Evaluating the efficacy and safety of Danning Pian in the short-term treatment of patients with non-alcoholic fatty liver disease: a multicenter clinical trial.

BACKGROUND: Currently, the management of non-alcoholic fatty liver disease (NAFLD) is less than certain. Some choleretic might be of potential benefit and deserve further evaluation. This multicenter clinical trial was designed to evaluate the efficacy and safety of Chinese herbal medicine Danning Pian (composed of rhubarb, grant knotweed, dried green orange peel and dried old orange peel) in the short-term treatment of patients with NAFLD. METHODS: The efficacy and safety of Danning Pian in the short-term treatment of NAFLD were investigated in 232 patients by a multicenter clinical trial during the period of July 1999 to February 2000. The patients consisting of 189 males and 43 females with an average age of 46.1+/-8.7 years were given 3-5 tablets of Danning Pian orally thrice daily for 3 months in addition to the other comprehensive therapy. The effects of Danning Pian on NAFLD were evaluated by the improvement of clinical symptoms, blood lipids, hepatic enzymes and liver ultrasonographic features. The drug safety was monitored by physical examinations, vital signs, and laboratory tests in addition to the assessment of the adverse events. RESULTS: All the enrolled patients completed the study except one whose serum ALT level was moderately increased during the therapy with Danning Pian. The effective rate of Danning Pian for the improvement of clinical symptoms, serum ALT levels, blood lipid and fatty liver was 85.8%, 78.2%, 39.6% and 34.0% respectively after the therapy for 3 months. However, the reduction of excessive body weight and waistline did not reach the significant level on the whole after the therapy. The general mild adverse events included diarrhea, skin rash and mild to moderate elevation of serum ALT level. The incidence of adverse reaction was 15.1%. CONCLUSION: The data of this trial indicate that Danning Pian is effective and safe, generally well-tolerated without severe adverse events, in the treatment of patients with NAFLD over a 3-month period.

[Antioxidative activity of water-soluble extracts from Fructus Polygoni Orientalis].

OBJECTIVE: To study the antioxidative activity of the water-soluble extracts from Fructus Polygoni Orientalis. METHODS: Malondialdehyde and superoxide dismutase were measured by thiobarbituric acid assay, nitrite method, respectively. The activity of glutathione peroxidase was determined by 5,5'-dithionbis (2-nitrobenzoic acid) photometric method. Lipofuscin was determined by Sohal method in the brain of senile model mice induced by D-galactose. The extracts of 0.8, 1.6 and 3.2 g.kg(-1).d(-1) were administered to the mice separately through oral gavage daily for 7 weeks. RESULTS: Malondialdehyde content increased, activities of superoxide dismutase and glutathione peroxidase in the heart, liver and kidney decreased. Levels of lipofuscin in the brain of senile model mice induced by D-galactose increased. The three doses of the extracts could inhibit the levels of lipofuscin in the brain, malondialdehyde in the heart, liver and kidney. These results showed that there was a negative dose-effect relationship. The extracts also up-regulated the levels of superoxide dismutase and glutathione peroxidase in the heart, liver and kidney with a positive dose-effect relationship. CONCLUSION: The water-soluble extracts exert its antioxidative activity by eliminating free hydroxyl radicals, increasing activities of superoxide dismutase and glutathione peroxidase.

Lipophilicity, antifungal and antioxidant properties of persilben.

The lipophilicity of persilben, an important parameter influencing the penetration of the compound through biological membranes, was determined experimentally by dynamic method and was theoretically calculated according to the fragmentation methods introduced by Crippen, Broto and Viswanadhan. The higher value of partition coefficient (log P = 3.89) obtained for persilben than that for resveratrol points to potentially higher ease of penetration of persilben into cells of living organism. Antimicrobial and antioxidant activities of persilben were tested. The obtained data suggest that this compound possesses some antioxidant activity. Persilben appears to have also some inhibitory effect against some species of dermatophytes from Tnichophyton genus but only at high concentrations.

[Effect of prepared radix Polygoni multiflori on the elevation of extracellular hydroxyl radical in striatum of rats induced by intracerebral perfusion of 6-hydroxy dopamine].

OBJECTIVE: To investigate the effect and mechanism of prepared Radix Polygoni Multiflori (RPM) on the elevation of extracellular hydroxyl radical in striatum of rats induced by intracerebral perfusion of 6-hydroxy dopamine (6-OHDA). METHODS: Cerebral microdialysis was used to establish the model. Hydroxyl radical was captured by salicylic acid, and 2,3-dihydroxy benzyl acid (2,3-DHBA) and 2,5-dihydroxy benzyl acid (2,5-DHBA) formed by hydroxyl radical in vital brain were measured by high performance liquid chromatography-electrochemical detector (HPLC-ED). RESULTS: After perfusion of 6-OHDA in brain of rats, the levels of 2,3-DHBA and 2,5-DHBA in the model group increased rapidly. The former was higher during the whole course of observation (P<0.01), while the latter was higher at most time points than that in the control group (P<0.05 or P<0.01). The level of 2,3-DHBA in the RPM group was lower than that in the model group at 5 time points (P < 0.05, P < 0. 01). CONCLUSION: RPM could inhibit the elevating of extracellular hydroxyl radical in striatum of rats induced by intracerebral perfusion of 6-OHDA, indicating one of the brain protective mechanisms of RPM may be related to its anti-oxidation effect.
