Field characteristics of geometric unsharpness due to the x-ray tube focal spot.

The basic theory of field characteristics is discussed. It is shown that the point spread function and optical transfer function (OTF) of geometric unsharpness at any arbitrary field position can be derived from those at the central beam position. At all locations in the radiation field, the OTF in a direction parallel to the x-ray tube cathode-target axis does not depend on the field position X, which is perpendicular to the tube axis, whereas the OTF in a perpendicular direction does not depend on the field position Y, which is parallel to the tube axis. The theoretical result is used to demonstrate the field characteristic of the uniform, square focal spot.

Irradiation of intense characteristic x-rays from weakly ionized linear molybdenum plasma.

In the plasma flash x-ray generator, a high-voltage main condenser of approximately 200 nF is charged up to 55 kV by a power supply, and electric charges in the condenser are discharged to an x-ray tube after triggering the cathode electrode. The flash x-rays are then produced. The x-ray tube is a demountable triode that is connected to a turbo molecular pump with a pressure of approximately 1 mPa. As electron flows from the cathode electrode are roughly converged to a rod molybdenum target of 2.0 mm in diameter by the electric field in the x-ray tube, weakly ionized linear plasma, which consists of molybdenum ions and electrons, forms by target evaporation. At a charging voltage of 55 kV, the maximum tube voltage was almost equal to the charging voltage of the main condenser, and the peak current was about 20 kA. When the charging voltage was increased, the linear plasma formed, and the K-series characteristic x-ray intensities increased. The K lines were quite sharp and intense, and hardly any bremsstrahlung rays were detected. The x-ray pulse widths were approximately 700 ns, and the time-integrated x-ray intensity had a value of approximately 35 micro C/kg at 1.0 m from the x-ray source with a charging voltage of 50 kV.

Operant conditioning and behavioral analyses using a minicomputer.

An operant conditioning and data analysis software system was developed for use on a PDP-12A minicomputer. The operant software functions in quasi time-sharing fashion to control and acquire data from any peripheral device that operates in the binary mode. In addition to independently running different experiments in near simultaneous fashion, the program also provides information on the current status of each experiment using a cathode ray-tube display. Response data from each experimental subject is stored on magnetic tape and analyzed, off-line, by the data analysis portion of the software system. A discussion of the operation of this system is given for one possible application: visual discrimination training.

Screening for sight-threatening diabetic retinopathy: comparison of fundus photography with automated color contrast threshold test.

PURPOSE: To compare tritan contrast threshold (TCT) with fundus photography in screening for sight-threatening diabetic retinopathy (STDR), before significant visual loss. DESIGN: Prospective, comparative study. METHODS: A total of 510 consenting diabetic patients attending a hospital-based photographic screening clinic were recruited over a 2-year period. Exclusion criteria included visual acuity of worse than 6/9, previous photocoagulation, and a history of previous eye disease known to affect color vision. The automated TCT test was performed using a computerized cathode ray tube-based technique. Retinal photography was performed using a Polaroid mydriatic fundus camera with a 45 degrees field. Grading of diabetic retinopathy was carried out by an ophthalmologist using slit-lamp biomicroscopy and a 78-diopters lens. Assessments of sensitivity, specificity, and predictive values for both fundus photography and the TCT test were made. RESULTS: Both the fundus photography and TCT test correlated significantly with the presence of STDR (P <.0001, chi(2) test). The TCT test yielded a sensitivity of 94% (95% confidence interval [CI], 73%- 100%) and a specificity of 95% (95% CI, 92%- 96%) for detection of STDR compared with a sensitivity of 88% (95% CI, 66%-97%) and a specificity of 95% (95% CI, 93%-97%) with fundus photography. Combined modality improved overall screening performance. CONCLUSION: The TCT assessment is an effective and clinically viable technique, in comparison with fundus photography, to screen for STDR among a diabetic population. Additionally, our results also showed that combining the TCT test with fundus photography greatly increases the performance of screening for STDR.

Heads-up intraoperative endoscopic imaging: a prospective evaluation of techniques and limitations.

OBJECTIVE: Endoscopes have been used adjunctively for spinal and cranial microsurgical procedures and directly for ventricular exploration, fenestration, and catheterization. Technological advances now allow for multi-imaging technologies, including a so-called "heads-up display," allowing the surgeon to view the operative field and the endoscopic image simultaneously. INSTRUMENTATION: A high-resolution, active matrix liquid crystal display is built into the frame of the eyewear, with a display density of 182,000 pixels (280 x 650 pixels) and a resolution of 200 lines. The display occupies approximately 20% of the visual field. The headgear weighs 4 oz, with dimensions of 9 x 18 x 17 cm. The modular interface weighs 7 oz. Two different visualization systems can be used. The first uses a single integrated interface (IM 300), whereas the second (remote 900) is a 900-MHz frequency modulation wireless system that provides for a line-of-sight link between the NTSC source and the modular interface (range = 100 ft). RESULTS: Heads-up adjunctive endoscopy was used in 60 patients during 18 months. The procedures included 16 craniotomies, 1 intradural lumbar biopsy, and 42 ventriculoperitoneal shunt placements. Follow-up was obtained at a minimum of 1 year. Patient age ranged from 1 month to 58 years. The only complication was a transient loss of auditory evoked potentials after contact of the eighth nerve by the endoscope. CONCLUSION: We describe a portable, light-weight heads-up display imaging system, which we have used in 60 operative procedures. Benefits of the heads-up system include portability and a high-resolution digital monocular image, which reduces eye strain and vertigo. The ideal headgear will likely be an extremely high-resolution liquid crystal display-based or cathode ray tube-based semi-immersive system, with all of the benefits currently described for two-dimensional heads-up systems (i.e., light weight, portability, image quality, and the avoidance of complications associated with immersive systems).

Chromatic contrast thresholds as a prognostic test for visual improvement after macular hole surgery: color vision and macular hole surgery outcome.

PURPOSE: To evaluate the relationship between preoperative chromatic contrast thresholds, postoperative visual acuities (VA), and visual improvement after macular hole surgery. METHODS: A consecutive series of patients with Stage II to IV macular holes was studied before macular hole surgery. Preoperative chromatic contrast thresholds, VA, and reading acuity were analyzed in relation to the postoperative visual function. The chromatic contrast thresholds were measured using a computerized cathode ray tube-based system along red-green and tritan confusion axes. RESULTS: Preoperative and postoperative chromatic contrast thresholds were elevated significantly in affected eyes (P < 0.001). Preoperative VA showed a strong correlation with postoperative VA (r = 0.66, P < 0.001) but a weak correlation with visual improvement (r = -0.33, P = 0.03). Red-green contrast threshold correlated strongly with both the distance visual improvement (r = -0.77, P < 0.001) and reading visual improvement (r = -0.61, P < 0.001). Tritan contrast threshold, however, showed a slightly weaker correlation (distance: r = -0.63, P < 0.001; reading: r = -0.47, P < 0.005). CONCLUSIONS: These results suggest that chromatic contrast thresholds, especially the red-green contrast threshold, represent a better prognostic guide for visual improvement after macular hole surgery than VA measurement.
