Neuroticism, symptom reports, and type A behavior: interpretive cautions for the Framingham Scale.

The Framingham Type A Scale (FTAS) is one of three primary measures of Type A behavior. Unlike the structured interview (SI) and Jenkins Activity Survey (JAS), the FTAS is correlated with neuroticism. Further, neuroticism and FTAS scores predict angina-like chest pain complaints but not more definitive coronary heart disease (CHD) end points. Thus, the FTAS may be unique among Type A measures in its susceptibility to the neuroticism-symptom reporting confound. The present study examined associations of the SI, JAS, and FTAS with neuroticism and symptom reporting in two independent samples of undergraduate males. Unlike the SI and JAS, the FTAS was correlated with neuroticism and symptom reporting. Further, the correlations of FTAS scores and symptoms were due to shared variance with neuroticism. The findings are discussed in terms of a possible alternative interpretation of the Framingham Study and the need to consider neuroticism in studies of personality and health.

The relative safety of an oxygenator.

Analysis of prospectively registered incidents related to cardiopulmonary bypass (CPB) was initiated to establish the incident rate for 10 different oxygenator brands employed over a seven-year period in 5000 clinical perfusions. A general safety index (SI) was defined as the number of recorded incidents in a given series of oxygenators divided by its total number and multiplied by 100. Specific SI was calculated for each of the following categories: high-pressure drop, debris, gas exchange, leakage, material failure and oxygenator change-out. An SI of 0.2 was arbitrarily set as a reference and an acceptable safety level. An estimate of the relative risk for a particular oxygenator brand was compared with the reference by calculating the odd's ratio with a 95% confidence interval. The mean SI was determined to be 1.6, ranging from 0 for the Maxima CBAS and the Cobe CML to 2.92 for the Safe oxygenator. The dominating specific type of incident was HPD with an SI of 0.81 followed by debris, SI = 0.71. A systematic analysis of adverse events in CPB may be used to evaluate and to set standards, a method already employed in the pharmaceutical industry. Our results indicate that oxygenator safety margins may vary between different brands.

Association between insulin sensitivity and post-glucose challenge plasma insulin values in overweight Latino youth.

OBJECTIVE: To determine associations between directly measured insulin sensitivity (SI) and oral glucose tolerance test (OGTT)-derived plasma insulin values, or calculated SI indices, in overweight peripubertal Latino children at risk for type 2 diabetes. RESEARCH DESIGN AND METHODS: Thirty overweight Latino children with a family history of type 2 diabetes, aged 8-13 years, Tanner stages 1-2, underwent an OGTT. Fasting and 2-h plasma insulin values and OGTT-derived SI indices were compared with SI derived from a frequently sampled intravenous glucose tolerance test (FSIGTT) with minimal model analysis, before and after adjustment for total body fat and lean tissue mass, or BMI. RESULTS: FSIGTT-derived SI for all subjects was 1.62 +/- 0.78 x 10(-4) min (-1) x (microU/ml), with no sex differences. SI correlated (all P values < 0.001) with fasting (r = -0.57) and 2-h (r = -0.58) plasma insulin and all SI indices (r = -0.57 to 0.67). After adjusting for total body fat and lean tissue mass, or BMI, the associations between SI and either fasting insulin or fasting SI indices were no longer significant. However, the 2-h insulin and post-glucose challenge SI indices maintained significant independent associations with SI, even after adjustment for body composition. CONCLUSIONS: In overweight, peripubertal Latino children at risk for type 2 diabetes, the 2-h plasma insulin value and postchallenge SI indices are better independent correlates of SI than fasting values, after accounting for body composition. The 2-h insulin may therefore be superior to fasting insulin as a single blood sample value for clinical or epidemiological estimates of SI, especially when combined with assessment of body composition.

Characterization of rainbow trout egg quality: a case study using four different breeding protocols, with emphasis on the incidence of embryonic malformations.

The aim of this study was to set up a methodology to accurately evaluate the effects of various husbandry practices on a fish broodstock based on the developmental potential of the egg. For that purpose, long-short photoperiod manipulations (tested twice, PM1 and PM2 groups), spawning induction by injection of a GnRH analog (SI group), and a 16-day post-ovulatory ageing of eggs (POA group) were used in rainbow trout (Oncorhynchus mykiss). Females without any treatment were used as a control group. Survival at eying (E) and yolk-sac resorption (YSR) were recorded and malformations at YSR were monitored according to a detailed typology that included cyclopia, torsion, incomplete YSR, prognathia, and others. Egg weight was also monitored. A deleterious effect of photoperiod manipulation was observed on egg quality in both PM1 and PM2 groups. Incomplete YSR appeared as the predominant malformation while cyclopia type was nearly absent. In the SI group, a limited effect on egg quality was observed in comparison to the other experimental groups, although the percentage of normal alevins at YSR was significantly lower than in the control group. Finally, the most important effects on egg quality were observed in the POA group. The percentage of normal alevins was only 14+/-6% (mean+/-95% confidence interval) while the percentage of malformed embryos reached 49+/-11%. The proportion of cyclopia was significantly higher than in the control group. In conclusion, the type of egg quality alteration is extremely dependent on the applied breeding protocols, and the proposed methodology is able to discriminate those experimental conditions even when the impact on egg quality is limited.

Lipoprotein lipase (LPL) mass in preheparin serum reflects insulin sensitivity.

Lipoprotein lipase (LPL) is one of the enzymes regulated by insulin and its plasma activity reflects insulin sensitivity. Although intravenous heparin injection is required to measure LPL activity, we can detect LPL mass in preheparin serum (Pr-LPL mass) by immunoassay. In this study, we examined whether Pr-LPL mass reflects insulin sensitivity. We measured Pr-LPL mass, insulin sensitivity (Si), and acute insulin release in response to a glucose bolus (AIRg) in subjects with normal glucose tolerance (NGT; n = 23), impaired glucose tolerance (IGT; n = 10), and Type II diabetes mellitus (DM; n = 48). Si and AIRg were determined by minimal model analysis. We also compared Pr-LPL mass with the homeostasis model assessment of insulin resistance (HOMA-R) and the urinary excretion of C-peptide (urine CPR). We found that Pr-LPL mass correlated significantly with Si ( r = 0.354, P < 0.01) in all the subjects. This correlation was still significant in the NGT group (P < 0.472, P < 0.05), DM group (r = 0.311, P < 0.01), and DM group with a fasting plasma glucose >150 mg/dl ( n = 20, r = 0.459. P < 0.05). Moreover, Pr-LPL mass correlated negatively with HOMA-R (r = -0.272. P < 0.05) and fasting IRI (r = -0.256, P < 0.05). By contrast, Pr-LPL mass was not correlated with either urine CPR or logAIRg that reflect the ability to secrete insulin. In conclusion, Pr-LPL mass reflects insulin sensitivity. We speculate that Pr-LPL mass might be used to assess insulin sensitivity not only in the general population but also in advanced diabetic patients.

Participation of the matrix metalloproteinase inhibitor in Thy-1 nephritis.

What influence would be shown in Thy-1 glomerulonephritis when the synthetic matrix metalloproteinase (MMP) inhibitor SI-27 is administered? Five groups of 80 male Wistar rats were studied: healthy group; treated healthy group; nephritic group; pretreated nephritic group; and post-treated nephritic group. SI-27 treatment of nephritic animals was initiated either 2 days before or 2 days after anti-Thy-1.1 antibody injection. On days 7, 14, 26 and 42 after disease induction, we examined renal histology, extracellular matrix (ECM) constituent, and MMP activity. SI-27 treated Thy-1 groups resulted in significant reduction of glomerular cells including alpha-smooth muscle actin (alpha-SMA) positive mesangial cells and suppressed expression of type IV collagen at 7 days. Moreover, type I collagen was also decreased by SI-27 at 42 days. However, glomerular cell numbers did not show any significant changes at 14, 26 and 42 days. In gelatin zymography, the gelatinolytic band for MMP-9 was expressed in SI-27 treated Thy-1 nephritis groups, although it was not expressed in the nephritic group at day 7. However, the expression of MMP-9 was no longer seen at 14, 26 and 42 days. The bands for an active form of MMP-2 were expressed throughout the experimental period in the Thy-1 nephritic groups. These results suggest that MMP plays an important role in the development of Thy-1 nephritis, and even if the synthetic MMP inhibitor intercepts the initial increase of glomerular cells and matrices, it does not inhibit recovery to normal glomerular capillary structures in Thy-1 nephritis.

Relationship of AcrySof acrylic and PhacoFlex silicone intraocular lenses to visual acuity and posterior capsule opacification.

PURPOSE: To compare the changes in visual acuity and the development of posterior capsule opacification (PCO) with AcrySof acrylic intraocular lenses (IOLs) (Alcon Laboratories) and second-generation PhacoFlex silicone IOLs (Allergan ). SETTING: Eye Associates of New Mexico and Southwest Colorado, Albuquerque, New Mexico, USA. METHODS: Medical charts of patients having cataract extraction with implantation of an AcrySof MA30BA or MA60BM (MA30/60) or PhacoFlex SI-30NB or SI-40NB (SI-30/40) IOL between January 1995 and June 1997 were abstracted. Analyzed were the changes in visual acuity and development of PCO 1 month postoperatively and at the last available ophthalmologist visit or the visit before neodymium:YAG (Nd:YAG) capsulotomy. RESULTS: Patients with MA30/60 acrylic IOLs were significantly older, had a worse preoperative best corrected visual acuity (BCVA), and had more concomitant ocular diseases than those with SI-30/40 silicone IOLs. The change in BCVA from preoperatively to 1 month postoperatively was equivalent in the 2 lens groups. The BCVA declined from 1 month postoperatively to the last recorded or pre-Nd:YAG visit. This decline was greater in eyes with SI-30/40 silicone IOLs than in those with MA30/60 acrylic IOLs. Although the decrease in BCVA between IOL types was not significantly different, eyes with a SI-30/40 silicone IOL were significantly more likely to develop PCO and have Nd:YAG capsulotomy. Eyes developing PCO had a statistically significant decline in BCVA from 1 month postoperatively to the last/pre-Nd:YAG visit. CONCLUSIONS: The MA30/60 acrylic lenses were associated with lower PCO and Nd:YAG capsulotomy rates than second-generation SI-30/40 silicone IOLs. Patients with MA30/60 IOLs also tended to have less of a decrease in visual acuity than patients with SI-30/40 silicone lenses, probably as a result of the difference in PCO rates between groups.

Assessing the role of neutrophil apoptosis in the resolution of particle-induced pulmonary inflammation.

Following inflammatory-cell recruitment in the lung, neutrophil apoptosis and subsequent engulfment by macrophages are regarded as important components in the resolution of pulmonary inflammation. The goal of this study was to further investigate the role of apoptosis and its influence, if any, on the pulmonary inflammatory process following exposures to the following particulate types: amorphous (AMO) or crystalline silica (Si), lipopolysaccharide (LPS), or pigment-grade titanium dioxide (TiO2). Rats were intratracheally instilled either with TiO2, AMO, or Si particles at doses of 1 or 5 mg/kg or 6 microg LPS. Following exposures, bronchoalveolar lavage fluids and lung tissues were collected and evaluated at 12, 24, 48, or 168 h (i.e., 1 wk). At the 1 mg/kg dose, AMO instillation produced the highest pulmonary inflammatory response, concomitant with a rise in apoptotic cells that mirrored temporally the transient nature of the inflammatory response. At 5 mg/kg, amorphous silica and crystalline silica particles induced substantial pulmonary inflammation [approximately 50-60% neutrophils (PMNs)] at 12 h postexposure (pe). A fundamental difference between the two inflammatory patterns, however, was the subsequent reversibility of inflammation in the AMO-exposed rats at 168 h postexposure and the sustained inflammatory effect in the Si-exposed animals measured through 168 h pe (approximately 40% PMNs). Pulmonary apoptotic responses in AMO-exposed rats mirrored temporally and correlated with the time-course reduction of inflammatory responses, leading to resolution. In the Si-exposed rats, apoptotic levels remained elevated, concomitant with sustained inflammation measured through 168 h pe. High doses of TiO2 particles produced transient lung inflammation, but with low levels of apoptosis. In addition, instillation of LPS produced a transient inflammatory response which mirrored the time course of apoptosis levels and was resolved by 168 h pe. cDNA microarray methods demonstrated that gene expression was altered for several apoptosis-related genes in AMO-, Si-, and LPS-exposed animals at 24 h pe. The results of these studies demonstrate that, following exposures, the resolution of lung inflammation correlated temporally with apoptotic levels of neutrophils in AMO- and LPS-exposed rats. Alternatively, instillation of crystalline silica resulted in sustained pulmonary inflammation and measurable apoptosis at 1 wk postexposure, but the apoptotic cell processes were not effective in resolving the inflammatory response. The findings suggest that the coordination between the resolution of inflammation and inflammatory cell apoptosis in the lung is dependent on the particle-type and that other factors, such as particle cytotoxicity, may also be important.

Kinetics of picosecond laser pulse induced charge separation and proton transfer in bacteriorhodopsin.

Bacteriorhodopsin (BR) films oriented by an electrophoretic method are deposited on a transparent conductive ITO glass. A counterelectrode of copper and gelose gel is used to compose a sandwich-type photodetector with the structure of ITO/BR film/gelose gel/Cu. A single 30-ps laser pulse and a mode-locked pulse train are respectively used to excite the BR photodetector. The ultrafast falling edge and the bipolar response signal are measured by the digital oscilloscope under seven different time ranges. Marquardt nonlinear least squares fitting is used to fit all the experimental data and a good fitting equation is found to describe the kinetic process of the photoelectric signal. Data fitting resolves six exponential components that can be assigned to a seven-step BR photocycle model: BR-->K-->KL-->L-->M-->N-->O-->BR. Comparing tests of the BR photodetector with a 100-ps Si PIN photodiode demonstrates that this type of BR photodetector has at least 100-ps response time and can also serve as a fast photoelectric switch.

Responses of monkey prefrontal neurons during a visual tracking task reinforced by substantia innominata self-stimulation.

The role of the substantia innominata (SI) in the generation of a behavior mediated by the prefrontal (PF) cortex was examined in two Japanese monkeys. PF neuronal activities related to a visual tracking task by wrist movement were recorded and neural responses to SI stimulation were analyzed. Sixty-six neurons showed task-related activity and were classified into 3 types. Type 1 (n = 31) showed transient activation during the movement. Type 2 (n = 26) showed gradually increasing activity before the reward presentation. Type 3 (n = 9) showed tonic activation from the GO signal to the reward presentation. Antidromic and orthodromic responses to SI stimulation were observed in every type. In Type 1, the percentage of antidromically activated neurons (26%) was similar to that of orthodromically activated ones (16%), but in Type 2, 83% of responding neurons showed antidromic responses, and 56% of Type 3 showed orthodromic responses. These results show that the different types of PF neurons have different anatomical relations to SI. Although orthodromically activated neurons were fewer than antidromically activated neurons, many orthodromically activated neurons showed movement-related activity. This suggests that ICSS at SI facilitates the generation of the behavior through the afferent pathway to PF.

O triclusters revisited: classical MD and quantum cluster results for glasses of composition (Al(2)O(3))2(SiO(2)).

The (17)O NMR spectrum of CaAl(2)Si(2)O(8) glass shows two types of O sites that are not present in the crystalline material. One of these, with (17)O NMR parameters C(Q) = 2.3 MHz and delta = +20 ppm, has been assigned to a "tricluster" O, a local geometry in which the O is coordinated to three tetrahedrally coordinated atoms, either Al or Si. For crystalline CaAl(4)O(7), a tricluster site (with three Al linkages to O, i.e., OAl(3)) has been characterized experimentally, with a C(Q) of 2.5 MHz and a delta of about +40 ppm. Thus, a C(Q) value of 2.5 MHz or less seems to be a characteristic of such sites, although they may show a range of delta values. However, several different quantum chemical cluster calculations employing energy-optimized geometries for various tricluster species have given C(Q) values considerably larger than that seen experimentally in the CaAl(2)Si(2)O(8) glass (with minimum C(Q) values of 3.0 MHz even for all Al species). We have recently shown that for edge-sharing geometries, in which the tricluster O atoms participate in "two-membered rings" of composition Al(2)O(2), the calculated C(Q) values are considerably lower, in the range identified in the glass. However, such two-membered ring geometries had been observed only in crystalline inorganic alumoxanes. Ab initio MD calculations on related compositions, such as the calcium aluminosilicate, CAS, (CaO)(0.21)(Al(2)O(3))(0.12)(SiO(2))(0.67), show a small percentage of O triclusters, but none in two-membered rings of the Al(2)O(2) type, and the calculated C(Q) values for the triclusters that do exist are higher than seen in the original experiments on CaAl(2)Si(2)O(8) glass and not significantly different from those for two-coordinate O in Si-O-Al sites. However, a classical MD simulation of the structure of glassy aluminum silicate AS2, (Al(2)O(3))2(SiO(2)), gave a predominance of O triclusters within two-membered rings, with structures much like those seen in the alumoxanes. We have now calculated (17)O nuclear quadrupole coupling constants and NMR shielding values for clusters extracted from these simulations, using standard quantum chemical methods. The calculated C(Q) values for these O triclusters are now in the range observed experimentally in the CaAl(2)Si(2)O(8) glass (around 2.3-2.6 MHz) when the tricluster O is surrounded by three Al, two of which are part of an Al(2)O(2) ring. This supports the experimentalists' contention that such tricluster O species do exist and have been seen by (17)O NMR.

Seabed classification from acoustic profiling data using the similarity index.

We introduce the similarity index (SI) for the classification of the sea floor from acoustic profiling data. The essential part of our approach is the singular value decomposition of the data to extract a signal coherent trace-to-trace using the Karhunen-Loeve transform. SI is defined as the percentage of the energy of the coherent part contained in the bottom return signals. Important aspects of SI are that it is easily computed and that it represents the textural roughness of the sea floor as a function of grain size, hardness, and a degree of sediment sorting. In a real data example, we classified a section of the sea floor off Cheju Island south of the Korean Peninsula and compared the result with the sedimentology defined from direct sediment sampling and side scan sonar records. The comparison shows that SI can efficiently discriminate the bottom properties by delineating sediment-type boundaries and transition zones in more detail. Therefore, we propose that SI is an effective parameter for geoacoustic modeling.

Association of IL-7 with disease progression in Chinese HIV-1 seropositive individuals.

BACKGROUND: Elevated levels of interleukin-7 (IL-7) have been correlated with CD4(+) T cell depletion and the emergence of syncytium-inducing (SI) variants in human immunodeficiency virus type-1 (HIV-1) infection, and suggested as an indicator of acquired immunodeficiency syndrome (AIDS) disease progression. Therefore, we investigated the effects of IL-7 on disease progression and virus phenotype in Chinese HIV/AIDS patients. METHODS: In a cross-sectional study of 71 untreated HIV-1 seropositive individuals and 12 healthy donors, plasma IL-7 levels were determined by an ultra sensitive enzyme-linked immunosorbent assay (ELISA), and its relations to CD4(+) T cells, CD8(+) T cells, plasma viral loads and HIV phenotypes were analyzed. RESULTS: Significant higher IL-7 levels were found in Chinese HIV/AIDS patients [(3.33 +/- 3.60) pg/ml] than those of health controls [(1.2 +/- 0.81) pg/ml] (P < 0.05), and IL-7 levels were inversely associated with CD4(+) T cell counts (r = -0.497, P < 0.01). Furthermore, IL-7 levels were significant higher in patients with SI variants [(9.12 +/- 4.55) pg/ml] than those with non-syncytium-inducing variants [(1.50 +/- 2.69) pg/ml] (P < 0.01). CONCLUSIONS: Increased IL-7 levels were found in Chinese HIV/AIDS patients and significantly associated with disease progression, thus increased IL-7 plasma levels may indicate disease progression.

Distribution of GAD-immunoreactive neurons in the first (SI) and second (SII) somatosensory cortex of the monkey.

The distribution of neurons immunoreactive for glutamic acid decarboxylase (GAD), the synthesizing enzyme of gamma-aminobutyric acid (GABA), was examined in the first (SI) and second (SII) somatosensory cortex of monkeys. GAD-like immunoreactive (GAD-LI) somata and puncta were present in all layers of SI and SII. All GAD-LI somata were identified as non-pyramidal neurons and were most numerous in layer IV of SI and in layer III of SII. Layer IV of SI also contained the highest density of GAD-LI puncta. In SII, GAD-LI puncta were distributed more homogeneously and did not show a dense band of labelled puncta in layer IV. The major and minor diameters of GAD-LI somata in SII ranged from 6.9 to 26.2 micron and from 6.2 to 19.0 micron, respectively. The major diameters of GAD-LI somata in SII were significantly smaller than those in SI in layers I, III and IV. Differences between the distributions of GAD-LI puncta and somata in SI and SII may be accounted for by differences in the number and/or distribution of different types of GABAergic neurons. Functional differences of neurons in SI and SII may be related to the differences in GABAergic inhibitory mechanisms and reflected in the distribution of GABAergic neurons.

Storage (irritative) and voiding (obstructive) symptoms as predictors of benign prostatic hyperplasia progression and related outcomes.

OBJECTIVES: To assess the utility of voiding and filling symptom subscores in predicting features of benign prostatic hyperplasia (BPH) progression, including acute urinary retention (AUR) and prostate surgery. METHODS: The Proscar Long-term Efficacy and Safety Study (PLESS) was a 4-year study designed to evaluate the effects of finasteride versus placebo in men with lower urinary tract symptoms (LUTS), clinical evidence of BPH, and no evidence of prostate cancer. A self-administered questionnaire was employed to quantify LUTS at baseline. Receiver operating characteristics (ROC) curves were used to assess baseline characteristics from patients treated with placebo as predictors of outcomes. The characteristics assessed included the overall symptom score (Quasi-AUA SI), separate voiding and filling subscores, prostate volume (PV) and serum prostate-specific antigen (PSA) levels. RESULTS: PV and PSA were superior to the symptom scores at predicting episodes of spontaneous AUR and all types of AUR. The Quasi-AUA SI and the filling and voiding subscores were effective at predicting progression to surgery; however, PSA was more effective at predicting this outcome. To better evaluate symptoms as predictors of surgery, patients who experienced a preceding episode of AUR were excluded from the surgery analysis. In the absence of preceding AUR, the best predictors of future surgery were the Quasi-AUA SI and the filling subscore. CONCLUSIONS: Among men with LUTS, clinical BPH and no history of AUR, the overall symptom score and storage subscore are useful parameters to aid clinicians in identifying patients at risk for future prostate surgery. PV and PSA were the best predictors of AUR, while PSA was the best predictor of prostate surgery (for all indications).

Stroke volume response to progressive exercise in athletes engaged in different types of training.

Using the impedance cardiography method, heart rate (fc) matched changes on indexed stroke volume (SI) and cardiac output (CI) were compared in subjects engaged in different types of training. The subjects consisted of untrained controls (C), volleyball players (VB) who spent about half of their training time (360 min.week-1) doing anaerobic conditioning exercises and who had a maximal oxygen uptake (VO2max) 41% higher than the controls, and distance runners (D) who spent all their training time (366 min.week-1) doing aerobic conditioning exercises and who had a VO2max 26% higher than VB. The subjects performed progressive submaximal cycle ergometer exercise (10 W.min-1) up to fc of 150 beats.min-1. In group C, SI had increased significantly (P < 0.05) at fc of 90 beats.min-1 (+32%) and maintained this difference up to 110 beats.min-1, only to return to resting values on reaching 130 beats.min-1 with no further changes. In group VB, SI peaked (+54%) at fc of 110 beats.min-1, reaching a value significantly higher than that of group C, but decreased progressively to 22% of the resting value on reaching 150 beats.min-1. In group D, SI peaked at fc of 130 beats.min-1 (+54%), reaching a value significantly higher than that of group VB, and showed no significant reduction with respect to this peak value on reaching 150 beats.min-1. As a consequence, the mean CI increase per fc unit was progressively higher in VB than in C (+46%) and in D than in VB (+105%).(ABSTRACT TRUNCATED AT 250 WORDS)

Modulation of adoptively transferred viable motheaten pathology in sublethally irradiated normal recipient mice by normal hematopoietic cells.

"Adoptive mev" chimeras were created by grafting hematopoietic cells (HC) from B6 viable motheaten (mev) mice into bg or wild sublethally irradiated (SI) mice: the chimeras developed mev-type symptoms such as paw inflammation and necrosis, lung damage, thymus atrophy, and high serological IgM concentration and autoantibody levels as well as rapid death. The phenotype of adoptive mev mice could be obtained even after two to three successive passages into SI mice (Kuntz et al., 1991. Immunology 73, 356). In the present study, mixed HC transfer experiments showed that bg or wild HC could not prevent or delay neither the serological symptoms nor the pathology conferred by cotransferred mev HC. Nevertheless, when cotransferred with adoptive mev chimera HC, bg or wild HC could block the development of the pathology and the morbidity, although only delayed the emergence of the serological abnormalities. This shows that the differentiation of mev HC in a bg or wild normal-type environment does not allow the maintenance of all mev HC-dependent abnormalities.

Detection of Barrett's adenocarcinoma of the gastric cardia with sucrase isomaltase and p53.

BACKGROUND: Routine surveillance for dysplastic epithelium in patients with Barrett's esophagus has markedly improved prognosis. Many patients with short segments of Barrett's mucosa near the esophagogastric junction remain undiagnosed and at risk for the development of Barrett's adenocarcinomas (BA). Sucrase isomaltase (SI), an intestinal enzyme, is highly expressed in intestinal-type Barrett's mucosa and frequently expressed in dysplastic Barrett's mucosa and BA. Sucrose isomaltase is not expressed in normal esophageal or gastric mucosa. Alterations in the p53 tumor suppressor gene are frequent events in dysplastic Barrett's mucosa and BA and result in nuclear protein accumulation. The purpose of this study was to determine the presence or absence of these markers of Barrett's mucosa in adenocarcinoma of the esophagogastric junction or cardia. METHODS: Expression of SI and p53 were examined in 40 BAs and 25 cardia adenocarcinomas using immunohistochemical techniques. RESULTS: Sucrose isomaltase analysis revealed positive staining in 55% (22/40) of the BAs and 44% (11/25) of the cardia adenocarcinomas. Of 14 cardia adenocarcinomas that were SI negative, 100% (14/14) had no associated Barrett's mucosa. However, in 21 cardia adenocarcinomas with no associated Barrett's mucosa, 7/21 (33%) were SI positive. This suggests that SI-positive tumors may represent BA without the standard definition of Barrett's esophagus being met. P53 was present in 65% of BAs and 64% of cardia adenocarcinomas, demonstrating the importance and similarity of this gene alteration in both tumor types. Staining was positive for SI or p53 in 77% (50/65) of all tumors. Tumors of lower stage expressed SI more often than higher stage tumors. CONCLUSIONS: These data suggest that a subset of cardia adenocarcinomas represent BAs. Surveillance endoscopy incorporating additional esophagogastric junction biopsies and assessment of SI or p53 may improve detection of intestinalized Barrett's mucosa and early dysplastic changes.
