Neuropathological and aetiological studies of sporadic non-suppurative meningoencephalomyelitis of cattle.

Sporadically occurring non-suppurative encephalitis appears to be a frequent condition of Swiss cattle. Fifty-one such cases diagnosed over a period of 10 years were examined retrospectively to investigate whether they constituted one or more distinct diseases, and to search for aetiological agents. Three cases were characterised by periventricular granulomatous encephalitis, and most probably represented a different disease, but the remaining 48 cases had disseminated non-suppurative encephalitis with widespread neuronal changes. Neuronal degeneration was very marked in the hippocampus of 10 cases and in the cerebellar Purkinje cells of 11. It was thought that the latter cases represented morphological variations of the same disease rather than a different disease because of their overlapping morphological features. The 48 cases had the following features in common: the disease had primarily neurological signs affecting mostly adult cattle, it was a sporadic condition, and there was a clear tendency for it to have a subacute to chronic course. Polymerase chain reaction (PCR) amplification for chlamydial DNA was negative except in one of 32 specimens, and immunohistochemistry did not demonstrate the presence of chlamydial antigens either in the one PCR-positive case or in the other cases examined. Immunohistochemistry for rabies virus, Borna disease virus, and central European tickborne encephalitis virus was negative. In four cases, immunolabelled cells were found in the lesions with antibodies against paramyxovirus antigens.

Comparative perilymph permeability of cephalosporins and its significance in the treatment and prevention of suppurative labyrinthitis.

Cephalosporins are nonototoxic antibiotics that provide excellent coverage for almost all bacteria that can cause suppurative labyrinthitis. In this study we performed comparative perilymph permeability determinations of the three cephalosporins that we deemed to have the most clinical potential in these varied situations. Perilymph pharmacokinetic profiles were established for ceftazidime, cefuroxime, and cefotaxime and its metabolite desacetylcefotaxime in 36 guinea pigs by using the technique of high-performance liquid chromatography. At 1, 2, 3, 4, and 6 hours after intravenous administration of the three cephalosporins at a dose of 100 mg/kg of body weight, ceftazidime consistently exhibited the highest perilymph concentration. Desacetylcefotaxime showed the next highest capacity for penetration into perilymph. Keeping in mind that the choice of drug for the treatment of suppurative labyrinthitis should be based foremost on culture and sensitivity studies, we consider ceftazidime to be the first-line agent for treatment and prevention of both meningogenic labyrinthitis and labyrinthitis complicating acute or chronic otitis media.

Routes of infection of acute suppurative thyroiditis diagnosed by barium examination.

A fistula between the left pyriform sinus and the left thyroid gland region has been reported to be the route of infection in several cases of acute suppurative thyroiditis. Another four patients with acute suppurative thyroiditis have been encountered in whom a fistula from the hypopharynx was demonstrated radiographically. In three patients the fistulous tract was situated on the right side, and in one of these cases the fistula originated from the epiglottic vallecula rather than the pyriform sinus. These locations have not been reported earlier. Endoscopic examination was valuable for confirming the diagnosis.

Typhoidal focal suppurative lymphatic abscess.

We describe a submandibular suppurative lymphatic abscess caused by Salmonella typhi in an 8-year-old child. The diagnosis was confirmed by repeated isolation of S. typhi from the abscess. A literature search found no previous report of a similar nature and this therefore seems to be the first case report of focal suppurative typhoidal lymphatic abscess. The child responded to cephalexin and surgical drainage.

Treatment of suppurative arthritis by percutaneous catheter drainage.

Percutaneous catheter drainage to treat suppurative arthritis was performed in five joints in five patients. Joints drained included the hip in two cases and one case each of a hip joint prosthesis, an ankle joint, and a glenohumeral joint. Organisms isolated from the joints included Staphylococcus aureus in one hip joint and the hip prosthesis, and Haemophilus influenzae in the ankle joint. Specific organisms were not isolated in the other hip joint or in the shoulder joint. Systemic antibiotic therapy was used in all five patients, and in two patients gentamicin was instilled through the catheters. Joint infection was managed successfully with catheter drainage and antibiotics in three patients. In all three cases, the range of motion was restored and the patients became free of pain after catheter drainage. These three patients remained asymptomatic at follow-up ranging from 3 weeks to 9 months. In two patients, percutaneous drainage failed. In one patient, the catheter positions could not be maintained and the catheters repeatedly became dislodged. In the other, superimposed osteomyelitis necessitated surgical debridement. No complications occurred. Our experience suggests that suppurative arthritis can be successfully treated with drainage of the joint via a percutaneous catheter in combination with antibiotic therapy.

Acute suppurative thyroiditis in a primary intrathoracic goiter.

OBJECTIVE: To describe a case of acute suppurative thyroiditis and focal abscess formation in a patient with a primary intrathoracic goiter. METHODS: We summarize the initial clinical manifestations, the pathologic findings, and the surgical treatment of a woman with an enlarging mediastinal mass. RESULTS: A 41-year-old woman had a fever and a sudden increase in severity of compressive symptoms after a remote urinary tract infection. Smears from an intraoperative fine-needle aspiration biopsy showed necrotic debris, polymorphonuclear leukocytes, hemosiderin-laden macrophages, and degenerated follicular epithelial cells. Urine and tissue cultures grew Escherichia coli. The patient underwent resection of an 800-g primary intrathoracic goiter with associated degeneration, necrosis, hemorrhage, and abscess formation. CONCLUSION: To our knowledge, this is the first reported case of acute suppurative thyroiditis and abscess formation occurring within a primary intrathoracic goiter.

Nodular suppurative cutaneous cellulitis in a Galapagos sea lion.

Necropsy was performed on a sea lion (Zalophus californianus wollebaeki), sacrificed in an advanced (pre-terminal) stage of disease, possibly represenatative of the Galapagos epizootic of 1970-71. Predominant features of the disease were nonumbilicated multiple suppurative cutaneous nodules, debilitation and loss of motor power. Histopathological studies of the skin lesions disclosed suppurative cellulitis, with leucocytic invasion extending, in some instances, to all layers of the epidermis. Pseudomonas aeruginosa was recovered from cultures of blood and pus.

[Clinical x-ray characteristics of acute suppurative destructions of the lungs of a nonstaphylococcal nature in children].

The authors conducted complex clinico-roentgenological and immuno-bacteriological studies in 63 children, aged from 14 days to 14 years, with acute suppurative destructions of the lung, complicated with the development of pyothorax. As a result of these investigations, it was found that clinico-roentgenological symptomatics in children with acute suppurative destructions of the lung of the non-staphylococcal nature (Proteus, pyocyanic bacillus and others) show evident peculiarities and differ from typical manifestations of staphylococcal destruction of the l,ng in children. Complex clinico-roentgenological and immuno-bacteriological studies in children with the mentioned pathology enabled the authors to differentiate an infective nature of the disease.

[Use of chromato-mass-spectrometry for the differential diagnosis of suppurative and nonsuppurative inflammations in the maxillofacial area in children].

Ten children with inflammatory processes in the maxillofacial soft tissues and 10 healthy children were examined using chromato-mass-spectrometry of the chemical composition of oral cavity air. Air samples were collected by an adsorbent sample collector by individual sucking and analyzed on a Finnigan MAT chromato-mass-spectrometer. The concentrations of short-chain fatty acids and minor ketones were increased and normally absent isobutyl alcohol appeared in the oral cavity air in patients with serous inflammations. The content of valeraldehyde, isoprene, sulfur compounds increased significantly in suppurative inflammations; ramified long-chain aldehydes, absent in serous inflammation, appeared. The ratios of the following substance concentrations were estimated: acetaldehyde/valeraldehyde, pentane/dimethylsulfide, undecane/dimethylsulfide, which can be used for the differential diagnosis of suppurative and nonsuppurative inflammations of the maxillofacial soft tissues in children.

Mediastinitis after cervical suppuration.

The objective of this study is to decode the etiopathogenesis, the clinical feature, the diagnosis and the prognosis of the acute mediastinitis resulting of infectious processes of the cephalic segment. Three out of five patients studied presented focus of dental origin and two patients presented focus in the face area. One of them presented Ludwig's Angina before the suppurative process would spread over the mediastinum. In the others, there was fast expansion through the fascial spaces of the neck and, in two of them, besides the mediastinum, there was pleuropericardial involvement. Three patients died due to respiratory insufficiency and two survived with complications. The mediastinitis after cervical suppuration is a special and extremely serious kind of endothoracic infection. The pus reaches that area through the fascial spaces of the neck, taking the organism to an alarming toxemic feature. The rarity of the disease, the little is known about its physiopathology and the initial care of the patient in non-specialized services, which are not familiarized with this type of feature, are factors that can delay the diagnosis and worsen the prognosis.

Management of conductive deafness of non-suppurative aetiology in Nigeria.

A review of 39 cases of deafness of non-suppurative aetiology seen in the Department of Otolaryngology of the University College Hospital, Ibadan between 1966 and 1971 is presented. Deafness of this type is not as common as deafness of suppurative origin but it presents problems of its own. First there is the inability of many patients to give accurate histories and describe actual sensation in the ear. Secondly, there is the problem created by patients not turning up for regular follow up. Thirdly, there is the problem of unavailability of necessary equipment such as that highlighted by the use of straight polythene tubes instead of the shaped grommet in the management of serous otitis media and the non-availability of prosthesis to replace the stapes when stapedectomy may have helped the patients. Suggestions are also offered on the ways of improving the treatment of the patients.

[The treatment of suppurative and atonic wounds with the CO2 laser and the helium-neon laser].

The doctrine on wounds is the most important and current one in general surgery and reflects the level and development of medicine. Each surgeon should be well acquainted with all processes and stages of wound healing, because treatment of a suppurative wound requires keen knowledge and art. The basic principle of the local treatment of a septic wound is the thorough knowledge on the objective criteria underlying the course of the wound process and on the etiopathogenetic therapy. Laser therapy of wounds accomplishes adequate necrotomy, acts aseptically on the wound surfaces, inhibits the wound microbial flora and stimulates the regeneration processes. The authors record their experience in CO2 laser and helium-neon treatment of 120 patients with suppurative, atonic and decubital wounds over the period 1984-1990.

CO2 laser for suppurative hidradenitis of the vulva.

Chronic suppurative hidradenitis is a disease of the apocrine glands occurring predominantly in the axillae of the anogenital region. Treatment, to date, has been frustrating. Prolonged antibiotic therapy and multiple incisions and drainage have been the usual form of therapy, but those surgical procedures leave patients with ugly scars; retracted, hypertrophic skin edges; and pitted, indurated and disfiguring abscesses. The only successful treatment to date has been wide excision of the entire apocrine gland-bearing tissues, such as radical vulvectomy, which often necessitates grafts to cover the surgically exposed areas. That has left patients with unsatisfactorily functioning vaginas and a mutilated appearance. Recently we instituted the use of the CO2 laser to treat these patients with the goal of eradicating the suppurative sinus tracts and the infected apocrine glands without removing large volumes of adjacent, unaffected tissue. Over the past eight years we treated 11 patients who had extensive, chronic hidradenitis of the anogenital area with the CO2 laser. The use of laser therapy for these patients has produced effective and less-mutilating results. Such treatment has been a successful option for treating this debilitating disease.

[Clinico-morphologic evaluation of the course of the wound process after treatment of suppurative wounds with the CO2 laser].

The complex examination and treatment of 238 patients with suppurative wounds of the soft tissues were carried out. Treating a suppurative wound by means of CO2-laser leads to reduction in microbial colonization of the tissues in its depth, accelerates maturation of granulations, wound cicatrization. The use of CO2-laser permitted to achieve primary healing of 94.3% of the wounds. The average duration of hospital stay of the patients was (9.2 +/- 1.4) days, in control--(11.8 +/- 1.6) days.

[Genetic aspects of the frequency of occurrence of suppurative highmoritis].

One hundred and fifty patients with chronic suppurative maxillary sinusitis (89 men and 61 women), aged 15 to 50 years, were examined. A potential correlation between the incidence of the disease and blood genetic markers (ABO-H blood group and Rh-factor) was investigated. No correlation was found between the incidence and ABO-H in that group of patients as a whole. The patients with documented staphylococcal nature of suppurative maxillary sinusitis belonged for the most part to the A (II) - Rh (D) + blood group.

[The experimental and clinical use of gelatin sponges with kanamycin and gentamycin for the prevention and treatment of suppurative complications in neurosurgery].

Antiseptic gelatin sponges with kanamycin and gentamicin, which were studied in experiments on animals and used in the clinic, are a reliable measure for the prevention of suppuration in craniocerebral surgery in planned operations and in combination with debridement in penetrating injury of the skull and brain. For therapeutic purposes, local application of the agents is effective in focal suppurative processes in the skull and brain after surgical debridement of the purulent focus (abscess of the brain, osteomyelitis of the skull, etc.).
