Bone loss in patients with rheumatoid arthritis--effect of steroids measured by low dose quantitative computed tomography.

For 2 years bone density changes in patients with rheumatoid arthritis were monitored to determine the effect of low dose corticosteroid treatment. Sixteen perimenopausal females were studied. Of these nine were treated with 5 to 20 mg prednisone per day and seven had no steroids. The differential effects on trabecular bone density and on cortical bone in the radius and the tibia were examined with high precision peripheral quantitative computed tomography (QCT). Steroid therapy did not influence cortical bone loss and had only a minor influence on trabecular bone density. Of greater importance was the influence of estrogen depletion. Corticosteroids and estrogen depletion, however, cannot explain fully the yearly loss of 5.7% in the trabecular and 4% in the cortical bone of the radius.

Predicting the failure load of the distal radius.

The distal radius is an important site for the early detection of patients at risk for fracture. Since measuring bone strength in vivo is not possible, we evaluated which bone assessment method of the forearm would best predict failure load of the distal radius and computed a factor of risk for wrist fracture (Phi wrist). Thirty-eight cadaveric forearm specimens were measured by five different techniques to assess bone density, bone mineral content, geometry and trabecular structure at the distal forearm. The bone assessment techniques included dual-energy X-ray absorptiometry (DXA) of the radius, peripheral quantitative computed tomography (pQCT) of the 4% and 20% distal sites of the radius, DXA of the phalanges, digital X-ray radiogrammetry of the forearm (DXR-BMD), and quantitative ultrasound of the radius. The failure load of each excised radius was determined by simulating a fall on an outstretched hand. The pQCT measurements of polar stress-strain index and cortical content explained the greatest portion of variance in failure load (r2=0.82-0.85). Bone mineral content measures were generally better predictors of failure load (r2=0.53-0.85) than the corresponding volumetric or areal bone mineral density values (r2=0.22-0.69) measured by either pQCT or DXA. Multiple regression analysis showed that the addition of a bone geometry measure improved the ability of a bone density measure alone to predict failure load. There was high variability in the ability of different techniques and different variables within a given technique to predict failure load. Estimates of the factor of risk for wrist fracture (Phi wrist) revealed that the women in this study would have been likely to fracture their distal radius upon falling from a standing height (Phi wrist= 1.04), whereas the men would have likely withstood the impact without fracturing their wrist (Phi wrist= 0.79).

Differentiating empyemas and peripheral pulmonary abscesses: the value of computed tomography.

In 13 patients, conventional radiographic techniques and clinical histories were not sufficient to determine whether a peripheral cavitary lesion was an abscess or an empyema. However, after computed tomography (CT), eight patients were diagnosed as having abscesses and five as having empyemas. Abscesses had an irregular shape and a relatively thick wall which was not uniformly wide and did not have a discrete boundary between the lesion and lung parenchyma. In contrast, empyemas had a regularly shaped lumen, a smooth inner surface, and a sharply defined border between the lesion and lung. CT studies can help to distinguish between empyemas and abscesses, and treatment can be started sooner in difficult cases.

Multiple sclerosis lesion quantification using fuzzy-connectedness principles.

Multiple sclerosis (MS) is a disease of the white matter. Magnetic resonance imaging (MRI) is proven to be a sensitive method of monitoring the progression of this disease and of its changes due to treatment protocols. Quantification of the severity of the disease through estimation of MS lesion volume via MR imaging is vital for understanding and monitoring the disease and its treatment. This paper presents a novel methodology and a system that can be routinely used for segmenting and estimating the volume of MS lesions via dual-echo fast spin-echo MR imagery. A recently developed concept of fuzzy objects forms the basis of this methodology. An operator indicates a few points in the images by pointing to the white matter, the grey matter, and the cerebro-spinal fluid (CSF). Each of these objects is then detected as a fuzzy connected set. The holes in the union of these objects correspond to potential lesion sites which are utilized to detect each potential lesion as a three-dimensional (3-D) fuzzy connected object. These objects are presented to the operator who indicates acceptance/rejection through the click of a mouse button. The number and volume of accepted lesions is then computed and output. Based on several evaluation studies, we conclude that the methodology is highly reliable and consistent, with a coefficient of variation (due to subjective operator actions) of 0.9% (based on 20 patient studies, three operators, and two trials) for volume and a mean false-negative volume fraction of 1.3%, with a 95% confidence interval of 0%-2.8% (based on ten patient studies).

A comprehensive interactive on-line computer system for research and clinical practice in orthodontics.

An interactive on-line computer system for the measurement of cephalometric radiographs, and facial and plaster cast photographs is described. The details of the hardware (computer and peripherals) and software are provided. Numerical and graphical analysis and presentation of the resultant data are discussed. Many research and clinical applications of the system are outlined.

Managing predefined templates and macros for a departmental speech recognition system using common software.

The authors have developed a networked database system to create, store, and manage predefined radiology report definitions. This was prompted by complete departmental conversion to a computer speech recognition system (SRS) for clinical reporting. The software complements and extends the capabilities of the SRS, and 2 systems are integrated by means of a simple text file format and import/export functions within each program. This report describes the functional requirements, design considerations, and implementation details of the structured report management software. The database and its interface are designed to allow all radiologists and division managers to define and update template structures relevant to their practice areas. Two key conceptual extensions supported by the template management system are the addition of a template type construct and allowing individual radiologists to dynamically share common organ system or modality-specific templates. In addition, the template manager software enables specifying predefined report structures that can be triggered at the time of dictation from printed lists of barcodes. Initial experience using the program in a regional, multisite, academic radiology practice has been positive.

A Web-based home welfare and care services support system using a pen type image sensor.

A long-term care insurance law for elderly persons was put in force two years ago in Japan. The Home Helpers, who are employed by hospitals, care companies or the welfare office, provide home welfare and care services for the elderly, such as cooking, bathing, washing, cleaning, shopping, etc. We developed a web-based home welfare and care services support system using wireless Internet mobile phones and Internet client computers, which employs a pen type image sensor. The pen type image sensor is used by the elderly people as the entry device for their care requests. The client computer sends the requests to the server computer in the Home Helper central office, and then the server computer automatically transfers them to the Home Helper's mobile phone. This newly-developed home welfare and care services support system is easily operated by elderly persons and enables Homes Helpers to save a significant amount of time and extra travel.

A vascular catheterization simulator for training and treatment planning.

We have developed a working prototype of an augmented reality based vascular catheterization simulator. The simulator is called daVinci, and provides the interventional radiologists with a "hands on, user friendly, image based environment to augment training, enhance pretreatment planning and design interventional products and devices. daVinci is based on computational modeling of human anatomical images and provides the user with capabilities for realtime simulated navigation of catheters in both 2-D and 3-D modeled vessels. In addition, multiplaner CT, MRI and anatomical data sets are incorporated into the simulator to enhance the understanding of anatomical relationships associated with vascular catheterization procedures. The current prototype is designed for peripheral vascular applications. Additional systems are currently being considered for both interventional neuroradiology and cardiology.

Morphologic alterations in the cilia of a cat.

Based upon ultrastructural findings and computed tomography, a presumptive diagnosis of feline primary ciliary dyskinesia was made in a 2.5-year-old cat. The cat demonstrated morphologic alterations in the ultrastructure of oviductal cilia. In the oviduct, axonemal abnormalities were detected in 132 (20%) of 660 cross-sectioned cilia. The main ultrastructural ciliary defects were a lack of central microtubules, transposition of peripheral doublets to the center of the axoneme, supernumerary microtubules, and morphologic abnormalities of peripheral doublets. Computed tomography of the chest was consistent with early lesions of bronchiectasis, suggesting chronic stagnation of respiratory secretions, attributed to abnormal function of respiratory cilia. Specifically, the cranial branches of the cat's bronchi were wider and thicker than those of five healthy controls. Foci of pleural thickening and interstitial enlargement were also observed.

Influence of puberty on muscle area and cortical bone area of the forearm in boys and girls.

The aim of the current study is to analyze the interaction of the muscle and bone system (muscle-bone unit) during puberty in males and females by computed tomography of the nondominant forearm. The data presented here are the first results from 318 healthy children (159 boys and 159 girls), aged 6-22 yr, and 336 adults (parents) participating in the DONALD Study (Dortmund Nutritional and Anthropometric Longitudinally Designed Study). Cortical area (CA) of the radius representing bone strength and muscle area (MA) representing muscle strength were measured with peripheral quantitative computed tomography (XCT 2000; Stratec, Pforzheim, Germany). A single slice measurement at a site corresponding to 65% of the ulnar length proximal to the radial endplate was used. MA and CA of the radius have been determined by a built-in software algorithm using density differences. There was a strong correlation between MA (x) and CA (y) in all children, adolescents, and adults (y = 0.019x + 10.93; r2 = 0.77). Before puberty, boys and girls displayed a similar relation between MA and CA. CA in relation to MA was greater in girls than in boys during puberty. Analysis of covariance was performed investigating the dependency of CA on MA, five pubertal stages, sex, and interaction of sex and pubertal stages. MA representing muscle strength was the strongest predictor of CA (P < 0.001) representing bone mass. Pubertal stage (P < 0.001) and interaction of pubertal stage*sex (P = 0.002) also had a significant influence on CA. r2 of the model was 0.85. These data suggest that in pubertal girls and women rather than in pubertal boys and men an additional factor shifts the relationship between MA and CA to higher values of cortical area. The present data confirm previous studies of the influence of puberty and estrogens or related factors on the muscle-bone interaction.

Influence of time pressure and verbal provocation on physiological and psychological reactions during work with a computer mouse.

The overall aim of this study was to investigate whether time pressure and verbal provocation has any effect on physiological and psychological reactions during work with a computer mouse. It was hypothesised that physiological reactions other than muscle activity (i.e. wrist movements, forces applied to the computer mouse) would not be affected when working under stressful conditions. Fifteen subjects (8 men and 7 women) participated, performing a standardised text-editing task under stress and control conditions. Blood pressure, heart rate, heart rate variability, electromyography, a force-sensing computer mouse and electrogoniometry were used to assess the physiological reactions of the subjects. Mood ratings and ratings of perceived exertion were used to assess their psychological reactions. The time pressure and verbal provocation (stress situation) resulted in increased physiological and psychological reactions compared with the two control situations. Heart rate, blood pressure and muscle activity in the first dorsal interosseus, right extensor digitorum and right trapezius muscles were greater in the stress situation. The peak forces applied to the button of the computer mouse and wrist movements were also affected by condition. Whether the increases in the physiological reactions were due to stress or increased speed/productivity during the stress situation is discussed. In conclusion, work with a computer mouse under time pressure and verbal provocation (stress conditions) led to increased physiological and psychological reactions compared to control conditions.

Timing of EEG-based cursor control.

Recent studies show that humans can learn to control the amplitude of electroencephalography (EEG) activity in specific frequency bands over sensorimotor cortex and use it to move a cursor to a target on a computer screen. EEG-based communication could be a valuable new communication and control option for those with severe motor disabilities. Realization of this potential requires detailed knowledge of the characteristic features of EEG control. This study examined the course of EEG control after presentation of a target. At the beginning of each trial, a target appeared at the top or bottom edge of the subject's video screen and 1 sec later a cursor began to move vertically as a function of EEG amplitude in a specific frequency band. In well-trained subjects, this amplitude was high at the time the target appeared and then either remained high (i.e., for a top target) or fell rapidly (i.e., for a bottom target). Target-specific EEG amplitude control began 0.5 sec after the target appeared and appeared to wax and wane with a period of approximately 1 sec until the cursor reached the target (i.e., a hit) or the opposite edge of the screen (i.e., a miss). Accuracy was 90% or greater for each subject. Top-target errors usually occurred later in the trial because of failure to reach and/or maintain sufficiently high amplitude, whereas bottom-target errors usually occurred immediately because of failure to reduce an initially high amplitude quickly enough. The results suggest modifications that could improve performance. These include lengthening the intertrial period, shortening the delay between target appearance and cursor movement, and including time within the trial as a variable in the equation that translates EEG into cursor movement.

Evaluation of the Newton Pen-Pad as a tool for collecting clinical research data at the bed-side.

A protocol for the study of practice variation between Quebec Intensive Care Units in the treatment of myocardial infarction by thrombolysis was coded into the Newton Pen-Pad. This tool for the direct recording of clinical data was tested in the working environment by research nurses of 4 different teaching hospitals. Data was sent directly from the pen-pad by telecommunication to the information coordinating center. The results of this evaluation confirm the reliability and robustness of this approach which promises to be an important tool for applied clinical research.

Personal computer based programs for the handicapped.

This is an experience report concerning the utilization of personal (micro) computers for in and outpatients of The Burke Rehabilitation Center. Three Apple II Plus desk computers and peripherals are in use with the following objectives:-- To provide access via a variety of interface switches. To motivate and train child and juvenile amputees undergoing upper limb amputation rehabilitation. To provide a different approach to neuropsychological testing and behavioural assessment. To provide cognitive rehabilitation programs for patients with head trauma and stroke. To add another option for pre-vocational evaluation. The report discusses computer access technology along with a description of program applications. Conclusions based on the preliminary results of observing 101 patients are given.

Computed tomography of the abdomen and pelvis in non-Hodgkin lymphoma.

The experience of one institution using abdominal computed tomography (CT) for staging and restaging of non-Hodgkin lymphoma (NHL) patients is reported. In 49% of abnormal examinations no significant difference in quantity of involved nodes between diffuse and nodular lymphomas appeared. High paraaortic, iliac, and mesenteric nodal sites were most commonly involved. At restaging examination, usually less extent of disease was found. Abdominal involvement could not be predicted from the presence of peripheral adenopathy. Computed tomography changed the staged during initial workup significantly in 14%. During restaging, CT detected unsuspected active disease in 43% and changed the clinical impression from limited to extensive involvement in 16%. Computed tomography proved to be useful in the management of NHL patients and is recommended as the primary imaging procedure in the abdomen in this disease.

PTEVAL: a computerized home-based physical therapy intervention instrument.

The determination of physical therapy treatment protocols, based on an in-home evaluation of a patient, is a complex task. The specific rules for treatment indications are individually simple, but numerous and hence time-consuming and prone to error using pencil-and-paper methods. This paper describes PTEVAL, a computerized Point-of-Care instrument designed to support the intervention protocols of an ongoing clinical trial. With appropriate modifications and extensions, PTEVAL can be a model for instruments used in clinical practice.

A field trial of the NASA Telemedicine Instrument Pack in a family practice.

BACKGROUND: Previous studies of telemedicine applications have demonstrated that the technology is effective but inefficient. Little attention has been directed to the primary care portion of the connection, especially the use of the medical peripheral devices. This study used a telemedicine testbed that simulates a rural practice environment to describe the effectiveness and efficiency of the NASA Telemedicine Instrument Pack, a small self-contained system of medical peripheral devices. METHOD: This study was an 8-week field trial of a suitcase-sized pack containing a fundus camera, flexible nasopharyngoscope, dermatology macrolens, light source, and video monitor. The pack was first studied in specialty clinics and then was used in a family practice office connected to the consultant node by digital lines. Evaluations were obtained from technicians, patients, and consultants. RESULTS: During 20 video clinic sessions, 59 patients with 38 different diagnoses were examined. The ear, nose, and throat portion of the exam was effective, with some decrement in color and clarity with compression of the signal. The eye portion was marginally effective, limited by a field of view that was too narrow and also by rigorous technician requirements. The skin exam was largely unacceptable primarily because the macrolens did not meet the requirements for color or clarity prior to compression of the signal. CONCLUSIONS: Subsequent design efforts for medical peripheral devices for telemedicine use will require significant modifications to "off the shelf" equipment to be effective and efficient. A family practice telemedicine testbed provides the appropriate environment for such field trials.

Clinical implications of catheter variability on neonatal continuous arteriovenous hemofiltration.

The clinical impact of catheter diameter variability on performance of neonatal continuous arteriovenous hemofiltration (CAVH) systems was shown by demonstration of the effect of catheter inner diameter on catheter and CAVH system blood flow. Diameter of 3.5 Fr and 5 Fr catheters was determined by experimental measurement of catheter flow-pressure drop relationship with known equations for fluid flow. Physical measurements verified the accuracy of the calculated diameters. These diameters were then used in a mathematical simulation to describe blood flow through an entire neonatal CAVH system and predict clinical performance. Catheters of 5 Fr caliber had a 39% variation in diameter among manufacturers, resulting in a 370% variation in blood flows; 3.5 Fr catheters had a 29% variation in internal diameter with a corresponding blood flow variation of 290%. Computer simulation of a neonatal CAVH system revealed a maximum blood flow of 0.8-3.0 ml/min with available 5 Fr catheters. This wide variation is probably responsible for the lack of consistent results in neonatal CAVH systems to date.
