Transrectal US of the seminal vesicles and ejaculatory ducts: clinical correlation.

Transrectal ultrasonography (US) provides excellent anatomic detail of pathologic changes in the seminal vesicles and ejaculatory ducts. Fifty-two patients with US findings of seminal vesicle dilatation or cysts, ejaculatory duct cysts, or seminal vesicle or ejaculatory duct calculi were given questionnaires concerning a broad spectrum of genito-urinary symptoms. Compared with age-matched controls with normal US findings, patients with calculi in the seminal vesicles or ejaculatory ducts had a significantly increased prevalence of hematospermia and ejaculatory pain (P less than .01), and patients with cystic dilatation of the seminal vesicles were more likely to have perineal pain. Large midline cysts containing calculi or debris were symptomatic and probably represent müllerian duct remnants. Small cysts of the ejaculatory ducts were asymptomatic. Transrectal US may provide clinical insight into the causes of significant genitourinary symptoms that may previously have been ascribed to chronic nonbacterial prostatitis or have been considered to be idiopathic.

Ankylosis of the temporomandibular joint caused by rheumatoid arthritis: a pathological study and review.

Rheumatoid arthritis (RA) of the temporomandibular joint (TMJ) in a 59-year-old Japanese woman is reported, including details of clinical, histopathological and radiological findings. The patient had been diagnosed as having RA of the right knee joint 41 years previously, and suffered from arthralgia of the right TMJ. Radiological examination showed a radiopaque lesion of the mandibular head and mandibular fossa in the right TMJ and ankylosis of the right TMJ was diagnosed on the basis of the clinical and radiological findings. Condylectomy was performed. Pathological examination of material from the joint region revealed a marked increase of collagen fibers associated with slight capillary dilatation and hemorrhage. The final diagnosis was ankylosis of the right TMJ due to RA. The literature on TMJ ankylosis secondary to RA is reviewed and discussed.

Association between gallbladder cancer and anomalous union of the pancreaticobiliary ductal system.

Between 1978 and 1989, gallbladder cancer was diagnosed in 17 out of 48 patients with anomalous union of the pancreaticobiliary system. The clinical, surgical, and pathological features of these 17 patients are described. We classified them into two groups, those with and those without dilation of the common bile duct, and compared the clinicopathological features. Patients with dilation of the bile duct often had early onset of symptoms, so that cancer was detected at a relatively early stage. Most of those without dilation of the common bile duct already had a number of severe symptoms at the first consultation, and the cancer was usually at an advanced stage. We believe that early diagnosis of patients without dilation of the bile duct is an important clinical problem that needs to be resolved.

[Dilated form of hypertrophic cardiomyopathy].

To clarify pathological features of dilated form of hypertrophic cardiomyopathy (d-HCM), 14 autopsied hearts with d-HCM were studied. Prognosis of 23 cases with dHCM confirmed by myocardial biopsy specimens were evaluated. Pathological study revealed marked myocardial fibrosis(14/14), moderate to severe disarray(14/14), fibro-fatty degeneration of the outer layer of the left ventricle(13/14) and sclerosis of the intramural coronary arteries(mild, 5; moderate, 4). Degeneration of LV outer layer is an unique pathology in patients with d-HCM. Prognosis of 23 patients with d-HCM is longer than usually supposed(Ten-year-survival rate from the initial symptoms: 78%).

Peripapillary intrachoroidal cavitation in myopia.

PURPOSE: To report optical coherence tomography (OCT) findings in disorders recently described as peripapillary detachment in pathologic myopia (PDPM). DESIGN: Observational case report. METHODS: OCT, fluorescein, and indocyanine green angiography. RESULTS: A 69-year-old woman presented with bilateral yellow-orange peripapillary area at the inferior border of the myopic conus, typical of PDPM. OCT showed this area as a large intrachoroidal hyporeflective space located below the normal plane of the retinal pigment epithelium. There was no detachment of the retinal pigment epithelium which appeared flat. CONCLUSIONS: OCT findings suggest calling this anomaly peripapillary intrachoroidal cavitation, instead of peripapillary detachment in pathologic myopia.

Treatment of hematotrachelos after dilatation and curettage. A case report.

BACKGROUND: Hematotrachelos, accumulation of menstrual blood in a dilated cervix, is a rare disorder accompanying cervical stenosis. In this report, hematotrachelos was diagnosed by vaginal sonography and treated in a novel way. CASE: A 39-year-old woman with progressive hypomenorrhea following dilation and curettage for dysfunctional uterine bleeding was diagnosed with hematotrachelos using vaginal sonography. Cervical dilatation and evacuation of the hematotrachelos were performed. A latex nasopharyngeal airway was utilized as a temporary stent and allowed drainage and maintained cervical patency. Normal menstrual flow was resumed, and the hematotrachelos did not recur. CONCLUSION: Hematotrachelos may be a rare complication of routine dilatation and curettage. The diagnosis is confirmed by transvaginal sonography. Treatment is successful with cervical dilatation and placement of a temporary latex nasopharyngeal airway.
