Correlates of age at first sexual intercourse in a national sample of young women.

A subsample of 814 sexually experienced adolescent females from the 1979 U.S. National Survey of Young Women was analyzed to assess the correlates of age at 1st sexual intercourse. Multiple regression procedures were used to examine sets of variables sequentially. In the hierarchical regression model, the control variables (respondent's age, race, religion, and age at menarche), along with 3 independent variables (household income, ideal age at 1st marriage, and ideal age for 1st birth), predicted age at 1st intercourse. The control variables accounted for a major portion of the variance in the model. Of the controls, chronological age and age at menarche were highly significant across all models tested.

Probable Marden-Walker syndrome: evidence for autosomal recessive inheritance.

Two first cousins, the offspring of consanguineous marriages, had features suggestive of Marden-Walker syndrome. Phenotypic similarities and differences for Schwartz-Jampel syndrome have been discussed. Main features of the Marden-Walker syndrome are failure to thrive, marked motor and mental retardation, and multiple malformations in the form of peculiar facies associated wilth poor muscle mass, mild congenital joint contractures, pigeon breast, kyphoscoliosis and arachnodactyly. Peculiar facies is due to blepharophimosis, congenital ptosis, hypoplastic mandible and low-set and malformed ears. Posterior median cleft of the palate as well as cardiac and renal anomalies were noted in the case reported by Marden and Walker. Our Case 2 had dextrocardia. The present report suggests autosomal recessive inheritance of this syndrome.

[Study on the risk factors of 100 cases with vulvar dystrophy].

OBJECTIVE: To assess the risk factors for vulvar dystrophy. METHODS: An epidemiological study was carried out. Data on 100 cases with vulvar dystrophy was reviewed and face to face interviewed with a uniform questionnaire including the manner of work, environmental temperature, habit of eating, mood, underwear wearing, autoimmune diseases, marriage, menstrual age, the quantity of menses, orders of pregnancy, and labor trauma of vulvar during delivery, vulvitis and urethritis ect. Univariable analysis and multivariate logistic regression analysis were carried out with 1:1 case-control methodology. RESULTS: Multiple conditional logistic regression analysis showed that vulvar dystrophy was positively associated with hot food (OR = 2.55, 95% CI: 1.24 - 5.25), mood (OR = 4.27, 95% CI: 1.96 - 9.29), order of pregnancy (OR = 3.37, 95% CI: 2.11 - 5.40), vulvitis (OR = 6.74, 95% CI: 2.66 - 17.09) and urethritis (OR = 11.02, 95% CI: 1.01 - 120.19). Vulviitis or urethritis increased 6.74 or 11.02 times the incidence of vulva dystrophy. Anger or nervous state contributed to the incidence of vulva dystrophy (OR = 4.27). Addict to hot food and order of labor also increased risk ratio for 2.55 and 3.37 times, respectively. CONCLUSION: The risk factors of vulvar dystrophy were: addict to hot food, often holding a angry or nervous state, increase of labors, having vulvitis and urethritis.

Amobarbital treatment of multiple personality. Use of structured video tape interviews as a basis for intensive psychotherapy.

The case of a 30-year-old woman with five distinct personalities is presented. The patient was treated, using a system of structured video taped sodium amobarbital interviews, in which areas to be explored were developed in psychotherapy. Tapes were played for the patient after each session. The taped material was used as the basis for psychotherapeutic investigation. The patient evidenced many of the features previously reported in cases of multiple personality, specifically: being the product of an unwanted pregnancy in a repressively rigid family; emotional distancing by one parent; strong sibling rivalry with an adopted sib; family history of mental illness; a traumatic first sexual experience (rape); a marriage to a maladjusted individual in an attempt to escape the parental home; a high internalized standard of performance and an inability to display anger or negative feelings toward the parents. In the course of treatment, the patient's personalties fused and she was able to accept each component as part of herself. No further fragmentation has occurred during the year following discharge. The therapy technique minimized dependency, and the possiblity of addiction to amobarbital interviews permitted more active patient therapy involvement, and set clear-cut goals and expectations for improvement before further amobarbital interviews could be conducted.

Psychological wellbeing in later life: the longitudinal effects of marriage, widowhood and marital status change.

AIM: The study examined cross-sectionally and longitudinally psychological wellbeing in the context of marital status, gender, and age. METHOD: Measures were taken eight years apart for three groups: married at both interviews; widowed at both interviews; and married at first interview but widowed at third. Data were analysed using multiple regression models: dependent variables were morale and social engagement; independent variables were marital status, gender and age. RESULTS: Cross-sectionally significant differences were found for marital status and age for both morale and social engagement. Both widowed and newly widowed participants reported lower morale and social engagement than their married counterparts. In the longitudinal model, when prior levels of morale were taken into account, only age and being newly widowed contributed significantly to the variance (R2). The same pattern of results was found for social engagement. Gender was never significant. CONCLUSION: The results illustrate the value of modelling both cross-sectional and longitudinal data. The latter demonstrated that the size of the effect differed between those who had recently widowed and those who had been widowed for longer. The study shows that age needs to be taken into account when examining widowhood.

Case report of a rare syndrome associating amelogenesis imperfecta and nephrocalcinosis in a consanguineous family.

A rare syndrome associating amelogenesis imperfecta (AI) with nephrocalcinosis has been reported. The purpose of this study is to characterise the phenotype of a consanguineous family presenting amelogenesis imperfecta, delayed permanent teeth eruption and nephrocalcinosis. Six family members were examined. Ground sections of the case index deciduous teeth and biopsies of enlarged dental follicles were analysed. The patients's parents were first cousins. The case index had yellow discoloration and altered teeth shapes, retention of deciduous teeth, and delayed eruption. Panoramic radiographs revealed multiple enlarged pericoronal follicles in unerupted teeth and generalised intrapulpal calcifications. Renal ultrasound showed the presence of nephrocalcinosis. No other family members presented enamel defects or nephrocalcinosis. Histologically, the enamel appeared hypoplastic, and dental follicles indicated pericoronal hamartoma. The consanguineous marriage suggests an autosomal recessive mode of inheritance. Further studies are necessary to clarify the genetic defect behind this syndrome that associates AI, nephrocalcinosis and impaired tooth eruption.

Multivariate survivorship analysis using two cross-sectional samples.

As an alternative to survival analysis with longitudinal data, I introduce a method that can be applied when one observes the same cohort in two cross-sectional samples collected at different points in time. The method allows for the estimation of log-probability survivorship models that estimate the influence of multiple time-invariant factors on survival over a time interval separating two samples. This approach can be used whenever the survival process can be adequately conceptualized as an irreversible single-decrement process (e.g., mortality, the transition to first marriage among a cohort of never-married individuals). Using data from the Integrated Public Use Microdata Series (Ruggles and Sobek 1997), I illustrate the multivariate method through an investigation of the effects of race, parity, and educational attainment on the survival of older women in the United States.

[Adjustment disorders: apropos of an epidemiologic survey. Epidemiology and Psychiatry Group].

An epidemiologic survey performed in France in 1990 allowed us to analyze the epidemiologic characteristics of adjustment disorders (AD). AD were defined according to the DSM III-R guidelines. AD are strongly correlated to personality disorders. The study population was young (mean age: 39 years) and predominantly female (60.3%). Marriage or life with a partner, living in the country or in small towns, liberal profession and a high level of schooling are the most significant social and demographic characteristics that emerge from this study. These patients tend to have multiple medical contacts, seeing many doctors but consuming little medication (25% of them). Comorbidity with associated personality disorders (15%) and dependency behaviors (alcohol, drugs) are frequent (9.3%), although no causative link was found between these disorders. Alcoholism may have a tendency to protect against AD. There is no seasonal factor. The patients mostly see private physicians; their geographic distribution is the inverse of the distribution of alcoholism. Numerous social and demographic risk factors for AD were found. Among the personal psychiatric history, disorders of feeding behavior and early adjustment problems were prominent. The family history often shows the existence of mental disorders in the parents. The treatment history points to a lesser consummation of psychotropic drugs, mostly limited for two classes, i.e. antidepressants (50%) and tranquilizers (40.9%), although a different pattern of behaviour was found according to the clinical types of AD. The present therapeutic approach is recent and based on psychotherapy. It differs little according to the clinical forms, with the possible exception of co-prescribed medication. This study has thus allowed us to observe the epidemiologic characteristics of AD. Adjustment disorders appear to be frequent, but their particularities do not differentiate them significantly from other types of mental disorders.

Socio-cultural dynamics of female genital cutting: research findings, gaps, and directions.

The goal of abolishing female genital cutting (FGC, or also FGM or 'female circumcision') requires that the socio-cultural dynamics of the practice be well understood if behavioural change is to be accomplished. This paper, based on the literature and the author's ethnographic research in Sudan, reports on the research issues of studying the variation in and complexity of cutting practices and their cultural correlates, arguing for multiple approaches and methods. It highlights directions for future research.

Multiple roles and well-being among midlife women: testing role strain and role enhancement theories.

OBJECTIVE: Research on women's multiple roles frequently adopts one of two perspectives: role strain, which argues that assuming multiple roles is detrimental to mental well-being, or role enhancement, which argues that engaging in multiple roles enhances mental well-being. We argue that the relationship between role occupancy and well-being is manifested through multiple dimensions of role experiences. We investigate the association between depressive symptomatology and various dimensions of the roles of wife, mother, paid worker, and informal caregiver to aging parents. METHODS: Data are from the 1992 wave of the Health and Retirement Study. Depressive symptomatology, measured by a subset of the CES-D scale, is the dependent variable. To assess the robustness of findings relative to different functional forms of the dependent variable, we estimate multiple regression, log-linear regression, and multinomial logit models. Independent variables include demographic characteristics, measures of role occupancy, role demands, and role satisfaction. RESULTS: Although the number of roles women assume affects their reports of depressive symptoms, once the demand and satisfaction associated with these roles is controlled, number has no effect; that is, the effect of the number of roles is indirect. DISCUSSION: Our results highlight the importance of women's perceptions of the quality of their roles in relation to their overall well-being. Future investigations of women's multiple roles should examine how roles may provide rewards, impose constraints, or generate conflict, as well as the extent to which the willingness to assume multiple roles and the reported levels of role satisfaction and mental well-being may be jointly endogenous.

Personality and multiple divorce. A prospective study.

A review of cross-sectional and prospective research in both normal and clinical samples suggests that increased risk of divorce is associated with socially nonconforming, impulsive, and stimulus-seeking personality traits. This exploratory study addresses the question of whether individuals who divorce more than once are especially likely to exhibit such general personality dispositions. Subjects were male physicians (N = 431) who had completed the MMPI before entering medical school and who were followed up more than two decades later by a mail questionnaire that inquired about their health status, health practices, subjective well-being, and marital history. As indicated by higher scores on the MMPI Psychopathic Deviate (Pd) scale and greater likelihood of concurrently reporting several negative health practices (e.g., cigarette smoking), multiply divorced physicians did tend to exhibit greater nonconforming, impulsive, and risk-taking tendencies than both never-divorced and once-divorced physicians. Possible social-psychological processes linking such personality dispositions to the risk of multiple divorce are discussed, along with suggestions for further research.

Severe pre- and postnatal growth retardation, developmental delay with hypotonia and marked hypotrophy of the distal extremities, dental anomalies, and eczematous skin. A new autosomal recessive entity.

We report on three young children, two girls and one boy, with pre- and postnatal growth deficiency, hypotonia, psychomotor retardation with notably impaired speech development, hypotrophy of the distal extremities, small hands and feet, small and widely spaced teeth, eczematous skin, and, in two of them, a partial agenesis of the corpus callosum. To our knowledge this specific combination of features has not been reported before. Since the two girls are sisters and the boy is the product of a consanguineous marriage, the inheritance of this new syndrome appears to be autosomal recessive.

Choanal atresia and lymphedema.

The coexistence of bilateral bony choanal atresia and lower-extremity lymphedema was observed in a family with several consanguineous marriages. The combination of these two rare anomalies, in a hereditary form, has not been described before.

Intimate partner violence against women in eastern Uganda: implications for HIV prevention.

BACKGROUND: We were interested in finding out if the very low antenatal VCT acceptance rate reported in Mbale Hospital was linked to intimate partner violence against women. We therefore set out to i) determine the prevalence of intimate partner violence, ii) identify risk factors for intimate partner violence and iii) look for association between intimate partner violence and HIV prevention particularly in the context of the prevention of mother-to-child transmission of HIV programme (PMTCT). METHODS: The study consisted of a household survey of rural and urban women with infants in Mbale district, complemented with focus group discussions with women and men. Women were interviewed on socio-demographic characteristics of the woman and her husband, antenatal and postnatal experience related to the youngest child, antenatal HIV testing, perceptions regarding the marital relationship, and intimate partner violence. We obtained ethical approval from Makerere University and informed consent from all participants in the study. RESULTS: During November and December 2003, we interviewed 457 women in Mbale District. A further 96 women and men participated in the focus group discussions. The prevalence of lifetime intimate partner violence was 54% and physical violence in the past year was 14%. Higher education of women (OR 0.3, 95% CI 0.1-0.7) and marriage satisfaction (OR 0.3, 95% CI 0.1-0.7) were associated with lower risk of intimate partner violence, while rural residence (OR 4.4, 95% CI 1.2-16.2) and the husband having another partner (OR 2.4, 95% CI 1.02-5.7) were associated with higher risk of intimate partner violence. There was a strong association between sexual coercion and lifetime physical violence (OR 3.8, 95% CI 2.5-5.7). Multiple partners and consumption of alcohol were major reasons for intimate partner violence. According to the focus group discussions, women fear to test for HIV, disclose HIV results, and request to use condoms because of fear of intimate partner violence. CONCLUSION: Intimate partner violence is common in eastern Uganda and is related to gender inequality, multiple partners, alcohol, and poverty. Accordingly, programmes for the prevention of intimate partner violence need to target these underlying factors. The suggested link between intimate partner violence and HIV risky behaviours or prevention strategies calls for further studies to clearly establish this relationship.

Prevalence of anaemia in pregnant & lactating women in India.

BACKGROUND & OBJECTIVES: The prevalence of anaemia during pregnancy and lactation was significantly lower in the National Family Health Survey 1998-1999 (NFHS-2), using the hemocue method for haemoglobin estimation compared to earlier surveys. The present study selected seven States and used the same districts and villages studied in the NFHS-2, to see if the reported reduction in prevalence of anaemia was due to health and nutrition inputs and/or due to a different method for haemoglobin estimation. METHODS: A total of 1,751 women (1,148 pregnant and 603 lactating- exclusively breastfeeding up to 3 months of age), from seven States- Himachal Pradesh and Haryana in north; Assam and Orissa in east; Kerala and Tamil Nadu in south and Madhya Pradesh in central India, were selected. Haemoglobin was estimated by the cyanmethaemoglobin method, so that comparison was possible with earlier studies. Data on socio-demographic characteristics, pregnancy, nutritional status and dietary intakes were collected. RESULTS: Prevalence as well as severity of anaemia was significantly higher in the present study as compared to the NFHS-2 study data. The difference could be due to haemocue method, which gives higher haemoglobin values. The contributing factors found on multiple regression analysis for anaemia in pregnancy and lactation were: literacy, occupation and standard living index of the study women; their awareness about anaemia, its prevention by regular consumption of ironfolate tablets and increase in food intake. Maternal height, age of marriage, parity and foetal loss also contributed to haemoglobin level. There were interstate differences; lower fertility, higher literacy and better diet was observed in Himachal Pradesh as compared to Haryana. The literacy and nutritional status of women in Tamil Nadu was lower than Kerala. The remaining 3 states had poor fertility, lower social living index and nutritional status with >90 per cent women being anaemic in pregnancy and lactation. Low prevalence of severe anaemia in Orissa as compared to Assam was due to availability and consumption of iron folate tablets. The antenatal services in the first trimester and checkup by a doctor, along with availability and consumption of iron folate tablets over 3 months in all the States influenced haemoglobin levels. INTERPRETATION & CONCLUSION: Despite the measures taken to control anaemia in pregnancy and lactation in the last two decades, the severity of nutritional anaemia continues to remain a public health issue of great magnitude, suggesting that these measures have been largely ineffective. The present findings also showed interstate differences particularly in fertility, women education, nutrition status and occupation; availability of antenatal services and iron folate tablets as possible factors responsible for differences in prevalence of anaemia.

Home predictors of young adolescents' school behavior and academic performance.

The purpose of the present study was to examine the relation between home variables and young adolescents' school behavior/performance. 46 mother-father-adolescent triads (ages 11.5 years to 14.9 years) served as participants. Home factors included the adolescents' relation with each parent as well as a measure of each parent's personal adjustment and perception of conflict in the marriage. School measures consisted of grades and a teacher-completed assessment of internalizing and externalizing problems demonstrated by the adolescent. The results indicated that externalizing problems were related to the mother's depression level, the adolescent's relationship with the mother, and the adolescent's relationship with the father. The first 2 variables (maternal depression and mother-adolescent relation) entered into a multiple regression equation with sex of adolescent and accounted for 35% of the variance in externalizing problems. The relationship with the father was related to school grades and, in a multiple regression, combined with birth order of the adolescent to account for 15% of the variance. The results were discussed in terms of the importance of considering not only the mother-child relationship when examining cross-setting influences in the school but also the mother's personal adjustment and the father's relationship with the child.

Marital status, indicators of sexual activity and prostatic cancer.

STUDY OBJECTIVE: To analyse the relationship between marital status, indicators of sexual activity, history of urological and venereal diseases, and the risk of prostatic cancer. DESIGN: Case-control study. SETTING: A network of cooperating hospitals from northern Italy. PARTICIPANTS: A total of 271 patients with histologically confirmed prostatic cancer and 685 controls in hospital because of acute, non-neoplastic, non-genital or urological conditions. MEASUREMENTS AND MAIN RESULTS: Relative risks (RR) and the corresponding 95% confidence intervals (CI) derived from multiple logistic regression equations, including terms for age, area of residence, and education were determined. The risk of prostatic cancer was lower in never married than in married men (RR = 0.6), but not significantly so. Cases reported a significantly higher number of marriages than control subjects, and the RR was 3.2 (95% CI = 1.2, 8.9) for two or more marriages compared with never married men. Prostatic cancer patients also reported being significantly older at the time of their first marriage: compared with men who first married under age 25 years, the RR was 1.6 for marriage at age 25 to 29, and 1.8 for age 30 or more. With regard to urological or venereal diseases, only cystitis and nephrolithiasis were more frequently reported by cases, although there was no tendency for the risk to increase with the number of cystitis episodes and the RR decreased for longer periods since the first episode. CONCLUSIONS: Although these results do not show a totally cohesive picture, they confirm that some aspects of sexual lifestyle are associated with prostatic cancer in Italy.
