfully treated with high parenteral penicillin doses, in one instance followed by tetracyclin. "Erythema migrans disease", differing from Lyme disease described in the U.S.A. in only a few aspects, apparently cannot be successfully treated with low oral doses of penicillin, but can in certain circumstances be favourably influenced by high parenteral doses of penicillin G.

Recurrent annular erythema with anti-SSA/Ro and anti-SSB/La antibodies localized on palms and fingers in an elderly man.

A 69-year-old Japanese man with recurrent annular erythema localized on palms and flexor surface of fingers with anti-SSA/Ro and anti-SSB/La antibodies, is reported. The present case indicates that even in an elderly man with atypical localization, annular erythema with autoimmunity may be considered and serological analysis should be performed. Systematic studies will be required to understand the relationship between 'recurrent annular erythema associated with anti-SSB/La antibodies' in Japanese, subacute cutaneous lupus erythematosus (SCLE) and 'annular erythema associated with Sjogren's syndrome'.
