Throwing light on DARC.

Chemokines play a key role in directing and driving leucocyte trafficking. The efficient regulation of leucocyte recruitment by chemokines requires their appropriate localization in functional micro-anatomical domains, as well as setting limits to their effects in space and time. Both processes are influenced by silent chemokine receptors (interceptors), including DARC (Duffy antigen receptor for chemokines). Increasing experimental evidence suggests that DARC is involved in accumulation of extravascular chemokines in endothelial cells, chemokine transcytosis and presentation on their luminal surface, leading to leucocyte adhesion and emigration. Additionally, DARC is expressed on erythrocytes and can act as a sink for chemokines in blood. This limits the dissemination of chemokines through blood into distant organs and tissues as well as reducing their effects on the circulating leucocytes.

Don't throw away the ring: indications and use.

Protective rings are a valuable adjunct in revision acetabular surgery. Two types of rings are used. The roof ring, which is the smaller of the 2 rings, provides support from the ilium to the inferomedial part of the acetabulum. The reconstructive ring or cage provides support from the ilium to the ischium. Both devices can be used to protect morcellized or structural bone-graft. The roof ring can be used with morcellized graft if the ring can make contact with host-bone at the superior and inferomedial aspect of the acetabulum. If the defect is more global but still contained so that the entire acetabulum is filled with morcellized bone, the larger reconstructive ring should be used. Small structural grafts that support <50% of the cup may be protected by a roof ring. Larger structural grafts that support >50% of the cup should be protected by a reconstructive ring.

Dietary intake, anthropometric measurements and plasma lipid levels in throwing field athletes.

Since little descriptive data for field athletes is available, the anthropometric measurement, dietary intake and plasma lipid levels of 22 male and 15 female field athletes (throwers) are reported. Percentage body fat was calculated by using four skinfold thicknesses. Using this measurement as criterion, 53% of the females were obese. Percentage body fat and body mass index showed a positive correlation with total plasma cholesterol and plasma triacyglycerol and a negative correlation with percentage HDL cholesterol for males and females. Five of the males and three of the females were hypercholesterolaemic. The seven day estimated dietary record was used to determine their dietary intake. The males as well as the females consumed a diet that was high in fat and cholesterol content. Their diets were low in carbohydrate. It is recommended that these athletes increase their carbohydrate intake and lower their total fat intake. It is also recommended that the obese subjects should lose weight.

Relationship between throwing mechanics and elbow valgus in professional baseball pitchers.

Valgus elbow stress leads to medial tension and lateral compression injuries in baseball pitchers of all ages. This study was undertaken to investigate the relationship between elbow stress in professional baseball pitchers and the kinematic parameters of pitching mechanics. This was done in an attempt to understand valgus extension overload better and in an effort to improve preventive and rehabilitative protocols. High-speed video data were collected on 40 professional pitchers in game situations during the 1998 and 1999 Cactus League season in Arizona, as part of Major League Baseball Spring Training. A multiple linear regression analysis was used to relate elbow valgus to kinematic parameters of pitching mechanics. The resulting analysis produced an adjusted multiple R2 value of 0.974, indicating that nearly 100% of the variance in valgus stress on the elbow was explained by the parameters in the regression equation. This ability to explain over 97% of the variance in valgus stress is significant. The parameters of pitching mechanics related to elbow valgus may be assessed and optimized, if necessary, in order to decrease the magnitude of elbow stress in pitching.
