[Success in the induction of labor by peroral administration of Prostin E 2 in relation to Sandopart].

The results of the use of Prostin E2 in the induction of labour in 46 pregnant women were compared with the results obtained by Sandopart in 40 pregnant women. To avoid the effect of amnyotomy, both groups were examined without carrying out routine amnyotomy. The results have shown that a significantly better efficacy in the induction of labour was obtained with Prostin E2. This efficacy was very good regardless of the cervix assessed by the Bishop score, while the reaction of the uterus to Sandopart proved the poorer the lower the Bishop score. The mother's condition, established by clinical and laboratory examinations, did not change significantly in the course or after the administration of Prostin E2. Nor did the condition of infants, assessed by the Apgar score, differ from the condition of children from the group of labour induced by Sandopart. This shows that the induction by Prostin E2 is just as efficacious as the induction by Sandopart if the cervix is mature; if the cervix is not mature, the induction by Prostin E2 is even more efficacious.

Control of parturition in ruminants.

Parturition is a process which, when set into motion, occurs to completion. This review concerns the control of parturition in ruminants. Parturition is an endocrine event, dependent upon the activation of the fetal hypothalamus-pituitary-adrenal (HPA) axis. In sheep and other ruminants, increases in plasma concentrations of cortisol induce the activity of 17-hydroxylase and 17,20 lyase in the placenta, increasing the biosynthesis of oestrogen relative to progesterone. The increase in the so-called E:P ratio increases myometrial activity and culminates in labour and delivery. Much work has been done to identify the mechanism of the endogenous activation of the fetal HPA axis. Recent work suggests that production of prostanoids within the fetal brain influences fetal ACTH secretion, and that induction of prostanoid biosynthesis at the end of gestation might be important in the process of parturition. Oestrogen and androgens, secreted by the placenta at the end of gestation, augment activity of the fetal HPA axis by increasing fetal ACTH secretion and by decreasing negative feedback sensitivity to cortisol. Although significant progress has been made concerning the neuroendocrinology of parturition, many significant questions remain. Is parturition regulated or simply programmed? Is parturition the ultimate result of neuronal maturation within the fetal hypothalamus, or is there a complex interplay between the placenta and fetal hypothalamus? Answers to these and other important questions await further research, but may provide key information which will prove useful in understanding general principles of parturition in many mammalian species.

[The role of leukocytic factors in the preparation for and induction of labor].

Preparation for labour and its induction are controlled by an integrative but not single process. The primary stimulus that comprises this integrative hormonal prostaglandin-leukocytic mechanism is a suppression of local progesterone action with further inhibition of perivascular prostaglandine dehydrogenase and sharp increase of E2 and F2 prostaglandine content in uteroplacentary mechanism bed. Vasodilatory effect of prostaglandines along with synergication of interleukine 8 leads to leukocyte emigration and formation of infiltrates consisting of neutrophilic granulocytes and certain number of macrophages and mast cells. Hydrolytic enzymes released by neutrophilic granulocytes cause degradation of collagenic tissue in uterine cervix promoting its ripening. The factors mentioned also influence loosening of choriodecidual interface. Further amplification of effect of prostaglandine including those produced by neutrophilic granulocytes along with the other uteroconstrictors increase myometrium contractions which leads to labour induction.

Supportive care during labor: a guide for busy nurses.

Removing the barriers to the provision of supportive nursing care during labor will require much effort by multidisciplinary teams within each institution. Yet even without such reform, an individual nurse can easily provide crucial elements of supportive care. This article suggests many practical techniques that nurses can use to support women during the stresses of labor. Each requires only a minute or two, and all are partly based on recruiting and teaching the partner to carry out some of them.

Clinical significance of the floating fetal head in nulliparous women in labor.

OBJECTIVE: To examine the course of labor in nulliparous women in active labor with a floating fetal head. STUDY DESIGN: A prospective, cohort study of nulliparous women presenting in active labor at term with a floating fetal head (station > or = -3, n = 108) or engaged fetal head (n = 241). All patients were examined by a senior physician. Assignment to the study or control group was noted in the investigator's records. However, management of labor was at the discretion of the labor ward team on duty. RESULTS: Cesarean section rates for failure to progress were significantly higher in the study group (17.1% versus 4.2%, P < .0001), and the second stage of labor was prolonged (65.3 +/- 27.1 versus 54.9 +/- 30.2 minutes, P < .03). None of the women who had a persistently floating fetal head at 7 cm of cervical dilation delivered vaginally. Birth weights were larger (P < .03) and Apgar scores lower (P < .0001) in the study group. The lengths of the active phase and instrumental delivery rates were similar in the two groups. CONCLUSION: Nulliparous women presenting in active labor at term with a floating head are at substantially increased risk of cesarean section for abnormal progress of labor. However, the majority of patients will still deliver vaginally. A persistently floating head with advanced cervical dilation (7 cm) should prompt consideration of cesarean section since little is to be gained by waiting.
