Laparoscopic-assisted percutaneous endoscopic gastrostomy: its role in providing enteric access when percutaneous endoscopic gastrostomy is not possible.

Percutaneous endoscopic gastrostomy (PEG) replaced open surgical gastrostomy (OSG) as the preferred method for enteric access soon after its introduction in 1980. Since that time, laparoscopic gastrostomy (LG), percutaneous radiologic gastrostomy (PRG), and laparoscopic-assisted PEG (LAPEG) have been introduced. PEG and PRG have been found to be over 95 per cent successful, convenient, economical, and associated with less morbidity than OSG. However, there are patients that are not appropriate candidates for, or have failed attempts at, PEG or PRG placement. At one time, OSG was the only option left for these patients, but they may be better served by LAPEG or, in some cases, LG. LAPEG offers less morbidity than OSG by having less pain and wound complications, and potentially may avoid the use of general anesthesia. We present a series of patients that underwent successful LAPEG placement after an unsuccessful attempt at PEG placement, and we describe its role in patient care.

[A study on endoscopic hemostasis from the viewpoint of morphologic traits of Dieulafoy ulcer--an attempt at endoscopic hemostasis by local injection of aethoxysklerol and ethanol].

Characteristics of the vascular course of ruptured vessels in the submucosal layer of Dieulafoy ulcer were studied, and endoscopic hemostatic treatment was performed using Aethoxysklerol and ethanol. 1) Observations of vessel courses in serial sections revealed that ruptured vessels in Dieulafoy ulcer exhibit rhexis in the lateral portions of the vessel and long courses over the ulcer margin in the submucous layer. 2) More dependable hemostasis seemed to be provided by thrombosis not only at the source of bleeding. i.e., at the site of vessel rupture, but also in the region of the whole ruptured vessel in the submucous layer. 3) Hemostatic treatment by local injection of Aethoxysklerol and ethanol was performed in 4 patients with Dieulafoy ulcer, and complete hemostasis was achieved in all 4 patients.

[Complications in endoscopic and endoscopic biopsy studies--analysis of a 1-year patient sample].

We report on a prospective study of complications in 2,133 bronchoscopies, 61 transthoracic lung biopsies, 32 mediastinoscopies and 6 thoracoscopies, performed between 1985 and 1986. There were no deaths or life threatening complications. The total rate of essential complications, e.g. pneumothorax, haemorrhage, laryngospasm, and bronchospasm of all patients with bronchoscopies was 10.5%. Excluding small endobronchial haemorrhages, without influence on the patients state or endoscopic procedures the final rate of complications was 1.9%.

[A new TV-bronchoscopy method using a video endoscope and digital processing of the endoscopic color image].

The TV-bronchoscope dose not have an image fiber bundle, which is instead replaced by a monochromatic C.C.D. (charge-coupled device). This system consists of a bronchofiberscope, a computer, a TV camera, a frame memory and a color CRT display. The enhancement of an endoscopic color image is one typical example of color image processing. Firstly a color image is divided into luminance information and color information, and only the former is processed for enhancement. The luminance information is filtered so as to eliminate the fiber bundle structure, and the spatial frequency band characterizing the structure is then emphasized. Finally the processed luminance information and the non-processed color information are combined to reconstruct the color image. In this way, a processed color image can be obtained without changing the color balance of the original image. Using this process, blood vessels and surface irregularities are emphasized by the newly developed digital processing method. We have applied it for localization of hilar-type early lung cancer, which progresses on the surface, and have obtained good results.

[Endoscopic resection in the treatment of non-invasive excreto-urinary carcinomas of the bladder 146 endoscopic resections carried out in 53 patients].

The authors divided their cases of excreto-urinary tumours of the bladder using the classification of Chome and Algazi. The diagnosis was made in 8 cases out of 10 on the basis of the cystogram obtained by IV pyelography. The upper urinary tract was free of disease in all cases. All patients were treated by endoscopic resection with or without the addition of intravesical instillations of triaziridinyl phosphine sulphide. The effectiveness of such instillations would not appear to have been conclusively demonstrated at the present time. Recurrences of intravesical tumours are extremely common (33/53 patients), but were always at the initial stage: i.e. highly differentiated non-invasive carcinoma. Up to the present none of these 53 patients (maximum follow-up 10 years) has died of an invasive carcinoma of the bladder.

[Sonographic follow-up of endoscopic papillotomy, percutaneous transhepatic drainage and endoscopically implanted bile duct prosthesis].

Normal transpapillary bile flow after EPT with or without implantation of bile duct draining catheters can be checked by sonographic demonstration of air in the bile ducts and/or normal calibers of the bile duct system. The position of percutaneous transhepatic catheters within the liver can be shown by sonography after insufflation of air into the catheter in most cases; this will allow to ascertain the correct location in the bile duct system or the dislocation of the catheter into the abdominal cavity. After EPT, PTD or endoscopic implantation of bile duct draining catheters sonography should be the first line procedure in controlling the success of the intervention before x-ray examination or endoscopy are applied.

Endoscopic anterior cruciate ligament reconstruction. Endobutton/button--a method of endoscopic fixation.

1. Reconstruction of the anterior cruciate ligament may be accomplished in many ways using a variety of autologous as well as allograft tissue. 2. A method of fixation uses an Endobutton for femoral fixation and a 19 mm Hewson Ligament Button for the tibial fixation of the graft. 3. This method of fixation will permit the surgeon to choose the graft best suited to the patient's needs while allowing endoscopic reconstruction of the anterior cruciate ligament, regardless of the choice of graft. Fixation of the graft need no longer be a variable in anterior cruciate ligament reconstruction.

[Endoscopic surgery for early gastric cancer by endoscopic mucosal resection (EMR)].

Endoscopic mucosal resection (EMR) enables one to examine resected specimens histologically and judge the curability by the histological findings. A mucosa and a part of submucosa can be resected by EMR. Therefore, mucosal cancers are indications for EMR. The present study showed that better indications for EMR were as follows: lesions less than 10 mm in size, lesions without ulceration, and lesions in the antrum or at the greater curvature. However, it is difficult to judge correctly before EMR whether the lesions are completely resected or not. The lesions with better indications are not always resected completely, and, conversely, the lesions more than 11 mm in size are completely resected at times. Therefore, it is most important to know how to deal with resected specimens. Specimens must be dealt with in strict conformity with the regulations. At present, when EMR may be regarded as the least invasive modality of curative sugeries, a new systematic treatment combining surgical laparotomic treatment and EMR is desirable for curative treatment of early gastric cancer.

[Concrement in the biliary tract-Problems of endoscopic and radiological diagnosis-Possibilities of endoscopic therapeutic measures].

The scope of diagnosis of diseases of the biliary tract has been considerably amplified by the addition of retrograde endoscopic cholangiography. With appropriate indications and technique, it is superior to the conventional radiological methods. The method is particularly indicated in biliodigestive anastomoses, the probing of which usually presents no problems, The diagnosis and therapeutic possibilities-removal of foreign material from the biliary tract after radiological visualisation in a single operation-are discussed with reference to three cases. Useful additions to the conventional equipment are the modified Dormia basket and the Fogarty catheter.

[Reversal of portal blood flow in cirrhosis. Clinical, endoscopic and ultrasound endoscopic correlations in 72 patients].

From January to December 1991, the portal venous system was evaluated by Doppler ultrasonography in 72 patients with liver cirrhosis. The objectives of this study were to evaluate the prevalence of spontaneous reversal of blood flow in the portal vein and to assess the relationship between Doppler ultrasound investigation and clinical, biochemical, endoscopic (70 patients), and endosonographic (44 patients) features. Reversed flow was quite frequent (alternating: 17%, permanent: 22%) and its prevalence did not differ in relation to age, sex, serum gammaglobulin concentration and Child-Pugh class. In patients with reversed portal venous flow, the prevalence of hepatic encephalopathy was higher (39% vs 13.5%, P < 0.05), but the prevalence of esophageal or gastric varices was not related to that pattern. Endosonography detected gastric wall abnormalities in a higher proportion of patients with reversed portal flow than in patients without it (P < 0.05). This study suggests that reversal of flow in the portal vein could play a role in the development of the gastric wall abnormalities in liver cirrhosis, which are detected by endosonography but not by endoscopy.

[Present status of endoscopic therapy for esophageal varices--endoscopic variceal ligation (EVL)].

Recently, EVL was developed to provide the endoscopist a safer method for the treatment of esophageal varices. The purpose of this report is to reveal our data and the present status. From 1989 to 1995, we performed EVL with EIS using Aethoxysklerol in a total of 214 patients who had bleeding and/or risky esophageal varices. Our technique of EVL is as follows; For prophylactic and elective patients, a spiral ligation method was employed and followed by EIS to obtain the complete eradication. When active bleeding was observed, direct ligation was performed onto the bleeding point and if the bleeding point was not found, spiral ligation method was done. Eradicating effect was observed in 97% of the entire patients and 29 active/spurting bleedings were well controlled by this technique. Complications were experienced in 12 cases (2 stenosis and 10 post EVL ulcedr bleeding), however, there were no patients who could not be managed by endoscopic treatment. Variceal recurrence after EVL/EIS were occurred in 36 of 214 patients (16.8%). Recurrent period was ranged from 1 month to 24 months and mean period was 10 months. We conclude that EVL/EIS is safe, easy and effective mean for the treatment of esophageal varices.

Endoscopic ultrasound-guided fine-needle aspiration biopsy and interventional endoscopic ultrasonography. Emerging technologies.

Endoscopic ultrasonography (EUS) has been shown to be useful in the detection and staging of gastrointestinal and pancreatic malignancies. However, EUS has some limitations including suboptimal specificity in lymph node staging. This is one factor that has curtailed its widespread use. The emergence of EUS-guided fine-needle aspiration (FNA) and other interventional applications of EUS appear likely to increase the clinical utility and cost-effectiveness of this modality. This article reviews recent literature describing the role of EUS in guiding FNA, detecting choledocholithiasis and assisting in interventions such as pseudocyst drainage.
