Vulvar dermatoses.

Benign and malignant vulvar disorders may look alike, making diagnosis difficult for even experienced vulvologists. Understanding basic principles of differential diagnosis and diagnostic testing is necessary for good medical or surgical management. Skin diseases do not respond quickly even when treatment is appropriate. Therefore it is especially important to establish the correct diagnosis as early as possible. This group of dermatoses demands careful distinction of sometimes subtle differences, but the physician and patient will be rewarded by a sure response to correct therapy.

[X-ray irradiation of dermatoses in the genital area].

Report on empirical data of X-ray treatment of dermatoses in the genital area, concerning indications, technique and results. Irradiations are mainly successful in large patches of Bowen's disease and Bowen's carcinoma as well as in Queyrat's erythroplasia and Paget's disease. Carcinomata of the penis should not exceed the stages T1-T2. Regression of Peyronie's disease is accelerated. Psoriatic lesions disappear following Grenz-rays without further topical treatment. Grenz-rays can also prevent or delay local recurrences of herpes simplex eruptions in the genital area.

Assessment of psychiatric and psychosocial factors disposing to chronic outcome of dermatoses.

In many skin diseases, itching and scratching is a vicious circle, which prolongs the disease. The aim of this study was to investigate the mechanisms which make itching skin diseases more chronic. The patients consisted of seven diagnostic groups--79 inpatients all together. The dermatoses were: dermatitis herpetiformis, lichen ruber planus, chronic eczema, atopic eczema, neurodermatitis circumscriptus, prurico psychogenous and lichen obtusus corneus. Both psychiatric and dermatological examinations were performed. Psychiatric disturbance was clearly greater than in the average population. The chronifying mechanisms were the following: personality disorder as a treatment problem; emotional infantility, which makes the illness itself an important security factor; itching and scratching as pleasure and content of life; the accumulation of various other diseases, both somatic and psychiatric; and untreated depression. Information was obtained on the possibilities of psychiatric treatment and psychosocial rehabilitation.

[Occupational dermatoses among workers in slag separation plants of the low-carbon ferrichrome industry].

Slag separation shops in low-carbonaceous ferrochromium production, although provided with ecologically and economically justified closed waste-free technological scheme, are marked by occupational dermato-pathologies among the workers (epidermitis, contact dermatitis, allergic dermatoses) as well as by certain prevalence of some non-occupational skin and fat-tissue disorders caused by hazardous labour conditions. The article reveals the complex characteristics of slag's affecting the skin, which include primary irritating factors (alkali) and allergenic factors (chromium-hapten). The contributors propose a set of diagnostic and preventive measures with due account of the occupational diseases' specificity.

[Pyogenic dermatoses in the workers of the Apsheron oil production industry and the days of incapacity because of them].

A total of 2820 oilmen regularly exposed to industrial oil were examined. Folliculites were detected in 151, furuncles in 63, hydradenitis in 58, phlegmons in 35, panaritium in 30 subjects. The number of invalidity days due to pyogenic dermatoses was 2-3 times higher among the workers exposed to deep oils than among those handling surface oils; this difference is explained by chemical composition and physicochemical characteristics of the extracted oil.

Oral cyclosporine in the treatment of inflammatory and noninflammatory dermatoses. A clinical and immunopathologic analysis.

Cyclosporine is known to be effective in the treatment of psoriasis. In this study, we have used oral cyclosporine (6 mg/kg per day) given for 5 to 30 weeks to 24 patients for the treatment of 12 different dermatoses. Patients with the following diseases demonstrated a marked response or total clearing: 1 patient each with pyoderma gangrenosum, pityriasis lichenoides chronica, and psoriasis of the acrodermatitis continua of Hallopeau type. Moderate to marked response occurred in both patients with epidermolysis bullosa acquisita and the patient with hidradenitis suppurativa. Minimal to moderate responses were obtained in both patients with granuloma annulare, 1 of 2 with acrodermatitis continua of Hallopeau, both patients with Darier's disease, and 1 of 6 patients with vitiligo. Little or no response was noted in both patients with sarcoidosis, all 3 patients with pityriasis rubra pilaris, 5 of 6 patients with vitiligo, 1 patient with pemphigus foliaceous, and 1 with pemphigus vulgaris. Clinical side effects were mild and transient and included dysesthesia, fatigue, hypertrichosis, nausea, and flushing. The most frequent clinically significant abnormalities were hypertension and renal dysfunction, with all factors normalizing within 1 month of discontinuation of cyclosporine therapy.

The photoaggravated dermatoses.

The photoaggravated dermatoses are diverse diseases adversely affected by sunlight or artificial ultraviolet (UV) exposure. The role of UVR is often well-recognized, as in lupus erythematosus (LE) and bullous pemphigoid (BP). Immunologic theories for the cause of the photosensitivity in lupus erythematosus are the production of UV-DNA antibodies with subsequent immune complex formation, a partial defect in DNA repair following UV exposure, and the release or stimulation of potent cutaneous immunologic mediators. However, mechanisms are still not fully elucidated. Photosensitivity has also been described in erythema multiforme, actinic lichen planus, viral exanthems, cutaneous T-cell lymphoma, eczema, and psoriasis. Treatment is restriction of light exposure, use of high protection sunscreens, and treatment of the underlying disorder.

The behaviour of collagen peptidase in the serum of different dermatoses.

In 150 patients with different dermatoses and 15 controls the activity of collagen peptidase in serum was determined. No relation to specific skin diseases was found. Negative findings were also observed in the group of collagenoses: only 4 of 14 patients with progressive systemic sclerosis showed an increase of this enzyme activity. These negative results seem noteworthy because it thus is demonstrated that alterations of collagen structure are not necessaryly associated with alterations of collagen peptidase activity.

A new antihistamine hc20-511 compared with dimetinden (fenistil retard) in the treatment of chronic urticaria and other pruritic dermatoses.

The new antihistamine, HC20-511 (Sandoz), was compared with Dimetinden (Fenistil retard) in a single-blind comparative study in 42 patients with dermatoses, 28 of whom suffered from chronic urticaria. HC20-511 had a better effect, especially in chronic urticaria, where pruritus, erythema and papules quickly disappeared. The effect appeared somewhat faster than and lasted as long as that of Dimetinden, although HC20-511 is not a retard-preparation unlike the Dimetinden preparation used for comparison. HC20-511 also caused less side effects.

[The therapeutic efficacy of tigazon in treating dermatoses].

Forty patients with various dermatoses (psoriasis, ichthyosis, Darier's disease, discoid lupus erythematosis) were followed-up over the course of treatment with tigason, an aromatic retinoid. The effect was satisfactory in the majority of patients. Side effects were recorded in 16 cases.

Demonstration of leukotriene B4 in the scale extracts of psoriasis and inflammatory pustular dermatoses. Correlation with leukocyte chemotactic activity and C5a anaphylatoxin.

We carried out quantification of the levels of leukotriene B4 (LTB4), a highly potent cell membrane-derived leukocyte chemotactic factor, in scale extracts of psoriasis and related inflammatory dermatoses characterized by sterile subcorneal pustule formation by using radioimmunoassay. A small amount of LTB4 was demonstrable even in extracts from non-inflammatory stratum corneum, but larger amounts were detected in the scale extract from pustular psoriasis and in those from psoriasis vulgaris. There was a significant correlation between the LTB4 and C5a levels in scale extracts, suggesting that complement activation and generation of LTB4 are a closely related event in psoriatic lesions. However, in contrast to highly significant correlation noted between the amount of C5a and chemotactic activity for polymorphonuclear leukocytes (PMN) detectable in scale extracts, LTB4 levels correlated only marginally with the chemotactic activity.

Carcinomas following Grenz ray treatment of benign dermatoses.

During the years 1977-1986 five patients were referred to the Department of Plastic Surgery. University Hospital, Arhus, for treatment of carcinomas occurring in the skin on the sites previously treated with Grenz rays for benign dermatoses. Grenz ray treatment had been given for eight to nineteen years. The dose given, in one case, was very extensive. Otherwise a dose between 10,000 rad (100 Gy) and 29,300 rad (293 Gy) was given. The tumours were squamous cell carcinomas in four cases and basal cell carcinoma in one case. One person developed both squamous and basal cell carcinomas. The tumours behaved aggressively and in one young patient the dura and the parietal bone had to be replaced by tensor fasciae latae graft and a free latissimus dorsi flap. None of the patients had been exposed to other known carcinogens. Caution in applying Grenz ray treatment is stressed.

Occupational dermatoses from colour developing agents. Clinical and histopathological observations.

Exposure to colour developing agents that are derivatives of p-phenylenediamine, e.g. CD-2 and CD-3, are known to cause lichenoid lesions and allergic contact dermatitis. This study describes the clinical picture, course and patch test results in 24 patients with occupational skin disease caused by colour developing agents. Biopsies for histopathology of the occupationally-induced lesions and/or patch test reactions were obtained in 14 cases. The skin lesions cleared rapidly after exposure to colour developers had ceased. Sixteen patients had to leave their exposed work due to the skin disease. Contact allergy to colour developing agents was found in 23 cases. The clinical and histopathological findings showed that lichen planus and lichen planus-like lesions were common among the dermatoses caused by colour developers. It was not possible to provoke lichenoid lesions by patch testing. The histopathology of the patch test reactions showed dermatitis.

[Cytochemical and immunological indices of patients with occupational dermatoses of chemical etiology].

A total of 161 male patients, aged 18 to 56, suffering from occupational dermatitis (n = 81) and eczemas (n = 80), have been examined with the use of cytochemical and immunological methods in an outpatient rehabilitation centre in the town of Mingechaur, the Azerbaijan SSR. The patients have been engaged in the manufacture of fiber glass, insulation material, and rubber articles; the dermatoses they have developed have been due to exposure to allergic and irritating components of nonconsolidated synthetic polymers and chemical ingredients of industrial rubber production. The findings evidence a significant increase of the neutrophil metabolic and functional activities in dermatitis patients and a rise of the lymphocyte activities in eczema patients. A complex of methods for the studies of leukocyte function and of changes in the immunity status, both on the cellular and subcellular levels (sensitization, inflammation, toxicity) is recommended for examinations of such patient populations.

[Occupational dermatoses: clinical picture, noxious compounds, prevention].

Various occupational substances may cause dermatological lesions. Toxic, toxic-degenerative and allergic eczemas have to be mentioned on the first hand. The prevention of dermatoses ranges among the most important tasks of occupational medicine. The first measure should be a minimization of the direct skin contact with dermatotoxic substances. Here, adequate cleaning products play an important part. --When working with potentially toxic substances, the use of protective devices must be recommended. There are two possibilities: wearing gloves or application of protective ointments. In both instances, numerous disadvantages have to be considered. The main point is the reduced quality of work. Recently, a foam product for external protection has been developed. Clinical experiments and first practical experiences permit the assumption that this foam product which forms a biphasic lattice in the outer layers of the stratum corneum may ameliorate the situation.

Detection of transferrin and C3d receptors in the skin of patients with various dermatoses.

The localization of transferrin and C3d receptors in various skin lesions and normal appearing skin have been studied on sections with the PAP technique. The transferrin receptor was recognized in the lower epidermis from psoriatic plaques. Here it was more evident than in other inflammatory or hyperproliferative disorders where it was mainly detected on the basal cells. In healthy skin or lesions of lichen planus, scleroderma and ichthyosis the transferrin receptor was not detected in the epidermis. The C3d receptor was in normal skin found on the basement membrane and on elastic fibres in the papillary dermis. The basement membrane was strongly marked in pemphigoid but was not seen in lichen planus and Ehlers-Danlos syndrome. In patients with urticaria factitia, contact dematitis, psoriasis and Darier's disease the suprabasal cells also expressed C3d whereas in other dermatoses the epidermis was negative. Colloid bodies in lichen planus and GVH reactions expressed both the transferrin receptor and C3d.

[Clinico-immunologic evaluation of the efficacy of bikarfen in allergic dermatoses].

It was found that the new native antihistamine and antiserotonin drug bicarphen possesses in patients with allergic dermatoses (eczema and neurodermatitis) more active antihistaminic, anti-inflammatory, antipruritic and antiallergic properties than commonly used drugs, particularly in cases with prevalence of exudative and allergic reactions of the immediate type. Normalization of rosette-forming T-lymphocytes and A and G immunoglobulins was essential.
