[Immunological efficacy of smallpox vaccination in children with relative contraindications to vaccination].

Children (aged from 3 to 9 years) vaccinated against smallpox by multiple puncture (3 punctures by bifurcation needle), by a single cut and with vaccine diluted with a double dose of the solvent were observed. Vaccination by the methods tested considerably diminished the vaccinal reactions and failed to influence appreciably the formation of a full-value humoral immunity; only dilution of the vaccination material reduced the dynamics of the immunogenesis at the early periods after the vaccination. In children aged 3--5 and 6--9 years general vaccinal reactions were of the same character, whereas local reactions and immunological response were more pronounced in older children. A method of multiple puncture is recommended for mass vaccinations and also for vaccination of children with relative contraindications to the vaccinations.

[Anesthesiological aspects in determining indications and contraindications for surgical interventions in patients with severe wounds and injuries in the post shock period].

Approaches to validation of indications and contraindications for interventions in wounded patients and victims were developed on the basis of many-year experience. The desirability of intervention is determined primarily by strategic tasks which it solves. The operation is to be performed despite severe clinical condition if it removes the cause of critical state or creates conditions for preventing life-threatening complications of the traumatic disease. All the rest interventions can be postponed till complete or partial compensation of vital systems.

[Endoscopic local injection of anticancer drugs bound to carbon particles for treatment of advanced rectal cancer, when surgical treatment was contraindicated].

Generally, patients with advanced rectal cancer in whom surgical treatment is contraindicated receive radiation or chemotherapy. In such patients, we have administered local injection of methotrexate and mitomycin C bound to activated carbon particles. Four patients received intratumoral injection of the dosage formulation (total dose 100-400 mg of methotrexate or 8-32 mg of mitomycin C) under colonofiberscope. After the treatment, bleeding and pain were lessened in all 4 patients. In two patients, the tumor markedly decreased in size and there was no regrowth prior to death 12-14 months after the treatment. In another patient, bleeding and pain disappeared until the patient died of pulmonary and liver metastases. The fourth patient is alive without regrowth 5 months after treatment. Side effects were not severe.

[Myocardial infarction as a work accident in Polish Supreme Court decisions. Part II. Job contraindications due to an employee's state of health].

This paper presents the second part on opinions given by the Polish Supreme Court on the acceptance of myocardial infarction as work accident. This it can be seen that, in certain cases, the judgements of the Court are contradictory. For in some cases, an external cause is recognized in the act of permitting employees to work whose state of health would contraindicate the performance of their duties, while in others, the principle is accepted that work duties not considered harmful for healthy people cannot be deemed an external cause for people with health problems. An external cause according to the Supreme Court may also be the act of allowing an employee to work without a valid medical examination, or even by not conducting routine required work-related medical examinations on time.

Wireless capsule endoscopy in a case of small bowell obstruction: not always contraindicated.

Wireless capsule endoscopy is a new diagnostic technique used especially for investigating the entire small bowel. Recent studies have demonstrated its superior ability to detect and evaluate small bowel lesions compared to other radiological examinations. Performing video capsule endoscopy is regarded as contraindicated when an intestinal occlusion is documented. We present a case of hidden intestinal bleeding which underwent wireless capsule endoscopy and was treated successfully despite documented ileal stenosis.

[Indications and contraindications to extraperitoneal closing external intestinal fistulas].

Prognostic factors of recurrent external intestinal fistulas after extraperitoneal closing them were studied in 74 patients operated on in the Sverdlovsk region clinical hospital N1 in 1990-2004. There were no lethal outcomes. Good results were noted in 40 out of 74 (54.05%) patients. Postoperative complications developed in 34 (45.95%) patients, including 21 (28.38%) recurrent intestinal fistulas. Using the methods of mathematical analysis it was found that the recurrent intestinal fistulas resulted from persisting inflammatory alterations in the tissues of the peritoneal and intestinal walls and also from appearing due to operation mechanical injuries of patency of the intestine canal in the places of suturing the fistulas or the initial ones - in the abducent parts of the intestine. The indications and contraindications are proposed concerning the using, rational technical means of extraperitoneal closing the external intestinal fistulas.

[Effective microbiologic spectrum of gyrase inhibitors--indications and contraindications].

Ciprofloxacin is an essential enrichment of the antibacterial therapy. The activity spectrum comprises enterobacteria, haemophiliacs and Neisseriae, furthermore Legionellae and species of Pseudomonas (except Ps. maltophila). In the gram-positive area staphylococci, Listeriae and corynebacteria are covered, whereas species of streptococci including pneumococci are only moderately sensitive or resistent. Infections of the urinary tract, of the respiratory tract and the biliary tract as well as enteral infections are the main indications. Furthermore, an application in osteomyelitis and arthritis, in chlamydial infections and mycoplasmal infections, in legionellosis and for the selective decontamination in threatening granulocytopenia is justified. Pregnancy, period of growth and cerebral convulsive disorders are contraindications.

[Indications and contraindications for analgesics and antibiotics in pediatric dentistry].

A general overview upon most used products in odontopediatrics is presented; the factors which influence the determination of the adequate doses in children and some useful recommendations in dental clinical practice are described. The most prescribed products in pediatric odontology are summarized into two groups: a) benzodiazepine, for anxiety and lidocaine, aspirin and acetaminophen, dipirone and naproxen, for pain; and b) penicillin, erythromycin, for treatment of infections. Doses, pharmacologic effects, clinical indications, side effects and contraindications of the chemicals mentioned are described. Also, the article presents some of the aspects which justify the use, in our country, of certain medicaments prescribed in other nations reporting, the recommendations in order to prevent the use of other chemicals because of their lack of therapeutical advantages over the ones of first choice.

[Contraindications regarding total prosthesis of the hip joint].

The authors have made an analysis of unsatisfactory remote results in 83 (21.5%) out of 387 patients operated for endoprosthezing of the hip joint which resulted from shaking loose the endoprostheses in the bone tissue. A conclusion is made that the following factors may be considered as contraindications to endoprosthesis of the hip joints: coxarthrosis against the background of arthroso-arthritis, neglected stages of protrusive coxarthrites, deforming osteoarthrosis due to the use of corticosteroids, pronounced senile osteoporosis.

[Possibilities and contraindications of manual therapy in geriatrics (author's transl)].

Complaints of the locomotor apparatus increase in the old age. The arising of these disturbances is discussed and their effect to the locomotor apparatus with the clinical findings are described. The clinical findings are diagnosed by the methods of the Manual Medicine. The manual therapy enables the treatment of the reflectoric expression of these disturbances. The contraindications are mentioned. The "soft Tissue Technics" and the "Technics of Mobilisation" are of great importance for the treatment of complaints of the locomotor apparatus in the old age. Examples of those technics are described.

An attempt at applying epiadriamycin to the patients for whom doxorubicin is cardiologically contraindicated.

Administrating Epi-ADR (Epiadriamycin) to the patients for whom ADR was cardiologically contraindicated did not cause significant complications. No complications occurred in the patients treated with Epiadriamycin, after they had completed a full course of maximal dose of ADR. The trial was conducted in close cooperation with a cardiological intensive care unit which is an essential arrangement in such clinical situation, until the optimal therapeutical method of administrating Epi-ADR is defined with special attention devoted to its critical dose.

[Are auditory prostheses a contraindication to magnetic resonance imaging?].

Prior to the use of nuclear magnetic resonance imaging (NMRI) in patients with metallic ossicular prostheses their compatibility must be investigated with the intense magnetic fields generated by NMRI. Implanted auditory prostheses are an absolute contraindication to NMRI, but an experimental study of its possible interaction with metallic stapedial prostheses has shown absence of their displacement. The artefact induced may alter images of middle ear but do not affect internal auditory canal. Nevertheless, manufacturers should conduct tests of magnetism of prostheses before they are passed as satisfactory.

Hepatic hydrothorax is a relative contraindication to chest tube insertion.

Two patients with known chronic ascites developed new massive right-sided pleural effusions. Chest tube placement led to massive protein and electrolyte depletion and death of both patients. Patients who have cirrhosis and massive right-sided pleural effusions, in general, have congenital diaphragmatic defects that predispose them to life-threatening fluid depletion when chest tubes are inserted. Hepatic hydrothorax is a relative contraindication to chest tube insertion.
