[Assessing the efficacy of antimicrobial agents based on the Japanese Respiratory Society Guidelines for a community-acquired pneumonia].

We evaluated the efficacy of antimicrobial agents used for patients with community-acquired pneumonia (CAP) based on the guidelines of Japanese Respiratory Society. A total of ninety-nine hospitalized patients who were suspected to have bacterial pneumonia at the time of admission between January, 1998 and December, 2000 were assessed. Our conclusions were as follows. 1) The rate which was considered as effective by the guidelines were 83.3% (5 of 6 cases), 98.7% (74 of 75 cases), 85.7% (12 of 14 cases), and 100% (4 of 4 cases) for penicillin, cephem, carbapenem and tetracycline, respectively. 2) These guidelines were useful and practical for identifying the etiological organisms, determining the severity of pneumonia and evaluating the efficacy of chemotherapeutic agents in CAP. 3) Gram-positive organisms were isolated more frequently among the "mild" group. The number of isolated gram-negative was increased significantly as the severity of pneumonia progressed from "mild" to "severe" group. 4) Factors such as body temperature and white cells count were not always applicable for evaluating the clinical effect in elderly cases. 5) Risk factors such as underlying diseases, or complications, age (over eighty years old) and specific etiological organisms might influence the ineffectiveness of antimicrobial agents among the cases that had no clinical response. 6) The choice of antibiotic agents for the treatment of CAP should be made with consideration to the local-specific profiles of each medical facility as described in the guidelines.

Acoustically coupled gas bubbles in fluids: time-domain phenomena.

In previous work [C. Feuillade, J. Acoust. Soc. Am. 98, 1178-1190 (1995)] a coupled oscillator formalism was introduced for describing collective resonances, scattering, and superresonances, of multiple gas bubbles in a fluid. Subsequently, time-domain investigations of the impulse response of coupled systems have disclosed the exact conditions which determine whether the ensemble scattering behavior should be described using: either (a), a multiple scattering; or (b), a self-consistent methodology. The determining factor is the Q of the individual scatterers, and their typical spatial separations in the medium. For highly damped or sparse systems, e.g., scattering from loose schools of swimbladder fish, or from a gassy seabed containing entrained bubbles, the multiple scatter counting approach should be applicable. For more strongly coupled systems, e.g., a dense cloud of resonating bubbles in the water column, energy exchange may be due primarily to radiative cycling rather than scattering, in which case a self-consistent approach is indicated. The result has implications for both volume and bottom scattering applications.

In vivo standardization of cutaneous bactericidal activity of antiseptics by using monoxenic hairless mice.

This study was designed to investigate the bactericidal activities of antiseptics on the cutaneous flora of hairless mice monoxenic to Staphylococcus epidermidis, Staphylococcus aureus, or Pseudomonas aeruginosa in vivo. A standardized method for testing such antiseptics to compare their bactericidal effectiveness in humans in described. Seven antiseptics belonging to seven different chemical groups (iodine derivatives, alcohols, mercury compounds, quaternary ammonium salts, biguanides, phenols, and carbanilides) were used as recommended by the manufacturers (conditions of contact or prolonged contact time followed by washing with distilled water). Germfree hairless mice were infected with various bacterial strains by gastric intubation, producing levels of about 10(3) CFU/cm2 of skin. The antiseptic under test was placed on the right or left side of the ventral region. The contralateral side served as a control. In order to standardize the method, a number of crucial parameters were carefully controlled: the amount of antiseptic applied, the area of skin treated, the duration of treatment, and the washing procedure. Skin samples were obtained by cutaneous biopsy, which effectively removed all the bacteria along with the sample. The bacterial populations were counted before and after application of the antiseptic. Reductions of between 0.5 and 1.9 log units were obtained; these are comparable to those observed in humans. The standardization of our procedure and the use of animals with a strictly controlled flora eliminated much of the variability and sources of error inherent in human studies. This model could be of value for the study of resistant bacterial strains responsible for nosocomial infections and for investigations of damaged skin.

[Relationship between an offensive smell given off from human foot and Staphylococcus epidermidis].

The bacteria isolated from foot skins of 17 volunteers by the swab sampling method were mostly gram-positive cocci, which were identified as Staphylococcus epidermidis by the ID-kit SP-18 (Nissui Co., Ltd). After incubation of S. epidermidis on agar plates containing oleic acid and Tween 80 for 24 h at 35 C, the smell noticed was similar to an offensive smell of human pes. However, under the same conditions, the smell of another staphylococcal species was different from that of S. epidermidis. Except for the staphylococcal species, the colonies isolated from the skins were mostly those of yeast (unidentified) and gave off no offensive smell. From these results, it was considered that the smell of human pes might be given off by S. epidermidis, and if this species is inhibited, the smell would also be inhibited. A selective bactericide for gram-positive bacteria, which is a lotion containing deoxycholic acid, was applied to the feet of the 17 volunteers. The experiments showed that the application obviously decreased the counts of colonies of S. epidermidis and inhibited the smell as compared with controls.

Evaluation of argyrophilic nucleolar organizer region staining in predicting the behavior of meningiomas.

BACKGROUND: The number of argyrophilic nucleolar organizer regions (AgNOR) correlates with cellular proliferative activity. Comparing nonrecurrent, recurrent, atypical and malignant meningiomas we studied the value of the routine applicability of the AgNOR count in the prognostication of this tumor. PATIENTS AND METHODS: Two hundred and thirty-eight meningiomas were reviewed blindly and graded using WHO grading schema. Eighty-one cases were selected and arranged in six groups according to clinical data and grading: 14 benign non-recurrent meningiomas; 14 primary benign recurrent meningiomas and their subsequent benign recurrences; 14 atypical; 11 malignant and 14 spinal meningiomas. Silver-stained slides were prepared and mean, median and standard deviation of AgNOR dots determined. RESULTS: There was a proportionate increase of AgNOR dots with increasing tumor grade. There was a significant difference between benign non-recurrenttumors versus benign recurrent (P<0.0001) and atypical or malignant (P<0.0001) tumors. A difference was also noted between the recurring tumors versus malignant ones (P = 0.002) but no significant difference was seen between recurrent and atypical; atypical and malignant; intracranial and intraspinal; and primary of recurring meningiomas with their subsequent recurrences. A mean AgNOR count of <2.3 could separate benign tumors from atypical or malignant meningiomas with 93% specificity; and 93% of tumors with benign histology had no recurrence potential if their mean AgNOR count was less than 1.8. CONCLUSION: This study indicates that in meningioma, the AgNOR count has a good correlation with tumor grading and recurrence, which may aid pathologists and clinicians in predicting tumor behavior.

[On the correlation between the texture of contrast agent ultrasonic image and the intracardiac pressure].

Based on the application of box-counting fractal model to the texture analysis of ultrasonic image after intravenous injection of Levovist acoustics contrast agent in a dog, this paper presents a method to calculate the fractal dimension(D) and assistant characteristic (C(L)) of the ultrasonic images of left ventricle. It was found that the D and C(L) changed regularly in the continued cardiac cycles, that is, the maximum value of D appears during diastasis, the minimum value appears during end-systole; as the ventricular systole begins, D changes from maximum to smallness, and as the diastole begins, D changes from smallness to maximum; the maximum value of C(L) appears around end-systole, the minimum value appears around diastasis; from the start of left ventricular systole to end-systole, C(l) shows a tendency to change from smallness to maximum, and from the start of diastole to diastasis, it shows a tendency to change from maximum to smallness. The changes of D and C(L) are very similar to the changes of left ventricular pressure. It is thus evident that there is correlation between the fractal texture characteristics of the contrast agent ultrasonic images and the changes of intracardiac pressure. The results demonstrate that it is possible to measure the intracardiac pressure noninvasively.

Radionuclide ventriculography in the assessment of diastolic function in hypertension.

Radionuclide ventriculography has been used for the assessment of left ventricular diastolic function in hypertension. Compared to cardiac Doppler ultrasound techniques, the radionuclide method offers the advantages of allowing numerous consecutive cardiac cycles to be examined, and of deriving the data of these examinations from automated software computer programs. For better precision, however, adequate instrumentation is required. In addition, correction factors should be applied to overcome the influence of elements such as heart rate, left ventricular end-diastolic volume, as well as the technical influence of the efficiency of counting among individuals. Its application to hypertensive patients enabled the diagnosis of early abnormalities of left ventricular filling rate in about 30% of the population studied before any alteration in indices of systolic function were detected. The mechanism of the alteration of these indices of diastolic filling rate in hypertension, however, is still not well understood.

Fetal movement monitoring: clinical application.

Although many factors influence fetal movements, maternal perception of gross fetal movement appears to be an accurate reflection of fetal activity. Observation in humans and studies in animals indicate that the compromised fetus reduces its oxygen requirements by reducing activity. This fact has enabled perceived fetal movement to become a useful adjunctive test of fetal asphyxia in high-risk pregnancy. Its application to low-risk pregnancy has many attractions, particularly as approximately 50 per cent of stillbirths occur without obvious cause in normal pregnancies. Although studies suggest that such application would be beneficial, questions about acceptability, the burden of further testing precipitated by reports of diminished movement, and the perinatal implications of unwarranted early intervention need to be clarified before the universal application of fetal movement counting protocols can be recommended.

Importance of Solcoseryl in infertility. 2. Effects of Solcoseryl with pathological spermiogram findings in vivo.

44 patients with different spermiogram findings were treated with Solcoseryl¿ (Messrs. Solco, Basel, Switzerland). Among the findings were 39 cases of oligozoospermia, 26 cases of asthenozoospermia and 7 cases of hypospermia. 20 subjects had already received other treatment without success. Treatment in these 20 cases had been terminted at least six months prior to our studies. All patients received 10 injections comprising 1 ampule each of Solcoseryl¿ at intervals of 1 day with breaks at the weekends. The spermiograms were analysed upon completion of treatment and two weeks, four weeks, three months and six months after completion of treatment. The effects of Solcoseryl¿ proved to be completely independent of previous treatment. The most noticeable improvement in he findings following Solcoseryl¿ application was the sperm count. This effect was observed already upon completion of treatment and reached a maximum two weeks later. The effect then decreased. In a few cases, however, sperm numbers continued to decline for up to two weeks after therapy before subsequently increasing. The findings for 44% of the patients with asthenozoospermia were at least temporarily normal after the treatment. In a few of these patients, however, the number of dead sperm also increased immeditely after therapy. Severe cases of asthenozoospermia were not influenced by Solcoseryl¿ treatment. Improvement was also found in the case of hypospermia. In view of the objective improvement of the parameters of the ejaculate and the fact that gravidity has so far occurred in 11 (25%) of the wives of the patients treated with Solcoseryl¿ (in some cases after the first three-monthly examination), attempted treatment with Solcoseryl¿ is recommended.

Counting absolute number of lymphocytes in quail whole blood by flow cytometry.

In a previous study, we reported a new method for counting quail blood cells. After quail blood cells were stained with fluorescent lipophilic dye (DiOC6(3)), absolute counts of erythrocytes, granulocytes, and monocytes were obtained by means of flow cytometry (FC). The FC method has the potential for application to avian blood cells count; however, the method was unable to distinguish between lymphocytes and thrombocytes. In the present study, we improved the FC method to obtain separate counts of lymphocytes using DiOC5(3). After quail blood cells were stained with DiOC5(3), the cells were measured with FC. Each blood cell type was distinguished by means of their typical FL-1 (green fluorescence) and SSC (side scatter). Absolute numbers of erythrocytes, granulocytes, monocytes and lymphocytes in whole blood were obtained. The improved FC analysis worked equally well with chicken (Gallus gallus) and goose (Anser cygnoides) blood.

Effects of postchill application of acidified sodium chlorite to control Campylobacter spp. and Escherichia coli on commercial broiler carcasses.

Experiments were performed to assess the reduction of Campylobacter spp. and Escherichia coli in commercial broiler carcasses by postchill dip applications of acidified sodium chlorite. Carcass rinses were collected before the inside-outside-bird washer (IOBW), post-IOBW, postchill, and after the postchill application of acidified sodium chlorite. Prevalence and counts of Campylobacter spp. and E. coli were determined. The mean values for Campylobacter spp. and E. coli counts differed significantly at sampling sites. The IOBW reduced the bacterial counts significantly in only one experiment. The chiller reduced Campylobacter counts significantly in both experiments but failed to significantly reduce the counts of E. coli in one experiment. No major reduction in the prevalence after enrichment for Campylobacter spp. was detected post-IOBW or postchill. However, a significant reduction in Campylobacter spp. and in E. coli counts and Campylobacter spp. prevalence was seen after the postchill application of acidified sodium chlorite. These results demonstrate that the antimicrobial effect of acidified sodium chlorite applied postchill may be used to significantly reduce Campylobacter spp. and E. coli in commercial broiler carcasses. Postchill systems may eventually be used in different applications, such as mist, spray, or bath, which could be applied closer to the final stages in processing.

[Statistical study of the optimal conditions of the spiral plating method for counting bacterial numbers in river water].

The application of the spiral plating method, a rapid and labor-saving technique for enumerating bacteria in food, to evaluate bacteria in river water, was examined. Standard plate counts and the numbers of heterotrophic bacteria, coliforms and Flavobacterium spp. in the water were tested by the spiral plating method, the results were compared with those of the conventional method and the optimal conditions for using the spiral plating method were considered and selected to fit the conventional methods. The optimal conditions selected were as follows: by laser colony counter after incubation at 35 degrees C for 38h. for standard plate counts; by laser colony counter after incubation at 25 degrees C for 48h. for the numbers of heterotrophic bacteria and Flavobacterium spp.; by colony viewer counting reddish colonies > or = 0.25mm in diameter after incubation at 35 degrees C for 22h. for the number of coliforms. The numbers of bacteria, except for coliforms, determined by this spiral plating method were found to be closely related to those from conventional methods (r > or = 0.91), and the replicating variances of both methods were not significant. The counts of bacteria by laser colony counter gave results similar to those by colony viewer counting by visual inspection. This spiral plating method saved time, money and labor in the evaluation of bacteria in river water as comparable to that in food. These results indicate that the spiral plating method can be used in place of conventional methods in evaluating the number of bacteria in river water.

Electron microscopical particle counting in virology. III. Parameters governing the efficiency of sedimentation techniques and its improvement with the application of poly-L-lysine.

In the preceding study the degree of adsorption of virus to electron microscope grids has been shown to be an important parameter governing the efficiency of sedimentation techniques used for particle counting. In the present study the application of poly-L-lysine not only improves the binding ability but makes it more reproducible. In addition the distribution of adsorbed particles between grid bars and holes is characterized and its importance as an additional parameter for such techniques discussed. For accurate particle titre estimation and for assessing the efficiencies of sedimentation techniques in virus diagnostics the product of these parameters represents a correction factor which must be taken into consideration. Furthermore it could be demonstrated, using the technique here described over the surface of a poly-L-lysine treated grid after drying. This phenomenon makes possible the estimation of a titre through particle counting on gravimetrically determined volumes.

G-CSF-stimulated PMN in immunotherapy of breast cancer with a bispecific antibody to Fc gamma RI and to HER-2/neu (MDX-210).

Myeloid cells can mediate tumor cell cytotoxicity via certain receptors for immunoglobulins. Among the different Fc receptors, the high-affinity IgG receptor (Fc gamma RI, CD64) is a promising trigger molecule because it is selectively expressed on effector cells, including monocytes/macrophages and granulocyte colony-stimulating factor (G-CSF)-primed neutrophils. In vitro, a bispecific antibody (BsAb) (MDX-210, constructed by chemically cross-linking F(ab') fragments of monoclonal antibody (mAb) 520C9 to HER-2/neu and F(ab') fragments of mAb 22 to Fc gamma RI) mediated effective lysis of HER-2/neu overexpressing breast cancer cell lines. HER-2/neu (c-erbB2) is overexpressed in approximately 30% of breast and ovarian carcinomas and is a target for immunotherapy in clinical trials. In vitro assays showed Fc gamma RI-positive neutrophils to constitute a major effector cell population during G-CSF therapy. Based on these preclinical data and a preceding study at Dartmouth (New Hampshire) with a single dose of MDX-210 alone, a combination of G-CSF and MDX-210 is tested in a phase I study in breast cancer patients. In this study, patients receiving G-CSF are treated with escalating single doses of MDX-210. This therapy was generally well tolerated by the treated patients, some of whom reacted with fever and short periods of chills, which were temporally related to elevated plasma levels of IL-6 and TNF-alpha. After MDX-210 application, a transient decrease in the total white blood count and absolute neutrophil count (ANC) was observed. During G-CSF application, isolated neutrophils were highly cytotoxic in the presence of MDX-210 in vitro. These data indicate a potential role for G-CSF and BsAb in immunotherapy.

A simple method for measuring DNA synthesis in epidermis and sebaceous glands and its application in chalone assays.

Liquid scintillation counting and autoradiography were used to measure DNA synthesis (3H-TdR uptake and incorporation) by the epidermis and the sebaceous glands in mouse pinna skin. Using this approach it has been possible to show that a chalone-like inhibitor of DNA synthesis acts rapidly to restrict the flow of epidermal G1 cells into S-phase. The point of action within the cell cycle is probably at the G1-S phase 'boundary'. The technique described is sufficiently sensitive to be used for measuring small alterations in DNA synthesis in short-term experiments and for long-term experiments in which the size of the progenitor cell population is altered by systemic or topical administration of compounds influencing DNA synthesis.

Detection of false-positives among total and fecal coliform counts by factorial analysis of correspondence.

Application of an analysis of correspondence to the biochemical characteristics of total and fecal coliforms isolated in the Ivory Coast permitted us to separate two small clusters of isolates different from the main clusters, which included isolates from human and animal feces. The isolates grouped in the small clusters were from water samples. An analysis of the biochemical characteristics which permitted the segregation of the "water-specific" isolates from the main clusters indicates that water-specific total coliforms were citrate positive, indole negative, and amygdaline positive. Water-specific fecal coliforms were either citrate positive, indole negative, amygdaline positive, and inositol negative or indole negative, amygdaline positive, and inositol positive. Any isolates not fitting the above patterns could be considered of fecal origin. If this observation is confirmed under temperate climates and for a greater number of isolates, these simple tests could be used to confirm the fecal origin of coliforms.

Influence of insulin-like growth factor-I (IGF-I) on nerve autografts and tissue-engineered nerve grafts.

To overcome the problems of limited donor nerves for nerve reconstruction, we established nerve grafts made from cultured Schwann cells and basal lamina from acellular muscle and used them to bridge a 2-cm defect of the rat sciatic nerve. Due to their basal lamina and to viable Schwann cells, these grafts allow regeneration that is comparable to autologous nerve grafts. In order to enhance regeneration, insulin-like growth factor (IGF-I) was locally applied via osmotic pumps. Autologous nerve grafts with and without IGF-I served as controls. Muscle weight ratio was significantly increased in the autograft group treated with IGF-I compared to the group with no treatment; no effect was evident in the tissue-engineered grafts. Autografts with IGF-I application revealed a significantly increased axon count and an improved g-ratio as indicator for "maturity" of axons compared to autografts without IGF-I. IGF-I application to the engineered grafts resulted in a decreased axon count compared to grafts without IGF-I. The g-ratio, however, revealed no significant difference between the groups. Local administration of IGF-I improves axonal regeneration in regular nerve grafts, but not in tissue-engineered grafts. Seemingly, in these grafts the interactive feedback mechanisms of neuron, glial cell, and extracellular matrix are not established, and IGF-I cannot exert its action as a pleiotrophic signal.
