The pulp. A big issue about a little tissue.

Difficulties in correlating pulp symptomology with available histopathological and biochemical data have hampered understanding of pulp reaction to stimuli. Some of the less well known biochemical cell and tissue regulatory systems are appraised. Pathways whereby such systems could be implicated in pulp function are suggested.

[Methodological bases for the study and the patterns of the seasonality of viral hepatitis A].

On the basis of the study of the data on the incidence of viral hepatitis A in one of the districts of a big city for 20 years the authors come to the conclusion on the advisability of a comparative study of the monthly changes in the incidence of hepatitis A within individual uniform morbidity cycles covering the periods of many years and the use of average monthly data for many years for plotting the typical curve, as well as the use of the simplified for the calculation of the upper limit of annual morbidity. Similarities and differences in the monthly dynamics of morbidity in the years of high and low morbidity levels have been revealed, and the age group of the population (20-39 years) which ensures the continuity of the epidemic process all the year round has been determined. The factors contributing to the seasonal activation of the epidemic process start operating in June among schoolchildren aged 11-14 years, and later their operation spreads to other groups of the population.

What's ahead in glucose monitoring? New techniques hold promise for improved ease and accuracy.

Advances in blood glucose monitoring have made it easier, more comfortable, and more practical for patients to monitor frequently. The new meters for intermittent monitoring are smaller and less dependent on technical aptitude than older models. They require less blood, and many provide downloadable information for glucose analysis. Data systems used with new meters provide valuable information that can dramatically improve glycemic control. Continuous glucose sensing (figure 4) is another major breakthrough in management of diabetes. Current systems allow only retrospective analyses, but real-time readings should be available in the near future. Such technological advances hold promise for preventing both hypoglycemia and hyperglycemia and for reducing the risk of long-term complications associated with diabetes. An artificial, mechanical islet cell may be the big next step toward bringing this disease under control. By combining continuous glucose monitoring data with continuous insulin delivery via an external or an implantable insulin pump, the outlook promises to be much brighter for patients with type 1 diabetes.

Dog bites: how big a problem?

OBJECTIVE: To estimate the magnitude of the dog bite problem in the US. METHODS: Data on dog bites were gathered as part of a 1994 national telephone survey of 5,238 randomly dialed households. Data were weighted to provide national estimates. RESULTS: The weighted total number of dog bites was 4,494,083 (estimated incidence = 18/1,000 population); of these, 756,701 persons sustained bites necessitating medical attention (incidence rate = 3/1,000). Children had 3.2 times higher medically attended bite rates than adults (6.4/1,000 children v 2/1,000 adults). CONCLUSIONS: More attention and research needs to be devoted to the prevention of dog bites. Potential prevention strategies include: educational programs on canine behavior, especially directed at children; laws for regulating dangerous or vicious dogs; enhanced animal control programs; and educational programs regarding responsible dog ownership and training. Unfortunately, the relative or absolute effectiveness of any of these strategies has not been assessed. Continuing surveillance for dog bites will be needed if we are to better understand how to reduce the incidence of dog bites and evaluate prevention efforts.

Use of generalized linear mixed models in analyzing mutant frequency data from the transgenic mouse assay.

The transgenic mouse assay is now widely used for the study of mutagenesis in diverse rodent tissues and to test chemicals for genotoxic potential. This kind of assay generally involves nested observations at several levels of sampling, e.g., animals, packaging reactions, and plates. Due to the common origin, the mutant frequency (MF) in tissues from the same animal are likely to be positively correlated, inducing extra variation relative to the common binomial variation. In this article, a generalized linear mixed model is used to analyze the overdispersed binomial data on mutant frequency from the transgenic mouse assay, with a random effect for each level of the sampling hierarchy. This is a comprehensive framework within which different sources of variation in the data can be evaluated in nested factorial experiments and treatment effects can be assessed simultaneously. It avoids the current practice of repeated testing for excess binomial variability at each level of the sampling hierarchy and aggregating data up the levels, but fits the data with one single model. Parameters associated with the fixed effects, particularly dose, and the variance components for the random effects (e.g., animals, packages, and plates) can be estimated and tested for significance. Data previously reported in the literature involving the lacl gene from the Big Blue mouse are used to illustrate the proposed method.

The vasoconstrictor action of big endothelin-1 is phosphoramidon-sensitive in rabbit saphenous artery, but not in saphenous vein.

The effects of big endothelin-1 and endothelin-1 on vascular reactivity were compared in isolated rabbit saphenous artery and vein. The contractile potency of endothelin-1 was three times higher in the vein than in the artery. In contrast, big endothelin-1 was two times more effective in the artery. Phosphoramidon (100 microM), a metalloproteinase inhibitor, antagonised the contractile action of big endothelin-1 in the artery but not in the vein. These data suggest that the biosynthetic pathway leading to the formation of endothelin differs in arterial and venous vessels.

The economics of physical activity: societal trends and rationales for interventions.

What are Americans doing with their time and their money and what has changed in recent decades? Do changes suggest interventions that will lead to healthier lifestyles? This paper analyzes several different data sets that reveal some surprising (and some less surprising) insights. The big growth areas, both in terms of expenditure and time allocation, have been leisure time and travel/transportation. Leisure-time industries outpace gross-domestic-product growth for both "active" (sporting goods, dance studios, gyms) and "sedentary" industries (spectator sports, cable TV), although industries associated with more sedentary lifestyles grow the fastest. Overall time spent in productive activities, whether at home or work, has declined by several hours each week for both men and women compared to 40 years ago. Reduced physical activity by itself is not a reason for intervening, as many changes improved overall quality of life (even if not necessarily health-related quality of life). But other trends are more likely to reflect poorly functioning markets, leading to worse economic and health outcomes. Market failures that lead to less physical activity or unhealthy nutrition justify interventions, both from an economic and a public health perspective.

Microarrays in Parkinson's disease: a systematic approach.

Neurological disease (ND) is one of the greatest challenges facing our population, from medical, financial, and social perspectives. The application of new research approaches to understand the underlying pathogenesis of ND is critical. In this article, we review the use of microarray analysis in Parkinson's disease (PD). Microarrays have tremendous power, simultaneously querying the expression of tens of thousands of genes from a given biological sample. Coupled with impressive advances in statistical tools for analyzing large, complex data sets, well-designed microarray experiments are poised to make a big impact in the field of ND. Parkinson's disease is a devastating neurodegenerative disease well suited to a systems-based microarray analysis. Genetic and environmental rodent models of PD emulate many of the cardinal features of human PD, providing the unique opportunity to compare gene expression profiles from different etiologies of the same disease. The elucidation of important gene expression patterns during disease will make possible identification of genetic susceptibility markers, biomarkers of disease progression, and new therapeutic targets.

Penetration of macromolecules into contracted blood clot.

The effectiveness of thrombolytic therapy is determined by accessibility of thrombus compartments to plasminogen activators and, therefore, depends on permeability of thrombus to blood born macromolecules. Accumulation of 125I labeled proteins with molecular massess ranging from 150 to 450 kD into partly contracted blood clot or plasma clot was consistent with diffusion coefficients 3.2 x 10(-11) and 2.7 x 10(-11) m2 s-1, respectively. So far as the model conditions imitated those for venous thrombi, these data indicate that such thrombi are porous enough for immunoconjugates of relatively big size.

Inadequacy of sample sizes in clinical trials of laboratory parameters attributable to invalid statistical assumptions.

Clinical trials often determine the sample size based on the use of statistical methods such as analysis of variance, t tests, and rank sum tests, which compare mean or median values. The resulting studies rarely are big enough to show that the method is based on mistaken assumptions. Data from a recent clinical trial of nephrotoxicity associated with the use of contrast agents during angiography found a significant difference on the order of the difference that previous studies had intended to detect. It also showed that a central assumption does not apply to changes in serum creatinine. As a result, the previous studies had considerably lower power than believed. Their lack of significance reflected only the mistaken assumption. Analysis of variance, t tests, and rank sum tests may be just as invalid for other clinical parameters. Lack of significance cannot be automatically taken to imply a small treatment effect.

[Nitrate water pollution of drinking water in the Bydgoszcz district].

The study was undertaken to evaluate the degree of drinking water pollution in the towns and countryside of the Bydgoszcz District, supplied by various installations, on the basis of the results of investigations of sanitary-epidemiological stations. Evaluation was done according to the state in 1985 as compared with that of 1981 to establish the eventual dynamics of the changes. The data presented in the tables indicate that the water supplied by the big water lines (public and institutional) does not contain nitrate quantities exceeding the norm (above 10 mg N/dm3). Their presence at a 20 mg N level/dm3 was found only in four (1.5%) lines supplying large state farms. Small installations were much more frequently polluted with nitrates, especially public wells and those of local institutions, a total of 8.8% in towns and as many as 17% in the country, where a tendency to deterioration of this state is also visible in contrast to the towns. Among the plants and institutions supplied by these water sources the situation is most unfavourable in agriculture, schools, educational and food producing and distributing establishments. The nitrate concentration in the analysed water varied in general within the limits of 10-30 mg N/dm3, maximal amounts within 60-200 mg N/dm3, showing an about 50% decrease in the compared time periods. Nitrates occur most frequently and in highest concentrations in water of wells belonging to individual households which supply about 50% of the rural population and about 11% of town dwellers.(ABSTRACT TRUNCATED AT 250 WORDS)

Building deals on bedrock.

The headlines are filled with the sorry tales of companies like Vivendi and AOL Time Warner that tried to use mergers and acquisitions to grow big fast or transform fundamentally weak business models. But, drawing on extensive data and experience, the authors conclude that major deals make sense in only two circumstances: when they reinforce a company's existing basis of competition or when they help a company make the shift, as the industry's competitive base changes. In most stable industries, the authors contend, only one basis--superior cost position, brand power, consumer loyalty, real-asset advantage, or government protection--leads to industry leadership, and companies should do only those deals that bolster a strategy to capitalize on that competitive base. That's what Kellogg did when it acquired Keebler. Rather than bow to price pressures from lesser players, Kellogg sought to strengthen its existing basis of competition--its brand--through Keebler's innovative distribution system. A company coping with a changing industry should embark on a series of acquisitions (most likely coupled with divestitures) aimed at moving the firm to the new competitive basis. That's what Comcast did when changes in government regulations fundamentally altered the broadcast industry. In such cases, speed is essential, the investments required are huge, and half-measures can be worse than nothing at all. Still, the research shows, successful acquirers are not those that try to swallow a single, large, supposedly transformative deal but those that go to the M&A table often and take small bites. Deals can fuel growth--as long as they're anchored in the fundamental way money is made in your industry. Fail to understand that and no amount of integration planning will keep you and your shareholders from bearing the high cost of your mistakes.

[Sweet's syndrome. A study of 10 cases and review of the literature].

Ten cases of acute febrile neutrophilic dermatosis or Sweet's disease have been studied clinically and histologically. Seventy percent of the patients were females with mean age of 43.1 +/-9 years. All of them presented the typical skin lesions consisting of papules and painful erythematous--edematous plaques in face, neck and upper chest. Fever was observed in seven patients and painful joints in four cases. One case presented polyarthritis of the big joints and there was one other case of conjuntivitis. The analytical data revealed a constant increase in sedimentation rate observed in 90% of patients. Leukocytosis was observed in 30% of patients and neutrophilia in 40%. Histologically, the lesions showed a neutrophilic infiltration of the skin without signs of vasculitis. Eight patients received treatment with Prednisone per os, one of whom, because of his relapses, was later given Potassium Iodine. Another patient was treated with Indomethacin, and one patient did not received any treatment. The evolution was favorable in all cases with sustained remissions. Sweet's Syndrome has been described associated mainly with acute myeloid leukemia in 10-20% of patients and in isolated instances with other systemic and neoplasic diseases. The concomitant conditions in 50% of our cases were: Ulcerative colitis, nodular sclerotic Hodgkin disease, infiltrative ductal carcinoma of the breast, carcinoma of the uterus neck and Crohn's disease; these last two associations had not been previously described in the literature.
