Prosthetic treatment and rehabilitation: use in patients with cancer of the head and neck.
Dental management of the patient with cancer, especially when the disease process involves the head and neck region, should be an integral part of the patient's overall treatment plan. By utilizing the team approach and detailed advance planning, stressing both immediate treatment to control the patient's disease and short- and long-term rehabilitation, the best possible treatment for each patient can be formulated with a minimum of posttreatment complications. If an institution is to make the commitment to treat patients with cancer, a comprehensive dental support program with a maxillofacial prosthodontist is a necessity. As the benefits of a dental department are recognized, the utilization of this service increases and the overall treatment and rehabilitation of the patient with cancer is improved.
Rehabilitation of a patient with complete mandibulectomy and partial glossectomy.
Following a number of radiologic and surgical procedures for the treatment of oral cancer, a patient with severe facial disfigurement and alteration of the vocal tract acquired acceptable speech. Consultation among referring physicians and speech pathologists can aid such a patient by facilitating the rehabilitative process through improvement of communicative skills.
Reconstruction of mandibular discontinuity defects using autogenous grafting and a mandibular reconstruction plate: a prospective evaluation of nine consecutive cases.
This article presents the clinical results of a prospective evaluation of nine cases of mandibular discontinuity that were reconstructed using autogenous grafting and a reconstruction plate. All patients were allowed to function immediately postsurgery. Acceptable esthetic and functional results were obtained with minimal donor and recipient site complications. All cases showed complete graft incorporation and restoration of osseous continuity.

New concepts in the reconstruction of mandibular defects following tumor resection.
The results of experiments with a new reconstruction plate used to bridge defects in irradiated and nonirradiated mandibles of dogs are presented. The principles of fixation of the plate with hollow titanium powder-sprayed screws and a titanium mesh for stabilizing the autologous cancellous bone are discussed. The direct contact between the surface of the screw and the bone, as well as proliferation of bone into the hollow screw, are demonstrated by light and scanning electron microscope studies. Autologous cancellous bone fixed with the new mesh leads to bridging of the defect even in irradiated areas.
Mandibular resorption from silicone chin implants in children.
Silicone implants were placed in the nasal, cheek, and chin areas in children with Down's syndrome to improve facial appearance. Clinical and radiologic follow-up evaluation revealed significant crater-like bone resorption in the mandibular symphysis in 75% of the cases. All of these implants were removed, and the bone showed regrowth to the presurgical state. No sign of bone damage was present in the malar or nasal areas.
Analysis of reconstruction of mandibular defects using single stainless steel A-O reconstruction plates.
PURPOSE: This report reviews the results and complications of immediate reconstruction using a single stainless steel A-O plate after resection of mandibles invaded by malignant tumors. PATIENTS AND METHODS: Thirty-four patients were retrospectively evaluated. According to the extent of the tumor, hemimandibulectomy or segmental resection were performed in 4 and 30 cases, respectively. Five of 34 cases required a myocutaneous flap to close soft tissue defects. RESULTS: Nine of the 34 patients died during the follow-up period. Complications occurred in 7 of these patients: plate exposure was caused by local tumor recurrence (4 cases), screw loosening (1 case), and temporomandibular joint pain (2 cases). Twenty-five of the 34 patients (73.5%) were alive, with a mean follow-up of 47.4 months (range 6 to 115 months). Eighteen of the 25 surviving patients with the single A-O plate reconstruction did not show any complications. Thirteen of these (52%) patients had not undergone secondary reconstruction. Five of the 25 patients received secondary reconstruction for cosmetic or masticatory function reasons. Postoperative complications occurred in 7 of the 25 surviving patients (28%): extraoral plate exposure (2 cases), fracture of the plate (1 case), mandibular fracture (1 case), screw loosening (3 cases), and screw fracture (1 case). CONCLUSIONS: On the basis of the findings, it was concluded that a single A-O reconstruction plate can be used for temporary, or sometimes even permanent, reconstruction after mandibular resection.
